1.Expression,identification and purification of the resuscitation-promoting factor A of mycobacterium tuberculosis

Objective:To construct the Resuscitation-promoting factor A(RpfA) expression plasmid of Mycobacterium tuberculosis, and express and purify it in E.coli.Methods:RpfAgene of Mycobaeterium tuberculosis was amplified with PCR method from the H37Rv,cloned into pET32a(+) vector,and then transformed into E.coli Top10.After identifying by restriction digestion and DNA sequencing,the constructed recombinant plasmid was transformed into E.coli BL21,and expressed the recombinant protein under the control initiated by T7 after induction with IPTG.The target protein was identified by His-tag in-gel Stain and purified by using affinity chromatograghy.Results:The size of the RpfA gene digested by restricted enzymes accorded with the theoretical size and the DNA sequence with the reported one in literatures.66ku protein was found by SDS-PAGE and His-tag in-gel Stain.Conclusion:The recombinant expression plasmid pET32a(+)-RpfA was successfully cloned and constructed, and the recombinant protein with high purity was obteined;which laid the basis for further study.

3.Factors of Nosocomial Infection of 1014 Disabled Children

ObjectiveTo explore the causation of nosocomial infection in disabled children.MethodsThe causation of nosocomial infection in disabled children (n=1014) was analyzed retrospectively. The relationship between nosocomial infection and the factors, such as constituent ration in the infections parts, time in hospital, age and contact history separately were analyzed.ResultsOut of the 1014 patients, 291 cases were infected (the rate was 29%). The main site was in the respiratory system (276 cases, 94.8%). Among that, 73.2 % in upper respiratory tract and 21.6% in lower respiratory tract. The infection of digestive canal was only 7.6 %. The nosocomial infection rate in children under 1, 1~3, and above 3 years old were 44.6%, 37.8% and 27.5% respectively. The rate of those with contagious contact history was 31.6%, significantly higher than the ones without contact history (9.1%) (P≤0.05). There was no significant difference between infection rate and the time in hospital.ConclusionThe nosocomial infection is related to the age, contagious contact history, hospital environment and body nutritional situation, but not with time in hospital.

5.Methylprednisolone combined with puerarin in the prevention of spinal cord ischemia-reperfusion injury

Objective To investigate the effect of methylprednisolone (MP) combined with puerarin in preventing the spinal cord ischemia-reperfusion injury (SCII).Methods The model of ischemia-repeffusion injury was established by clipping the abdominal aorta of rats for 30 min.Eighty male SD rats were randomly and equally divided into 4 groups: the blank group in which the abdominal aorta was only exposed,the positive drug group in which intravenous MP injection (30 mg/kg) was conducted 30 min before the establishment of the ischemia-reperfusion injury model,the therapy group in which intravenous MP (10 mg/kg)and puerarin (30 mg/kg) injection was conducted 30 min before the establishment of the model,and the model group in which the physiological saline equivalent to the volume of MP in the positive drug group were injected into the caudal vein of rats 30 min before the establishment of the model.Ten rats were taken from each group and the homogenation of the spinal cord was used to test expression level of superoxide dismutase (SOD) and sodium-potassium adenosinetriphosphatase (Na-K ATPase).For another 10 rats in each group,the BBB scoring system was used to evaluate the neural function of the hind limb immediately after recovery from anesthesia,12 h and 24 h after reperfusion,and the spinal cord was obtained 24 h after reperfusion to observe the morphous of the spinal cord neurons.Results The expression levels of SOD and Na-K ATPase in the model group,positive drug group and therapy group 3 h after reperfusion were significantly lower than that in the blank group; the expression levels of SOD and Na-K ATPase in positive drug group and therapy group were significantly higher than that in the model group,and there was no significant difference between the positive drug group and the therapy group with regard to the levels of SOD and Na-K ATPase.The BBB score was better in the positive drug group and the therapy group than those in the blank group and model group.Microscope observation showed that the degree of injury between positive drug group and therapy group was equivalent,while each of them was lower than that in the model group.Conclusion MP combined with puerarin plays a great role in the prevention of the SCII.
6.The design of meticulous maintenance management system in medical equipment

