2.St udy on rta ditional Chinese med icine constitution in the patients with type I diabeet s in Baoshan District,Shanghai

Objectvi e To understand the correlation between the diabetes and the complications and the constitution of traditional Chinese medicine ( TCM ) in Baoshan District in Shanghai , and to provide scientific evidence for carrying out the targeted prevention and intervention measures.M ethods TCM body mass table was used to distinguish the TCM constitution of the diabetic patients and the control group .The relationship between TCM constitution and diabetes and its complications was analyzed by single factor and multiple factor logistic regression . Results There were less patients with yin-yang harmony constitution and yang deficiency constitution in the case group than in the control group , while more with qi deficiency constitution and phlegm-damp constitution than in the control group .Among a total of 879 cases of diabetic patients with1 235 physical elements , the top four biased constitution elements were identified including constitution of qi deficiency ( 15.22%), yin deficiency ( 8.42%), yang deficiency ( 8.42%) and phlegm-damp ( 8. 34%).The yin deficiency, qi deficiency and phlegm dampness constitution elements in the case group were significantly higher than in the control group .Among 875 cases in the control group with 1 129 physical elements , the top four biased constitution elements were yang deficiency (12.40 %), qi deficiency( 9.65%), yin deficiency (5.05%) and phlegm-damp (4.52%), which yin-yang harmony constitution and yang deficiency constitution elements were significantly higher than those of the case group .The incidence of complications of the diabetic patients with biased constitution was significantly higher than those with the yin-yang harmony constitution . Conclusion Phlegm dampness constitution , qi deficiency constitution and yin deficiency constitution are the main dangerous constitution of diabetes mellitus .The protective constitution of diabetes is yin-yang harmony constitution and yang deficiency constitution .The biased constitution elements are the risk factors of diabetes complications .
3.Analysis of influence of sarcopenia on early postoperative function of femoral neck fracture by hip arthroplasty.

OBJECTIVETo determine the association of sarcopenia with short-term postoperative function after hip replacement for femoral neck fractures.
METHODSA prospective study of 181 consecutive patients with femoral neck fractures who underwent hip replacement from May 2014 to January 2017 were performed, including 58 males and 123 females aging from 53 to 92 years old. The general conditions were collected before surgery, skeletal muscle index(ASMI), handgrip strength were measured. Clinical outcomes were followed up including postoperative complications, time of on-site, Harris score (postoperative 2 weeks, 3, 6 months), hospitalization costs, and hospital stay. According to handgrip strength and ASMI, the patients were divided into the sarcopina group and the non-sarcopina group; according to the Harris score at the 6-month follow-up, the patients were divided into good prognosis group and poor prognosis group. Univariate analysis and binary logistic regression analysis were used to investigate whether sarcopenia was a risk factor for poor postoperative hip joint surgery.
RESULTSAll patients were followed up at 2 weeks, 3 and 6 months, postoperative early complication included wound infection in 16 cases, thrombus of lower extremity veins in 14 cases, no dislocation, prosthetic loosening and prosthesis related infections occurred. Sarcopenia was present in 82 of 181 patients(45%), Compared with non-sarcopenic patients, sarcopenic patients had a higher risk of postoperative complications, longer postoperative hospital stay, more hospital costs and lower harris scores. In Binary logistic analysis revealed that sarcopenia(=0.08), hemiarthroplasty(<0.001), diabetes(=0.016) and infection(=0.018) were important predictors of unsatisfactory postoperative function.
CONCLUSIONSSarcopenia is an important predictor of poor postoperative prognosis in patients with femoral neck fractures after hip replacement. The treatment for sarcopenia maybe an important way to protect patients with femoral neck fractures from poor prognosis after hip replacement.
