2.The role of CD4+CD25+Foxp3 regulatory T cells in patients with chronic hepatitis B

Objective To investigate the role of CD4+CD25+Foxp3 regulatory T cells in chronicity of hepatitis B and viral clearance of hepatitis B virus(HBV).Methods Nineteen patients with chronic active hepatitis B(CAH).21 HBV carriers(AsC)and 12 patients with resolved HBV infection and 1 5 healthy controls were enrolled.The frequency and phenotype of peripheral CD4+CD25+Foxp3+ T cells were detected by flow cytometry.CD4+CD25+T cells were sorted by magnetic-activated cell sorting(MACS)assay.Level of Foxp3 mRNA in CD4+CD25+T cells was examined by real time polymerase chain reaction(PCR)assay.The data were analyzed by one-way ANoVA or nonparametric statistics.Results Both frequencies of CD4+CD25+Foxp3+T cells and levels of Foxp3 mRNA in CD4+CD25+T ceils in patients with CAH or AsC were significantly higher than those in healthy controls Or resolved HBV infection(F=6.8,F=3.72,respectively;both P<0.05).Accumulation of Foxp3+T cells in liver tissue of CAH patients was higher than that of healthy controls,while that in AsC was lower than CAH.The frequency of CD4+CD25+Foxp3+T cells of hepatitis B e antigen(HBeAg)positive patients(including CAH and AsC)was significantly higher than that of HBeAg negative patients(t=2.3,P<0.05),and that of antFHBe negative patients were significantly higher than anti-HBe positive patients(t=2.4,P<0.05).Furthermore,the frequency of CD4+CD25+Foxp3 regulatory T cells was positively correlated with serum HBV DNA level of patients with chronic hepatitis B(r=0.56,P<0.01).Conclusion The findings have important implication in the understanding of the role of CD4'CD25'regulatory T cells in chronicity and viral clearance in HBV infection.

4.Case-control study on transcatheter artery embolization for massive bleeding due to pelvic fractures.

OBJECTIVETo investigate the techniques and curative effects of transcatheter artery embolization (TAE) for massive bleeding due to pelvic fractures.
METHODSThe clinical data of 92 patients with haemorrhage due to pelvic fractures from March 1998 to February 2008 were retrospectively analyzed. Among 53 patients treated conservatively such as massive transfusion and fluid infusion in the control group, 43 patients were male and 10 patients were female, ranging in age from 27 to 61 years, averaged (37.2 +/- 5.7) years. Among 39 patients who were hemodynamically unstable or had evidences of ongoing hemorrhage required TAE, 26 patients were male and 13 patients were female, ranging in age from 26 to 62 years, with a mean age of (35.3 +/- 9.5) years. The clinical date such as blood or fluid transfusion volume, shock redress time and survival rate were compared between the two groups. The hemostatic efficiency and complications of the surgery were also analyzed.
RESULTSThe average hemostasis time of TAE group was 2 hours. There were no intraoperative injuries of blood vessels, nerve or vital organs. Three patients had lower limbs numbness and 5 patients had gluteal skin redness after the operation. The blood transfusion or fluid infusion volume, shock redress time and survival rate were all significantly better than those in the conservative group.
CONCLUSIONSTAE is an early,rapid and effective method in controlling haemorrhage due to pelvic fractures.

5.Influence of individualized nursing intervention on postoperative rehabilitation of the patients with isehemic cerebral vascular diseases treated with endovascular intervention

Objective To observe the influence of the application of individualized nursing intervention on postoperative rehabilitation of the patients with ischemic cerebral vascular diseases treated with endovascular intervention.Methods Selected 53 patients with ischemic cerebral.vascular diseases as control group and 67 patients with ischemic cerebral vascular diseases as observation group,who treated with endovascular intervention from January 2010 to December 2010 and from January 2011 to December 2011 respectively.Nursing intervention was adopted in the observation group and routine nursing care was adopted in control group.The sleeping time,nursing measure leakages,postoperative complications and patient satisfaction were observed.Results In the observation group,the sleeping time were all increased in preoperative one night and the postoperative night (t =3.482,P =0.001 ; t =2.533,P =0.013),the nursing measure leakages were reduced(3.0% vs 13.2%,P =0.043),the incidence of postoperative complications was reduced(3.0% vs 15.1%,P =0.022),and the patient satisfaction was improved(97.0% vs 81.1% ;x2 =8.294,P =0.004) significantly.Conclusions The individualized nursing intervention can effectively reduces the postoperative complications,and improves the nursing quality,promotes the rehabilitation of patients,improves their life quality and satisfaction.
6.Inhibition of Curcumin analogue FM0807 on pannus formation in rheumatoid arthritis

