1.Analysis of position of one-piece soft intraocular lens after implantation of intraocular lens with Pentacam

Background The aspheric intraocular lenses(IOLs)can reduce ocular spherical aberration to some degree.However,the clinical effect depends more on the IOL proper alignment.It becomes more important to study the IOL position in eye,Objective This study was to analyze the position alteration of IOL after phacoemulsification combined with implantation of one-piece soft IOLs.Methods In this prospective control study,80 eyes of 40 patients with age-related cataract were enrolled.The phacoemulsification with IOL implantation was performed in all the eyes.Decentration and tilt of IOL in the nasal superior,superior temporal,inferior temporal and nasal inferior quadrants(the intersection point of the system optical axis and the IOL maximum cross plane were regarded as the ordinate origin)were measured by rotating Scheimpflug camera(Pentacam Oculus)in 3 months postoperatively under the mydriasis condition.Written informed consent was obtained from each subject prior to this trial.Results In the right eye group,the IOL decentered toward temporal in 26 eyes(65%)and infratemporal in 16 eyes(40%).IOLs tilted temporally in the horizontal plane in 37 eyes(92.5%)and tilted inferiorly in the vertical plane in 34 eyes(85.0%).In the left eye group,IOLs decentered temporally 33 eyes(82.5%)and 20 IOLs (50%)infratemporally,IOLs tilted temporally in the horizontal plane in 37 eye(92.5%)and 36 IOLs(90%)tilted inferiorly in the vertical plane.There was no statistical difference for the intercomparsion of horizontal/vertical decentration in various quadrant in the right eye(F =0.221,0.792,P>0.05).The obvious elevated horizontal decentration was found in the supertemporal and infratemporal quadrants compared with supernasal quadrant in the left eyes but there was no significant difference in the vertical decentration among 3 quadrants(F=0.576,P>0.05).Decentrations were positively correlated with the tilt in both horizontal and vertical plane(right eye horizontal plane:r=0.374,P=0.002;right eye vertical plane:r=0.402,P=0.001 ;left eye horizontal plane:r=0.377,P=0.002;left eye vertical plane:r=0.347,P=0.002).Conclusions The one-piece soft IOLs(Adapt AO)decenter toward temporal mostly in 3 months after surgery,especially infratemporally in the eye.And the optical axis of the IOL tilt toward infratemperol mostly in both right and left eyes.The decentration and tilt are consisted in the corresponding direction between the right and left eyes.The position of the IOLs showed mirror symmetry between right and left eyes.The IOLs decentration show the positively correlation to tilt whatever in horizontal and vertical plane.
2.Comparation of effect and cost-benefit analysis between acupoint catgut-embedding and electroacupuncture on simple obesity.

OBJECTIVETo compare the clinical effect of acupoint catgut-embedding and electroacupuncture on simple obesity and evaluate the economics benefit by cost-benefit analysis.
METHODSSixty cases were randomly devided into an acupoint catgut-embedding group and an electroacupuncture group, 30 cases in each group. Zhongwan (CV 12), Tianshu (ST 25), Daheng (SP 15), Shuifen (CV 9), Qihai (CV 6), Guanyuan (CV 4), Zusanli (ST 36) and Ashi acupoints were selected as the main acupoints in both groups. The acupoint catgut-embedding group was treated with acupoint catgut-embedding, once each week, four weeks as a course for two courses. The electroacupuncture group was treated with electroacupuncture, three times each week for eight weeks.
RESULTS1) The total effective rate in the acupoint catgut-embedding group was 90.0% (27/30) and in the electroacupuncture group was 86.7% (26/30), with the similar therapeutic effect between the two groups (P > 0.05). 2) The body mass, body mass index (BMI), waistline, hip circumference and waist-to-hip ratio in the two groups were all decreased significantly (all P < 0.05). 3) The total medical treatment cost in the acupoint catgut-embedding group was 61 500 yuan and the cost per patient was 2050 yuan, and in the electroacupuncture group, the total cost was 117 210 yuan and the cost per patient was 3907 yuan. The cost effect analysis showed that there were 1857 yuan of the cost per patient in the acupoint catgut-embedding group less than that in the electroacupuncture group.
CONCLUSIONAcupoint catgut-embedding has significant effect in treating simple obesity with low cost and fine economics benefit.

