1. HPLC-MS in evaluating bioequivalence of two kinds of aniracetam capsules in healthy volunteers

Objective: To establish a HPLC-MS method for determinating the concentration of 4-p-anisamidobutyric acid (ABA), the main active metabolite of aniracetam capsules in the plasma, and to compare the bioequivalence of two aniracetam capsules in healthy volunteers. Methods: Twenty-four healthy male volunteers were randomly given an oral single dose of 200 mg test or reference aniracetam capsules in a crossover manner. The concentrations of ABA were assayed by HPLC-MS at different time points. The main pharmacokinetic parameters and the relative bioavailability of two preparations were calculated, and their bioequivalence was evaluated. Results: The pharmacokinetic parameters of test and reference preparations were as follows: Cmax, being (9.30±5.13) and (8.70±3.17) μg/ml; tmax being (38.41±17.89) and (39.09±19.92) min; t1/2 being (37.21±10.51) and (38.45±9.24 ) min; AUC0-t being (555.21±157.10) and (545.39±97.22) μg/(ml·min), and AUC0-∞ being (566.24±158.01) and (554.71∞100.32) μg/(ml·min), respectively. Relative bioavailability F0-t and F0-∞ values of the test preparation were (101.22±17.17)% and (101.52±16.63)%, respectively. No significant differences were found in the main pharmacokinetic parameters between the two preparations. Conclusion: The two aniracetam preparations tested in the present study are bioequivalent.

BACKGROUND:Along with the medicine development.the prevention and treatment of the atrophy of denervated skeletal muscle have made a remarkable progress,but the clinical curative effect is still not satisfied OBJECTIVE:To summarize the current status of the prevention and treatment methods of denervated skeletal muscle atrophy,and to find a more effective method for the prevention and treatment of the denervated of skeletal muscle atrophy METHODS:Using"denervation,muscle atrophy,treatment"in English for the search term,Medline database from January 1998 to January 2008 was retrieved.Using"denervation,muscle atrophy,treatment:in Chinese for the search term,the Chinese Journal Full-text Database from January 1998 to January 2008,Wanfang Database from January 1998 to January 2008,and Journal of Clinical Rehabilitation and Tissue Engineering Research from January 1 998 to January 2008 were searched.Literature was limited to English and Chinese languages.The muscular endurance and contractile force,muscle wet weight of denervated nerve,and skeletal muscle repair served as the evaluation index The studies on the micro-surgical procedures,physical therapy,biological and chemical therapies,gene therapy for the atrophy of denervated skeletal muscle were included.Except the above methods,other therapies to the denervated skeletal muscle atrophy were excluded.RESULTS AND CONCLUSION:After peripheral nerve damage.the skeletal muscle is denervated and then inevitably atrophy occurs Therefore,the exploration of prevention and treatment of denervated skeletal muscle atrophy has attracted the interests from many domestic and foreign scholars.will certainly become the important task and the research hot spot in the 21st century in the peripheral nerve domain The microsurgery surgery,physiotherapy,biology and chemotherapeutics,gene therapy are the main effective prevention methods for the atrophy of denervated skeletal muscle.At present,this domain.s research already presents the multiple perspectives and various tendencies.The prevention and treatment of denervated skeletal muscle atrophy has already given the pointed management,but in the improvement microcycle,but much works need to carry on in fields of improving microcirculation,preventing cell apoptosis,suppressing the collagen excessive growth,as well as adjusting myogenic regulatory factors expression using gene therapy method in the gene level,but also had the.Along with the tissue engineering,cell culture science,molecular biology,genetic engineering and so on,an unceasing development will be obtained regarding the prevention and treatment of denervated skeletal muscle atrophy.
3.Efficacy and Health-Related Quality of Life for small cell lung cancer patients after prophylactic cranial irradiation

Objective To analyze the survival,brain metastases and health-related quality of life (HRQOL) for small cell lung cancer patients after prophylactic cranial irradiation(PCI).Methods From Aug 2007 to Apr 2011,42 small cell lung cancer patients were eligible for analysis.Overall survival rate was estimated by the Kaplan-Meier method.The HRQOL scores before and after PCI were compared by the nonparameter wilcoxon test.Results The median survival time from the start of PCI was 23 months(95% CI,15.05-30.95),progression free survival time was 17 months (95% CI,14.33-19.67),1-,2-and 3-year survival rates were 85.5%,45.8% and 36.6%,respectively.The median time from PCI to brain metastases was 15 months(95% CI,12.44-17.56),1-and 2-year brain metastases rates were 77.6% and 54.1%.PCI had a little influence on HRQOL scales except for cognitive function.Conclusions For patients with limited stage small cell lung cancer who achieve complete or nearly complete remission after initial treatment as well as patients with extensive stage who respond to initial chemotherapy,PCI is effective in decreasing the rate of brain metastasis and improving survival,while the adverse effects is acceptable.There is no significant impact on HRQOL scales during short term.
4. MiR-34b suppresses the proliferation of bladder cancer BIU-87 cells

