1.Comparison study of corneal epithelial remodeling after TransPRK and Epi-LASIK for myopia

Abstract?AIM: To compare the changes in epithelial thickness profile following TransPRK and Epi-LASIK for myopia.? METHODS: In this prospective non -randomized controlled study, 76 right eyes of 76 myopic patients with the spherical equivalent refraction -1.25 to -6.00D were included under the informed consent. The eyes were divided into TransPRK group for 43 eyes and Epi-LASIK group for 33 eyes. Epithelial thickness was measured using spectral-domain optical coherence tomography at different corneal zones ( central, 2mm; paracentral, 2-5mm;and mid-peripheral, 5-6mm) preoperatively and at 1, 3, and 6mo postoperatively. The results were compared between the two groups.?RESULTS: The epithelium were thicker at 3 and 6mo after surgery compared to preoperative measurements in the two groups (all P<0.05).In TransPRK group, the epithelial thickness at 3 and 6mo demonstrated a negative meniscus-like lenticular pattern with lesser thickening centrally and progressively great thickening centrifugally (F3mo =-2.687,P=0.027;F6mo =-2.908,P=0.000).No statistically significant change was detected among the three zones in Epi-LASIK group (F=1.365, P=0.237). The epithelial thickness was thicker in the TransPRK group compared to the Epi-LASIK group mid-peripherally ( P<0.05) .? CONCLUSION: Significant epithelial thickening was observed after TransPRK and Epi-LASIK.It was showed a lenticular change with more thickening mid-peripherally after TransPRK than Epi -LASIK. Wound healing and inflammation may account for differences in the effect on epithelial thickness change by both surgeries.
2.Application of modular cementless femoral stems in complex revision hip arthroplasty.

OBJECTIVETo investigate and clarify the key surgical techniques and evaluate clinical effectiveness of modular cementless femoral stems(LINK MP modular stem) in complex revision hip arthroplasty.
METHODSFrom January 2002 to December 2013, 470 patients in Chinese PLA General Hospital received revision hip arthroplasties using MP stems. Among them, 246 patients were followed for an average of 5.7 years (ranged, 0.5 to 11 years). There were 148 males and 98 females, with an average age of 62.5 years old (ranged from 26 to 83 years old). The most common reason for revision was aseptic loosening of femoral stems (107 cases, 43.5%), followed by prosthetic joint infection (104 cases, 42.3%). Other reasons included unexplained hip or thigh pain, periprosthetic femoral fracture, prosthesis wear and dislocation. According to Paprosky classification of femoral deficiency, there were 171 cases of type IIIA (69.5%), 14 cases of type I, 39 cases of type II, and 22 cases of type IIIB. The average preoperative Harris hip score (HHS) was 37.4 ± 7.5. In addition, radiographic presentation, degree of patients' subjective satisfaction, and clinical effectiveness were assessed as well.
RESULTSThe average Harris score was im- proved to 87.8 ± 4.2 after operation,with significant statistical difference when compared to preoperative score (t = 92.13,P = 0.00). Analysis of patients' subjective satisfaction for leg length discrepancy, stability and overall degree of satisfaction showed that the number of patients with great satisfaction were 27, 60, 61 respectively. The most common complication was the periprosthetic fracture (intraoperatively 21 cases and postoperatively 8 cases). Other complications included infection (7 cases), dislocation (5 cases), numbness possibly caused by incomplete sciatic nerve injury (6 cases) and thigh swelling (3 cases). The X-ray taken at the final follow-up revealed that prosthesis subsidence occurred in 4 cases, 2 of which needed a second revision.
CONCLUSIONThe application of modular cementless femoral stems (LINK MP modular stem) in complex revision hip arthroplasty (especially in cases with the serious defect of proximal femoral bone) shows good results at both the short-term and long-term follow-up, especially in the revision for prosthetic infection with the high success rate. In order to avoid complications and improve clinical outcomes,the surgeons should carefully select proper surgery candidates,perform the procedure with care, and master the key surgical techniques.