Natural products are important sources for the discovery of anti-tumor drugs. Evodiamine is the main alkaloid component of the traditional Chinese herb Wu-Chu-Yu, and it has weak antitumor activity. In recent years, a number of highly active antitumor candidates have been discovered with a significant progress. This article reviews the research progress of evodiamine-based antitumor drug design strategies, in order to provide reference for the development of new drugs with natural products as leads.
4.Effect of Changtong oral liquid on fibroblast proliferation in normal and adhesive rat peritoneal tissues.

OBJECTIVETo investigate the effect of Changtong oral liquid (CTOL) on the proliferation of cultured fibroblasts derived from normal peritoneum (NFs) and adhesive peritoneum (AFs) of rats.
METHODSTwenty male SD rats were randomized into 4 groups, including a normal serum group and 3 CTOL groups with CTOL treatment at low, medium or high doses. Serum samples were obtained from the abdominal arteries of the rats after oral treatment with CTOL for 7 days. The fibroblasts were isolated from the peritoneum by means of tissue culture, and the passage 3-8 cells were cultured with the sera of the normal control and CTOL groups for 24, 48, 72 and 96 h. MTT assay was used to observe the proliferation of the fibroblasts.
RESULTSThe dose of CTOL was inversely correlated to the absorbance but positively to the growth inhibition rates. Compared with the NFs cultured in normal control rat serum, the NFs in serum from CTOL groups showed no obviously changes in the absorbance at 24 and 48 h, but displayed significant reduction at 72 and 96 h (P<0.01). Compared with the AFs in normal rat serum, the AFs in the 3 CTOL groups all showed significantly decreased absorbance at 24, 48, 72 and 96 h (P<0.05). At the same time point, the inhibition rate of AFs in low-dose CTOL group showed no significant difference from that in the normal control group, but CTOL at a medium dose resulted in a significantly higher inhibition rate of AFs at 72 h (P<0.05). High-dose CTOL produced significant differences in the inhibition rates of AFs and NFs (P<0.05).
CONCLUSIONCTOL can inhibit the proliferation of AFs and NFs in vitro. AFs appear to be more sensitive to CTOL, which has a dose-dependent inhibitory effect of AF proliferation.

5.Preparation of gemcitabine polybutylcyanoacrylate nanoparticles.

OBJECTIVETo optimize the preparation process of gemcitabine polybutylcyanoacrylate nanoparticles (GCTB- PBCA-NP).
METHODSAccording to the particle size, the entrapment efficiency and the loading quantity of GCTB-PBCA-NP, single factor analysis was carried out to optimize the component composition and preparation process based on an orthogonal design.
RESULTSThe mean particle size of the NP was (112-/+9) nm with an entrapment efficiency of (54.12-/+2.43)% and drug loading of (11.08-/+0.89)%.
CONCLUSIONAn optimized nanoparticular drug delivery system is obtained by emulsion polymerization.

6.Research and practice on Mini-CEX in standardized training for residents

Objective To observe whether Mini-Clinical Evaluation Exercise ( Mini-CEX ) is effective and feasible for improving the clinical competency of residents receiving standardized training. Methods 40 residents receiving standardized training in the Second Affiliated Hospital of Amy Medical university were randomly divided into control group and experimental group, with 20 trainees in each group. From October 2015 to April 2018, the experimental group was trained by the traditional teaching methods combined with Mini-CEX, and the other was trained by traditional teaching method. At least 2 Mini-CEX were conducted for each trainee of the experimental group in each department. The Mini-CEX, defense of case report and OSCE evaluation were carried out to assess the clinical competence of residents after the standardized training. Satisfaction surveys were conducted for teachers and students who had used Mini-CEX . SPSS 18 . 0 was used to analyze the data . Independent samples t test was used for inter-group comparison and paired sample t test was used for intra-group comparison. P<0.05 was statistically significant. Results Mini-CEX showed that the scores of the two groups in the clinical competence assessment were improved; the scores of the experimental group were higher than those of the control group , and the improvement in scores of experimental group was higher than that of the control group (the differences were statistically significant, P<0.05). The average score of defense of case report of the experimental group was (81.16 ±3.75),while that of the control group was (70.13 ±3.88) (t=-9.140, P=0.000). The average total score of OSCE evaluation of the experimental group was (96.300±4.681), while that of the control group was (91.775 ±3.227) (t=-3.559, P=0.001). Satisfaction rate of teachers and students were 95.0% and 92.5%respectively. Conlusion Mini-CEX can improve the clinical competence of residents receiving standardized training.
7.On angiotensin II receptor distribution after myocardial infarction in dogs.