Objective: To investigate the effect of microRNA-34b (miR-34b) on the proliferation of bladder cancer BIU-87 cells, and to exploreObjective: To investigate the effect of microRNA-34b (miR-34b) on the proliferation of bladder cancer BIU-87 cells, and to explore Methods: The expression levels of miR-34b in bladder cancer BIU-87 cells, SV-HUC-1 cells and renal tubular epithelial HK-2 cells were determined by real-time fluorescent quantitative PCR. After transfection with miR-34b mimic, the proliferation rate and clone formation rate of BIU-87 cells were detected by MTT assay and clone formation assay, respectively. After co-transfection with miR-34b mimic and ubiquitin-specific peptidase 22 (USP22)-wide type (WT) or USP22-mutation type (Mut), the specific binding ability of miR-34b to 3'-untranslated region (3'-UTR) in USP22 gene was examined by luciferase reporter system. The expression levels of USP22 mRNA and protein in BIU-87 cells after transfection with miR-34b mimic were detected by real-time fluorescent quantitative PCR and Western blotting, respectively. Results: The expression level of miR-34b in human bladder cancer BIU-87 cells was lower than those in SV-HUC-1 cells and renal tubular epithelial HK-2 cells (both P < 0.05). The proliferation rate and clone formation rate of BIU-87 cells in miR-34b mimic transfection group were lower than those of blank control cells (BIU-87 cells without any transfection) and negative control cells [BIU-87 cells transfected with miRNA mimic negative control (miRNA mimic NC)] (all P < 0.05). The result of luciferase reporter system indicated that miR-34b targeted 3'-UTR in USP22 gene. The luciferase activity of BIU-87 cells in co-transfection with miR-34b mimic and recombinant vector USP22-WT group was decreased (P < 0.05). The expression level of USP22 protein in BIU-87 cells in miR-34b mimic transfection group were lower than those in the blank control and the negative control groups (all P < 0.05). Conclusion: miR-34b can suppress the proliferation of bladder cancer BIU-87 cells. This effect may be related to the expression of USP22.

6.Learning curve of full endoscopic technique for the surgical treatment of lumbar disc herniation

ObjectiveTo evaluate the learning curve of utilizing the full endoscopic technique for the surgical treatment of lumbar disc herniation.MethodsFrom August 2008 to February 2009,30 patients with lumbar disc herniation underwent lumbar discectomy by the full endoscopic-only approach were retrospectively analyzed.The patients were divided into three groups of 10 sequential cases each.Group of early time consisted of the first 10 cases,Group of medium term the subsequent 10 cases,and Group of later time the last 10 cases.The clinical evaluation data included operative time,length of hospital stay,leg and back pain visual analogue scale (VAS),and complications.ResultsAll patients were observed prospectively for (1.61±0.22) years.There was no measurable intraoperative bleeding,no postoperative infections and symptomatic recurrences in the three groups.Compared to group of early time,the operative time in group of medium term was significantly decreased(P<0.05).The patients in group of later time had much less operative time than that in group medium term(P<0.05).There was no significant difference with length of hospital stay between the three groups (P>0.05).The improvement of leg and back VAS in each group was similar:there was a significant improvement 3 months post-operation compared with pre-operation (P <0.05),but no statistical difference between 3 months post-operation and final foliow-up(P>0.05).The complication rate was 12.5% for group of early time,10% for group of medium term,and 0 for group of later time.There were two cases conversed to an open procedure for group of early time compared with 0 cases in both other groups.ConclusionExcellent clinical and minimally invasive outcomes can be obtained in the surgical treatment of lumbar disc herniation via the interlaminar approach assisted by full endoscopic technique.However,attention must be paid to the steep learning curve with use of this complex technique.Obtaining microsurgical experience and suitable patient selection can help shorten the learning curve and decrease the complications.
7.The correlation between NPHS2 polymorphism and IgA nephropathy in northern Chinese patients

