2.Study on the Effects of Total Cost Prospective Payment System on the Consumption of Drugs

OBJECTIVE:To investigate the consumption of drugs after the inplementation of total cost prospective payment sys-tem(PPS),and to provide reference for hospital drug cost control. METHODS:The data of prescriptions collected from 9 hospital during 2007-2014 were divided into group A,B and C according to PPS,and then summarized statistically in respects of increase rate of total consumption sum,consumption sum ratio of major category;the drug cost per time of outpatient department,emergen-cy department and inpatient department were calculated as well as consumption sum ratio of self-paid drugs;the dosage per time of drugs in blood pressure and diabetes prescription were also calculated. RESULTS:The total consumption sum of drug increased slowly after the implementation of total cost PPS;the consumption sum ratio of major category kept stable,while that of anti-infec-tive agent decreased;the emergency drug cost per time achieved a negative growth,and outpatient and inpatient drug cost per time increased slightly. The proportion of self-paid drugs was relatively stable. The dosage per time of drugs in hypertension and diabetes prescriptions was stable,too. CONCLUSIONS:Total cost PPS is useful in controlling the rapid growth of drug costs,and promote the reasonable drug use. The consumption sum of self-paid drugs are well controlled. The increase of drug cost per time in outpa-tient and inpatient should arouse the attention of the relevant departments. In addition,it has no effect on drug dosage for the pa-tients with hypertension and diabetes.

6.The Distribution of Body Weight Related Index and Relationship with Cardiovascular Diseases in Rural Residents of Yunnan Province

Objective The aim of this study was to investigate the distribution of body weight related index and to explore the relationship between weight related index and cardiovascular diseases in rural residents in Yunnan Province. Methods By random cluster sampling method,rural residents were selected in Yunnan Province. The data were collected by face-to-face questionnaire and medical examination. Results 4070 rural residents in Yunnan participated in this study. The survey results showed that gender,age,education level and ethnicity affected both the distribution of body weight related index and low body weight,overweight,obesity,and central obesity(P < 0.05). Compared with low body weight,non overweight,non obesity and non central obesity group,non low body weight,overweight,obesity and central obesity group had higher prevalence rate of hypertension(P < 0.01). Among central obesity group,the prevalence rate of coronary heart disease was higher than non central obesity group(P < 0.01). Conclusion The distribution of body weight related index in rural residents of Yunnan province showed some special characteristic. The overweight,obesity,and central obesity were related to cardiovascular diseases. Change unhealthy living habits or behavior as well as improving their cultural quality have effects on controlling body weight related index and help to reduce the prevalence rates of overweight,obesity and central obesity which can further reduce the prevalence rates of cardiovascular diseases.
7.Treatment of 60 Cases of Senile Herpes Zoster by Encircled Acupuncture plus Valaciclovir

To observe the clinical therapeutic effect and safety of local encircled acupuncture plus valaciclovir in treating senile herpes zoster.Methods:Sixty senile patients with herpes zoster were divided into two groups.In acupuncture and medicine group,the patients were treated by encircled acupuncture plus valaciclovir.The needle was inserted about 0.8 cun away from herpes and to form an angle of 15°with the skin around the skin lesion.During treatment,valaciclovir was taken orally 300 mg every time,twice every day for successive 10 days.In westem medicine group,valaciclovir was taken orally 300 mg every time,twice every day for successive 10 days.Results:The time of stopping herpes,relieving pain andscabbing in acupuncture and medicine group was significantly lower than that in western group.Conclusion:Local encircled acupuncture plus valaciclovir in treating senile herpes zoster got effects quickly and could effectively shorten the course of disease and reduce the incidence rateofresidual neuralgia.
8.Correlation between coronary artery disease and levels of glycosylated hemoglobin and endothelin

Objective To analysis the correlation between coronary atherosclerosis and levels of glycated hemoglobin (HbA1c) and endothelin(ET ) ,furthermore to explore the clinical significance of the two indexes in the department of cardiology .Methods 572 inpatients were divided into four groups according to the blood glucose level and coronary angiography results :negative control group with 40 patients ,simplex DM group with 50 patients ,simplex CHD group with 262 patients ,CHD+DM group with 220 pa-tients .The HbA1c and ET levels in the four groups were monitored ,and the correlations between the two indexes and coronary ath-erosclerosis were analysed by Logistic regression analyses .Results HbA1c level :control group and CHD group < DM group
9.Effect of Tanreqing injection on pulmonary function, blood gas index and clinical symptoms in chronic obstructive pulmonary disease

Objective To investigate the effect of Tanreqing injection on pulmonary function, blood gas index and clinical symptoms in patients with chronic obstructive pulmonary disease.Methods 90 cases of chronic obstructive pulmonary disease patients from January 2010 to December 2013 in Xinjiang Tuha Petroleum Hospital were selected as the research object.They were randomly divided into the observation group and the control group, 45 cases in each group.The control group received routine western medicine, on the basis of this, the patients in the observation group were treated with Tanreqing injection.Before treatment and 15 days after treatment, compared the two groups of patients with pulmonary function , blood gas index and the improvement of clinical symptoms.Results Before treatment, there were no significant differences between the two groups of patients with FEV1 , FVC, FEV1/FVC and other pulmonary function index, PaO2 , PaCO2 , SaO2 and other blood gas index and the scores of clinical symptoms.15 days after treatment, the two groups of patients with FEV1 , FVC, FEV1/FVC and other indicators of pulmonary function significantly increased, while its PaO2 , SaO2 levels also increased significantly, the level of PaCO2 in the two groups was decreased, and the clinical symptom scores were significantly decreased.However, the improvement rates of the observation group were significantly higher than that of the control group, the differences between the groups were statistically significant (P<0.05).Conclusion On the basis of conventional western medicine treatment, add Tanreqing injection to treat chronic obstructive pulmonary disease, can significantly improve the pulmonary function and blood gas index level, the clinical symptoms have been effectively relieved.
10.Discussion of vancomycin dosage in Chinese adult patients

Objective To study whether vancomycin dosage 2 g daily in Chinese adult patients would achieve the target serum trough concentration of 10-20 mg/L.Methods The data of 122 adult inpatients under therapeutic drug monitoring while receiving vancomycin were collected from September 2011 to December 2012.The correlation between vancomycin trough concentration and dosage,age,sex,body weight,concomitant medications were analyzed retrospectively.Results Vancomycin trough concentration was below the target concentration in 62.1% of the included patients.Age is an independent risk factor for attainting target concentration.Serum concentration was below target concentration in 70.0% of the patients below 60 years of age.Serum concentration was above target concentration in 62.5% of the patients above 60 years of age.Conclusions Clinicians and pharmacists should assess the dosage regimen of vancomycin cautiously,and age should be considered especially as an independent risk factor.
