3.A Case of Leg Ulcer in Systemic Scleroderma Treated with Pinch Grafts.

The pinch graft is one type of skin graft and is indicated in small, chronic stubborn wounds or ulcers where cosmetic results are not paramount. It is a common procedure for the rapid healing of leg ulcers in countries where these are very common, such as in western society. However, it is an unfamiliar procedure to dermatologists in Korea where leg ulcers are quite rare. We treated a case of leg ulcer in systemic sclerosis using pinch grafts with satisfactory results. The pinch graft may be a useful treatment method for skin ulcers in dermatologic surgery under adverse conditions.

4.Venous Leg Ulcer in a Sarcoidosis Patient: A Case Report.

Venous leg ulcers, the most common form of leg ulcers, are relevant to the pathogenicity of pericapillary fibrin cuff. Sarcoidosis, a multiorgan granulomatous disease, causes fibrin deposition in tissues. We report a case of a 50-year-old man with venous leg ulcers coexisting with sarcoidosis. On the basis of the histologic findings, we propose the hypothesis that sarcoidosis patients are prone to the development of venous leg ulcers.

5.A Hydroxyurea-induced Leg Ulcer.

Hydroxyurea is a cytostatic agent that has recently become the drug of choice in the treatment of various myeloproliferative diseases. The cutaneous side effects of hydroxyurea include xerosis, hyperpigmentation, nail discoloration, and scaling. Leg ulcers have only rarely been reported in association with hydroxyurea treatment. A 75-year-old woman presented with leg ulcers, nail discoloration, and xerosis. The leg ulcers were refractory to conventional treatment. She had been taking oral hydroxyurea since being diagnosed with essential thrombocytosis in 2002. Hence, we suspected hydroxyurea-induced leg ulcers and discontinued her hydroxyurea treatment; the ulcers gradually healed thereafter. We present a rare case of hydroxyurea-induced leg ulcers in Korea.

6.Systemic Blastomycosis with Osseous Involvement of the Foot: A Case Report.

We report a patient who presented with three months of foot pain, lytic navicular bone lesions in the foot, and a painless ipsilateral leg skin ulcer. Bone and skin biopsies revealed organisms compatible with Blastomyces. Systemic blastomycosis is very rare, especially with bone involvement in the foot.

The antiphospholipid syndrome is a multiple-system disorder characterized by persistently elevated antiphospholipid antibodies and/or arterial or venous thrombosis, thrombocytopenia, or recurrent spontaneous abortion. Various cutaneous manifestations have been associated with this disease and may be the first sign of the underlying problem. We report a case of antiphospholipid syndrome with cutaneous ulcer on the left calf of a 21-year-old woman, who has been treated with deep vein thrombosis.

We report a case of systemic lupus erythematosus (SLE) and antiphospholipid syndrome with cutaneous ulcer on the right ankle. Histopathologic finding of noninflammatory vascular thrombosis suggested that antiphospholipid antibodies played a pathogenetic role in the described skin manifestation of anti-cardiolipin antibody and lupus anticoagulant positive SLE patients.

9.Marjolin's Ulcer Presenting with In-Transit Metastases: A Case Report and Literature Review.

Marjolin's ulcer is an aggressive cutaneous malignancy common in previously traumatized or chronically inflamed skin. It has high regional metastasis and fatality rates. Our patient presented with subcutaneous nodules and ulcerations on the right limb. He had a history of osteomyelitis of the fifth toe. Histopathological examination of the nodule and ulceration demonstrated squamous cell carcinoma. The nodules and ulcerations were in-transit metastases of Marjolin's ulcer. Here, we present a case of squamous cell carcinoma arising at a site of a chronic osteomyelitis with resultant in-transit metastases.

10.A Case of Prolonged Skin Ulceration Caused by Jellyfish Stings.

A 32-year-old Korean man was stung by a jellyfish on his right lower leg while scuba diving at the Philippine seashore. The stung lesion rapidly became painful, erythematous and swollen. Seven days later, an irregular shaped ulcer developed at the pervious sting site. The histopathological findings showed focal perivascular infiltration of chronic inflammatory cells in the upper and mid dermis, fibrosis and focal necrosis in the subcutis. The patient had been treated with antibiotic ointment, topical application of tetrachlordecanoic acid solution and 800mg pentoxifylline for 2 months. During treatment, leg swelling had recurred intermittently. After 2 months, the lesion healed leaving an erythematous atrophic scar. We report a rare case of delayed cutaneous reactions with prolonged skin ulceration and chronic recurrent swelling due to lymphatic obstruction.
