2.Evaluation of nephron-sparing surgery for renal cell carcinoma

Objective To evaluate the indications and intraoperative management of nephron-spar- ing surgery (NSS) for renal cell carcinoma (RCC).Methods A total of 53 patients (32 men and 21 women;age range,21 -76 years) with unilateral RCC and a normal contralateral kidney underwent NSS.The disease course ranged from 1 week to 13 months.The unilateral RCCs were detected by B-uhrasonography. Partial nephrectomy was performed on 38 cases of polar RCC and wedge nephrectomy was performed on 15 cases of central RCC.Both renal vein and artery were blocked for 10 - 15 min,and unclamped for 1 -2 min if necessary.The resections were finished within 7 -21 min (mean,12 min).No inosine and regional hypo- thermia were used during operation.Absorbable hemostatic gauze and terylene flake were used to suture renal wound surface.After operation all patients were treated with interferon hypodermic injection of 5 000 000 U every other day for 3 months.Results The diagnosis of RCC with negative cutting margin was confirmed by pathological examination in all patients.The greatest dimension of the renal cancers were<4 cm except for one of 6 cm.The clinical stage was T_1N_0M_0 in all patients.During the follow-up of 6 -48 months,all pa- tients were alive without recurrence and had normal renal function.Conclusions NSS can be rapidly per- formed for T_1 stage renal cancer<4 cm or single clearly localized cancer>4 cm in diameter.The follow-up results are satisfactory.
3.Comparative study of absorbable screw and metallic internal fixation in treatment of tibial plateau fractures

Objective To assess the effectiveness of absorbable screws and metallic devices for treating tibial plateau fractures.Methods Fifty-seven cases of tibial plateau fractures were treated with internal fixation using absorbable screws(Group A),cannulated screws(Group B)and condylar buttress plates(Group C)in our hospital.According to Schatzker classification,there were 15 cases with typeⅠfractures,21 with typeⅡ,nine with typeⅢand 12 with typeⅣ,who were followed up for 6-51 months. Statistical analysis was performed to compare bone union and functional restoration of the knee between three groups.Results Bone union was achieved in all cases.Based on Merchant scale for knee func- tion,excellence rate was 84%(16/19)for Group A,92%(11/12)for Group B and 85%(22/26) for Group C,with no significant statistical difference.Conclusion Absorbable screws is as effective as metallic devices for treating tibial plateau fractures and has advantages of minimal invasion,few complica- tions and avoiding secondary operation.
4.Multiple factors logistic regression analysis on the basic syndromes related factors in patients with chronic prostatitis.

OBJECTIVETo explore the basic syndrome related factors in patients with chronic prostatitis (CP).
METHODSUsing questionnaire to collect data of CP patients from three hospitals in Beijing, Epidata 3.02 to establish database, the uni-variate and multiple Logistic regression analysis was performed with SPSS 17.0 software to determine the basic syndrome related factors.
RESULTSEngagement in brainwork was the main risk factor for suffering from dampness-heat downward-flow syndrome; CP of type IIIA (classified by Western medicine), employed on brainwork or physical work, and working time < or =8 h were risk factors for suffering from qi-stagnancy and blood-stasis syndrome; illness duration >12 months, uncomfortable habitat, dislike for irritative beverages and poor digestive function were risk factors for Gan-qi stagnation syndrome; and aging, decreased burden from work, winter onset of the illness were those for Shen-yang deficiency syndrome.
CONCLUSIONSThe basic syndrome related factors in patients with CP are age of patient, duration and type of illness, occupational type, daily working time, burden from work, onset season of illness, living amenity, state of digestion, and relish for irritative beverage.

5.Clinical efficacy and safety of butylphthalide in the treatment of patient with transient ischemic attack