Objective To observe the inhibitory effect of Curcumin analogue FM0807 on pannus formation and its possible mechanism.Methods 40 male SD rats were used to establish the adjuvant-induced arthritis rats model,and then randomly divided into model group,control group,test A group,test B group and test C group,8 rats in each group.Another 8 healthy SD rats were used as blank control group.7 days after modeling,test A group,testB group and test C group were given 25,50,100 mg · kg-1 FM0807 through gavage ouce a day;control group was given 0.2 mg · kg-1 dexamethasone intraperitoneal injection,once a day;model group and the blank control group were given 0.9% NaCl gavage,once a day.The rats in the six groups were treated for 21 days.Observe the content of interleukin-6 (IL-6) and tumor necrosis factor -or (TNF-α) in serum by enzyme-linked immunosorbent assay (ELISA) and determinec-Jun N-terminal kinase (JNK) protein expression by western blotting.Results The contents of TNF-α and IL-6 in serum of test A group,test B group,test C group,control group,model group and blank control group were (106.10 ± 7.15),(88.12 ± 12.75),(67.18 ±11.95),(40.96 ±9.40),(123.76 ±5.05),(18.02±10.97) pg · mL-1and (1.61 ±0.14),(1.48±0.15),(0.73 ± 0.16),(0.41 ± 0.12),(1.88 ± 0.09),(0.27 ± 0.10) pg · mL-1 respectively.JNK protein expression was (7.00 ± 1.22),(3.40 ± 0.54),(2.60 ± 0.55),(1.40 ± 0.55),(7.20 ± 1.30),(1.20 ± 0.27).The differences between test C group and model group were statistically significant (all P < 0.01).Conclusion Curcumin analogue FM0807 may inhibit the formation of pannus and relieve the symptom of rheumatoid arthritis by inhibiting JNK pathway,decreasing the release of inflammatory mediators and reducing inflammatory cell stimulation.
8.Clinical effect of superselective arterial embolization for the treatment of massive haemorrhage from pelvic fracture.

OBJECTIVETo investigate feasibility and curative effect of superselective arterial embolization for the treatment of massive haemorrhage from pelvic fracture.
METHODSFrom March 2008 to February 2016, clinical data of 65 patients with massive haemorrhage from pelvic fracture were collected and analyzed, and patients were divided into non-embolic and embolic group according to whether perform vascular thrombosis. Thirty-three patients were in non-embolic group including 26 males and 7 females aged from 21 to 64 years old with an average of(39.2±5.7) years old, the time from injury to operation ranged from 1.1 to 4.8 h with an average of (2.2±0.4) h; 12 cases were type B and 21 cases were type C according to AO/Tile classification; injury severity score (ISS) ranged from 25 to 42 with an average of (37.7±7.5); shock index score ranged from 1.7 to 2.4 with an average of 2.1±0.3; treated with blood transfusion and fluid infusion. Thirty-two patients in embolic group, including 25 males and 7 females aged from 22 to 65 years old with an average of(38.1±4.5) years old; the time from injury to operation ranged from 1.2 to 4.8 h with an average of (2.1± 0.5) h; 14 cases were type B and 18 cases were type C according to AO/Tile classification; ISS ranged from 26 to 43 with an average of 38.9±4.5; shock index score ranged from 1.6 to 2.4 with an average of 2.2±0.2; treated by blood transfusion and fluid infusion with superselective arterial embolization. Blood transfusion volume, fluid infusion volume, shock correction time and survival rate were observed and compared, effective rate of hemostasis and postoperative complications were compared.
RESULTSThirty-seven artery were injured in embolic group, hemostasis were controlled at 3 h after operation, and hemodynamics turned to stable. There were significant difference in blood transfusion volume, fluid infusion volume, shock correction time between non-embolic and embolic group, and embolic group performed better. Survival rate in embolic group was also better than that of non-embolic group, and had significant difference. While there was obvious differences in complications(χ²=4.03,=0.045).
CONCLUSIONSSuperselective arterial embolization for massive haemorrhage from pelvic fracture could effective hemostasis, reduce blood transfusion and fluid infusion volume and occurrence rate of shock, moreover improve survival rate and deserves promotion.
9.Identification and analysis of anti-diabetic drugs based on terahertz time-domain spectroscopy

Objective To study the vibration frequencies and resonant peaks of different molecular groups in anti-diabetic drugs and to investigate the absorption spectra of these drugs in the range of terahertz, so as to accurately and efficiently identify similar drugs and provide foundation for understanding the contribution of such vibration in different molecular groups to pharmacology. Methods Using terahertz time domain spectroscopy (THz-TDS), six kinds of similar diabetes pills for treating diabetes mellitus (DM), including gliquidone, glipizide, gliclazide, glimepiride, repaglinide and metformin were tested to obtain their absorption spectra from 0.3~3.0 THz. Support vector machines (SVM) method was employed to separate these anti-diabetic drugs by selecting the experimental data from 1.5~2.0 THz. Results For gliquidone, glipizide, gliclazide, and glimepiride, an obvious resonance peak was found at 1.37 THz. According to the THz spectra, repaglinide and metformin could be easily separated from sulfonylurea drugs. Furthermore, with the help of SVM, the accuracy of discrimination for four kinds of sulfonylurea could reach 100%. Conclusions THz technology, as a new testing method, shall play a positive role in the drugs for their identification, quality control and distinguishing their chemical bonds/functional group.

