1.Efficacy of cold magnesium sulfate saturated pad in nursing care after episiotomy

Study on 410 pregnant women had babies at Obstetric Department, Sa Dec General Hospital between March and August 2002, who underwent episiotomy or had perineal laceration, followed by swelling, edema, and painful sutures. All participants were divided into 2 groups: group of women had born babies at even dates were treated by 25% magnesium sulfate cold solution, and group of women had born babies at odd dates treated by cold placebo pad. Results: magnesium sulfate solution has endosmotic effect, leads to reduce swelling and edema. Pain relief effects were similar between 2 groups but magnesium sulfate cold solution was likely to have faster improvement in perineal edema and urination. Using cold magnesium sulfate saturated pad for nursing care after episiotomy is simple, easy method and has fast effects.

2.Synthesis of polyrotaxane-camptothecin conjugates and evaluation of its anti-tumor effect

To prepare polyrotaxane-camptothecin conjugates and evaluate its anti-tumor effect, polyrotaxane-camptothecin conjugates were successfully synthesized, and the release behavior was performed; MTT assay and cell morphology were used to examine the inhibition of cells' proliferation effect in vitro. The experimental study of the antitumor effect on S180 mice in vivo was also performed to further evaluate the anti-tumor effect of conjugate. The result showed polyrotaxane-camptothecin conjugates can effectively inhibit the proliferation in a dose dependent effect. In vivo study and cell morphology observation of S180 mice showed significant decrease in growth of tumor, degree of tumor infiltration and blood vessel number. The result indicated anti-tumor mechanism may be through affect the angiogenesis and reduced blood supply to tumor cells and then leading to necrosis.
3.Extended \ufffd?spectrum \xdf \ufffd?Lactamase in E.Coli, K.Pneumoniae and Enterobacter spp.

Background:Resistance to antibiotics due to extended \ufffd?spectrum \ufffd?Lactamase (ESBL) which increased quickly, made treatment much more difficult. However, this matter was not enough to be concerned in our country. Objectives: To investigate the prevalence of ESBL producing among clinical isolates of E.coli, K.pneumoniae and Enterobacter spp and the classification of ESBLs gene by PCR. Subjects and method: 663 strains, including 248 E.coli, 393 K.pneumoniae, 22 Enterobacter spp, isolated from patients in Viet Tiep hospital (Hai Phong), Bach Mai and Pediatric hospital (Ha Noi). ESBLs were detected using modified double \ufffd?disc method. The classification of ESBLs producing strains was implemented by PCR. Results:the percentage of ESBL producing in E.coli, K.pneumoniae and Enterobacter spp is 20.2; 18.3 and 36.4%, respectively. The ESBLs producing strains were co \ufffd?resistant to most of the tested antibiotics. These strains were prevalent in intensive care units (sputum or respiratory fluid samples). TEM, SHV, CTX \ufffd?M, OXA were 87.7; 62.3; 24.6 and 12.3%, respectively. They were detected alone or in combination in the same strain. Conclusion: The rate of ESBLs producing strains is high. ESBLs were marker for multi \ufffd?drug resistance. TEM and SHV type ESBLs are most prevalent in the tested strains.

4.Non-recoverable low radial nerve palsy and the outcomes of Smith's procedure of tendon transfer

Background: In the procedure of tendon transfer to treat low radial nerve palsy, recovering movement amplitude for toes is not difficult. It is more important to select the appropriate motive muscle. Previously, pillar muscle was commonly used as motivation for stretching fingers, but many authors opposed this because it\u2019s affecting the function of the hand. Objectives: To evaluate the clinical features of low radial nerve palsy and the outcomes of the treatment of non-recoverable low radial nerve palsy by Smith's procedure of tendon transfer. Subjects and methods: 36 patients suffering from irreversible low radial nerve palsy were treated by Smith's procedure of tendon transfer from Jan 2000 to Jan 2008. Mean follow-up was 23 months. Outcomes assessment was based on Tajima's criteria. Results: All 30 patients were satisfied with the operation and were greatly improved both their functional and aesthetic aspects. In term of biomechanics, the Flexor Carpi Radialis (FCR) is more suitable for reconstruction of the Extensor Digitorum Communis (EDC) in comparison with the flexor carpi ulnaris (FeU) or the power density spectrum. The patients showed good movements of the wrist and fingers/ the thumb moved independently with satisfactory extension and abduction/ the grip strength was rapidly improving. Conclusions: The use of the FCR for reconstruction of the EDC is a suitable choice and Smith's procedure of tendon transfer is a good procedure for the treatment of irreversible radial nerve palsy.