Objective To examine the polymorphism in NPHS2 gene of IgA nephropathy in northern Chinese patients and to investigate the possible association of the NPHS2 polymorphism with the development of IgA nephropathy, as well as its clinical and histologic manifestations.Methods The polymorphism of NPHS2 was analyzed by direct DNA sequencing in 32 northern Chinese patients with IgA nephropathy (16 with heavy proteinuria and 16 with isolated hematuria).According to preliminary results, a total of 537 IgA nephropathy patients were genotyped for the NPHS2 C357T polymorphism by PCR combined with restriction fragment length polymorphism (PCR-RFLP). We collected clinical and histologic manifestations for gene analysis in patients with IgA nephropathy, such as age, sex, urine protein excretion and so on.ResultsEight NPHS2 polymorphisms (-931A>T, -601C >T, 19G>T, 171A>G, 357C > T, IVS3-21C > T, 1023C > T and 1107A > G) were identified.The preliminary results of gene sequencing showed that the frequency of 357T allele in nephrotic syndrome group was obviously lower than isolated hematuria group (0.038 vs 0.125, P <0.05).In 537 IgA nephropathy patients with clinical and histologic data, the average urinary protein excretion in the patients with the 357CT/TT genotype was less (P =0.023).The incidence of urinary protein of more than 3.5 g/d was significantly lower in patients with T allele and TT/CT genotype, respectively (P =0.017 and 0.011).The logistic regression analysis indicated that, even after adjusting for the effect of hypertension and age of patients, the CT/II genotype of NPHS2 C357T was an independent protective factor for the urinary protein excretion more than 3.5 g/d(P =0.012,OR = 0.485, 95% CI 0.275-0.84).ConclusionsEight NPHS2 polymorphisms were identified in northern Chinese IgA nephropathy patients. The frequencies of NPHS2 T allele and TT/CT genotype were the protective factors for urinary protein, especially with that of more than 3.5 g/d.

Pharmacology , pharmacokinetics , clinical study and safety of enzalutamide in the treatment of castration-resistant pros-tate cancer (CRPC) were reviewed in the paper.Enzalutamide, a pure second-generation androgen receptor antagonist , can inhibit multiple points in the androgen receptor signaling pathway , including the inhibition of androgen binding to the androgens receptor , nu-clear translocation of the androgens receptor complex and binding of the androgens receptor complex to deoxyribonucleix acid ( DNA) . The phase 3 clinical trials have revealed that enzalutamide is effective to delay the progression of metastatic CRPC , reduce the prostate-specific antigen (PSA) levels, decrease the progression time of PSA and prolong the overall survival time and the time of first skeletal -related events .Enzalutamide has become a novel treatment option for CRPC patients .Enzalutamide is well tolerated with low incidence of side effects .
9.The correlation between femoral popliteal artery stent fracture and in-stent restenosis

Objective To investigate the correlation between femoral popliteal artery stent fracture and in-stent restenosis (ISR) in patients with arteriosclerosis obliterans of superficial femoral artery and proximal popliteal artery after receiving stent implantation.Methods The clinical data of a total of 97 consecutive patients with arteriosclerosis obliterans of superficial femoral artery and proximal popliteal artery (107 diseased limbs in total),who were treated with primary stent implantation during the period from March 2012 to March 2016,were retrospectively analyzed.The imaging materials,including Doppler ultrasonography,plain radiography,contrast-enhanced CT scan,DSA,etc.were collected,and Kaplan-Meier survival analysis and other statistical analysis methods were used to analyze the related data.Results During the follow-up period,71 patients (72 limbs in total) developed ISR and the incidence of ISR was 67.3% (72/107).The incidences of ISR in the stent-fracture group and non-fracture group were 84.2% (32/38) and 58.0% (40/69) respectively,the difference between the two groups was statistically significant (P=0.01).Conclusion After stent implantation of femoral popliteal artery,fracture of stent is one of the important risk factors for the occurrence of ISR.
10.Evaluation of radiation dose in chest scan with enhanced dual-source computed tomography in children with congenital heart disease

Objective To evaluate the radiation dose from enhanced dual-source computed Tomography(DSCT) scan on children with congenital heart disease(CHD). Methods Seventy children with CHD, age from 1 month to 8 years old, were scanned with enhanced DSCT. Children were divided by age into < 1 year old group, 1-5 years old group, > 5 years old group. The differences among three groups were tested by F test. Then, the SNK test was used to compare the difference between each group. Multiple linear regression analysis was used to test the relationship of dose length product (DLP) with the age,weight, voltage, current, pitch and scan sheet. Results The average value of DLP was (144.46 ±test showed statistical differences of DLP between < 1 year old group and 1-5 years old group, and between <1 year old group and >5 years old group(q =5.21,6.52,P =0.009,0.004). The difference of DLP between 1-5 years old group and > 5 years old group did not reach significant ( q = 0.28, P = 0.48 ). The differences of ED was not statistically significant among the three groups [< 1 year 3.20 mSv, 1-5 years (5.17 ± 1.98 ) mSv, > 5 years ( 3.74 ± 1.33 ) mSv, F = 0.54, P = 0.59]. DLP was positively correlated with age (4.3 years,r = 0.541 86, P = 0.0008 ), weight ( 12.1 kg, r = 0.563 71, P = 0.0004 ), voltage [(95.48±6.99) kV,r =0.632 69, P<0.01], current [(138.55±40.67) mA,r =0.796 08, P<0.0001] and scan sheet (236.10 ±46.51 ,r = 0.721 92, P < 0.01 ). DLP was negative correlated with pitch (0.48 ± 0.03,r = - 0.466 93, P = 0.0047 ). Conclusion Higher DLP was observed in children over 1 year old under enhanced DSCT scan, but ED was not statistically significant among the three groups due to the higher K value in the children under 1 year old.
