2.Improvement of the survival rate and life quality of patients with local recurrence of rectal cancer

Despite progress in the understanding of perirectal anatomy and subsequent adoption of total mesorectal excision,local recurrence after curative resection for rectal cancer continues to be a significant concern.Local recurrence of rectal cancer is associated with a wide spectrum of clinical symptoms and morbidities,and decreased quality of life.The management of locally recurrent rectal cancer is challenging because of the altered pelvic anatomy,postoperative adhesion and difficulty in obtaining a definite diagnosis before surgery.Surgery is the mainstay treatment for locally recurrent rectal cancer,and the use of radiation and chemotherapeutic agents provides significant improvement in survival rate and quality of life.
3.Applying PFGE to the investigation of molecular epidemiology of Enterobacter cloacae in nocosomal infections

Objective To develop a fast typing method with good reproducibility and discriminatory power for epidemiology investigation of Enterobacter cloacae. Methods A method combining pulsed field gel electrophoresis (PFGE) with low-frequency-cleavage restriction endonucleases was established to analysis DNA of Enterobacter cloacae. Cells were imbedded in low melting point agarose. After purification of DNA in situ, it was digested with low-frequency-cleavage restriction endonucleases in situ. The intact DNA was electrophoresed in pulsed field. Relatedness of isolates was demonstrated by comparison of its ‘fingerprints’. Results 17 clones were found among 28 Enterobacter cloacae isolated from nocosomal infections. Outbreak was not registered during the period of six-months (from Jan. 1999 to Jun. 1999). But two small clone transmission happened during this period. They were clone A and clone B, five strains were implicated respectively. Conclusion PFGE is a useful technique with good discrimination and reproducibility for investigating the epidemiology of Enterobacter cloacae in microbiology laboratory.

5.Methodological Validation on Microbil Limit Test of Dental Ulcer Pellicles of Sunset Abelmoschus Flower

OBJECTIVE:To validate the test condition and test method of the microbil limit test of the Dental ulcer pellicles of Sunset Abelmoschus Flower.METHODS:The methodological validation was carried out in accordance with the standard for microbil limit test stated in the appendix of China Pharmacopoeia(2005 Edition,VolumeⅠ).RESULTS:By membrane-filter procedure,the recovery rates of different test strains in the samples were all over 70%.CONCLUSION:The membrane-filter procedure can more effectively eliminate the bacteriostatic action of Dental ulcer pellicles of Sunset Abelmoschus Flower so as to result in accurate microbil limit test results.

The question regarding how to avoid stoma complication and improve quality of life should be considered when stoma creation is planned. The benefit of stoma should be balanced with complications brought about by stoma creation.

7.Comparison of 23 G and 20 G vitreous cut system in silicone oil extracting operation

AIM: To evaluate the safety and effectiveness of 23G and 20G vitreous cutting system in silicone oil extracting operation.
●METHODS:A total of 98 cases (98 eyes) patients, after 3- 6mo silicone oil filling with retinal attachment and suitable for retrieving silicone oil. Patients were randomly divided into two groups. 20G vitreous cutting system group including 48 cases ( 48 eyes ) and 23G vitreous cutting system group including 50 cases ( 50 eyes ) . A series of comparative analyses were conducted on the times for set up and closing the passage and the time for taking the oil time on the two groups:intraocular pressure of preoperative and of postoperative for 1, 3d, 1wk, 3 and 6mo. The comparisons were also made between the best corrected visual acuity of preoperative and postoperative. The section inflammation of postoperative 1, 3d, 1mo were compared. Complications were compared between intraoperative and postoperative.
●RESULTS:Establishing surgery channel for 20G and 23G group were ( 243. 54 ± 51. 17 ) s and ( 91. 16 ± 21. 37 ) s respectively;closing wound time were (235. 04 ± 42. 89) s and (86. 04±21. 76)s. Extracting oil time were (6. 7±1. 65) min and ( 7. 35 ± 2. 02 ) min. There was a significant difference ( P<0. 01 ) between establishing the operation channel time and close the incision time. There was no significant difference between two groups in the extracting oil time (P>0. 05). The first day was statistically different ( P< 0. 05 ) but the rest of the time point of intraocular pressure had no statistical difference ( P >0. 05). The best corrected visual acuity for 20G and 23G group preoperative and postoperative after 6mo were:4. 21 ± 0. 61, 3. 91 ± 0. 64; 4. 03 ± 0. 46 and 4. 22 ± 0. 39 respectively. There was no significant difference between the two groups (P>0. 05). There was significant difference ( P<0. 05) of cells in the anterior chamber between the 20G group and 23G group of postoperative 1 and 3d. There was no statistical difference after 1mo (P>0. 05). The findings also reported that 20G and 23G group had no intraoperative complications and postoperative. For 23G group, 5 cases of temporarily had lower intraocular pressure, 1 case was retinal again, 1 case was choroid detachment. For 20G group, 1 case of temporarily had lower intraocular pressure, 3 cases were retinal detachment and 2 cases were choroid detachment.
●CONCLUSION:23G vitreous cutting system has the advantages of low operation steps, intraoperative injury. The postoperative complications of low probability, used in silicone oil is safe and effective.
8.Efficacy Observation of Ultrasonic Conductometric Acupoint Penetration of Wenhua Zhitong Recipe for Adenomyosis Patients.