Objective To observe the clinical efficacy and safety of butylphthalide in the treatment of patient with transient ischemic attack.Methods Ninety -one cases with transient ischemic attack were ran-domly divided into control group ( n=45 ) and treatment group ( n=46 ).Patients in the control group were administered of ozagrel sodium 80 mg+0.9%NaCl 150 mL, ivgtt, bid; patients in treatment group adminis-tered of butylphthalide 25 mg, ivgtt, bid, two weeks a cycle.After 2 cycles treatment , the clinical efficacy of the two groups , the levels of serum fibrinogen(FIB) and triglyceride (TG) before and after treatment, the carotid intima-media thickness ( cIMT) and the area of the carotid artery, the area of the soft plaque and the incidence of adverse reactions were compared.Results Total clinical efficacy were 86.96%( 40/46 ) and 66.67%(30/45) respectively with statistical difference (P<0.05).The serum levels of TG and FIB were significantly decreased after treat-ment in both the control and treatment group ( P<0.05 ).The serum le-vels of TG and FIB were significant lower than control group after treat-ment ( P<0.05 ).Carotid artery IMT and plaque area in two groups had no significant changes of the two groups ( P >0.05 ).No significant adverse reaction in two groups occurred , only two patients with medication ncrease in transaminase , after 1 month, the aminotransferase levels recovery to normal.Conclusion Significant clini-cal efficacy and safety was found for butylphthalide in the treatment of transient ischemic attack.
6.Exogenous Local Hyperthermia at 41o C Is Effective to Eliminate Mouse Model of Sporotrichosis, Independent of Neutrophil Extracellular Traps Formation

Background:
Local hyperthermia is recommended for the treatment of patients with fixed cutaneous sporotrichosis, though the effectiveness and mechanisms of action remain elusive. While neutrophils represent the main inflammatory cells associated with sporotrichosis lesions, the issue of whether hyperthermia is involved with interactions between neutrophils and Sporothrix globosa remains unclear.
Objective:
To evaluate the effect of local hyperthermia on sporotrichosis and determine whether local hyperthermia involves effects of neutrophils against Sporothrix.
Methods:
For the in vivo study, mice were infected with yeast cells of S. globosa followed by treatment with local hyperthermia. In vitro, an isolated S. globosa strain was co-cultured with or without neutrophils and subjected under different temperatures. Immunofluorescence was used to assess the formation of neutrophil extracellular trap (NETs) were formed under these different culture conditions and the number of fungi colony forming units were compared. Results: Hyperthermia was significantly more effective in clearing the lesions in the mouse model, as compared to sham treatment. Neutrophils failed to exert any fungicidal effects against S. globosa in response to hyperthermia. Moreover, NETs were formed after interactionwith S. globosa, and the percentage of NETs formed was not significantly different at 41o C or 37o C.
Conclusion
While hyperthermia could serve as an effective therapy for fixed cutaneous sporotrichosis, this ability does not involve the formation of NETs.
7.A clinical study of prostat combined with an antibiotic for chronic nonbacterial prostatitis.

OBJECTIVETo evaluate the efficacy and safety of Prostate in combination with an antibiotic for the treatment of chronic nonbacterial prostatitis.
METHODSA double-blind, parallel contrasted, multi-central method was applied in the study. After the Stamey test and expressed prostate secretion (EPS) examination, 160 patients with prostatitis were recruited and randomized into a trial group (80 cases with 1 case missing) and a control group (80 cases). In the trial group, the patients used the levofloxacin and Prostate during the first 4 weeks and Prostate only during the following 4 weeks. In the control group, the patients used the levofloxacin and placebo during the first 4 weeks, and placebo only during the following 4 weeks. Before and 4 and 8 weeks after the treatment, the patients were visited and evaluated by the national institute health-chronic prostatitis symptom index (NIH-CPSI), EPS, and asked about the side.
RESULTSAfter 4-week and 8-week treatment, the pain index dropped by 3.34 +/- 2.45 and 4.06 +/- 3.03 in the trial group, and effects. 2.28 +/- 2.42 and 3.30 +/- 3.29 in the control; the voiding index dropped by 2.22 +/- 1.79 and 2.77 +/- 2.04 in the trial group, and 1.24 +/- 1.67 and 1.83 +/- 2.25 in the control respectively. There was significant difference between pre-treatment and post-treatment in both the two groups (P < 0.01), while the difference was not significant between 4-week and 8-week post-treatment (P > 0.05). And there was significant difference between the two groups in the pain index and voiding index (P < 0.01), but not in the white blood cell count and lipid in the EPS (P > 0.05). No serious side effects were recorded, and the tolerance to Prostate and placebo showed no significant difference.
CONCLUSIONProstate in combination with an antibiotic can effectively relieve the pain and voiding symptoms and improve the life quality of the patients with nonbacterial prostatitis and well deserves to be recommended in clinical practice.

8.Studies on inherited coagulation factor VII deficiency and tissue factor abnormality in a pedigree.

