1.Change of serum FIB ,PCT and hs-CRP in children patients with infectious pneumonia and its clinical significance

Objective To observe the change of serum fibrinogen(FIB) ,procalcitonin(PCT) and hypersensitve C reactive protein (hs-CRP) in children with infectious pneumonia and its clinical significance .Methods One hundred and four cases of children with bacterial infectious pneumonia (bacterial group) ,109 cases with non-bacterial infectious pneumonia(non-bacterial group) and 110 healthy children under going physical examination (control group) were included into this study ,the serum FIB ,PCT and hs-CRP level were observed in each group .Results The FIB ,PCT and hs-CRP levels in the bacterial group were higher than those in the non-bacterial group and control group (P<0 .05);the FIB ,PCT and hs-CRP had no statistical difference among the virus group , mycoplasma group and chlamydia group had no atoctistical(P>0 .05) .The positive rate of combined detection of FIB ,PCT and hs-CRP in the non-bacterial group and bacterial group was significantly higher than that of single index detection (P<0 .05) .The FIB level in the bacterial group was positively correlated with PCT and hs-CRP(r=0 .445 ,0 .396 ,P<0 .05) .PCT and hs-CRP showed a positive correlation (r=0 .487 ,P=0 .021) .Conclusion The combined detection of FIB ,PCT and hs-CRP can effectively identify bacterial infection and non-bacterial infection and which has an important clinical significance for the diagnosis and treatment of pathogenic infection of pneumonia .

Objective To study the treatment of bladder neck obstruction. Methods Thirty nine cases of bladder neck obstruction were reviewed from 1990 to 1999. There were 25 males and 14 females. 39 cases were treated with Transurethral resection of the bladder neck. Results Symptoms were disappeared in 37 cases. Conclusions Transurethral resection is effective for bladder neck obstruction.

Methods In 50 patients with primary diagnoses as unstable angina that were undergone coronary angiography, the serum CRP levels were measured by using particle enhanced immunonephelometry. The relationships between the serum CRP level and the risk factors of CHD as well as the severity of coronary lesions were assessed. 4.Budd-Chiari syndrome in youth: clinical features and interventional therapy

Objective To present the clinical features and to evaluate interventional therapy for Budd-Chiari syndrome in Chinese youth.Methods From January 1990 to April 2012,227 cases who hospitalized at the age < 29 underwent color Doppler ultrasound scan and digital subtraction angiography (DSA).87 cases were with occlusive inferior vena cava (IVC type),105 cases with occlusive hepatic veins (HV type) and 35 cases with occlusive inferior vena cava and hepatic veins (MIX type).The occlusive veins were opened by percutaneous transluminal angioplasty (PTA),endovascular stent placement,intravenous catheter thrombolysis or combination.Postoperative anticoagulation was given to all patients.Results The symptoms and signs of portal hypertension disappeared or were alleviated in successful cases.Technical success was achieved in 210 patients.The success rate was 100% in IVC type,85.7% in HV type and 94.3% in MIX type.IVC pressure decreased from (26.52 ± 8.16) cm H2O to (14.28 ±4.08) cmH2O(P < 0.05) and HV pressure dropped from(35.70 ± 13.26) cm H2O to(18.36 ±8.16) cm H2O (P <0.05).Restenosis or occlusion was found in 21.4% (45/210) patients after a follow-up of 1 month to 15 years.The rate was 13.8% (12/87) in IVC type,31.1% (28/90) in HV type and 15.2% (5/33) in MIX type.These patients were managed by interventional procedures.Technical successwas achieved in 44 cases with restenosis.Conclusions Hepatic vein occlusion was the most common type of BCS in Chinese youth.The symptoms and signs of portal hypertension were the initial clinical manifestations.Postoperative recurrence rate in HV type was higher than that in the other two types.

6.Relationship between rotatioanal deformity and operational correction rate in scoliosis

Objective To study the relationship between rotational deformity and operational correction rate in scoliosis. MethodsFrom January 2005 to December 2006,44 cases of adolescent idiopathic scoliosis with operational correction were reviewed.Before operation,all the patients were performed MRI scan of the whole spine and the rotation angle of superior and inferior endplate of nine vertebraes with the center of apical vertebrae were measured so that the mechanical torsion angle of vertebrae and disc were calculated,and the rotation compliance of the spine was obtained.In addition,preoperative plain,lateral and bending film of thoracic-lumbar vertebrae were taken to measure the Cobb angle,and the items above were compared with those of post-operation to calculate the scoliosis flexibility and correction rate.Correlation analysis was conducted between the rotation compliance,flexibility and operational correction rate in scoliosis.Results There were significant differences in the mean coronal and sagittal Cobb angle before and after the operation(P
7.Different doses of atorvastatin for the treatment of chronic heart failure in patients

