1.Value of CT in the postoperative evaluation of patellar dislocation triple surgery

Objective To study the role of CT in the evaluation after patellar dislocation triple surgeries.Methods Retrospectively analysed the CT measurements of 60 recurrent patellar dislocation patients (61 knees),who had undergone patellar dislocation triple surgeries,within one month before and after procedures.The measurements included patellar tilt angle,lateral patellar displacement,trochlear con-gruence angle,Tibial Tuberosity-Trochlear Groove distance,Insall-Salvati Ratio and Caton-Deschamps Index.Moreover,the troch-lear dysplasia classifications of all patients were evaluated.In addtion,an analysis has also been completed for 40 (41 knees)of 60 patients who had second postoperative CT examinations as follow-up assessment 3 to 6 months after surgeries.Results All one-month postoperative measurements decreased obviously with statistical significance(P <0.05 ).Furthermore,one case of subluxa-tion,1 1 cases of abnormal Tibial Tuberosity-Trochlear Groove distance,13 cases of abnormal Insall-Salvati Ratio and 14 cases of Ca-ton-Deschamps Index abnormities were also observed.Besides that,it was also found that all patients had trochlear dysplasia and 5 cases were diagnosed with patellar subluxation or dislocation at the second postoperative follow-up assessment.Conclusion The patients with redislocation or risks for patellar instability can be detectecd by CT which provides objective evidence for postoperative evaluation and further treatments.
2.Study on Quality Standard for Yunüjian Granules

Objective:To establish the quality standard for Yunüjian granules. Methods:Rehmannia glutinosa, rhizoma anemar-rhenae and achyranthes in the granules were identified by TLC. The HPLC method was adopted. A SunfireTM C18 column (250 mm × 4. 6 mm, 5 μm) was used with the mobile phase consisting of acetonitrile-0. 1% acetic acid solution(16∶84) at the column tempera-ture of 30 ℃. The flow rate was 1. 0 ml·min-1 ,the detection wavelength was 334nm, and the imjection volume was 10 μl. Results:The TLC spots of verbascoside, and sarsasapogenin in rhizoma anemarrhenae and cyasterone in achyranthes were quite clear with good seperation. The linear range of verbascoside was within the range of 0.163-0.612 μg(r=0.999 9), and the average recovery was 101. 3%(RSD=2. 7%,n=6). Conclusion:The method is simple, reliable and accurate, and can be applied in the quality control of Ynnüjian granules.
3.The results of surgery on HIV carriers with urinary system disease.

OBJECTIVETo approach characteristics of performing operation on HIV carriers with urinary system diseases.
METHODSTo summarize author's experiences of surgery on 41 HIV carriers suffering urinary system diseases abroad from April 1996 to May 2004.
RESULTSThe 41 HIV carriers received HAART and were performed with corresponding operations, followed up from 4 to 30 months post-operatively. The 31 carriers have recovered well up to date, while 4 carriers died of AIDS. Among them, 2 patients with penis cancer who received a partial peotomy and a patient with renal tuberculosis receiving left nephrectomy were died of AIDS within 4-8 months after operations whose CD4+ T lymphocyte number was below 0.2 x 10(9)/L.
CONCLUSIONPrior to operation, HIV carriers should receive HAART ordinarily to control copy of the virus. The CD4+ T lymphocyte number is important for selecting a proper time for operation and deciding the further after surgery. We also take note to CD4+ T lymphocyte number to monitor progress of the AIDS. For those HIV carriers, endourologic surgery and laparoscopy should be taken so far as possible. Meanwhile, medical stuffs must pay more attention to preventing occupational infection during surgery.

4.Expressions of NOS isoforms in the cavernous tissues of diabetic rat models.

OBJECTIVETo investigate the mechanism of diabetic erectile dysfunction (ED) and find new methods for its treatment by detecting the changes in nitric oxide synthase (NOS) isoforms and erectile function of diabetic rats and observing the effects of insulin and alpha-lipoic acid (LA) on it.
METHODSFifty male Sprague-Dawley rats were divided into Groups A (normal control, n=10), B (non-intervention diabetes mellitus, n=13), C (insulin intervention diabetes mellitus, n=12), and D (insulin + LA intervention, n=15). And the diabetic models were made by intraperitoneal injection of streptozocin (STZ). Eight weeks later, the erectile function of the rats was assessed following apomorphine injection and the contents of NOS isoforms in the erectile tissues measured by immunohistochemical staining.
RESULTSAll the rats of Group A showed a normal erectile function (100%). In comparison, those in Groups B, C and D exhibited a significantly decreased rate, 28.6% in Group B, 62.5% in Group C and 80.9% in Group D. The numbers of positive nNOS fibers and eNOS in the penile tissues per visual field were 86.7 and 9.6 in Group A, but only 36.5 and 3.3 in Group B, 52.7 and 5.7 in Group C, and 71.4 and 7.4 in Group D (P < 0.05). However, the expression of iNOS was significantly lower in Group A (6.9) than in Groups B (43.6), C (36.2) and D (19.3) (P < 0.05). Compared with Groups B and C, the erectile function and the expressions of nNOS and eNOS were markedly increased, while the expression of iNOS significantly decreased in Group D (P < 0.05).
CONCLUSIONDiabetes mellitus severely affects penile erectile function and the expressions of NOS isoforms in the cavernous tissues, for which hyperglycemia is mainly responsible. LA is proved obviously efficacious for diabetic ED, which might be related to its antioxidant effect.

5.Expression of nerve growth factor in cavernous tissue and its effects on the treatment of rats with diabetic erectile dysfunction.

