1.The prognosis and relationship between soluble differentiation antigen of cluster designation 14 and the injury severity score of patients with multiple injuries

ObjectiveTo explore the prognosis and relationship between soluble differentiation antigen of cluster designation 14 (sCD14) and the injury severity score (ISS) of casualties with multiple injuries.Methods A total of 86 casualties with multiple injuries were enrolled from October 2009 to March 2010,and the severity of trauma of casualties were assessed in ISS score within the first 24 hours after accident,and the patients were divided into survival and non-survival groups as per the outcomes in 28 days after accident. Another 20 healthy subjects served as control group. In multiple injuries group,sCD14 concentrations were detected and APACHE Ⅱ scores were calculated on the 1st,3rd,5th and 7th days after accident.The related coefficient between sCD14 and ISS was calculated and then the values of sCD14 in predicting prognosis were analyzed by ROC curve. ResultsCompared with control group, sCD14 concentrations were obviously higher in casualties of multiple injuries group at all observation intervals ( P <0.01). Compared with survival group, casualties of non-survival group had more higher sCD14 concentrations and APACHE Ⅱ scores ( P < 0.05 ).The sCD14 concentration was correlated with ISS and the related coefficient was 0.469 ( P < 0.01 ). ROC curve analysis suggested sCD14 had values in predicting prognosis of casualties with multiple injuries and area under ROC curve was 0.820.Conclusions The concentration of sCD14 is correlated with ISS and can be used for predicting the prognosis of casualties with multiple injuries.
2.Prediction of Charlson weighted index of comorbidities on evaluating common diseases on > 60 years old patients with rectum cancer

Objective To investigate the effect of common diseases by the Charlson weighted index of comorbidities(WIC) in predicting 1-year survival rate of older patients with rectum cancer.Methods Retrospective analyzed 160 patients with rectum cancer who were admitted in general surgery of Beijing Chaoyang Hospital Affiliated to Capital Medical University from Jun.2011 to May 2012.The patients were divided into two groups according to 1-year survival status:survival group (n =123) and death group (n =37).The data were recorded including age,gender,cancer diagnosis,underlying diseases,operation mode (laparoscopic or open) The WIC and the Acute physiology and chronic health Ⅱ (APACHE Ⅱ) score were calculated.Logistic regression analysis was used to determine the independent predictors for 1-year survival rate.Receiver operating characteristics(ROC) curve was used to evaluate the value of WIC in predicting 1-year survival rate.Results Of the 160 patients,123 cases survived,37 cases died at 1 year after treatment.WIC score and APACHE score in death group were (2.2 ± 1.3) and (11.5 ± 5.1),higher than those of survival group (1.3 ± 1.2,9.5 ± 4.3 ;P =0.021,0.014).Multivariate Logistic regression analysis indicated that age,mode of operation,WIC score and APACHE score were related to the 1-year survival status of patients with rectal cancer(OR(95% CI):1.053 (1.018-1.178,1.021 (1.011-1.906),1.786 (1.203-2.235),1.019 (1.004-1.628).ROC curve area of forecasting of death of the WIC score,APACHE Ⅱ score and the combination of the two are 0.731,0.828,0.959.Conclusion The WIC scoring system can be a good evaluation method for 1-year survival rate in old patients with rectum cancer.
3.Pharmacokinetics and Relative Bioavailability of Acyclovir Tablets in Men

AIM to compare the bioavailability and pharmacokinetics of two acyclovir tablets. METHODS Concentrations of acyclovir in 10 men serum after po 600 mg of two acyclovir tablets were determined , in random 2-way crossover design. Pharmacokinetic parameters were also estimated. RESULTS The peak concentrations of test and reference tablets were 1.24±0.49 and 1.17±0.20 μg/ml, the AUC 7.56±1.80 and 7.56±2.10μg*h/ml, respectively. The relative bioavailability was 101.36%±11.62%. The test formulation was found bioequivalent to the reference in AUC and Cmax by two one-side t test. CONCLUSION Two tablets were bioequivalent.
4.Minimally invasive bi-endoscopic treatment of deep submandibular gland calculu.