Objective: Compare and analyze the results of the domestic Lanyi AH600 glycated hemoglobin analyzer and other different detection systems to understand the comparability of the detection results of different detectors, and establish the best cut point of Lanyi AH600 determination of haemoglobin A1c (HbA1c) in the diagnosis of diabetes. Methods: Multi center cohort study was adopted. The clinical laboratory departments of 18 medical institutions independently collected test samples from their respective hospitals from March to April 2022, and independently completed comparative analysis of the evaluated instrument (Lanyi AH600) and the reference instrument HbA1c. The reference instruments include four different brands of glycosylated hemoglobin meters, including Arkray, Bio-Rad, DOSOH, and Huizhong. Scatter plot was used to calculate the correlation between the results of different detection systems, and the regression equation was calculated. The consistency analysis between the results of different detection systems was evaluated by Bland Altman method. Consistency judgment principles: (1) When the 95% limits of agreement (95% LoA) of the measurement difference was within 0.4% HbA1c and the measurement score was≥80 points, the comparison consistency was good; (2) When the measurement difference of 95% LoA exceeded 0.4% HbA1c, and the measurement score was≥80 points, the comparison consistency was relatively good; (3) The measurement score was less than 80 points, the comparison consistency was poor. The difference between the results of different detection systems was tested by paired sample T test or Wilcoxon paired sign rank sum test; The best cut-off point of diabetes was analyzed by receiver operating characteristic curve (ROC). Results: The correlation coefficient R2 of results between Lanyi AH600 and the reference instrument in 16 hospitals is≥0.99; The Bland Altman consistency analysis showed that the difference of 95% LoA in Nanjing Maternity and Child Health Care Hospital in Jiangsu Province (reference instrument: Arkray HA8180) was -0.486%-0.325%, and the measurement score was 94.6 points (473/500); The difference of 95% LoA in the Tibetan Traditional Medical Hospital of TAR (reference instrument: Bio-Rad Variant II) was -0.727%-0.612%, and the measurement score was 89.8 points; The difference of 95% LoA in the People's Hospital of Chongqing Liang Jiang New Area (reference instrument: Huizhong MQ-2000PT) was -0.231%-0.461%, and the measurement score was 96.6 points; The difference of 95% LoA in the Taihe Hospital of traditional Chinese Medicine in Anhui Province (reference instrument: Huizhong MQ-2000PT) was -0.469%-0.479%, and the measurement score was 91.9 points. The other 14 hospitals, Lanyi AH600, were compared with 4 reference instrument brands, the difference of 95% LoA was less than 0.4% HbA1c, and the scores were all greater than 95 points. The results of paired sample T test or Wilcoxon paired sign rank sum test showed that there was no statistically significant difference between Lanyi AH600 and the reference instrument Arkray HA8180 (Z=1.665,P=0.096), with no statistical difference. The mean difference between the measured values of the two instruments was 0.004%. The comparison data of Lanyi AH600 and the reference instrument of all other institutions had significant differences (all P<0.001), however, it was necessary to consider whether it was within the clinical acceptable range in combination with the results of the Bland-Altman consistency analysis. The ROC curve of HbA1c detected by Lanyi AH600 in 985 patients with diabetes and 3 423 patients with non-diabetes was analyzed, the area under curve (AUC) was 0.877, the standard error was 0.007, and the 95% confidence interval 95%CI was (0.864, 0.891), which was statistically significant (P<0.001). The maximum value of Youden index was 0.634, and the corresponding HbA1c cut point was 6.235%. The sensitivity and specificity of diabetes diagnosis were 76.2% and 87.2%, respectively. Conclusion: Among the hospitals and instruments currently included in this study, among these four hospitals included Nanjing Maternity and Child Health Care Hospital in Jiangsu Province (reference instrument: Arkray HA8180), Tibetan Traditional Medical Hospital of TAR (reference instrument: Bio-Rad Variant Ⅱ), the People's Hospital of Chongqing Liang Jiang New Area (reference instrument: Huizhong MQ-2000PT), and the Taihe Hospital of traditional Chinese Medicine in Anhui Province (reference instrument: Huizhong MQ-2000PT), the comparison between Lanyi AH600 and the reference instruments showed relatively good consistency, while the other 14 hospitals involved four different brands of reference instruments: Arkray, Bio-Rad, DOSOH, and Huizhong, Lanyi AH600 had good consistency with its comparison. The best cut point of the domestic Lanyi AH600 for detecting HbA1c in the diagnosis of diabetes is 6.235%.
