2.Effect of BuShen Prescription on metabolism of collagen Ⅰ in ovariectomized rats

Objective:To study the effect of BuShen Prescription on metabolism of collagen Ⅰ in ovariectomized rats,which will provide experimental evidence for treating osteoporosis. Methods:50 Ovariectomized SD rats were randomLy divided into 5 groups,including normal control group,model group,positive control (OVX) group,Zuogui Wan group and Yougui Wan group,each group with 10 rats. After 12 weeks' routine feed,normal control group and model group were intragastric administrated with pure water,positive control group was intragastric administrated with conjugated estrogens water-solution,Zuogui Wan group was intragastric administrated with Zuogui Wan decoction,and Yougui Wan group was intragastric administrated with Yougui Wan decoction,then the PINP,DPYD/Cr,NTX/Cr were detected after ovariectomized rats were intragastric administrated with the schedule for 12 weeks. Ionizing radiation can directly attack biological macromolecules including DNA, protein and lipids. Radiation can also damage cells indirectly by the formation of lots of free radicals through the radiolysis of surrounding water molecules. As the result, ion-izing radiation may make serious injury in human immune system, reproductive system and nervous system. Several synthetic radiopro-tectors have been used in clinic. However, most of them have some undesirable side effects. Hence, natural radioprotectors with high efficiency and low toxicity have been the focus of radioprotection field in recent years. In the paper, the recent research progress in the protection and repair effects of polysaccharides on radiation damage was reviewed.
4.Access and effect of catheter-directed thrombolysis for treating acute deep venous thrombosis of the lower extremity

Objective To evaluate the therapeutic effect of catheter-directed thrombolysis (CDT) for the treatment of acute deep venous thrombosis (DVT) of the lower extremity.Methods Clinical data of 195 patients of acute DVT treated by CDT and adjunctive angioplasty and stenting from August 2010 to February 2014 were analyzed retrospectively.CDT by antegrade puncture of popliteal vein,CDT by great saphenous vein and CDT by retrograde puncture of contralateral femoral vein were used in these cases.Venous recanalization was graded by a thrombus score based on pre-and post-treatment venography.Results Technique success rate,clinical success rate,in popliteal vein group,great saphenous vein group and contralateral femoral vein group were 94.6%,72.2% and 90.3%,97.3%,83.3%,and 90.0%.Patent rate of deep venous,patent rate of stenting and mild post-thrombotic syndrome (PTS) rate were 88.6%,66.7% and 75.0%,83.3%,57.1% and 88.9%,8.3%,26.7% and 20.8%.Conclusion CDT by antegrade puncture of popliteal vein group combined with adjunctive angioplasty and stenting is safe and effective with higher clinical success rate and better long term results than other approaches for the treatment of DVT patients.
5.Clinical features and follow-up study of neonatal asymmetric crying facies.

OBJECTIVETo evaluate the clinical characteristics and short-term outcomes of neonatal asymmetric crying facies (ACF), in order to improve recognition of the disease.
METHODSThe clinical data of 11 infants with ACF between January 2010 and February 2012 were retrospectively studied. Physical and neurological development were followed up at correct gestational age 44 weeks and 3 months.
RESULTSOf the 11 infants with ACF, 4 had ipsilateral ear malformation, 2 had congenital heart disease and 1 had syndactyly and polydactyly. Of the 11 infants, 8 were male and 3 were female. Eight infants presented with lesions on the left side and 3 presented with lesions on the right. The fathers were aged over 35 in 8 cases and the mothers were over 30 in 7 cases. Eight mothers had a history of at least 3 pregnancies and 2 infants were born to mothers with diabetes mellitus. Physical index was below P10 in 1 case and 2 cases showed a low NBNA score and mild abnormal GMs (poor repertoire PR) during the writhing period at correct gestational age 44 weeks. Physical index was between P10-P90 and GM assessment during the fidgety period showed normal movements in all infants at correct gestational age 3 months, but they still had ACF.
CONCLUSIONSACF is associated with a high rate of other congenital malformations. The short-term outcomes of ACF infants are satisfactory, but long-term follow-up and interdisciplinary cooperation are necessary to improve prognosis.

6.Analysis of self-control trial results of narrow band imaging and white light in transurethral resection of bladder tumor.

