1.The allergen analysis and nursing of abnormal rhinitis

Objective To find out the common allergen of abnormal rhinitis in Shenzhen.Methods To take in process of allergopharma skin prick test(ASPT) for 855 dubitable patients with abnormal rhinitis to determine the main allergen.Results 578 patients in the 855 dubitable patients of abnormal rhinitis were positive(67.6%).These allergens were mostly inhalant.Among the 578 patients,51 cases were caused by single allergen;527 cases caused by more than two kinds of allergens;520 cases(90%) were positive in dermatophagoides pteronyssinus;488(84.4%) cases were positive in dermatophagoides farinae.Conclusion According to the common allergen of abnormal rhinitis in Shenzhen,and education to patients underwent ASPT can improve the accuracy of the test,lighten the symptom of abnormal rhinitis and raise the cure rate.
2.Clinical research in application of alginate dressing in fat liquefaction of abdominal incision

Objective To assess the efficacy of alginate dressing in fat liquefaction of abdominal incision. Methods 76 patients with fat liquefaction of abdominal incision in our hospital were divided into the experimental group (42 patients) and the control group(34 cases). Both groups were given preventive antibiotics. The penetration, times of dressing change, interval of dressing change, healing cost, wound infection, period Ⅱ wound healing, time for the medicine to start working, time of healing were observed in the two groups. All results underwent t test and χ2 test. Results No distinct difference of healing cost between the two groups was observed. The experimental group treated with alginate dressing was better than the control group in other clinical indexes after treatment. Conclusions Application of alginate dressing is beneficial to management of fat liquefaction of abdominal incision and reduce time of healing and hospital stay.
3.Influence of nursing interventin on compliance of patients with allergic rhinifis undergoing immunotherapy

Objective To study the influence of nursing interventin on compliance of patients with allergic rhinitis undergoing immunotherapy.Methods Patients with allergic rhinitis undergoing immunotherapy from October of 2003 to October of 2005 and from Novermber 2005 to August 2007 were named the control group and test group respectively.Only the test group received nursing intervention.The compliance of the patients was investigated with subsequent cause analysis.The compliance de-gree and its influencing factors in the two groups were compared.Results The complete complianceof the test group was 80.5%,which was statistically different from that of the control group(68.9%,P<0.01).The difference between adult patients of the two groups was also evidently different(P<0.05).But the difference between children patients of the two groups was not evidently different(P>0.05).Statistical difference existed between children and adult patients in the control group as well as in the testgroup(P<0.01).The uncontrollable factors in the test group reduced while the controllable factors increased.Conclusion The nursing intervention could improved patients compliance with immunotherapy in that it reduced the uncontrollable factors among the reasons causing uncompliance.It worth widely being applied in clinic.
4.Influence of nursing intervention on perioperative psychological status and treatment compliance of patients udergoing breast augmentation

Objective To investigate the influence of nursing intervention on perioperative psychological status and treatment compliance of patients undergoing breast augmentation. Methods 82 patients were investigated with self-rating anxiety scale (SAS), self-rating depression scale (SDS), self-designed mental activity questionnaire on patients with breast augmentation. And they were divided into the intervention group(42 patients) and the control group(40 patients) randomly. The control group was given routine treatment and nursing. The intervention group received systematic nursing intervention such as psychological needs,confidentiality, the relevant specialist knowledge and explanation and prevention o complications besides routine treatment and nursing care. The perioperative psychological status and treatment efficacy were compared between the two groups. Results The worrying degree about operation - related problems was lower in the intervention group compared with that of the control group, but SAS and SDS score showed no significant difference one day before operation between the two groups. SAS and SDS score of the intervention group were improved before discharge compared with those before operation and were also better than those of the control group. Conclusions Patients undergoing breast augmentation surgery subject to a variety of perioperative psychological barriers, systematic nursing intervention can significantly improve the psychological state and treatment compliance, make them to acquire the unification of perfect body shape and psychology with best physical and mental status during perioperative stage.
5.Effect of emblic leafflower fruit on total antioxidation and levels of malondialdehyde as well as endothelin in plasma in rabbits with atherosclerosis

