This paper introduced some definition of the public health approved by most of researchers and 9 basic functions and task of the public health including the monitoring and analysis of the health situation, epidemiological supervision and disease prevention and control, policy and plan for management of public health system, regulation and practice of the public health law, development of the human resources, health improvement, assurance of quality of the health services and solutions for renovation of the public health.

In this descriptive-comparative study, 480 blood donors (211 professional blood donors and 269 voluntary blood donors) were interviewed, and there were 17 deep interviewing for representatives of local authorities, women union, and health staff working in blood transfusion system in 3 provincial hospitals. The result of study is as follows: 26.1% of PBD group and 14.9% of VBD group were poor understandings about blood donation. 87.2% of PBD group giving blood for money. PBD group has been giving blood for many years. They often give their blood at many different hospitals, and the time interval between two times of blood donation of PBD group was not sufficient

Background: Flail chest is a severe condition of thoracic trauma, and it requires diagnosis and treatment as soon as possible. From year 2000, we developed an improved technique of open fixation in order to adapt the situations of Vietnam health settings. Objectives: To report preliminary results of application of improved open fixation technique in Viet Duc Hospital from 2001 to 2006. Subjects and method: This descriptive, prospective and retrospective study involved 19 patients with flail chest due to closed thoracic trauma or multiple traumas, treated by open fixation technique. The parameters included features of patients, characteristics of this technique and postoperative progression.Results: Of 19 patients, there were 16 males and 3 females, mean age: 47.8 years. All patients presented obvious signs of flail chest preoperatively, but only 9 cases were treated by open fixation technique and pleural drainage immediately. This technique canbe applied in all sugical settings with single local anaesthesia, and any kind of common surgical threads. The most suitable initial pulled weight is 2000 g. All cases of flail chest required pleural drainage. All patients had good outcomes with open fixation technique, no death. There were 3 patients with postoperative complications, all were severe multiple trauma patients. Conclusion: Improved open fixation technique applied in Viet Duc Hospital is simple, effective and safe, and can be widely used in all surgical settings.

Background: Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease caused worldwide. Objective: To research the epidemiology of COPD in the population of Bac Giang city and finding out the risk factors of COPD. Subjects and method: Cross-sectional survey of general population sample of 2,104 men and women with the age \ufffd?0 years in 30 wards of 11 communes of Bac Giang city. Data on respiratory symptoms, diseases, and risk factors were collected. Lung function tests were performed with Spiroanalyzer snoo, Japan. Results and conclusions: The incidence of COPD was 2.3%; in men 3.0% and in women 1.7%. The incidence of simple chronic bronchitis was 6.4%. People who smoked had higher rate of COPD than the others (OR = 2.8). The incidence of smoking in COPD was 55.9%. The three most important risk factors of COPD was the age \ufffd?0; smoking >15 pack/year and medical history of asthma.

Background: Fungal keratitis is a serious ocular infection that can cause corneal scarring and blindness. Currently, diagnosis of fungal pathogens remains a difficult problem. Objectives: To investigate the application of semi-nested PCR targeted ITS genes for detection of fungal agents causing keratitis. Material and method: Ten identified fungal strains, 4 bacterial strains, 20 scraping samples from patients with suspected fungal keratitis and 2 scraping samples from patients with suspected bacterial keratitis were tested using semi-nested PCR. Results: Semi-nested PCR showed positive results for the samples of identified fungal strains and for the 20 scraping samples from patients with suspected fungal keratitis. Neither samples of bacterial strains nor scraping samples from suspected bacterial keratitis patients gave positive PCR results. Conclusion: Semi-nested PCR is a robust tool for specific and rapid detection of fungal agents causing keratitis.

Background: Pediatric closed abdominal trauma is a common disease in surgical practice, in which spleen is the most vulnerable organ. As adults, diagnosis of pediatric splenic trauma is not difficult but treatment attitude of splenic trauma in children having more important differences compared with in adults, tending to conservative treatment without surgery. Objectives: To assess the results of pediatric splenic trauma treatment in Hue Central Hospital from 2001 to 2005. Subjects and method: To study on 43 pediatric patients (23 male, 20 female), mean age 11.35\xb13.51 years, were diagnosed with splenic trauma due to closed abdominal trauma, treated in Hue Central Hospital from September, 2001 to September, 2006. Results: 72.1% hospitalized patients with maximum blood pressure in normal limitation. There was not a statistic significant association between hypotension and indicated surgery. The rate of patients with splenic trauma due to traffic accidents, living accidents and sport activities were 62.8%, 32.6% and 4.7%, respective. 2 patients with combined traumatic brain injury without surgery (4.65%), 3 patients with combined hand fracture (6.97%). 3 patients in conservative treatment group changed to surgery (8.33%). A total of 76.7% patients treated with successful conservation. Conclusion: More than 60% patients with splenic trauma caused by traffic accidents and having stable hemodynamic index during hospitalization. The difference between position of splenic trauma and rupture degree of spleen both in surgical treatment group and successful conservative treatment group is statistically significant.