Objective:To investigate the clinical efficacy of different doses of atorvastatin in the treatment of chronic heart failure.Methods:A total of 100 patients with chronic heart failure who received treatment in the Third People's Hospital of Bengbu Medical College from June 2019 to June 2020 were included in this study. They were randomly divided into a control group (conventional treatment + atorvastatin calcium tablet 10 mg, n = 49) and an observation group (conventional treatment + atorvastatin calcium tablet 20 mg, n = 51). Before and after 12 weeks of treatment, serum levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) and inflammatory cytokines such as high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), interleukin-10 (IL-10) as well as ventricular remodeling (echocardiographic index) were compared between the two groups. Results:After treatment, serum level of NT-proBNP was (506.56 ± 62.37) pg/mL and (660.85 ± 74.55) pg/mL, serum level of hs-CRP was (14.74 ± 2.69) mg/L and (23.31 ± 3.45) mg/L, serum level of IL-6 was (36.77 ± 4.78) ng/L and (43.12 ± 5.22) ng/L, serum level of IL-10 was (12.36 ± 2.63) ng /L and (15.39 ± 3.56) ng/L, left ventricular end diastolic diameter was (37.74 ± 6.83) mm and (42.36 ± 7.73) mm, left ventricular ejection fraction was (45.78 ± 3.31)% and (42.63 ± 2.56)%, and the ratio of early (E wave) to late (A wave) diastolic velocities (E/A) was (1.44 ± 0.06) and (1.21 ± 0.03), respectively in the observation and control groups. There were significant differences in serum levels of NT-proBNP, hs-CRP, IL-6 and IL-10 as well as LVEDD and LVEF between the observation and control groups ( t = 3.73, 3.07, 3.58, 3.68, 2.99, 3.12 and 2.98, all P < 0.05). Conclusion:Atorvastatin is beneficial to the prognosis of patients with chronic heart failure. The therapeutic efficacy of 20 mg atorvastatin per day is better than that of 10 mg atorvastatin per day.
8.Treatment of thoracolumbar fractures by unilateral pedicle screw fixation plus short segment pedicle screw through para-vertebral muscles

A total of 33 non-neurological symptoms patients with thoracolumbar fractures underwent unilateral pedicle screw fixation plus short segment pedicle screw through para-vertebral muscles. Preoperative computed tomography ( CT) scan showed one side pedicle was complete.The average follow-up period was 22 (12-40) months.There was no internal fixation failure.The posterior paraspinal approach for unstable thoracolumbar fractures , retaining posterior ligament complex and fixation by unilateral pedicle screw fixation through the pedicle of fractured vertebra , is both safe and effective for thoracolumbar fractures.

OBJECTIVETo evaluate the feasibility and efficacy of unilateral pedicle screw fixation in treating thoracolumbar fractures through paraspinal approach.
METHODSFrom January 2006 to January 2009,21 patients with single level thoracolumbar fracture without neurological symptoms were treated with unilateral pedicle screw fixation through paraspinal approach. There were 14 males and 7 females,aged from 21 to 65 years old with a mean of 36.4 years. The duration from injury to operation ranged from 6 h to 5 d with an average of 3 d. According to the classification of Denis fracture, compression fractures happedned in 12 cases and burst fractures happened in 9 cases,including 1 case with T5 fracture, 2 cases with T7 fracture, 2 cases with T10 fracture, 3 cases with T11 fracture, 8 cases with T12 fracture, and 5 cases with L1 fracture. Based on the Flankel grade, all patients were classified as grade E. Anterior vertebral body height ratio, sagittal Cobb angle, condition of internal fixation failure, visual analogue score (VAS) were evaluated.
RESULTSAll patients were followed up from 12 to 36 months with an average of 20.5 months. No internal fixation failure was found. Anterior vertebral body height ratios at preoperative 3 days after operation and last follow-up were 54.3 +/- 2.8, 92.9 +/- 1.5, 93.8 +/- 1.7, respectively;sagittal Cobb angle at the three timepoints were (27.8 +/- 2.5) degrees, (5.3 +/- 0.8) degrees, (6.3 +/- 1.4) degrees, respectively; the difference was statistical significant (P < 0.05). VAS was (1.2 +/- 0.4) points at last follow-up and had obviously improved (P < 0.05).
CONCLUSIONTreatment of thoracolumbar fractures with unilateral pedicle screw fixation through paraspinal approach is safe with the advantages of micro-trauma and less blood loss,which can not only completely retain the posterior spinal complex structure, reinforce the spinal stability, raise the reductional quality, but also improve the strength of fixation and the distribution of stress force.

10.Effect of trachea intubation and mechanical ventilation on patient's discharge with successful resuscitation

Objective To study the effect of trachea intubation and mechanical ventilation on the prognosis and discharge rate of patients with successful cardiac-pulmonary resuscitation.Method The clinical data of 389 patients,who were admitted from January 2005 to February 2007,were retrospectively analyzed.The relation between trachea intubation time and discharge rate was studied.According to the time from cardiac arrest to finishing trachea intubation,patients were divided into group A (within 3 minutes,n=209) and group B (over 3 minutes,n=143);according to the time from reaching emergency medicine department to finishing trachea intubation,the rest patients were divided into group C (within 5 minutes,n=9) and group D (over 5 minutes, n=38) minutes.The discharge rate was calculated between groups.The software of SPSS 11.0 was used for statistical analysis.Results The successful rate was 9.75% (389/3988),and 59 patients were discharged, with discharge rate 1.48% (59/3988).The discharge rate of group A was 19.62% (41/209),and was significantly higher than that of group B 6.99 % (10/143) (P