BACKGROUND: Emblic leafflower fruit contains rich vitamin C, organic acid and mineral substance. It is indicated in modern research that emblic leaafflower fruit acts on preventing from cancer and anti-aging too. In recent years, more and more concerns have been paid to the researches on functional factor of emblic leafflower fruit and the development of the relevant functional food. It has been verified that emblic leafflower fruit acts on reducing lipid and anti-oxidation in vitro, but there are still lacks of enough basic researches to support it.OBJECTIVE: To probe into mechanism of emblid leafflower fruit on anti-oxidation and protection of vascular endothelial function in rabbits with hyperlipemia.DESIGN: Randomized controlled experimental study based on the experimental animals.SETTING: Atherosclerosis(As) experimental room in a university hospital.MATERIALS: Totally 24 normal NewZealand male rabbits, mass weighted (2.2±0.5) kg.METHODS: The experiment was performed in As Experimental Room of Beijing Anzhen Hospital from September 2001 to May 2002. Totally 24 New Zealand male rabbits were randomized into the control, the power of emblid leafflower fruit group(powder group)(4 g/kg day) and hypercholesterolemia model group (model group), 8 rabbits in each group. In both power and model groups, the rabbits were fed with hypercholesterol food. Oxidase method was applied to assay the content of serum lipid, chemical method to assay the plasma total anti-oxidation and the content of malondialdehyde (MDA), radio-immune method to assay the content of plasma endothelin-1 (ET-1), digoxin labeled probe and tissue hybridism in situ to assay the expression of endothelin mRNA of aortic tunica intima, and image analyzer to assay the area of aortic intimal atherosclerosis plaque and the ratio between the area of tunica intima and the area of tunica media.MAIN OUTCOME MEASURES: The primary outcomes included blood lipid level, serum MDA concentration and the comparison of artery plaque areas. The secondary outcomes included the changes in plasma ET-1.RESULTS: By the comparison between the powder group and model group in triglyceride(TG) and low density lipoprotein cholesterol(LDL-C) were significantly reduced [(11.70 ± 1.73), (14.32 ±2.22) mmol/L, P<0.05; (0.740 ± 0.107), (1.450 ± 0.220) mmol/L, P <0.01 successively]and thehigh density lipoprotein cholesterol(HDL-C) was increased MDA was significantly decreased [ (3.88 ± 0.51 ), (6. 29 ± 1.43 ) mmol / L,P < 0.01 ] and the total anti-oxidation significantly increased [ (10. 771der photo-microscope that the plaque area and the ratio between intima area and media area were remarkably decreased [(39.46±6.53), (50.69± 12.36)mmol/L, P < 0.05; (0. 62 ±0. 32), (1.38 ±0.38) mmol/L, Pgranules of aortic intima ET-1 gene were rem~kably decreased compared with the model group.CONCLUSION: Emblic leafflower fruit prevents from the formation of experimental atheromatous plaque in rabbits probably by regulating rabbit lipid metabolism, improving anti-oxidation to reduce lipid peroxidation and protecting endothelial function to inhibit the expression of artery intima ET-1 gene.
6.Establishment and evaluation of a multi-system chain transitional care programme for postpartum pelvic floor training

Objective To build a multi-system chain transitional care programme for postpartum pelvic floor training and test its effect.Methods By a prospective randomized controlled trial,the study group (n=60) received a multi-system chain transitional care programme which consisted of ward nurse,home visiting doctor,post-natal care clinic doctor,pelvic floor doctor and pelvic floor nurse.The control group(n=60) received routine care.The pelvic floor muscle strength,sexual function and patient satisfaction of baseline,on the 42th days,3rd months and 6th months were compared between two groups by short-term follow-up.Results There was no difference in baseline consisting of age,gestational weeks,birth weight and so on between groups.Participants in the study group had significantly better improvement and score in pelvic floor muscle strength,sexual thoughts,sexual arousal,sexual psychopathy and problems affecting sexual function.There was no difference in score of sexual pleasure and ejaculation time between two groups.Conclusions This study was an original effort to establish and evaluate a multi-system chain transitional care program for postpartum pelvic floor training.Results demonstrated that this transitional care was effective in improving pelvic floor function.
7.The efficacy and safety of donor lymphocyte infusion to treat Epstein-Barr virus associated lymphoproliferative diseases after allogeneic hematopoietic stem cell transplantation

