1.Therapeutic value of endoscopical therapy for small gastric stromal tumor

Objective To investigate the safety and effectiveness of endoscopical therapy for small grastric stromal tumor( d<2 cm) . Methods Clinical records of 80 patients dignosed as having small grastric stromal tumor by pathology and immunohistochemistry were retrospectively analyzed. Endoscopic or laparo?scopic or open surgery was performed, so patients were divided into endoscopic treatment group ( n=37) and surgical treatment group ( n=43) accordingly. Perioperative period data, effectiveness and follow?up data were analysed. Results Thirty?five cases( 94?6%) of endoscopic treatment group were successfully treated by endoscopic surgery. Intraoperative perforation occurred in 8 cases, among which 2 cases transferred for conventional operations.Pneumoperitoneum occurred in 2 cases (better after symptomatic treatment), while no serious infection or perioperative death occurred. There were no significant differences in the complete re?section rate[ 91?89% ( 34/37) VS 100?0%( 43/43) , P=0?095) , the incidence of postoperative complica?tions[5?71%(2/35)VS 2?33%(1/43),P=0?855)or the improvement of postoperative symptoms[93?10%( 27/29) VS 85?71% ( 30/35) , P=0?589] ,while statistically significant differences were found in operation time[(37?41±13?45)min VS(84?56±38?37)min, P=0?000], intraoperative blood loss[(5?65±5?88)ml VS(31?48 ±39?57)ml, P=0?000], postoperative feeding time[(2?47±0?61)d VS(3?26 ±1?27)d, P=0?001], postoperative hospital stay[(5?76 ±2?28)d VS(7?64±2?99)d,P=0?022],and medical expenses [(18 554?4±9 736?45)yuan VS(31 138?11±1 206?24)yuan, P=0?000]. Conclusion Endoscopic re?section is safe and effective for small grastric stromal tumor with minimal invasion,simple procedure, faster recovery and less economic burden for the patients.
2.Condition and Related Factors of Anxiety and Depression in Caregivers of Patients in Persist Vegetative State or Minimally Conscious State

Objective To explore the anxiety and depression in caregivers of patients in persist vegetative state and minimally conscious state, and discuss the related factors. Methods From January, 2010 to December, 2014, 60 caregivers of 60 patients in persist vegetative state and minimally conscious state were assessed with Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). The sex, ed-ucation level, relation to the patients and family income of the caregivers, and the age of the patients and number of the families of the pa-tients were recorded, and analyzed with Logistic regression. Results 55%caregivers were found in anxiety, and 48.4%in depression. The fe-male caregivers (OR>6.870, P<0.05), the number of the family of the patients (OR<0.233, P<0.05) and age of the patients (OR<0.294, P<0.05) were independent factors associated with anxiety and depression. Conclusion The caregivers of patients in persist vegetative state and minimally conscious state are troubled in anxiety and depression, especially the female, while age and the number of the families of patient may be important factors for it.
3.Relationships between seven single nucleotide polymorphisms and female breast cancer among Hui and Han nationalities

Objective To investigate the relationships between seven single nucleotide pdymorphisms(SNPs), including 6788895(SIAH2 gene), rs10941679(5p12), rs889312(MAP3K1), rs13387042(2q35), rs6504950(17q23), rs17530068(6q14) and rs2284378(20q11), and female breast cancer of Hui and Han nationalities.Methods A hospital-based case-control using age and nationallity as frequency matching conditions was conducted in Affiliated General Hospital of Ningxia Medical University.Results ①A total of 225 breast cancer patients and 225 controls were enrolled in this study.There was no significant differentce between case group and control group in age(48.27±9.27 vs 48.48±9.36, t=0.238,P=0.812) or nationallity (181 Han people and 44 Hui people vs 184 Han people and 41 Hui people, x2=0.131, P=0.718).②The subjects of study conformed to Hardy-Weinberg equilibrium principles.③The genotype and allele distribution of the above seven SNPs were not significantly different between cases group and control group of Hui and Han nationalities.Conclusion The seven SNPs mentioned above may not be related to breast cancer among people Hui and Han nationalities.

6.Manifestation study on depression in comprehensive hospitals

Objective To study and analyze clinical manifestation of depression in comprehensive hospitals in China.Methods Retrospective study was performed.3322 objects included in this research were measured by Minnesota Multiphasic Personality Inventory(MMPI).Results In Chinese comprehensive hospitals,the most common accompanying symptoms of depression were anxiety,fatigue,suicide inclination,apprehension,somatic discomfort,impotence or asexuality,hypochondriasis,somato-complaints,insomnia,self-accusmion and self-reproach,sense of dependence and so on.Depression was positively correlated with suicide risks,self-accusation,delusion of self-accusation,somatic discomfort,somato-complaints,impotence and asexuality,insomnia,headache,fatigue,apprehension,and family problems.Depression had negative correlations with self-strength,social status and economic status.The results had statistical significance.Conclusion Decreased self-strength,increased sense of dependence,low social status and economic status and family problems were the most common pathogenesis basis for depression.Depression can be manifested as various somatic disorders,especially insomnia.
7.Correlation Study of Depression, Anxiety and Sleep Disturbance

Objective To investigate the correlation between depression, anxiety and sleep disturbance. Methods 93 patients with the main complaint of insomnia were selected and performed a retrospective study. Results Four factors including anxiety factors, manifest anxiety, subjective depression and dominant depression of the insomnia patients were significantly higher than normal people, while the Ego Strength scores are significantly lower than the normal people. Conclusion When confronting patients with sleep disturbance, doctors must pay attention to identifying any psychological disorders including depression and anxiety in the patients, and psychological support should also be strengthened to such patients.
8.Clinical and pathologic features of small gastrointestinal stromal tumor

