1.Applying PFGE to the investigation of molecular epidemiology of Enterobacter cloacae in nocosomal infections

Objective To develop a fast typing method with good reproducibility and discriminatory power for epidemiology investigation of Enterobacter cloacae. Methods A method combining pulsed field gel electrophoresis (PFGE) with low-frequency-cleavage restriction endonucleases was established to analysis DNA of Enterobacter cloacae. Cells were imbedded in low melting point agarose. After purification of DNA in situ, it was digested with low-frequency-cleavage restriction endonucleases in situ. The intact DNA was electrophoresed in pulsed field. Relatedness of isolates was demonstrated by comparison of its ‘fingerprints’. Results 17 clones were found among 28 Enterobacter cloacae isolated from nocosomal infections. Outbreak was not registered during the period of six-months (from Jan. 1999 to Jun. 1999). But two small clone transmission happened during this period. They were clone A and clone B, five strains were implicated respectively. Conclusion PFGE is a useful technique with good discrimination and reproducibility for investigating the epidemiology of Enterobacter cloacae in microbiology laboratory.
2.Methodological Validation on Microbil Limit Test of Dental Ulcer Pellicles of Sunset Abelmoschus Flower

OBJECTIVE:To validate the test condition and test method of the microbil limit test of the Dental ulcer pellicles of Sunset Abelmoschus Flower.METHODS:The methodological validation was carried out in accordance with the standard for microbil limit test stated in the appendix of China Pharmacopoeia(2005 Edition,VolumeⅠ).RESULTS:By membrane-filter procedure,the recovery rates of different test strains in the samples were all over 70%.CONCLUSION:The membrane-filter procedure can more effectively eliminate the bacteriostatic action of Dental ulcer pellicles of Sunset Abelmoschus Flower so as to result in accurate microbil limit test results.
3.Efficacy Observation of Ultrasonic Conductometric Acupoint Penetration of Wenhua Zhitong Recipe for Adenomyosis Patients.

OBJECTIVETo observe the efficacy of ultrasonic conductometric acupoint penetration of Wenhua Zhitong Recipe (WZR) in treating adenomyosis.
METHODSTotally 40 adenomyosis patients with Shen-deficiency blood stasis syndrome were randomly assigned to the treatment group and the control group, 20 in each group. Patients in the treatment group received ultrasonic conductometric acupoint penetration of WZR from the 2nd day before menstruation to the end of menstruation, while those in the control group took WZR. All patients received 3 consecutive menstrual cycles. Endometrial contents of cyclooxygenase-2 (COX-2) and oxytocin receptor (OTR) were determined using SP immunohistochemical staining method before and after treatment. Clinical effect, efficacy of Chinese medical syndrome, changes of uterus volume, and contents of carbohydrate antigen 125 (CA125) were observed before and after treatment.
RESULTSThere was no statistical difference in the total clinical effect between the two groups (P = 0.357). Efficacies for relieving abdominal pain and lumbar soreness during menstruation were better in the treatment group than in the control group (P < 0.05). Uterus volume, endometrial levels and integrals of COX-2 and OTR decreased in the two groups after treatment (P < 0.05, P < 0.01).
CONCLUSIONUltrasonic conductometric acupoint penetration of WZR and oral administration of WZR both could effectively improve adenomyosis patients' symptoms such as Shen deficiency, and blood stasis, reduce the foci of uterus, and relieve the pain.

4.Surgical treatment of postinfarction ventricular septal defect

Objective To review our experience of surgical treatment for postinfarction ventricular septal defect. Methods From 1990 to 2004, 22 patients with postinfarction ventricular septal defect underwent surgical treatment. There were 15 males and 7 females with a mean age of (62.2?7.3) years (from 52 years to 72 years),19 had anterior ventricular septal defect and 3 had posterior VSD, and there were 18 cases with left ventricular aneurysm.10 patients also had introaortic balloon pumping in the early experience. In all the patients cardiac function were in NYHA class III~IV. Results There were 8 operative deaths (35%). There were 7 operative deaths before 1999 and 1 case after 1999. The other patients are clinically asymptomatic after surgery, and the cardiac function is in NYHA class I~II. Conclusion Postinfarction ventricular septal defect is an infrequent but serious complication of myocardial infarction. Surgery is the best way to treat this condition. A coronary angiography should be performed in all patients who can be stabilized hemodynamically and myocardial revascularization should be performed in patients with significant coronary artery stenosis.
6.Advancement in surgical treatment of intrahepatic cholangiocarcinoma

Intrahepatic cholangiocarcinoma (ICC) has the significant characteristics of regional spread and lymphatic metastasis. Radical resection provides the only chance for long-term survival of patients with ICC. Anatomic or extended hepatectomy is the most optimal treatment. The value of hepatic resection with lymph node dissection and the area of lymph node dissection are controversial. Palliative resection improves prognosis of some patients. Secondary hepatectomy also benefits some patients with late (>1 year) intrahepatic recurrence. Liver transplantation is only performed for highly selected prophase-patients or prudent clinical trials, and a combination with adjuvant treatment such as chemo-radiotherapy after operation is suggested.

8.Relationship Between Plasma Calcium Level and Fibrinogen, Homocysteine in Non-diabetic Elderly Patients

To explore the impact of normal range plasma calcium level changes on fibrinogen and homocysteine (Hcy) in non-diabetic elderly patients. Methods: A total of 1030 non-diabetic patients≥65 years were retrospectively studied. According to adjusted plasma calcium concentration (Cac), the patients were divided into 4 quartiles as Q1 group, Q2 group, Q3 group and Q4 group. The relationship between Cac and plasma levels of fibrinogen and Hcy were compared. Results: ① Plasma levels of fibrinogen and Hcy were increased by Cac elevation accordingly, P=0.010 and P<0.001 respectively. ② Cac was obviously related to the prevalence of high fibrinogen (OR=1.75, P=0.019) and high Hcy (OR=1.82, P=0.026). ③ Cac was positively related to fibrinogen (Standardized β=0.15, P<0.001) and Hcy (Standardized β=0.08, P=0.009). Conclusion: Normal range plasma calcium level changes were related to the elevation of plasma fibrinogen and Hcy in non-diabetic elderly patients.

This paper reports one case of overseas imported quartan malaria and the diagnosis and treatment process. By using dihydroartemisinin combined with piperaquine,the treatment results are satisfactory.
