2.Influence of with intrahepatic cholestasis on pregnancy outcome

Objective To investigate the pregnancy outcome of gestational intrahepatic cholestasis.Methods 70 pregnant women with intrahepatic cholestasis were selected as the study group.70 healthy pregnant women during the same period were selected as the control group.The pregnancy outcomes of two groups were compaind.Results (1) The maternal serum levels of bile acid,alanine aminotransferase,aspartate aminotransferase in study group were (65.61 ± 13.5) μmol/L,(134.31 ± 24.7) U/L,(97.35 ± 21.54) U/L,which were higher than those of the control group (3.34 ± 0.41) μ mol/L,(36.16 ± 4.15) U/L,(23.34 ± 4.45) U/L,and the differences were statistically significant(P <0.01).(2) In study group,the incidence rate of maternal gestational hypertension was 21.42%,premature rupture of membranes was 17.14%,incidence rate of postpartum hemorrhage was 15.71%,which were significantly higher than 7.14%,5.71%,4.29% in the control group,the differences were statistically significant(P < 0.05).(3) In study group,the incidence rate of neonatal asphyxia was 27.14%,therate of amniotic fluid contamination was 35.71%,fetal distress was 22.86% and 30.00% of the childrenwith low birth weight,whichwere higher than those of the control group,the differences were statistically significant(P <0.01).Conclusion Pre gnancy intrahepatic cholestasis can increase the incidence rate of perinatal complications,has serious impact on the prognosis of the fetus,and to strengthen the monitoring of pregnancy intrahepatic cholestasis has important clinical significance.
3.Effect of Radix Isatidis polysaccharide on nitric oxide and endothelin-1 after ortho-topic liver autotransplantation in rats

Objective To explore the effect of Radix Isatidis( indigowoad root,RI) polysaccharide on NO and ET-1 in orthopotic liver autotransplantation of rats.Methods The rats used in the experiment were randomly divided into three groups:normal control group (NC), autotransplantation group (AT) and RI polysaccharide group (RIPS).At 1, 6, 12 and 24 hours after autotransplantation, the content of NO,ET-1, alanine aminotransferase ( ALT), and aspartate amin-otransferase ( AST) in serum was assayed.Morphological observation of hepatic tissues was also performed.Results The serum level of ET-1 in RIPS group was significantly lower than that of AT group (P<0.05), and NO content in serum of RIPS group was increased significantly compared with AT group ( P<0.05) at 1, 6, 12 and 24 hours after autotransplanta-tion.The serum level of ALT and AST in RIPS group was lower than that of AT group (P<0.05) at each measurement point.The morphology of hepatocytes changed abnormally under light microscopy.The blood stasis and swelling of hepatic tissues, denaturalization and necrosis of hepatocytes in RIPS group were milder than that in AT group.Conclusion RIPS has a protective effect on hepatic ischemia reperfusion injury after orthotopic liver autotransplantation, which may be related to increased NO levels and lower levels of ET-1.
4.Effect on chronic urticaria and serum IL-4 and IgE in the patients treated with moving cupping therapy and autohemotherapy with acupoint inection.

OBJECTIVETo explore the clinical efficacy and effect mechanism on chronic urticaria treated with moving cupping therapy combined with autohemotherapy with acupaint injection for desensitization at acupoints.
METHODSOne hundred and four patients of chronic urticaria were randomized into a comprehensive therapy group and a medication group, 52 cases in each one. In the comprehensive therapy group, the moving cupping therapy along the governor vessel and bladder meridian of foot-taiyang was applied combined with autohemotherapy with acupaint injection for desensitization at acupoints. The self-venous blood was injected at bilateral Quchi (LI 11) and Zusanli (ST 36), 1 mL at each acupoint, once every 3 days. In the medication group, cetirizine tablets, 10 mg were prescribed for oral administration, once every day, and the compound dexamethasone acetate cream was used externally, once to twice a day. The clinical efficacy: was observed in 30 days of treatment in the two groups. The changes of serum interleukin 4 (IL-4) and immunoglobulin E (Ig E) before and after treatment were observed. The recurrence rate was compared between the two groups in 3 months after treatment.
RESULTSThe cured and markedly effective rate was 90.4% (47/52) in the comprehensive therapy group, which was higher obviously than 78.8% (41/52, P < 0.05) in the medication group. The levels of serum IL-4 and IgE were reduced in the patients of the two groups, indicating the significant difference in comparison before and after treatment (all P < 0.01) and the reducing degree in the comprehensive therapy group was much more significant than that in the medicine group (both P < 0.01). The recurrence rate was 19. 1% (9/47) in the comprehensive therapy group in the 3-month follow-up after treatment, and apparently lower than 51.2% (21/41, P < 0.01).
CONCLUSIONThe moving cupping therapy combined with autohemotherapy with acupaint injection for desensitization at acupoints achieves the better efficacy on chronic urticaria compared with the routine western medicine treatment and the recurrence rate is low. The effect mechanism is possibly related to the down-regulation of serum IL-4 and IgE in the patients.