OBJECTIVE:
To investigate the effect of NBI assisted white light transurethral resection of bladder tumor (TURBT) in the treatment of bladder urothelial carcinoma and to summarize the experience of narrow band imaging (NBI) operation.
METHODS:
Patients with bladder urothelial carcinoma were selected, and TURBT was performed after anesthesia. First of all, the bladder tumor was found and resected under white light. Then we replaced with NBI, looked for suspicious lesions and resected them, The specimens excised under white light and NBI were collected separately. The number, location and pathological results of the lesions under white light were recorded, and the residual lesions under NBI were also recorded. To evaluate the effect of NBI, the ratio of residual bladder tumor was calculated. The cases were divided into three groups according to the time sequence. The clinical data of each group were collected and the learning curve of TURBT under NBI assisted white light was observed.
RESULTS:
A prospective study of 45 patients with bladder tumor from April 2018 to January 2020, including 32 males and 13 females, aged from 23 to 89 years, with an average age of 65.2 years. All the operations were successfully completed, without obvious complications after operation. Nine cases were single and 36 cases were multiple. The maximum diameter of the tumors was 0.5 to 4.0 cm, with an average of 2.2 cm. The histopathology of the resected tissue under white light was urothelial carcinoma, and 19 cases (42.2%) were pathologically positive by NBI resection. The 45 cases were divided into three groups according to the time sequence, 15 cases in each group. The true positive rate of NBI was 33.3%, 46.7% and 46.7%, respectively, and the false positive rate was 60.0%, 46.7% and 26.7%, respectively in the three groups.
CONCLUSION
TURBT is an effective way to treat bladder urothelial cancer, NBI is an effective supplement of white light, which can increase the detection rate of bladder cancer and reduce post-operative recurrence. The NBI light source has a certain learning curve. With the increase of cases, the false-positive rate of NBI is gradually reduced. After the NBI operator has rich experience, the recognition degree of flat tumor is gradually improved under white light, and the residual rate of NBI is reduced after the removal under white light.

7.Application of flipped classroom based on WeChat and TBL in basic surgery teaching

There are many problems existing in the basic teaching of traditional surgery, such as much more content, difficult operation, fewer learning hours, more passive acceptance while less active learning and so on. Flipped class based on the WeChat and team-based learning (TBL) is based on the on-line and off-line class. We can use WeChat group and subscription to issue relevant learning materials, learning tasks and test before class. Also, we can summarize and explain incisively by discussion after field operation in class, and answer and test in the way of WeChat discussion after class. Finally, the three-dimensional hybrid teaching will be condensed and sublimated by brand competition based on the basis of surgery, achieving knowledge transfer before class, knowledge internalization in class, knowledge consolida-tion after class and brand competition sublimation, in order to improve the teaching effect of surgical basis.
8.Value of intraoperative Surgical Apgar Score in predicting postoperative delirium in patients undergoing pancreaticoduodenectomy

Clinical data of patients underwent pancreaticoduodenectomy due to malignant ampullary tumors from January 2013 and December 2016 in our hospital with full access to medical records were collected,and 144 patients were enrolled in total.Surgical Apgar Score (SAS) was calculated based on the intraoperative lowest mean arterial pressure,lowest heart rate and blood loss.The patients were divided into 2 groups depending on whether postoperative delirium developed or not within 7 days after surgery.The receiver operating characteristic curve of SAS in predicting postoperative delirium was drawn.The area under the curve,optimal cut-off value and sensitivity and specificity were calculated.Thirty-six patients developed postoperative delirium,and the incidence was 25.0%.The area under the curve of SAS in predicting postoperative delirium was 0.86 (95% confidence interval 0.79-0.91).The optimal cut-off value was 6 with a sensitivity of 86% and a specificity of 83%.In conclusion,intraoperative SAS can predict the development of postoperative delirium in patients undergoing pancreaticoduodenectomy.
9.The change of negative pressure in the cupping-cup and its influence on the depth of filiform-needle insertion.

This study was designed to observe the negative pressure in the cup during cupping, and to investigate the influence of negative pressure on the depth of filiform-needle inserted and retained. In the beginning, the change of pressure after cupping on acupoint BL23 for a span of 20 minutes was recorded. Acupuncture on acupoint BL23 was performed; the filiform-needle was retained and followed by cupping at different levels of negative pressure, and the changes of the depth of needle insertion was measured. The results showed: the absolute value of the negative pressure in the cup goes down with time and is significantly lower after 20 minutes (P < 0.05); the negative pressure in the cup causes the retreat of the inserted needle; and under the same pressure, there is a greater length measurement in needle retreat for male than for female (P < 0.01). The length of needle retreat is related to the pressure in the cupping-cup and to gender.