3.Chronic effects of percutaneous transmyocardial laser revascularization in patients with refractory angina.

OBJECTIVEConflicting results exist on the therapeutic effects of percutaneous myocardial laser revascularization (PMR) in patients with refractory angina pectoris. This study assessed the effects of PMR on myocardial innervation and perfusion in patients with refractory angina pectoris.
METHODSPatients with refractory angina unsuitable for standard revascularization treatment (PTI and CABG) were randomly divided into medication plus PMR (PMR, n = 17) and medication group (M, n = 13). Coronary sinus noradrenaline (NE) and epinephrine (E) levels, heart rate variability (HRV), total ischemic burden (TIB), and ischemic ST segmental events (STI), myocardial perfusion were evaluated at pre-, immediately post and 12 months post treatment (mean followed up time = 11.6 +/- 4.9 months).
RESULTSIn PMR group, one patient developed non-persistent ventricular tachycardia, 2 developed pericardial tamponade and another one patient developed heart failure at 24 h after operation. Coronary sinus NE and E were significantly lower 60 min post PMR compared to pre-PMR and HRV was significantly increased 24 h post PMR. One year post treatments, angina grade was significantly decreased in PMR (1.7 +/- 0.3) than that in M group (0.4 +/- 0.2, P < 0.05) while other parameters were similar between the groups.
CONCLUSIONSPMR induced an early transient denervation and decreased angina grade one year post treatment in patients with refractory angina.

4.An echocardiographic study of left ventricular torsion in patients with latent obstructive hypertrophic cardiomyopathy

Objective To explore the changes of left ventricular torsion function in patients with latent obstructive hypertrophic cardiomyopathy ( HCM ) ,and provide quantitative informations for clinical evaluation of cardiac function . Methods A total of 49 consecutive patients with HCM without left ventricular outflow tract obstruction at rest were enrolled . All subjects underwent exercise stress echocardiography . After exercise left ventricular outflow tract pressure gradient ( LVO T‐PG ) ≥30 mm Hg was positive for exercise stress test ( latent obstruction) ,w hile LVO T‐PG< 30 mm Hg was negative for exercise stress test ( non‐obstruction) . An ultrasound system obtained two‐dimensional ultrasound images of resting and moving peaks . The global longitudinal strain ( GLS ) ,global circumferential strain ( GCS ) , global radial strain ( GRS) of the left ventricle 16 segments and left ventricular rotation ,twist were analysis using off‐line EchoPAC software . T he differences of the above parameters were compared between the two groups . Results T here were no significant differences in GLS ,GRS ,GCS and Rotation‐B between the two groups in resting and peak period of exercise ( all P > 0 .05 ) ,GRS in both groups were significantly increased compared with that before exercise ( all P < 0 .05 ) . Compared with the negative exercise stress group ,the left ventricular twist and Rotation‐A were significantly increased in resting and peak period of exercise in the positive exercise stress test group( all P <0 .05) . Compared with before exercise ,Rotation‐A and left ventricular twist were significantly decreased in the positive exercise stress test group ( all P <0 .05) ,while no significantly difference was found in the negative exercise stress group ( all P > 0 .05 ) . Conclusions Left ventricular torsion function is significantly changed in rest and after exercise in latent obstructive HCM patients ,providing valuable quantitative information for clinical comprehensive evaluation of cardiac function .
5.18 F-FDG PET/CT in staging and metabolic activity assessment of multiple myeloma