5.Clonal analysis of endometriotic lesions using human androgen receptor gene

Objective To investigate characteristics of endometriotic lesions clone with various pathological subtypes by analyzing the polymorphism of X chromosome linked human androgen receptor allele (HUMARA). Methods Eight frozen tissues of endometriotic leisons were collected from 6 patients who received laparotomy or laparoscopy surgery in Peking Union Medical College Hospital from Nov. 2008 to Jun. 2009. Fifty specimens of epithelial cells from single endometrial glands were isolated and collected from endometriotic lesions by using laser capture microdissection. HUMARA was applied as the gene marker of clonal analysis. Nested palymerase chain reaction, double-enzyme digestion reaction with two methylationsensitive restriction endonuclease (Hha Ⅰ and Hpa Ⅱ), and the automated gene sequencing technique were utilized in this study to evaluate the characteristics of endometriotic lesions clone. Results Of 50 specimens of isolated glands, 34 were informative for clonal analysis, and all of which showed monoclonality. Of 3 ovarian endometriotic tissues, one tissue of HUMARA showed unuseful information, the other 2 ovarian endometriotic tissues respectively had 4 and 7 informative specimens of gland epithelial cells, and all of the glands from each tissue showed uniform clonal pattern. Two peritoneal endometriotic tissues had 1 and 5 informative specimens from individual glands, respectively; and the clonal patterns in 5 glands from the single lesion were divergent. Two abdominal wall endometriotic tissues had 7 and 6 informative specimens,respectively; and variable clonal patterns were seen in different glands from each lesion. One deep infiltrating endometriotic lesion had 4 informative specimens of isolated glands, and all of them showed unique clonal pattern. The disparate clonal patterns were found in endometriotic lesions with variable pathological subtypes, even arising from the same patient. Conclusions The epithelial cells from individual endometriotic gland showed monoclonality , and different glands from the same endometriotic lesion might show divergent patterns.
6.Therapeutic effect on high dose intravenous gamma globulin treating Guillain-Barre syndrome and multiple sclerosis

Objective To observe the therapeutic the effect and safety of high dose intravenous gamma globulin (GG) treating Guillain Barre syndrome (GBS) and multiple sclerosis (MS).Methods 17 patients with GBS and 12 patients with MS were treated with IVIG in a dose of 0.2 g/kg/d for 4 consecutive days (in a dose of 0.4g/kg for the first day), and 25 patients with GBS and 41 patients with MS were treated with steroid. Their therapeutic effectiveness was compared. 7.Study on clinic, pathology and image of sporadic Creutzfeldt-Jakob disease

Objective To investigate the clinical, pathological and imaging features of sporadic Creutzfeldt-Jakob disease (CJD). Methods The clinical, pathological, electroencephalogram (EEG) and imaging data of 12 cases of CJD were analyzed retrospectively.Results (1) 7 cases of the enrolled patients were male and 5 were female. The average onset age of them was 49 years old. 3 cases were acute onset by visual loss and 9 cases were subacute onset by hypophrenia, mental and behavior disorder or cerebellar ataxia. All the cases showed dementia, myoclonus and extrapyramidal sign. (2) 9 cases showed typical triphasic wave on EEG and 1 case presented atypical triphasic wave. (3) 5 cases showed abnormal hyperintense lesions in basal ganglia on T 2-weighted images (T 2WI). 8 cases in occipital and/or frontal cortex showed the abnormal hyperintense lesions on diffusion-weighted images (DWI), and 5 of them accompanied symmetric diffuse hyperintense lesions in basal ganglia. (4) 1 case with postmortem and 6 cases with biopsy of brain showed classic pathological features of CJD.Conclusion On the base of typical clinic manifestations, EEG and DWI examinations are useful in early diagnosis of sporadic CJD.
8.Study on Extraction Process of Yangwei Jinshi Granules

Objective To optimize the extraction process of Yangwei Jinshi Granules. Methods An orthogonal test was adopted in the study. With the paste-forming rate, the content of extract and the content of hesperidin of Yangwei Jinshi Granules as indices, data processing was carried out with grading method of synthesizing multiple indexes for optimizing the extraction condition. Results The best extraction technology was as follows:boiled 2 times, first with 10 times amount of water for 1.5 h, and then boiled with 8 times amount of water for 1 h. Conclusion The optimized process is reasonable, stable and economic for Yangwei Jinshi Granules.
9.Impact of biological function on ovarian clear cell carcinoma ES2 cell line with ARID1A gene expression down-regulating in vitro

