1.Study on the influence of lansoprazole combined with mosapride for the gastric and esophageal dynamics and gastric electrical activity indexes of patients with gastroesophageal reflux

Objective To study the influence of lansoprazole combined with mosapride for the gastric and e-sophageal dynamics and gastric electrical activity indexes of patients with gastroesophageal reflux,then to evaluate its clinical effects.Methods 108 patients with gastroesophageal reflux were randomly divided into the control group (lansoprazole)and observation group(lansoprazole combined with mosapride),54 cases in each group.The gastric and esophageal dynamics and gastric electrical activity indexes of the two groups before the treatment and at 3nd,6th and 12 th week after the treatment were compared.Results The gastric and esophageal dynamics and gastric electri-cal activity indexes of observation group at 3nd,6th and 12th week after the treatment were all obviously better than those of control group,those indexes of observation group increased progressively after treatment (6 weeks after treatment, tcontract =3.12,tmotion =2.97,tfrequency =2.36;tpressure =3.53,trelaxation rate =6.24,tshrinkage ratio =12.17;tfrequency preprandial =2.57,tfrequency postprandial =3.48,tamplitude preprandial =9.01,tamplitude postprandial =17.74;12 weeks after treatment,t contract =4.97,tmotion =4.53,tfrequency =2.99;tpressure =2.95,trelaxation rate =7.92,tshrinkage ratio =11.34;tfrequency preprandial =2.90, tfrequency postprandial =4.47,tamplitude preprandial =8.55,tamplitude postprandial =11.23,P <0.05 ).Conclusion The influence of lansoprazole combined with mosapride to the gastric and esophageal dynamics and gastric electrical activity indexes of patients with gastroesophageal reflux is great,so its treatment effect for the disease is better.
2.Relationship between polymorphisms of interleukin-4 receptor gene and childhood asthma in Harbin

Objective To investigate the relationship between polymorphisms of interleukin-4 receptor (IL-4R) gene (E375A, C406R, Q576R) and childhood asthma in Harbin. Methods Ninety-one (91) children with asthma were included, and 42 healthy children were enrolled as control. The genotype polymorphisms of IL-4R were determined by polymerase chain reaction-restriction fragment length polymorphism analysis. Results There was no significant difference in the distributions of the genotypes at both 1 124 A/C polymorphic sites (i.e. E375A polymorphism) and 1 902 A/G polymorphic site (i.e. Q576R polymorphism) in IL-4R between asthmatic group and control group (P > 0.05). There was significant difference in the distributions of the genotypes (CC, CR, RR) on 1 216 T/C polymorphic site (i.e. C406R polymorphism) in IL-4R between the two groups (P < 0.05). The R allele frequency was 22.94% in asthmatic group and 6.58% in control group, with significant difference between the two groups (P < 0.05), but the allele frequencies at this site did not satisfy Hardy-Weinberg equilibrium (P < 0.05). Conclusions Our data suggest that the E375A and Q576R polymorphisms in IL-4R is not associated with the development of asthma in children in Harbin. Further study is needed on the relationship between C406R polymorphism and asthma.
3.Effect of high fat and low sugar enteral nutrition support on immune function and ventilatory function of patients with chronic obstructive pulmonary disease undergoing mechanical ventilation

Objective:To explore the effect of high fat and low sugar enteral nutrition support on ventilatory function and immune function in patients with chronic obstructive pulmonary disease(COPD) undergoing mechanical ventilation.Methods:From January 2018 to December 2019, 96 COPD patients treated in Zhuji People's Hospital were divided into observation group and control group by simple random sampling method, with 48 cases in each group.The control group was treated with traditional nutrient solution, and the observation group was given high fat and low sugar enteral nutrition.The blood gas analysis, respiratory function, immune function, weaning time and weaning success rate were observed before and after treatment.Results:After treatment, the arterial oxygen partial pressure(PaO 2)[(10.08±0.49)kPa], the blood oxygen saturation (SpO 2)[(96.46±2.15)%] in the observation group were higher than those in the control group[(9.53±0.46)kPa, (95.04±2.08)%]( t=5.670, 3.289, all P<0.05), and the carbon dioxide partial pressure (PaCO 2) [(6.42±0.27)kPa] was lower than that in the control group [(7.33±0.25)kPa]( t=17.134, P<0.05). After treatment, the forced vital capacity(FVC) [(3.03±0.52)L], forced expiratory volume in one second(FEV 1)[(1.61±0.33)L], peak expiratory flow(PEF)(0.54±0.08) in the observation group were higher than those in the control group [(2.78±0.45)L, (1.42±0.31)L, (0.49±0.07)]( t=2.519, 2.907, 3.259, all P<0.05). The immunoglobulin G [(12.45±1.32)g/L], immunoglobulin M [(1.88±0.34)g/L] and immunoglobulin A [(2.68±0.47)g/L] in the observation group were higher than those in the control group [(11.66±1.28)g/L, (1.65±0.37)g/L, (2.38±0.45)g/L] ( t=2.977, 3.171, 3.194, all P<0.05). The weaning time of the observation group was (6.94±1.55)d, which was lower than that of the control group [(8.73±1.41)d] ( t=5.919, P<0.05). The success rate of weaning of the observation group (91.67%) was higher than that of the control group (72.92%) (χ 2=5.790, P<0.05). Conclusion:High fat and low sugar enteral nutrition support can improve the ventilation indicators and immune function indicators of COPD patients with mechanical ventilation, shorten the time of mechanical ventilation, and improve the success rate of weaning.