OBJECTIVETo investigate the mechanism of clinical haemorrhage in an inherited coagulation factor VII (FVII) deficiency and tissue factor abnormality pedigree.
METHODSAll exons, exon-intron boundaries and the 3', 5' untranslated sequences of FVII and tissue factor (TF) genes were amplified by PCR and sequenced directly. Any mutation identified by direct sequencing was confirmed by reverse sequencing. FVII cDNA of the proband was synthesized with random primers and amplified by nest PCR.
RESULTS55C-->T heterozygous mutation located in promoter of FVII gene was identified in the proband. The heterozygous mutation was derived from his mother. Tracing the other pedigree members found that his sister had the same heterozygous mutation and the others had wild-type FVII genes. A 9363 C-->T (Arg131Trp) heterozygous polymorphism in TF gene, which was 2.63% frequency of T allele polymorphism, was found in all of the pedigree members.
CONCLUSIONIt was the first report that the -55C-->T heterozygous mutation in FVII gene and the Arg131Trp heterozygous polymorphism in TF gene explained the clinical symptom of the proband.

9.Relationship between polymorphism of interleukin-8 and silicosis susceptibility.

OBJECTIVETo explore the relationship between the polymorphisms of interleukin-8 (IL-8) and the silicosis susceptibility.
METHODSThe case group consisted of 101 male patients with stage I silicosis diagnosed by the Pneumoconiosis Diagnosis Expert Panel according to the Chinese National Diagnosis Criteria of Pneumoconiosis (GBZ 70-2009). The control group consisted of 121 workers without silicosis exposed to same dusts. The cases and the controls had the same dust exposure history. The peripheral venous blood was drawn from each subject. DNA was extracted from leucocytes by the salting method. The polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) techniques and PCR were used to examine polymorphism of IL-8 (Met31Arg, 781C/T, -251A/T and RA+860).
RESULTSThere were no the differences of age, cumulative exposure time and smoking between the cases and the controls (P > 0.05). The frequencies of IL-8 (Met31Arg) GT genotypes in cases and controls were 12.87% and 2.48%, respectively, there was significant difference (P < 0.05). The frequencies of allele G in cases and controls were 6.44% and 2.07%, respectively, there was significant difference (P < 0.05). The frequencies of IL-8 (-251A/T) AA genotypes in cases and controls were 9.90% and 25.64%, respectively, there was significant difference (P < 0.05). The frequencies of IL-8 (781C/T) CC, CT, TT genotypes in cases and controls were 38.61%, 40.59%, 20.79% and 46.28%, 40.50%, 13.22%, respectively, there was no significant difference (P > 0.05). The frequencies of IL-8 (RA+860) GG, GC and CC genotypes in cases and controls were 75.25%, 21.78%, 2.97%, 80.17%, 14.88%, 4.96%, respectively, there was no significant difference (P > 0.05).
CONCLUSIONSIL-8 (Met31Arg and -251A/T) genetic polymorphisms might play a role in the development of silicosis. The risk of pneumoconiosis in workers carrying (Met31Arg) genotype GT is likely to increase. The risk of pneumoconiosis in workers carrying IL-8 (-251A/T) AA genotype is likely to decrease. The relationship between IL-8 781C/T and RA+860 genes polymorphisms and silicosis is not found.

10.The severity of psychic symptoms closely correlated with that of clinical ones in chronic prostatitis patients.

OBJECTIVETo study the correlation between the severity of psychic symptoms and that of clinical ones in CP patients.
METHODSWe investigated 300 CP patients with NIH Chronic Prostatitis Symptom Index (CPSI), Symptom Checklist 90 (SCL-90) and a self-designed questionnaire, and analyzed the correlation between the scores. Based on the scores on pain and urinary symptoms in CPSI, the patients were divided into a mild, a moderate and a severe group.
RESULTSA total of 288 questionnaires were collected. Compared with normal males, the SCL-90 scores of the CP patients were not different in obsessive-compulsive (P > 0.05), lower in interpersonal-sensitivity (P < 0.01), higher in paranoid ideation (P < 0.05) and significantly higher in other factors and the number of positive factors (P < 0.01). There were no statistical differences among the mild, moderate and severe groups either in somatization and interpersonal-sensitivity (P > 0.05) or in phobic anxiety (P > 0.05), but there were between the mild and moderate groups in phobic anxiety (P < 0.05) and significantly among the three groups in other factors and the number of positive factors (P < 0.01 or P < 0.05). No correlation was found between CPSI and SCL-90 in the total scores.
CONCLUSIONThere was a close correlation between the severity of psychic symptoms and that of clinical symptoms in CP patients. Psychotherapy, especially psychological intervention, plays an important role in the early stage of the disease.