OBJECTIVETo investigate the effects of valsartan on expression of angiotensin II receptors in different regions of heart after myocardial infarction (MI).
METHODSCanines were divided into sham-operated control group (n=7), infarction group (n=7) and Valsartan group (10 mg x kg(-1) x day(-1) for 4 weeks after MI operation, n=7). Four weeks after operation, Doppler tissue imaging (DTI) was used to evaluate regional ventricular function in the noninfarcted myocardium (apical and basal near to the infarction region). The mRNA and protein expressions of angiotensin II type 1 receptor (AT1-R) and angiotensin II type 2 receptor (AT2-R) on the corresponding regions were detected by competitive reverse-transcriptase polymerase chain reaction technique and immunohistochemical technique respectively. Results The protein and mRNA expressions of AT1-R were significantly increased in both apical and basal regions near to the infarction in dogs with MI compared with those in control group (P < 0.05) which could be downregulated by valsartan (P < 0.05). AT2-R expressions were significantly upregulated in infarction group in both apical and basal regions compared with those in control group and valsartan further increased AT2-R expressions in both areas (P < 0.05). Myocardial peak systolic velocity (Sm), myocardial peak early diastolic velocity (Em) and myocardial peak late diastolic velocity (Am) at both apical and basal regions near to the infarction regions were significantly lower in MI group than those in the control group which could be significantly improved by valsartan.
CONCLUSIONBoth mRNA and protein expressions of AT1-R and AT2-R are upregulated in noninfarcted regions near MI, valsartan improved myocardial function via inhibiting AT1-R upregulation and enhancing AT2-R upregulation.

8.Clinical application of lateral lag screw in fixing intracapsular sagittal condylar fracture.

OBJECTIVETo introduce the clinical application and curative effect of lateral lag screw technique in fixing condylar intracapsular sagittal fractures.
METHODSThirteen condyles with intracapsular sagittal fracture of 11 cases were fixed using lateral lag screw technique. Curative effects were observed by clinical and radiological follow-up for 6 approximately 30 months (mean 12 months) postoperatively.
RESULTSThe favorable results were obtained in all cases. All patients were satisfied with the clinical results. slight malocclusion existed in 2 cases. Radiological abnormalities were seen in 3 cases by CT scanning, but without any obvious function disturbances.
CONCLUSIONSLateral lag screw technique is a simple and effective treatment in fixing intracapsular sagittal condylar fractures.

9.Biomechanical stability study on different internal fixation for acetabular fracture.

OBJECTIVETo investigate the biomechanical stability of the acetabular fracture with three different internal fixation methods.
METHODSSixteen both-column acetabular fracture models were randomly divided into three groups:The specimens of 16 hip joints were randomly divided into 4 groups. Among them, 1 group of complete acetabulum were used as normal control group, and the other 3 groups simulated two column fracture models and were fixed in the following methods, respectively: anterior wall with screw and posterior with plate(SP), anterior wall with plate and posterior wall with screw(PS) and both wall with plate (PP). The degree of fracture displacement and the contact characteristics of the acetabulum were recorded by continuous vertical loading.
RESULTSThe mean longitudinal displacement under the load 800 N of SP, PS and PP three groups were (1.92±0.81), (2.09±1.13) and (3.44±0.75) mm, there was significant difference between SP and PP group (0.033). And the mean horizontal displacement of SP, PS and PP three groups were(0.63±0.33), (0.77±0.45) and (1.44±0.56) mm, there was significant difference between SP and PP group(0.047).Compared with normal control group in the acetabular area under the loading 800 N, the contact area of SP, PS and PP groups were increased by 6%, 9% and 27%, there was significant difference between PP and normal control group (0.027). Meanwhile, the mean stress of SP, PS and PP groups were increased by 4%, 29% and 39%, there was significant difference between PP and intact acetabulum group (0.003).
CONCLUSIONSAnterior column screw combined with posterior column plate has better biomechanical stability and contact characteristics than other two methods.
10.Effects of valsartan on angiotensinⅡreceptor distribution after myocardial infarction in dogs
