4.A Case of Sarcomatoid Carcinoma of the Larynx Mimicking Vocal Polyp.

Sarcomatoid carcinomas are biphasic tumors which have surface epithelial changes and an underlying spindle-shaped neoplastic proliferation. A 62-year-old male with hoarseness came to our hospital for evaluation. A single smooth polypoid lesion was detected on his right true vocal fold by larnygoscope. The patient was diagnosed with vocal polyp and treated with surgical excision. Pathology report of the excised specimen was compatible with sarcomatoid carcinoma. Therefore, further surgical resection was performed to secure safety margins. We report a case of a sarcomatoid carcinoma patient who was clinically diagnosed as vocal polyp.

6.The Significance of CD44v3 Expression in Squamous Cell Lesions of the Larynx.

BACKGROUND AND OBJECTIVES: It is known that a part of laryngeal premalignant lesions progresses to an invasive carcinoma. Despite many previous reports, conventional histology is not sufficient to predict such tumor progression. Herein, the authors investigated the role of CD44v3 as a biomarker in predicting the progression of laryngeal premalignant lesion to an inavasive cancer. SUBJECTS AND METHOD: Paraffin-embedded tissue specimens from 40 patients were diagnosed accordingly as laryngeal invasive squamous cell carcinoma (n=10), Carcinoma in situ (n=10), dysplasia (n=10), and hyperkeratosis (n=10) between 1993 and 2002. They were immunohistochemically stained for CD44v3 protein. RESULTS: In invasive squamous cell carcinoma, the expression of CD44v3 was diffused and gave a strong positive stain, and in carcinoma in situ, it was diffused and gave 3+-2+ stain. However, in dysplasia and hyperkeratosis, the proportion of CD44v3 expression was decreased by 2+-1+, and 1+-0, respectively. CONCLUSION: These results suggest that the expression of CD44v3 in laryngeal premalignant and malignant lesions can be associated with tumorigenesis and invasion. Those strong positive expressions of CD44v3 may represent more aggressive pathology of the larynx.

7.Pathologic Change of Perichondrium on Recurrent Otohematoma.

Otohematoma is a fairly common disease in otolaryngologic clinics. Many physicans have been challanged with various method of management on auricular hematoma. The initial treatment usually involves simple aspiration and a compression bandage. But most of these treatments are recurrent problems and eventually become the cauliflower ear. A treatment involves a total excision of the newly formed fibroneocartilaginous layer. If hematoma persists, perichondrium becomes irregular by thickned fibroneocartilage formation. We treated 11 patients with otoplasty to remove fibroneocartilage under local anesthesia. We found that the perichondrium formed severe fibroneocarilage and organization after 2 weeks. We studied the sequence of pathological changes that occurs after injury.

8.Laryngeal polypus at the ventricular band symptomized firstly by hemoptysis: a case report.

Published reports about polyps of larynx at the ventricular bands are extremely rare. The first symptom of this case presents recurrent hemoptysis. The video laryngoscope demonstrated that a smooth-faced and hyperemic polypoid lesion was found in the anterior part of the right false vocal cord, close to laryngeal surface of epiglottis. Postoperative pathological report revealed that the mass was a vascular polyp.

9.Poorly differentiated neuroendocrine carcinoma of the tongue base: a case report.

Neuroendocrine carcinoma(NEC) is a malignant tumor derived from neuroendocrine cells, with distinct clinical, morphological and immunohistochemical characteristics. Neuroendocrine carcinoma of the head and neck is very rare in clinic. Larynx is the most common affected site, and the root of the tongue is extremely rare. The clinical manifestations are mainly eating pain, cauliflower like mass in the mouth, and ulcerative lesions that have not healed for a long time. Maxillofacial MRI and contrastenhanced CT are the most commonly used examination tools for such diseases, which can detect the spaceoccupying lesions of tumors. Neuroendocrine granules found in the cytoplasm under pathological light microscope can be diagnosed as neuroendocrine carcinoma. However, for most cases, it is difficult to make a diagnosis only under light microscope, and it is often necessary to make a diagnosis by means of immunohistochemistry and other technical means. This paper reports a case of neuroendocrine carcinoma of the root of the tongue, introduces its characteristics, diagnosis and treatment, and reviews the relevant literature of this case.

10.Primary Malignant Laryngeal Melanoma: Report of a Case with Review of Literature.

A case of primary malignant melanoma of the larynx is reported. The patient, a 41-year-old Korean man, presented with dyspnea and blood tinged sputum. Physical examination showed blackish necrotic mass involving right side of the larynx. The mass proved to be malignant melanoma containing many melanin pigments. A description of the lesion and a review of literature are given.