To investigated the protective potential of ethanol extracts of Scutellaria baicalensis (S. baicalensis ) against lipopolysaccharide (LPS)-induced liver injury. Methods: Dried roots of S. baicalensis were extracted with ethanol and concentrated to yield a dry residue. Mice were administered 200 mg/kg of the ethanol extracts orally once daily for one week. Animals were subsequently administered a single dose of LPS (5 mg/kg of body weight, intraperitoneal injection). Both protein and mRNA levels of cytokines, such as tumor necrosis factor alpha, interleukin-1β, and interleukin-6 in liver tissues were evaluated by ELISA assay and quantitative PCR. Cyclooxygenase-2, inducible nitric oxide synthase, and nuclear factor-κB protein levels in liver tissues were analyzed by western blotting. Results: Liver injury induced by LPS significantly increased necrosis factor alpha, interleukin-1β, interleukin-6, cyclooxygenase-2, inducible nitric oxide synthase, and nuclear factor-κB in liver tissues. Treatment with ethanol extracts of S. baicalensis prevented all of these observed changes associated with LPS-induced injury in liver mice. Conclusions: Our study showed that S. baicalensis is potentially protective against LPS-induced liver injury in mice.

Background: Streptococcus pneumonia (S.pneumoniae) is the main cause of acquired pneumonia in the community along with otitis media, sinusitis, septicemia and meningitis. Objectives: The study determined antimicrobial resistance and serotype distribution of Streptococcus pneumonia isolates from hospitalized children at Hai Phong Children's Hospital, Vietnam. Subjects and method: From June 2006 to September 2007, 80 pneumococccal isolates were tested for susceptibility to the 13 antibiotics and 84 pneumococcal isolates were serotyped. Results:Seventy-five percent of strains showed multi-drug resistance. Ninety percent of strains showed resistance to penicillin (48% intermediate and 42% fully resistant). In addition, 100% of isolates were resistant to cotrimoxazole, 74% of isolates were resistant to cephalexin; 71 % of isolates were resistant to erythroomycin and 58% were resistant to cefuroxxime. Almost all the isolates were susceptible to amoxicillin, cefotaxime, ceftriaxone, ceefepime, ofloxacin and 100% of isolates were susceptible to vancomycin. Among the 84 serotypes, 82% were included in the 23 valent pneumococcal polysaccharide vaccine including: 19F (30%), 23F (21 %), 14 (13%) and 6B (13%). Six other serotypes (13, 15C, 18, 11A, 15B and 6A) accounted for 12% of strains and 9 (11%) strains were untypeable. Conclusion: Pneumococcal antibiotics is spreading most rapidly among children in Vietnam, especially strains typs 19F and 23 F. Objective: The purpose of this study was to identify factors associated with twin pregnancy following day 3 double embryo transfer (DET). Methods: This retrospective cohort study incorporated data from 16,972 day 3 DET cycles. The participants were women aged between 18 and 45 years who underwent in vitro fertilization with intracytoplasmic sperm injection (IVF/ICSI) at My Duc Assisted Reproduction Technique Unit (IVFMD), My Duc Hospital, located in Ho Chi Minh City, Vietnam. Results: Of the 16,972 day 3 DET cycles investigated, 8,812 (51.9%) resulted in pregnancy. Of these, 6,108 cycles led to clinical pregnancy, with 1,543 (25.3% of clinical pregnancies) being twin pregnancies. Factors associated with twin pregnancy included age under 35 years (odds ratio [OR], 1.5; 95% confidence interval [CI], 1.32 to 1.71; p<0.001) and cycles involving the transfer of at least one grade I embryo. Relative to the transfer of two grade III embryos, the risk of twin pregnancy was significantly elevated following the transfer of two grade I embryos (OR, 1.40; 95% CI, 1.16 to 1.69; p<0.001) or a combination of one grade I and one grade II embryo (OR, 1.27; 95% CI, 1.05 to 1.55; p=0.001). Conclusion By analyzing a large number of IVF/ICSI cycles, we identified several predictors of twin pregnancy. These findings can assist medical professionals in tailoring treatment strategies for couples with infertility.
