1.A Study of Correlation between Stage and Angiogenesis f Uterine Cervical Squamous Cell Carcinoma.

A variety of malignant neoplasms have been shown to induce neovascularization, and in some cases the degree of vascularization appears to correlate with an aggressive behavior and risk of metastasis. We compared the degree of vascularization in 11 benign and 33 cancerous lesions of the cervix. The microvessels were identified by immunohistochemistry using antibody to Factor VIII-related antigen in 44 hystrectomy specimens. Three highly vascularized microscopic fields were selected and counted the number of microvessels in 400 magnification. The proportion of the endothelial cell area was also quantified by using the CAS 200 image analysis system. All 33 cases of carcinomas demonstrated a significantly higher microvessel count and an endothelial cell area than those of the benign lesions (p<0.01). There were no significant difference in microvessel count and endothelial cell area among carcinoma in situ, microinvasive carcinoma and invasive carcinoma (p>0.05). Microvessel count and an endothelial cell area in invasive cancers were not correlated with tumor size, depth of invasion, or histologic type (p>0.05).This study showed cervical cancer induces neovascularization in an early stage but it is difficult to predict prognosis and metastasis with microvessel count and an endothelial cell area.

2.Clinical Usefulness of New Cervicography in Screening of Cervical Cancer.

OBJECTIVE: This study was performed to evaluate the usefulness of New cervicography by comparing cervicographic finding with Papanicolaou smear and result of colposcopically directed biopsy. METHODS: This study group consisted of 189 patients who visited the Department of Obstetrics and Gynecology, Chung-Ang University Pildong Hospital from September 1998 to August 1999. All women simultaneously underwent Papanicolaou smear and New cervicography. If either method was positive, the patient was referred for colposcopically directed biopsy. RESULTS: The New cervicography was significantly more sensitive than the Papanicolaou smear(92.2% vs 60.8%, p<0.01), whereas the Papanicolaou smear was significantly more specific than the New cervicography(99.3% vs 75.0%, p<0.05). The false positive rate of New cervicography was 75%, significantly higher than 0.7% of Papanicolaou smear. There was no significant difference between the negative predictive value of cervicography and Papanicolaou smear. When New cervicography and Papanicolaou smear were used together, the sensitivity was significantly higher than Papanicolaou smear used alone(98.1% vs 60.8%, p<0.01) and false negative rate was significantly lower than Papanicolaou smear(1.9% vs 39.2%, p<0.01). However the specificity, positive predictive value of the combined test were lower than Papanicolaou smear. CONCLUSION: Cervicography is one of the Papanicolaou smear adjunctive tests and a useful method to detect cervical cancer. Our study confirms the previously suspected low sensitivity of Papanicolaou smear and demonstrate that cervicography is more sensitive than Papanicolaou smear and less specific. From the above results, when New cervicography and Papanicolaou smear were used together, the detection rate of cervical cancer will be increased more easily since the cases missed by one method may be picked up by another.

3.Endoscopic Removal of Upper Gastrointestinal Tract Foreign Body.

The clinical review was done on 17 cases with the foreign body in the upper gastrointestinal tract who underwent endoscopic removal of foreign body at Kang Nam Sacred Heart Hospital from Dec. 1981 to Dec. 1991. The results were as follows: 1) The age distribution varies from 3 months to 72 years of age, being most prevalent under 5 years of age. 2) The incidence of foreign body in the order of frequency was coin, fish bone, safety pinring, peanut, keyring, food bolus, artificial teeth and medal. Among 17 cases, 9 cases(53.0%) were lodged in the esophagus and 8 cases(47.0%) were lodged in the stomach. 3) Dysphagia or globus was the common symptom in 8 cases(47.2%), followed by absence of symptom in 7 cases(41.2%), upper abdominal discomfort in 1 case(5.8%) and vomiting in 1 case (5.8%). 4) l0 cases(58.8%) of foreign body were removed within 72 hours and 7 cases(41.2%) of foreign body were removed after 72 hours.

5.Teh Effect of Local anesthetic on Degeneration and Regeneration of the Experimentally Traumatized Striated Muscle

The repair of striated muscle after acute trauma occurs simultaneously by the regeneration of disrupted muscle and formation of connective scar. But, there had been no attention concerning effects of local anesthetics on the degeneration and regeneration process of the injured muscle. The author induced a constant contusion injury on the calf of rats. Lidocaine and/or epinephrine was intramuscularly injected at the site of injury. Morphological and histochemical observation was conducted and the results were as follows: 1. Traumatized striated muscle showed evidences of the healing process for upto 90 days. 2. Morphological and histochemical changes in the traumatized muscle became more severe after the injection of lidocaine and/or epinephrine. 3. Formation of collagenous scar tissue significantly increased after injection of lidocaine and/or epinephrine. Injection of lidocaine or epinephrine developed similar amount of scar after 15 days. 4. Cross section area of the regenerated muscle fibers decreased in the injection groups than in the control groups. Atrophy was significant on day 90 in all types of fiber after injection of the mixture of lidocaine and epinephrine, and in type B after injection of epinephrine. In conclusion, injection of the lidocaine and epinephrine inhibited regeneration of the traumatized striated muscle. Mixture of the drugs showed more inhibitory effect than a single drug.

8.Supramalleolar Stepcut Osteotomy for Tibial Deformity in Vitamin D

In treatment of Vitamin D-resistant rickets, the authors supplemented supramalleolar stepcut osteotomy for complex deformity of tibia consisted of varus, anterior bowing and intemal torsional supramalleolar level which accompanied severe waddling gait. The results of 10 tibia operations from 5 patients were generally satisfactory in terms of appearance and function.

10.Metabolic Abnormalities of 24-hour Urinary Lithogenic Factors in Recurrent Stone Formers.

PURPOSE: To determine the metabolic characteristics of recurrent stone formers (RSF), we investigated urinary lithogenic factors. MATERIALS AND METHODS: The 24-hour urine samples of 151 RSF (114 men, 37 women) and 70 normal controls (33 men, 37 women) were analyzed for excretion rate differences of lithogenic and inhibitory constituents such as volume, sodium, phosphorus, uric acid, calcium, magnesium, oxalate, and citrate. The incidence and spectrum of metabolic abnomality in REF were aldo determined. RESULTS: The RSF showed significantly increased excretion of phosphorus (p=0.004), uric acid (p=0.003), and calcium (p=0.007) and decreased in that of citrate (p=0.044). No significant differences were found between the RSF and normal controls with regard to the excretions of sodium, magnesium, oxalate, and volume. The most frequent metabolic abnormality in RSF was hypocitraturia (43.7%), followed by hypernatriuria (41.7%), hypercalciuria (23.2%), and hyperuricosuria (20.5%). At least one metabolic abnormality was found in 118 of out of 151 RSF (78.1%). CONCLUSIONS: Our results showed that RSF had different urinary excretions of citrate as well as phophorus, uric acid, and calcium compared to the normal subjects and metabolic abnomalities were found up to 80% of them. The 24-hour urine study would be an effective means for metabolic evaluation in RSF although diagnostic accuracy increases with repeated examination.
