1.Investigation on heart rate turbulence in patients with decreased left ventricular diastolic function

Objective To assess the characteristics of heart rate turbulencer (HRT) in elderly patients with decreased left ventricular diastolie function.Methods 40 patients were divided into two groups:20 patients with enlarged left atrium and 20 patients without enlarged left atrium.20 healthy people were selected as controls. Turbu1ence onset (TO) and turbulence slope (TS) were measured,and correlation was analyzed between TO,TS and the E/A,index of Macruz and heart rate variability (HRV).Results TO was higher (P<0.05) and TS was lower in patients (P<0.01),TO was higber in patients with enlarged left atrium than in with out enlarged lef atrium people (P<0.05) and TS was lower (P<0.01).TO was positively (P<0.01) and TS Was negatively (P<0.05),correlated with the index of Macruz.TO Was negatively (P<0.05) and TS was positively (P<0.05) related to SDNN and SDANN.Conclusion HRT can be used as a potential risk predictor in decreased left ventricular diastolic function patients.

3.Nonsebaceous lymphadenoma of salivary gland:report of two cases and review of the litera-ture

Purpose To investigate clinicopathological features, diagnosis and differential diagnosis of non-sebaceous lymphadenoma of the parotid gland. Methods The histopathological morphology, immunohistochemical profiles and clinicopathological features were an-alyzed in two cases of NSL, along with review the related literatures. Results Two patients were female adults. Microscopically, The tumor was a well-circumscribed mass surrounded by a fibrous capsule of variable thickness and comprised a mixture of proliferating epi-thelium accompanied by a prominent lymphoid component, reactive lymphoid follicles were found in lymphoid stroma. The epithelial component took the form of anastomosing trabeculae, glands, solid basaloid islands or cyst formation. The cysts and glands were lined with luminal cells and abluminal cells, filled with eosinophilic secretions with occasional histiocytes. The epithelial cell was no seba-ceous differentiation, significant cytological atypia and mitotic activity. A fibrous capsule with subcapsular sinus was seen around the mass in one case. Immunohistochemically, the abluminal cells were positive for p63, CK34βE12 and CK5/6, while the epithelial cells were positive for CK(AE1/AE3) and CK7. Conclusion NSL is a very rare benign of salivary gland, which occuring in the lymph node lesions are less reported, knowledge of the wide histological spectrum of this rare tumor is important in order to avoid misdiagno-sis, particularly as malignant tumor.

Objective To investigate the possible roles of the different subnuclei of hypothalamic paraventricular nucleus (PNV) which may regulate splenic immune function. Methods 4 SD rats received splenic injection of pseudorabies virus(PRV).96 hours after injection, a block containing hypothalamus was taken and cut into 40 μm sections with cryostat. Immunohistochemical technique and stereology method were used to detect the distributing character of transsynaptic PRV infection neurons in the PVN and the localization relationship between PRV neurons and AVP neurons in the PVN.Results Transsynaptic PRV infection neurons distributed through out the PVN, but mainly concentrated in the lateral parvocellular (LP) subnucleus of PVN and posterior magnocellular (PM) subnuclei of PVN (occupied 73.49% of total surface area),a few distributed in the anterior, dorsal and periventricular parvocellular (AP,DP and PP)nuclei of PVN,few scatted in the rest of nuclei of PVN. In the LP and PM some of the PRV neurons distributed among the AVP neurons.Conclusions Besides the classical neuro-immuno-endocrine modulation, the PVN may regulate the splenic immune function through direct innervation .The AVP neurons in the PM and neurons projecting to the proganglionic neurons in the dorsal medulla and lateral horn of spinal cord in the LP,DP and AP regulate the immune function of spleen through sympathetic and para-sympathetic innovation.