Objective:Based on the hospital resource planning(HRP) system, design in delicacy maintenance information management system of medical equipment, aims to solve problems for preventive maintenance, repair decision support, repairing parts management, fault library, practical training, cost analysis, workload statistics and other aspects.Methods: The five issues including establishment of a standardized procedure for solving maintenance, electronic document management, scientific and material support, comprehensive personnel management, cost analysis, statistical and intelligent decision support by the Visual Basic for application programming technology were solved to realize the database interaction. Constructing standardization process of equipment maintenance management and making full use of the stored information can provide guidance for equipment maintenance management activities.Results: The equipment maintenance management and integrated decision analysis, provides a strong reference for all types of decision-making of the hospitals. To realize information sharing among departments make hospital equipments maintenance comply with a standardized and scientific management requirements.Conclusion: It can be widely applied in hospital equipment maintenance management process and is significant to the achievement of scientific management of medical equipment.
7.Clinical curative effect and safety evaluation of three oxide inorganic polymer repair the pulp cavity perfora-tion under oral microscope

Objective To explore the clinical efficacy and safety of mouth three oxide inorganic polymer under a microscope to repair the pulp cavity perforation.Methods 120 patients with medullary cavity perforation, caries origin were selected as the research subjects,they were randomly divided into oral microscope inorganic oxide polymer repair the pulp cavity perforation of the experimental group and the resin inlay repair the pulp cavity perfora-tion of the control group,each group had 60 cases.The postoperative occlusion pain,swollen gums and loose,fistula formation,the incidence of chewing function not completed and X -ray curtate alveolar bone density decrease the shadow changes were compared between the two groups.Results The incidence rates of bite pain,swollen gums, loosening of the observation group were 5.00%,3.33%,6.67%,which of the control group were 11.60%,13.33%, 18.33%,the differences between two groups were significant (χ2 =5.49,P <0.05;χ2 =4.76,P <0.05;χ2 =4.23, P <0.05).The fistula formation,chewing function not completed,X -ray curtate alveolar bone density to reduce shadow expand of the observation group were 3.33%,5.00%,5.00%,which of the control group were 10.00%, 30.00%,40.00%,the differences between the two groups were significant (χ2 =5.34,P <0.05;χ2 =4.12,P <0.05;χ2 =4.56,P <0.05).Conclusion The clinical curative effect of oral three oxide inorganic polymer under a microscope to repair the pulp cavity perforation is outstanding,compared with the resin inlay repair the pulp cavity perforation has many advantages,it can be used as a kind of ideal method used in the clinical work.

10.Imaging features of thirteen cases of coronary-to-bronchial artery micro-fistula and the related clinical analyses

Objective To investigate imaging features and clinical characteristics of the small coronary-bronchial artery fistula (CBF). Methods By retrospective analysis of image data from May 2007 to November 2011 for coronary angiography in 30 284 patients without chronic lung or pulmonary vascular disease patients, 13 cases of small CBF (diameter of the end of fistula near coronary artery< 2 mm) were detected. Incidence of CBF was counted, its morphology was described, and its clinical features were preliminarily analyzed and summarized. Independent sample t test and χ2 test were used to compare diameters and incidences of CBF originatng from left and right coronary arteries. Results In this study primary CBF detection rate was 0.043%(13/30 284). The ends of CBFs near coronary arteries were slender and their diameter average was (1.57 ± 0.75) mm. In this study all of the CBF were from the right coronary artery and left circumflex coronary artery, finding no cases starting from the left anterior descending coronary artery. There was no significant difference between incidence of CBFs originating from the right coronary artery and left circumflex coronary artery coronary and the diameters of CBFs. In 13 cases of CBF, 9 cases of myocardial ischemic symptoms were caused by coronary artery stenosis, which was proven by angiography;Four cases without myocardial ischemic symptoms or with very atypical symptoms were finally diagnosed as cardiac neurosis. After branching, the CBF diameters of these coronary arteries were seen no evindently reduced, which suggested that the coronary bypass had less blood flow, and therefore had no significant hemodynamic significance. Conclusions Congenital CBF can exist in populations without chronic lung or pulmonary vascular disease. The main image features are that their diametes were small and all originate from the right coronary artery and left circumflex coronary artery. The most fundamental clinical features are that there're no symptoms of myocardial ischemia due to coronary artery steal and no obviously hemodynamic significance. It is important to well recognize this type of CBF for enriching radiographic knowledge, identifying various anatomic variations, and carrying out clinical diagnosis and treatment.