Objective To investigate the clinical value of 18 F?FDG PET/CT in staging multiple myeloma ( MM) and evaluating the glucose metabolic activity of MM. Methods A total of 25 MM patients ( 13 males, 12 females, age:39-67 years) from May 2010 to April 2015 were enrolled in this retrospective study. The SUVmax of each patient was recorded. D?S plus staging according to 18 F?FDG PET/CT was com?pared with the traditional D?S staging. The SUVmax and the percentage of plasmacytes of bone marrow of phase Ⅲ and non?phase Ⅲ ( phaseⅠand Ⅱ) according to D?S plus staging were compared. Two?sample t test and Wilcoxon rank sum test were used to analyze the data. Results In 25 MM patients, the range of SUVmax of lesions was 1.8-12?0 and the mean value was 5.15±2.74. According to D?S staging, the numbers of patients with phase Ⅰ,Ⅱ andⅢwere 7, 4 and 14, respectively. While the numbers were 3, 1 and 21 by D?S plus staging. Based on the D?S plus staging system, stages of 7 patients ( 28%, 7/25 ) were changed. According to the D?S plus staging system, the SUVmax between phaseⅢand non?phaseⅢpatients was significantly different (5.75±2.54 vs 3.00±0?70; t=2.12, P<0.05), while the percentage of plasma?cytes of bone marrow between the 2 groups had no significant difference ( 17. 50%( 4. 25%-41. 75%) vs 11?15%(10.25%-36.57%);z=0.05, P>0.05). Conclusion 18F?FDG PET/CT is of clinical importance for MM staging and metabolic activity assessment of MM.
6.Self-adapted segmentation of pulmonary nodule based on region growing.

OBJECTIVETo improve the accuracy and efficiency of pulmonary nodule segmentation of thoracic CT image for computer-aided diagnostic (CAD) system, especially for those nodules adhering to the pleural or blood vessels.
METHODSWe proposed the automatic process of pulmonary nodule segmentation, and using region growing method based on the contrast and gradient, the pulmonary nodule images were acquired. A self-adapted morphologic segmentation algorithm was presented for the unsuccessful nodule segmentation using region growing.
RESULTS AND CONCLUSIONSExperiments on clinical 2D pulmonary images showed that the solitary pulmonary nodules and those adhering to the pleural or blood vessels could all be segmented. This pulmonary nodule segmentation algorithm is feasible for automated segmentation of the thoracic CT images.

7.Effects of low tidal volume mechanical ventilation and low level of positive end expiratory pressure on respiratory function in patients with asthma under general anesthesia

Objective · To investigate the effects of low tidal volume mechanical ventilation and low level of positive end expiratory pressure (PEEP) on respiratory function in patients with asthma under general anesthesia. Methods · Forty-five patients with asthma undergoing upper abdominal surgery under general anesthesia were selected and randomly divided into three groups, with 15 cases in each group. Group A received a regular tidal volume of 10 mL/kg, group B a low tidal volume of 6 mL/kg and group C a low tidal volume of 6 mL/kg combined with 5 cmH2O PEEP. Ppeak was recorded at different time points, including the time of induction under anesthesia and the time of 5, 30 and 60 minutes after anesthesia induction. Based on such parameters, Cdyn were also calculated. Moreover, blood gas analysis index (PaO2 and PaCO2) were documented at the time of deoxidation period for 5 minutes before induction and 60 and 120 minutes after induction of anesthesia. Results · The Ppeak values in goup C were significantly lower at all times (P<0.01) and Cdyn at the time of 60 and 120 minutes after induction were significantly higher (P<0.05) than group A. At the time of 60 and 120 minutes after induction, the levels of PaO2 were also increased (P<0.01) while the levels of PaCO2 was significantly decreased (P<0.05) in group C compared to group A and group B respectively. Conclusion · The intermittent positive pressure ventilation of low tidal volume combined with low level of PEEP is a safe and effective mechanical ventilation method for asthma patients under general anesthesia, which can reduce airway peak pressure, enhance pulmonary dynamic compliance and improve the oxygenation index.
8.Influence of T-bet/GATA-3 to Th1/Th2 of athma rats