OBJECTIVETo observe the efficacy of ultrasonic conductometric acupoint penetration of Wenhua Zhitong Recipe (WZR) in treating adenomyosis.
METHODSTotally 40 adenomyosis patients with Shen-deficiency blood stasis syndrome were randomly assigned to the treatment group and the control group, 20 in each group. Patients in the treatment group received ultrasonic conductometric acupoint penetration of WZR from the 2nd day before menstruation to the end of menstruation, while those in the control group took WZR. All patients received 3 consecutive menstrual cycles. Endometrial contents of cyclooxygenase-2 (COX-2) and oxytocin receptor (OTR) were determined using SP immunohistochemical staining method before and after treatment. Clinical effect, efficacy of Chinese medical syndrome, changes of uterus volume, and contents of carbohydrate antigen 125 (CA125) were observed before and after treatment.
RESULTSThere was no statistical difference in the total clinical effect between the two groups (P = 0.357). Efficacies for relieving abdominal pain and lumbar soreness during menstruation were better in the treatment group than in the control group (P < 0.05). Uterus volume, endometrial levels and integrals of COX-2 and OTR decreased in the two groups after treatment (P < 0.05, P < 0.01).
CONCLUSIONUltrasonic conductometric acupoint penetration of WZR and oral administration of WZR both could effectively improve adenomyosis patients' symptoms such as Shen deficiency, and blood stasis, reduce the foci of uterus, and relieve the pain.

9.Analysis of Prognosis of Partial Seizures and Its Prognostic Factors in Children

Objective To analyze the influence of clinical status, electroencephalography (EEG) and antiepileptic drugs on prognosis of partial seizure in children. Methods A total of 233 pediatric patients with partial seizures were included. Their data including EEG records, responses of medication were reviewed. The patients were classified into three groups: seizure free, improved control and poor control groups. The significance of therapeutic efficacy in initial presentation in different age were assessed with t test. Chi-square test was used to analyze the significance of clinical data,laboratory findings and result of treatment. Logistic regression was used to investigate independent effect of each risk factor. Results Complete seizure control was achieved in 167 patients, improved seizure control in 31 patients and poor seizure control in 35 patients. Patients with poor seizure control were in much younger age group (3 times/month ) was also a predictive factor of poor outcome (?~2=8.976 P=0). The results of treatment were significantly different among the patients with simple partial seizure(SPS), complex partial seizure, and partial seizure with secondary generalized seizure. There were no significant differences in drug responsiveness among the patients treated by phenobarbital, carbamazepine, or valproate (?~2=0.107 P=0.948). Multiple logistic regression analysis showed that outcome of treatment was correlated with age of onset, seizure frequency, and type of the seizure (P=0.053,0.007,0.054). Conclusions Poor seizure control is closely associated with a younger onset age, frequent seizures. The treatment responsiveness is the worst in patients of partial seizure with secondary generalized seizures. The best outcome is in patients with SPS. Phenobarbital, carbamazepine, and valproate are equally effective for treating childhood partial seizure.
10.Clinical analysis of cellular schwannoma in the gastric wall misdiagnosed as gastric stromal tumor

Cellular schwannoma is a rare benign tumor of the peripheral nerves.The tumor is consist of spindle cells which arranged in interwoven bundles,and lacking the mesh district and palisading structure.The tumor cells are abundant,with coarse chromatin,deeply stained nuclei and in multiforms.So the tumor may easily be misdiagnosed as various types of spindle cell sarcoma.Intraperitoneal cellular schwannoma may also be mistaken as gastrointestinal stromal tumors.In this article,the clinical characteristics,diagnosis and treatment of cellular schwannoma were analyzed based on the clinical data of a patient with cellular schwannoma in the gastric wall misdiagnosed as gastric stromal tumor.