Objective To investigate the efficiency of biological function of AT rich interaction domain 1A (ARID1A) gene silenced by small interfering RNA (siRNA) on ovarian clear cell carcinoma ES2 cell line. Methods (1) The three pairs ARID1A gene siRNA interference fragments siN1 (ARID1A-705), siN2 (ARID1A-1513), siN3 (ARID1A-2282) and one pair negative control were respectively designed, and transfected into ES2 cells by RNA interference max reagent transiently. Reverse transcription (RT)-PCR and western blot methods were used to detect the expression of ARID1A mRNA and protein in ES2 cells transfected with interference fragments respectively. So as to select the best silencing effect of siRNA interference fragment(that was siN3),and then was used in the following experiment. (2) The following experiment were divided into three groups, namely siN3 transfection group, negative control group and blank control group. The proliferative activity of three groups of cells after transient transfection ( 6, 24, 48, 72, 96 hours) was assessed by cell counting kit-8 (CCK-8) assay and expressed as absorbance (A) value; the apoptosis rate of three groups of cells transfected transiently with interference fragment was measured by flow cytometry with annexin V/propidium iodide (PI) staining;the ability of cellular invasion of three groups of cells transfected transiently with interference fragment was tested by transwell experiment;the expression of nuclear factor-kappa B (NF-κB), membrane type-1 matrix metalloproteinase (MT1-MMP) and matrix metalloproteinase-2 (MMP2) protein in ES2 cells transfected transiently with interference fragment was measured by western blot. Results (1) The RT-PCR results showed that the ARID1A mRNA relative expression levels in ES2 cells after transfected transiently with siN1, siN2 and siN3 were 0.007 8±0.005 7, 0.006 8±0.000 3 and 0.002 8±0.000 3 respectively. They were all apparently lower than that in the negative control group (0.034 6 ± 0.001 3;all P<0.01). The western blot results showed that the expression levels of ARID1A protein were 0.439 4±0.000 7, 0.424 4±0.005 0 and 0.386 0±0.005 8 respectively. They were also lower than that in the negative control group (0.732 4 ± 0.030 3; all P<0.01). The siN3 with the highest transfection efficiency was selected to use in the following experiment. (2) The CCK-8 method showed that the proliferative activity of siN3 transfection group cells after transfected transiently at 6 hours was not statistically significant difference compared with those in negative control group and blank control group (0.506 ± 0.010, 0.491 ± 0.006, 0.498 ± 0.009, respectively; all P>0.05). However, the proliferative activity of siN3 transfection group cells after transfected transiently at 24, 48, 72, 96 hours were higher than those in negative control group and blank control group (all P<0.01). The flow cytometry results showed that the apoptosis rate of siN3 transfection group cells was (20.0±3.9)%, which was significantly lower than those in negative control group and blank control group [(31.5 ± 5.0)%, (34.0 ± 4.2)%, respectively;all P<0.05]. The transwell experiment showed that the penetrated cell counts of siN3 transfection group was 60.4±2.9, which was apparently higher than those in negative control group and blank control group (54.2 ± 3.5, 52.1 ± 3.8, respectively; all P<0.01). Western blot experiment showed that the relative expression levels of NF-κB, MT1-MMP and MMP2 protein in siN3 transfection group were respectively 1.85 ± 0.16, 0.37 ± 0.08, 1.38 ± 0.11, which were apparently higher than those in negative control group (0.93±0.11, 0.17±0.05, 0.86±0.06;all P<0.05) and blank control group (0.94 ± 0.04, 0.15 ± 0.08, 0.85 ± 0.10, respectively; all P<0.01). Conclusions It would be to promote the cell doubling time, reduce cell apoptosis and increase the invasive capability in ES2 cells that ARID1A expression was down-regulating by ARID1A mRNA interference. The invasion mechanism may be related to the activation of NF-κB signal transduction pathway, up-regulation of MT1-MMP expression and then promoting the invasion of tumor cells via the up-regulation of MMP2 expression.
10.Neuroimaging and electroencephalogram features of a patient with Creutzfeldt-Jakob disease: a follow-up study

Objective To explore the characteristics of neuroimaging and electroencephalogram (EEG) features of sporadic Creutzfeldt-Jakob disease (sCJD). Methods CT scan was performed at the 6th week, MRI was performed at the 7th, 9th, 17th and 64th week, regular EEG recording was performed at the 9th week, contineous 24-hour EEG recording was performed at the 11th, 14th, 16th, 37th and 64th week after onset in a patient with the pathological diagnosis of CJD. Regular pattern was analyzed according to the neuroimaging results and EEG. Results a) Diffusion-weighted imaging (DWI) was more sensitive in diagnosis of sCJD than cranial CT, T1-weighted MRI (T1WI), T2-weighted MRI (T2WI), fluid-attenuated inversion recovery (FLAIR) MRI or contrast-enhanced MRI. Abnormalities would not be found in the early stage of CJD in CT, T1-weighted MRI, T2-weighted MRI, FLAIR or contrast-enhanced MRI, but they could be found in the middle stage of CJD. The CT scan and routine MRI might not show any specific feature for the sCJD. b) DWI abnormalities appeared in bilateral cortex and basal ganglia, and changed along with the progression of disease. In late stage of the disease, abnormal aignals in the cortex would disappear, but it was still present in basal ganglia. c) The appearance of periodic discharge of sharp wave complexes (PSD), which could be mathematically described by a sine curve, might be absent in the early and late stage of the disease. Along with the progression of CJD, PSD would be present in a part of a lobe at first and then involve more lobes, and finally the whole brain would be involved. Conclusion Repeated monitoring of DWI and EEG is significant for early diagnosis of CJD.