Objective To analyse our series patients' data to assess its efficacy and safety of donor lymphocyte infusion (DLI) for Epstein-Barr virus (EBV) associated post-transplant lymphoproliferative disorders (PTLD) after allogeneic hematopoietic stem cell transplantation (HSCT). Methods Patients received HSCT from November 2006 to November 2009 and diagnosed as EBV associated PTLD by pathological or clinical methods were enrolled in this study. Lymphocyte was prepared by COBE collector.Related haplo-donors were the alternative if the original donors was unavailable. A range of mononuclear cell (MNC) dose of (0.5-1.0) × 108/kg was designed and the expected number of T lymphocyte included was at level of 107/kg. Cyclosporine (CsA) trough concentration was kept in a therapeutic level. Results Nine patients with PTLD received DLI 13 times, the median number of PBMC infused was 0.8 (0.16-1.03) ×108/kg, CD3+T cell number was 4.2 (1.6-5.7) × 107/kg. Seven patients received peripheral blood mononuclear cells (PBMC) from original haplo-identical donors, with 7 response and 6 complete remission.Defervescence occurred after 2 ( 1-5 ) d, and adenopathy began to recover in 6 ( 1-14 ) d after the initial infusion of leukocytes. Graft versus host diseases (GVHD) occurred in 6 recipients out of 7 evaluable patients, and all were controlled successfully. Three patients survived for 38, 23 and 3 months after PTLD.Conclusion In this small series cases, infusion of controlled dose of lymphocyte from primary donor is an effective and safe therapy for EBV associated PTLD after mismatched/haploidentical HSCT while the optimal regimen needs to be further studied.

9.Assessment of polymerase chain reaction and serology for detection of chlamydia pneumoniae in patients with acute respiratory tract infection.

OBJECTIVETo study Chlamydia pneumoniae (C. pneumoniae) infection in 110 patients with respiratory tract infection admitted to our hospital from January to December 1995 in Nanjing.
METHODSSputum and throat swab specimens were taken and C. pneumoniae DNA was detected by using polymerase chain reaction (PCR) with the HM-1-HR-1 primer pair. At the same time, serum samples were taken and immunoglobulin G and M (IgG and IgM) fractions of antibodies to C. pneumoniae were studied by microimmunofluorescence test.
RESULTSPrevalence of specific IgG was 70% in patients with respiratory tract infection. Seventeen patients (15.5%) were serologically diagnosed as having recent C. pneumoniae infections and 12 patients (10.9%) had positive PCR in sputum and/or swab specimens. The total positive rate was 22.7% (25/110) detected by PCR combined with serological tests. Acute infection of C. pneumoniae was common in patients with asthma (57.1%), pneumonia (35.0%), COPD (25.9%) and bronchitis (25.0%). Clinical features between C. pneumoniae infection and non-C. pneumonia infection showed no significant differences.
CONCLUSIONSChlamydia pneumoniae is an important pathogen that causes infection of the human respiratory tract and attention should be drawn to this special illness.

10.Establishment and application of intelligent document management system for nursing system

Objective:To establish and apply an intelligent nursing system document management system to provide bedside guidance for clinical nurses and standardize clinical nursing services.Methods:The intelligent nursing system document management system was established and successfully applied in clinical practice. Its functions included upload management, intelligent retrieval, cross-specialty sharing, multi-terminal access, reading trace management, individualized document revision reminder and file validity management. The problem of time-consuming file management, file retrieval time, and problem occurrence rate in file management when collecting manual management paper system files and intelligent system management electronic system files was evaluated, and the application effect of the intelligent nursing system file management system was evaluated.Results:The average time taken by the file administrator to collect and recycle documents was (492.68±14.04) min and (195.43±8.12) min. After the system was established, the files could be updated and discarded directly on the computer, which saved the time of collection and collection. The time consumption of the document decreased from (82.72±7.47) s to (44.75±5.28) s, the difference was statistically significant ( t value was 34.242, P<0.01); the average incidence of dysfunction in the ward was 2.78%, and the average rate of inconsistent version was 5.56%. After that, the files were managed uniformly and synchronized, eliminating the lack of files and inconsistent versions; there was no need to print paper documents, which significantly reduced the cost of consumables. Conclusion:Through the establishment and application of the intelligent nursing system document management system, the document management process was rationally optimized, the accuracy and convenience of document use were effectively improved, the manpower and consumables cost of document management were reduced, and the efficiency and quality of document management were improved.