5.Correlation factors of cervical lymphatic metastasis in thoracic esophageal squamous cell carcinoma

ObjectiveTo investigate the correlation factors of cervical lymphatic metastasis in thoracic esophageal squamous cell carcinoma.Methods Data of 62 patients with thoracic esophageal squamous cell carcinoma were retrospectively analyzed.Factors associated with cervical lymphatic metastasis were analyzed by using x2 test and multivariate Logistic regression analysis.ResultsLymphatic metastasis was found in 36 cases (58.1% ),of which metastasis in cervical lymph node was found in 15 cases(24.2% ).A total of 1954 lymph nodes were excised,with an average of 31.5 per patient.Of all the lymph nodes,metastasis was found in 187 (9.6%) nodes.Logistic regression showed that tumor location,tumor length,thoracic lymphatic metastasis,the metastasis number of thoracic lymph nodes and thoracic lymphatic metastasis along recurrent laryngeal nerve were independent risk factors of cervical lymphatic metastasis [ OR:1.976 ( 1.436-2.538 ),P =0.035 ; 1.346(1.032-1.683),P =0.041;3.012( 1.572-6.825),P =0.012;2.023( 1.463-4.745),P =0.025 and2.254 ( 1.483-4.952 ),P =0.020,respectively ].Conclusion The independent risk factors of cervical lymphatic metastasis are foundation of selective 3-field lymph node dissection and their validities should be validated in further clinical trials.

8.Effect of Electro-acupuncture on Behavior Alterations in Mice with Isoflurane-induced Alzheimer's Disease and Its Mechanism

Objective To investigate the effect of electro-acupuncture on behavior alterations,the expression of amyloid precursor protein(APP),amyloid β-protein(Aβ) proteins and neuroapoptosis in the cerebral cortex in the APPswe/PS1dE9 double transgenic mice with Alzheimer's disease (AD) induced by isoflurane.Methods Sixmonth-old AD mice and wild-type mice at the same age were randomly divided into wile-type control group,AD group,isoflurane group,electro-acupuncture group (N =8).The mice were given pretreatment with electro-acupuncture at Baihui(GV20) acupoint and Yongquan(KI 1) acupoint once a day for 3 successive days,15 min each time.And then the mice in electro-acupuncture group and isoflurane group were exposed to a box full of 1.2% isoflurane for 4 hours.Morris water maze was used to test the learning and memory abilities of the mice,Western blotting method was used to detect the expression of APP-C83,APP-C99 and Aβ,and terminal deoxynucleotidyl transferase(TdT)-mediated dUTP nick end labeling(TUNEL) was used to detect neuroapoptosis in the cerebral cortex.Results The escape latency of AD group was longer than that of wild-type mice group(P<0.05),and the latency of isoflurane group was longer than that of AD group (P < 0.05),while the latency of electro-acupuncture group was shorter than that of isoflurane group(P < 0.05).The percentage of retention time in the target quadrant and the times for crossing the target quadrant in isoflurane group were lower than those of AD group (P < 0.05),but were higher in electro-acupuncture group than those in isoflurane group (P < 0.05).APPC83 expression level in isoflurane group was significantly lower than that in AD group (P < 0.05),while APP-C83 expression level in electro-acupuncture group was higher than that in isoflurane group (P < 0.05).APP-C99 expression level in isoflurane group was significantly higher than that in AD group (P < 0.05),and APP-C99 expression level in electro-acupuncture group was lower than that in isoflurane group (P < 0.05).The cortical apoptosis index in isoflurane group was significantly higher than that in AD group (P < 0.05),and the cortical apoptosis index in electro-acupuncture group was lower than that in isoflurane group (P < 0.05).The expression level of Aβ in AD group,isoflurane group and electro-acupuncture group was significantly higher than wild-type control group (P < 0.05).Conclusion Electro-acupuncture can relieve the AD-like neurotoxicity induced by isoflurane and inhibit the decline of learning and memory abilities of AD mice,and the mechanism is probably related with suppressing the overexpression of APP-C99 and reducing the production and accumulation of Aβ,thereby alleviating the neuroapoptosis.
9.Optimization of Processing Technology for Ginger Pinellia by Orthogonal Tests

Objective: To research the best processing method for ginger pinellia by orthogonal tests.Methods: The orthogonal tests included the soaking time, boiling water and cooking time as the influencing factors, an HPLC method was used for the determination of 4 nucleosides (uridine, guanosine, adenosine, inosine), and the alum limit and extract content were also studied.The results were evaluated by multi index comprehensive weighted score to optimize the processing technology of ginger pinellia.Results: The best processing technology of ginger pinellia was as follows: soaked for 60 hours, the proportion of boiling water and pinellia tuber was 15:1, and boiled for about 5 h.Conclusion: The optimum processing technology of ginger pinellia is reasonable, reliable and reproducible, which can be used as the reference for the processing standardization of Chinese crude drugs.
10.Ethical View on the Impact of Hand Hygiene Culture Construction on Hand Hygiene Compliance

Objective:To evaluate the effect of a hand hygiene culture program on hand hygiene compliance in a three grade A hospital .Methods:Direct observation methods were used to assess the hand hygiene compliance and hand hygiene quality .Results:After the hand hygiene culture program , the hand hygiene compliance rate and the hand hygiene accuracy rate of doctors raised from 30 .2%and 66 .7%to 65 .3%and 85 .8%.Those of the nur-ses raised from 52.2%and 80.0%to 87.6%and 93.3%.Conclusion:The hand hygiene program increased the hand hygiene compliance and reducing thd risk of hospital infection occurred .