OBJECTIVEA study was conducted to evaluate the clinical application of sialendoscopy combined with conven tional laparoscopy in a 30 degree angle in depth salivary calculus of submandibular glands.
METHODSA total of seventeen patients were presented with repeated swelling in the unilateral submandibular area from June 2013 to August 2015. All patients underwent CT examinations of the mandible and salivary gland function imaging preoperatively. Sialoendoscopy and conventional endoscopy were applied to do intraoral lithotomy. Among the study group, thirteen cases were males and four cases were females. A total of twelve were left cases and five were right cases. Results Seventeen patients had deep positive calculus in the submandibular glands under CT. Photo technetium function was basically normal, whereas secretion of ipsilateral decreased significantly in salivary function tests. Double endoscope assisted surgeries were performed successfully through the mouth to remove the stones. The operation time was (42+21) min. There were no obvious complications in perioperative periods.
CONCLUSIONIntraoral lithotomy assisted by sialoendoscopy and conventional laparoscopy for deep stones in the submandibular glands is a new technique. This technique will possibly lead to complete avoidance of gland resection because of early deep sialolithiasis.

5.Effect of dexmedetomidine sedation on perioperative hemodynamics and heart rate variability in the aged patients with cataract operation

Objective To investigate the effect of dexmedetomidine sedation on perioperative hemodynamics and heart rate variability (HRV) in the aged patients with cataract operation.Methods Fifty aged patients with cataract who accepted phacoemulsification operation in local anesthesia,were divided into experiment group(dexmedetomidine group) and control group (0.9% sodium chloride group) by random digits table method with 25 cases each.The experiment group was given local anesthesia,after dexmedetomidine load at 0.5 μg/kg intravenous infusion,dexmedetomidine continuous pumping [0.2-0.3 μg/ (kg ·h)] to maintain the depth of anesthesia.The control group was given the same dose 0.9% sodium chloride intravenous infusion.Local anesthesia method:2% lidocaine and 0.75% bupivacaine 3 ml mixing for retrobulbar anesthesia.The changes of HRV and hemodynamics in the 2 groups before operation (T0),beginning operation (T1),perioperative10min (T2),perioperative30min (T3),postoperative (T4) were recorded,and hemodynamies was analyzed.Results The normalized units of low frequency (LFnu) and low frequency (LF)/high frequency (HF) at T1,T2,T3 in control group were significantly higher than those at T0 [(73.9 ± 9.2)%,(74.5 ± 9.0)%,(75.8 ± 8.5)% vs.(61.3 ± 7.5)% and 2.4 ± 1.1,2.6 ± 1.3,2.7 ± 0.9 vs.1.6 ± 0.5],total frequency power (TP) was significantly lower than that at T0 [(1372 ± 419),(915 ± 243),(1389 ± 142) Hz vs.(2351 ± 608) Hz],there were statistical differences (P < 0.05).The LFnu and LF/HF at T1,T2,T3 in experiment group were higher than those at T0,but there were no statistical differences (P >0.05).The TP at T1,T2,T3 in experiment group was significantly lower than that at T0 [(1458 ± 325),(983 ±272),(1497 ± 210) Hz vs.(2235 ± 513) Hz],there was statistical difference (P < 0.05).There was no statistical difference in normalized units of HF at all time points between the 2 groups (P > 0.05).The LFnu,LF/HF and TP at T1,T2,T3 in experiment group were better than those in control group,there were statistical differences (P < 0.05).The fluctuation of hemodynamics in experiment group was significantly lower than that in control group,there was statistical difference (P < 0.05).Conclusion Dexmedetomidine sedation little effects on HRV during perioperative period in aged patients with cataract operation,can significantly reduce the influence of operation stimulation on autonomic nervous system and hemodynamics.
6.Thirty Cases of IgA Nephropathy Treated by Differential Diagnosis of Syndromes in TCM

Thirty caaes diagnosed as lGA nephroparthyby kidney biopay were treated bassed on differential diagnosis of syndromes the cases were divided into several stages via lesion in the lung -kidney (initial stage )lesion in the spleen-kidney (stable atage )and lesion in the liver -kidney (stable atage)and relapsing stage, and were treated by Chinese medicaments accordingly. The results revealed that 10 cases were totally mitigated; 13 cases, markedly alleviated; 4 cases, ameliorated, with only 3 cases ineffective. 8.Antibacterial activity and mechanism of baicalein.