Objective:To discuss the influence of dexamethasone(DXM) to T-bet/GATA-3 of athma rats.Methods: 30 SD rats were randomly divided into normal group,model group and DXM group,10 rats in each group.Ovalbumin was intraperitoneal injected on the 1th and 8th day and aerosol inhaled from the 15th day,once a day,14 days altogether.Dexamethasone was intraperitoneal injected in 0.5 mg/kg from the 15th day,once per day,a total of 14 times.Flow cytometry tested the content of Th1 and Th2 in spleen and peripheral blood,immunohistochemistry and Western blot measured T-bet and GATA-3 protein expression and RT-PCR tested T-bet and GATA-3 mRNA expression.Results: Compared model group with normal group,the contents of Th1,Th2 in spleen and peripheral blood had a very significant increase(P<0.01);expression of T-bet,GATA-3 protein and mRNA in the lung tissue also had very significant risen(P<0.01).While compared DXM group with model group,T-bet/GATA-3 rose significantly(P<0.05)or very significantly(P<0.01).Conclusion: T-bet/GATA-3 is the key transcription factors that influence the balance of Th1/Th2,while DXM can raise the ratio of T-bet/GATA-3,which reduce the severity of asthma.
9.Effect of Task-oriented Training on Upper Limb Function in Children with Spastic Hemiplegic

Objective To investigate the effectiveness of task-oriented bilateral arm training on upper limb function in children with spastic hemiplegia. Methods 27 children within 4-6 years with spastic hemiplegic were randomly divided into control group (n=13) and experiment group (n=14). The children in control group received routine occupational therapy. The bilateral arm task-oriented training was added for children in experiment group. The Peabody development motor scale-2 (PDMS-2) and Upper Limb Physician's Rating Scale (ULPRS) were used to assess the upper limb function before and after 10 weeks intervention. Results After treatment, the scores of grasping function and visual-motor integration in PDMS-2 were significantly higher than those before treatment (P<0.05), and the scores of experimental group werehigher than that of control group (P <0.05), and the difference of the added value between two groups were statistically significant (P<0.05).The ULPRS scores of both groups increased after treatment (P<0.05), scores between the treatment group and the control group were not statistically significant (P>0.05), and the difference of the added value between two groups was not statistically significant (P>0.05). Conclusion The task-oriented bilateral arm training can promote recovery for upper limb and hand function in children with spastic hemiplegia compared with routine occupational therapy.
10.Iodized salt consumption and iodine deficiency status in China: a cross-sectional study

Objective:To monitor iodized salt consumption and evaluate iodine deficiency status in 2014 in China.Materials and Methods:In 2014,a nationwide cross-sectional survey was conducted in 31 provinces (in this study,provinces,autonomous regions and municipalities in Mainland China were named as provinces).Probability proportional to size sampling method was adopted to recruit children ages 8-10 and pregnant women.47,467 children's and 18,994 pregnant women's urine samples were collected and 47,706 children's thyroid volumes were examined.Iodine content in salt was determined with 46,900 edible salt samples from children's households;urinary iodine concentration (UIC) was tested from children and pregnant women's urine samples;thyroid volume of children was assessed by ultrasound.Results:The national coverage rate of iodized salt and consumption rate of qualified iodized salt were 96.3% and 91.5%,respectively.Median iodine content in iodized salt was 25.2 mg/kg.In 22 of 31 provinces,the provincial coverage rates of iodized salt were over 95%.And consumption rates of qualified iodized salt were more than 90% in 21 provinces.In this study,the national median urinary iodine concentration (MUIC) of children in China was 197.9 μg/L.At the provincial level,MUIC of children in 19 provinces was 100-199 μg/L,which in 12 provinces was 200-299 μg/L.The national MUIC of pregnant women in 2014 was 154.6 μg/L,slightly higher than the lower limit of the WHO criteria for adequate (150-249 μg/L).At the provincial level,MUIC of pregnant women in 18 provinces was 100-149 μg/L,which in 13 provinces was 150-249 μg/L.The national prevalence of goiter among children in 2014 was 2.6%,of which only Shandong province (5.6%) exceeded the national standard (5%).Conclusions:In China,iodine deficiency disorders (IDD) has been eliminated since 2005.And in 2014,the IDD-free status still remained.