Objective To investigate the clinical and pathologic features of small gastrointestinal stromal tumors (small GISTs,d < 2.0 cm).Methods Medical records of 95 patients undergoing surgery (endoscopic surgery,thoracoscopic/laparoscropic surgery and open surgery)and diagnosed as having GISTs by pathology and immunohistochemistry in Nanfang hospital from October 2003 to June 2014 were retrospectively analyzed.Based on clinical and pathological results,correlation analyses between risk factors for endoscopic ultrasonography(EUS) and Mitotic count(MI),clinicopathologic parameter and NIH risk classification were performed.Results Among 95 cases (104 lesions),88 were single,while 7 were multiple;81.7% (85/104) small GISTs arose from stomach,including 87.1% (74/85)in middle-upper stomach;5 cases (5.3%) presented calcification of different degrees,3 cases(3.2%) presented local necrosis and 2 cases (2.1%) with arrangement of epithelioid cells;88 cases (92.6%) were very low grade of NIH risk classification,6 cases (6.3%) were intermediate risk and 1 case(1.1%) was high risk.Positive rates of CD34 and CD117 were 95.8% (91/95) and 96.8% (92/95) respectively.The risk factors (border,mucosal surface,echo and heterogeneity) of EUS had no correlation with mitotic count(P>0.05).The correlation analysis between clinicopathologic features and NIH risk classification revealed tumors more than 1.5 cm had a striking correlation with NIH risk classification (P< 0.05).Conclusion Most small GISTs,single or multiple,located at middle-upper stomach,were of very low or low risk,and have a favorable prognosis.But it has worse biological behavior and a higher grade risk when the diameter is more than 1.5 cm,intervention should be recommended.
9.Systematic Review of the Effect of Danhong Injection on Hemorheology in Patients with Angina Pectoris in Coronary Heart Disease

OBJECTIVE:To systematically review the effect of Danhong injection on hemorhelogy in patients with angina pec-toris in coronary heart disease(CHD),and provide evidence-based reference for clinical treatment. METHODS:Retrieved from PubMed,EMBase,Medline,Cochrane Library,CJFD,VIP database,Wanfang database,and retrieved relevant literatures manual-ly,the randomized controlled trials (RCT) about the effect of Danhong injection on hemorhelogy in patients with angina pectoris in CHD were collected. After the quality evaluation and information collection of clinical studies with inclusion criteria,Meta-analy-sis was conducted by using Rev Man 5.2 software. RESULTS:A total of 12 RCT were enrolled,involving 1 160 patients. Results of Meta-analysis showed the conventional treatment of Danhong injection could more effectively improve the high blood viscosity [MD=-0.87,95%CI(-1.24,-0.50),P<0.001],low blood viscosity[MD=-2.43,95%CI(-3.99,-0.87),P=0.002],hemato-crit value[MD=-0.05,95%CI(-0.10,-0.00),P=0.04],plasma viscosity[MD=-0.58,95%CI(-0.78,-0.38),P<0.001] and fibrinogen level[MD=-1.06,95%CI(-1.65,-0.47),P<0.001],compared with convertional treatment,there were significant dif-ferences. CONCLUSIONS:Based on the conventional treatment,Danhong injection has improvement effect on the related indica-tors of hemorhelogy in patients with angina pectoris in CHD. Due to the limit of methodological quality and sample size,it remains to be further verified with more rigorously designed and long-term follow-up of large-scale RCT.
10.Significance of ~(99m)Tc-Diethyl Iminodiacetic Acid Hepatobliliary Planar and Tomographic Scintigraphy in Diagnosis on Biliary Atresia

Objective To evaluate the clinical value of 6 h 99mTc-diethyl iminodiacetic acid (99mTc-EHIDA) planar hepatobiliary scintigraphy (HBS),6 h tomographic HBS and 24 h planar HBS in diagnosis on biliary atresia(BA).Methods Seventy cases(32 male,38 female) with continuous jaundice received planar and tomographic HBS in Beijing Friendship Hospital from Jan.2005 to Dec.2007.The mean age was 48.7 d (29 d-4 months).According to final diagnosis,all cases were divided into BA group (45 cases) and non-BA group (25 cases).All cases fasted at least 4 hours before HBS.The equipment was 3 head IRIX from Philips company with low energy high resolution collimator.The tracer was 99mTc-EHIDA and the radiochemistry purity was more than 95 percent.The dosage was 7.4 MBq/kg.All diagnosis demonstrated by operation pathology and clinical follow-up.All cases received HBS at 5,10,15,20,30 min and 1,6 h after tracer injection.HBS would ended if radioactivity appeared in gallbladder or intestine.These cases would received tomographic HBS and 24 h HBS if radioactivity did not appear in gallbladder or intestine at 6 h post injection.All these images were analyzed by 2 or more nuclear medicine physicians.Results There were not radioactivity appearing in gallbladder and intestine on planar and tomographic HBS of 27 cases,which suggested the BA.There were radioactivity appearing in gallbladder and intestine on planar and tomographic HBS of 30 cases,which suggested the non-BA.Positive rate of 6 h tomographic HBS was significantly higher than that of 6 h planar HBS and there was significantly difference between the 2 methods.Positive rate of 6 h tomographic HBS was significantly higher than that of 24 h planar HBS and there was significant difference between the 2 methods.Conclusions 99mTc-EHIDA HBS is a noninvasive,safety,valuable examing method and has definitely clinical value in the diagnosis on BA.The clinical value of 6 h tomographic HBS is significantly higher than that of 6 h planar HBS and 24 h planar HBS.