7.Clinical evaluation of iodine-125 brachytherapy in hilar cholangiocarcinoma

Hilar cholangiocarcinoma is a common bile duct cancer.The radical resection rate of end-stage malignancies reported in literature is about 20%.Half of the patients have lost the chance of operation at the time of care,while the average survival time in the patients who can not be operated is about six months.Therefore,for most patients,looking for a palliative treatment which can prolong the survival time is particularly important.From June 2013 to June 2014,our hospital applied iodine-125 brachytherapy,to treat 7 patients with unresectable hilar cholangio carcinoma.All the patients are still alive,the average survival time of 7 patients is 5.2 months up to now,with median survival time being 7.8 months,and longest survival time being 12 months.With good outcomes,few complications,as well as significantly prolonged survival time,iodine125 brachytherapy is regarded with broad clinical applications.
8.Mesohepatectomy for large and centrally located hepatocellular carcinoma

Objective To evaluate the safety and efficacy of mesohepatectomy for large and centrally located hepatocellular carcinoma.Methods A retrospective study was carried out on 136 pa tients who underwent mesohepatectomy for large and centrally located hepatocellular carcinoma at Xiangya Hospital,Central South University,from 2001 to 2007.Intraoperative/post operative data and long-term survivals were analyzed.Results Vascular occlusion time,operative time,intraoperative blood loss,intraoperative blood transfusion and hospital stay were (13.3 ± 9.1) min,(173.1 ±41.1)min,(548.7±320.5)ml,(511.4±231.7)ml and (18.6±8.8)d,respectively.Four patients developed major complications.There was no in-hospital death.The 1-,3-,and 5-year overall survival rates and disease-free survival rates were 71%,46%,29% and 65%,40%,24%,respectively.Conclusions Mesohepatectomy for large and centrally located hepatocellular carcinoma preserved the maximum amount of functional liver parenchyma.It is safe and reliable and may be used as the treatment of choice.
9.Clinical Observation of Acupuncture using Xingnao Kaiqiao Needling Method and Rehabilitation Training on Stroke

ObjectiveTo observe the clinical effection of acupuncture using Xingnao Kaiqiao needling method and rehabilitation training on patients with stroke.Methods70 patients with stroke were randomly divided into the observation group(n=35)and the control group(n=35), both groups receiving routine treatment and early rehabilitation training, while acupuncture using Xingnao Kaiqiao needling method was given additionally to the observation group, 4 weeks for a course. The Fugl-Meyer Assessment, assessment of neurological deficit, the Barthel Index and the clinical effect were compared among two groups.ResultsThe neurological deficit decreased obviously, and the Fugl-Meyer Assessment and Barthel Index increased significantly in two groups after the treatment(P<0.05). The theraputic effect was significantly better in the observation group than in the control group(P<0.05).ConclusionAcupuncture using Xingnao Kaiqiao needling method combined with rehabilitation training can more effectively promote the recovery of nerve function, enhance motor function, and significantly improve the quality of life and the prognosis of patients with stroke.
10.Analysis of the pathology of lymph nodes in 151 acquired immune deficiency syndrome patients with superficial lymphadenectasis

Objective To investigate the pathological types and features of lymph nodes in human immunodeficiency virus(HIV)/acquired immune deficiency syndrome(AIDS)patients with superficial lymphadenectasis.Methods The tissues of lymph nodes were obtained from 151 HIV/AIDS patients with superficial lymphadenectasis for pathological examination.The pathological results were observed by light microscope after Hematoxylin-Eosin(HE),acid-fast,periodic acid-Schiff (PAS),and digested-PAS(D-PAS)staining.The pathological results of lymph nodes were described and the correlation between pathological changes and CD4+T lymphocyte count was analyzed.Chisquare test was used for the statistic analysis.Results The benign lesions were found in 145 patients (96.0%),while the malignant tumors were found in 6 patients(4.0%).The pathological findings in the 151 HIV/AIDS patients included tuberculosis(72 patients),lymph node reactive hyperplasia(34patients),lymphatic fungal infections(23 patients,including penicillium diseases in 19 cases),AIDSrelated lymphadenectasis(14 cases),non-Hodgkin lymphoma(5 cases),benign fibrous histiocytoma (1 case).In addition,there were 83 patients(55.0%)with CD4+T lymphocyte count lower than 100×106/L.The frequency of penicillium diseases was higher in patients with lower CD4+T lymphocyte count(x2=7.757,P=0.021).Conclusions The major reasons for superficial lymphadenectasis in HIV/AIDS patients are infectious diseases,such as tuberculosis and fungal infections,which are common in patients with lower CD4+T lymphocyte counts.Non-Hodgkin lymphoma is the most common malignant tumor in this patient population.