Baicalein (BAI) is an effective bactericide. The antibacterial activity and mechanism experiments were carried out by determining conductivity and content of macromolecules of membrane penetrability, the oxidative respiratory metabolism and protein synthesis changes and the inhibition of DNA topoisomerase activities. Electrical conductivity and the number of large molecules of BAI increased 2.48% and 1.8%, respectively, than that of the control. However, the membrane integrity did not destroyed by BAI directly. With BAI treatment, inhibition rates of activities for SDH and MDH were 56.2% and 57.4%, respectively, demonstrating that BAI could inhibit cell respiratory. After treated with BAI for 20 h, the total soluble content of proteins decreased by 42.83%. Moreover, the activities of DNA topoisomerase I and II were inhibited completely by 0.2 mmol x L(-1) BAI. These results indicated that BAI had obvious antibacterial activity on Staphylococcus aureus. The mechanism is that it could affect bacterial membrane penetrability, inhibit protein synthesis and influence SDH, MDH and DNA topoisomerase I and II activities to exert its antibacterial functions.
9.The value of contact heat evoked potentials for the early diagnosis of diabetic peripheral polyneuropathy

Objective To study the value of contact heat evoked potentials (CHEPs) for the early diagnosis of diabetic peripheral polyneuropathy. Methods Seventy-five diabetic patients were examined by conventional nerve conduction velocity studies and then divided into 2 groups: diabetics with normal nerve conduction and diabetics with abnormal nerve conduction. Thirty-three normal subjects were used as controls. Toronto Clinical Neuropathy Sco-ring System was used to evaluate the patients. CHEPs were recorded using different stimulation intensities at different temperatures. Results The peak latencies of 45℃ , 50℃, 53℃ in diabetics with normal nerve conduction group were longer than those in normal control group, with a significant difference between the 2 groups(P<0.05). The peak latencies of diabetics were positively related to Toronto scores. Conclusion CHEPs could detect the impair-ment of diabetic peripheral nerve and reveal the impairment earlier than conventional nerve conduction velocity exami-nation, The prolongation of peak latency in diabetics group was significantly and positively related to clinical condi-tion.
10.Effect of insulin-like growth factor Ⅰ and basic fibroblast growth factor on proliferation and differentiation of human dental papilla mesenchymal cells

BACKGROUND: Previous research has indicated that both insulin-like growth factor Ⅰ (IGF-Ⅰ) and basic fibroblast growth factor (bFGF) play an important role in cell proliferation and differentiation. However, the effect on biological characteristics of human dental papilla mesenchymal cells (hDPMCs) still remains unclear. OBJECTIVE: To research the effect of IGF-Ⅰ and bFGF on the proliferation and differentiation of hDPMCs.METHODS: hDPMCs were isolated and cultured in DMEM/F12 culture media containing 1% or 10% fetal bovine serum. The fourth-passaged hDPMCs were incubated in culture media containing 0, 0.1, 1, 10 and 100 μg/L bFGF and 0, 25, 50, 75 and 100 μg/L IGF-Ⅰ(0 μg/L as control group), respectively. At 96 hours after culture, proliferative activity was measured with MTT assay. The corresponding growth factor culture media were used in 10 μg/L bFGF group, 100 μg/L IGF-Ⅰgroup, bFGF + IGF-Ⅰ group, and control group, respectively. At days 1, 3, 5, and 7 after culture, the proliferative activity was detected using MTT assay, and alkaline phosphatase (ALP) activity was measured using modified enzyme kinetics method. RESULTS AND CONCLUSION: At the 0-100 μg/L mass concentration scope, both bFGF and IGF-Ⅰcould accelerate proliferation of hDPMCs, and the proliferation ability of bFGF was superior to that of IGF-Ⅰ; moreover, the combination of bFGF and IGF-Ⅰcaused a synergetic action to proliferation of hDPMCs. The maximal valid concentration of bFGF was 10 μg/L, and the maximal action concentration of IGF-Ⅰwas 100 μg/L. At 0-7 days, the effect of bFGF on the ALP activity of hDPMCs was not obvious, but the effect of IGF-Ⅰon ALP activity of hDPMCs became greater with the time passing; furthermore, the combination of bFGF and IGF-Ⅰcould generate a synergetic action on increasing the ALP activity.