OBJECTIVETo investigate the content of nerve growth factor (NGF) in penis of rats with diabetic erectile dysfunction (DED) and apply rhNGF to treat the DED rat model to study the pathogenesis mechanism of DED and treatment effects by NGF for DED rat model.
METHODSFifty adult male SD rats were randomly selected to make up diabetic models. After 8 weeks, the mRNA and protein levels of NGF in rat penis were detected. Then the rest of rats were divided into 5 groups: normal control group, non-treated diabetes group, NGF treatment group, insulin treatment group, NGF plus insulin treatment group. After 8 weeks of treatment, intracavernous pressure (ICP) was measured. The content of neuronal nitric oxide synthase (nNOS) in the erectile tissue was also value- ated by immunohistochemistry staining.
RESULTSCompared with those of normal control group, the mRNA content of NGF and protein in the penis of non-treated diabetic rats is increased significantly. Compared with non-treated diabetic group, ICP was much higher in the treated groups which received NGF or/and insulin therapy, and the changes in nNOS staining of those groups were the same.
CONCLUSIONThe injury of pelvic splanchnic nerves in advanced stage of diabetes may lead to DED, which may be relevant to the abnormal of NGF level or the NGF receptors. The increased extent of NGF in penis of rat with DED suggests that erectile nerves be severely damaged by diabete and the increase of producing NGF couldn't compensate the needs for reproduction of the nerve fibers. The ability of combination of NGF with its receptor may also be damaged. It is helpful to use exogenous NGF to lessen the partly neuropathy and improve the erectile dysfunction of diabetic rats. The abnormal of NGF may play an important role in the pathogenesis of diabetic ED and its treatment.

6.Effects and mechanism of programmed cell death of human gastric cancer cell under the treatment of flavonoid compound GL-V9

To investigate the apoptotic effect of flavonoid compound GL-V9 on human gastric cancer cells and its potential mechanism, MGC-803 and BGC-823 cells were treated with GL-V9. MTT assay was performed to assess the growth inhibition effects on MGC-803 and BGC-823 cells under different concentrations of GL-V9. Annexin V-FITC/PI staining assay was employed to observe the apoptotic rate of GL-V9 cells with the treatment of GL-V9. DAPI staining was performed to observe the nuclear morphological changes using fluorescence microscopy. Activation of caspase-9 and caspase-3 was analyzed by Western blotting. Ca2+ concentration in gastric cancer cells was detected by Fluo-3 AM staining assay. Results showed that GL-V9 could inhibitcell viability, change the nuclear morphologyl, activate caspase-9 and caspase-3 and induce the apoptosis in gastric cancer cells. The mechanism of the induction of apoptosis in MGC-803 cells under the treatment of GL-V9 may aetivate the Ca2+ associated mitochondrial apoptosis pathway.
7.Risk factors analysis of pulmonary complications after video-assisted thoracoscopic surgery in elderly patients

Objective To analyze the incidence and risk factors of postoperative pulmonary complications(PPCs)in elderly patients undergoing video-assisted thoracoscopic surgery(VATS).Methods Elderly patients aged≥65 years who underwent VATS in Peking Union Medical College Hospital from January 2013 to December 2017,were reviewed retrospectively and divided into non-PPCs group and PPCs group.General information,past medi-cal history,and postoperative complications were recorded in a uniform case report form.The clinical factors with statistical significance in univariate analysis and important clinical significance according to experience were ana-lyzed with Logistic regression to evaluate the independent risk factors for PPCs.Results A total of 900 patients were included,and 48(5.3%)of them suffered PPCs.Multivariate logistic regression showed that pre-operative smoking history,history of stroke,chronic obstructive pulmonary disease(COPD)and pulmonary lobectomy were independent risk factors for PPCs in elderly patients after VATS.Conclusions Elderly patient is a high-risk group for PPCs after VATS.The risk factors include smoking history,stroke,COPD and scope of surgery.Therefore,perioperative management needs to optimize,and monitoring should be strengthened for these high-risk patients.
8.Anti-hemorrhoidal activity of Lian-Zhi-San, a traditional Chinese medicine, in an experimental hemorrhoidal model in rats.

OBJECTIVE:
Hemorrhoidal disease (HD) is the most common proctological disease, with an estimated prevalence rate of 4.4%, and a peak in individuals between 45 and 65 years of age. This study was done to evaluate whether Lian-Zhi-San (LZS), a clinically used anti-hemorrhoidal ointment could alleviate the inflammatory injury, with its associated changes of inflammatory cytokines and morphology of anorectal tissues, in an experimental model of HD in rats.
METHODS:
HD was induced by croton oil preparation (COP) applied to the anorectal region. Rats were then treated with cotton swabs soaked in LZS ointment, water or white vaseline, twice a day for 7 d. At the end of the experiment, HD was evaluated by measuring hemorrhoidal and biochemical parameters along with histopathological observations.
RESULTS:
In this study, COP induced a significant increase in the macroscopic severity score, anorectal coefficient and Evans blue extravasation, compared to normal rats. Additionally, it greatly enhanced the expression and secretion levels of some important inflammation-related cytokines along with marked histological damage, compared to normal rats. Rats treated with LZS ointment experienced significantly ameliorated Evans blue extravasation (P < 0.05), decreased macroscopic severity score (0.86 ± 0.14 vs. 1.65 ± 0.16) and the anorectal coefficient (P < 0.01); its use also attenuated tissue damage and inhibited the expression and secretion levels of inflammation-related cytokines (interleukin-1β, interleukin-6 and tumor necrosis factor-α).
CONCLUSION
This study validates a preliminary understanding of the use of LZS ointment to treat inflammatory factors and tissue damage in an experimental model of HD in rats.
