1.Telmisartan Reduce Serum Retinol Binding Protein 4 and Improve Insulin Resistance in Rats with Metabolic Syndrome

Background Retinol binding protein 4(RBP4) is a novel adipokine which has been related with insulin resistance.Objective To test the hypothesis that telmisartan improves glucose and lipid metabolism may be associated with inhibiting serum RBP4 and phosphoenolpyruvate carboxykinase activity in liver.Methods Thirty wistar male rats were received high fat diet to establish metabolic syndrome model and randomly to receive telmisartan [5 mg/(kg?d),n=8] or pioglitazone [20 mg/(kg?d),n=8] for 8 weeks or high-fat diet placebo(n=10).Plasma triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),fasting plasma insulin,serum RBP4,and phosphoenolpyruvate carboxykinase specific enzyme activity in liver(PEPCK) were determined.Results Telmisartan improved insulin resistance and the disorders of glucose and lipid metabolism in diet-induced insulin resistance rats [HOMA-IR,telmisartan group(3.4?1.2) vs high-fat diet group:(8.3?1.1),P
2.Advances in the studies on cytokine and chemokine gene polymorphisms associated with uveitis

·Uveitis is an inflammation of any or all parts of the uveal tract including the iris, ciliary body and the choroid. Despite current advances in diagnosis and management, visual loss occurs in 35%-45% of patients with uveitis. The etiopathogenesis of uveitis remains unknown; it may be associated with environmental and immunogenetic factors. Many studies have demonstrated polymorphisms in major histocompatibility complex (MHC) genes may determine involvement in uveitis. Recently polymor-phisms in non-MHC genes, including cytokine and che-mokine genes, have been reported to play important roles in the pathogenesis of uveitis. This article reviewed the advances in the studies on cytokine and chemokine gene polymorphisms associated with uveitis.
3.Synthesis and structure of the Salen-Mn compound

ObjectiveTo synthesize metal complex that exhibits superoxide dismutase (SOD)-like activity and determine its structure.MethodsThis chemical compound was synthesized by means of reflux in laboratory.Its structure was examined by infrared spectra (IR),nuclear magnetic resonance hydrogen spectra(H-NMR) and mass spectra (MS).The activity of the SOD was determine by xanthine oxidase (XOD).ResultsThe IR indicted that this complex showed a typical absorption band of -OH,C=N and benzene. The H-NMR reveals the existence of metal ion manganese (Mn3+ ,paramagnetic). The molecular weight was 365, which was obtained by EI-MS analysis. And this complex showed SOD-like activity.ConclusionA metal complex with low molecular weight and SOD-like activity has been synthesized.
4.Protective effects of the compound with low molecular weight of anti-free radical on brain injury in rats

Objective To study the protective effects of the compound with low molecular weight of anti free radical on brain ischemic and hypoxic injuries in rats.Methods The model of brain ischemic and hypoxic injuries in rats was established by unilateral carotid artery ligature just for 2 h. The compound was injected 30 minutes after the rats act on hypoxic state (10% O2+90%N2) for 1 h. Then the serum of rats was separated and the value of the superoxide dismutase (SOD) activity and malondiade (MDA) were determined, and the pathological changes were observed in brain tissue.Results Compared with rats in the model group, the compound decreased the concentrations of MDA in serum (P<0.05), and raised the level of SOD (P<0.05) significantly in therapy group. Otherwise, the pyramidal cells remained organized order, with only a few cells degenerated occasionally in rats of therapy group. Conclusion The compound has SOD like activity. It can scavenge the free radical and inhibit peroxidation of lipid, so it can prevent brain cells from the damage.
5.Application Research on Method of“Role of Identification”in Clinical Teaching

Objective To investigate the score changes of graduates majoring in clinical medicine after using the method of “role of identification”. Methods Sixty students who were undertaking internship in Endocrinology Department of Shanghai Traditional Chinese Medicine Hospital received three-week clinical teaching and examination. Then they got into one-week “role of identification”, and were appointed as clinical teachers to teach what they have learned during the last 3 weeks to the next batch of interns and make another examination. Scores of the two examinations were compared. Results Scores of the examination after received the method in“role of identification”improved significantly (P<0.01). Conclusion The method of“role of identification”can enhance clinical medicine graduates’ mastery of professional knowledge.
6.Value of DCE-MRI quantification study with exchange model in evaluation of liver cirrhosis

Objective To investigate the value of dynamic contrast-enhanced MRI (DCE-MRI) quantitation study with exchange model in evaluation of liver cirrhosis.Methods A total of 30 patients with liver cirrhosis confirmed to included and excluded criteria were divided into compensatory cirrhosis group (n=15) and decompensatory cirrhosis group (n=15).And 15 healthy volunteers were scrvcd as a control group.All subjects were performed liver DCE-MRI and quantitative pa rameters of three groups were obtained by exchange model,including volume transfer constant of the contrast agent (Ktrans),reverse reflux rate constant (Kep),volume fraction of EES (Ve),volume fraction of plasma (Vp),arterial perfusion index (HPI),blood volume (BV),blood flow(BF) and mean transit time (MTT).The parameters of three groups were compared by ANOVA analysis and LSD test for every two groups.The diagnostic efficacy of Ktrans,Ve,HPI,MTT for liver cirrhosis were analyzed by ROC.Results Ktrans,Ve,HPI,MTT and BF values in the control group,compensatory cirrhosis group and decompensatory cirrhosis group were (0.85 ± 0.34) min-1,(0.67 ± 0.36) min-1,(0.39±0.18)min-1;0.47±0.42,0.34±0.13,0.16±0.06;0.35±0.09,0.57±0.17,0.76±0.20;(17.38±15.38)s,(34.26111.37)s,(57.33±16.73)s;(97.22±36.29)ml/(min· 100 g),(69.30±35.22)ml/(min· 100 g),(24.64± 28.24) ml/(min · 100 g),respectively,and showing significant differences among three groups,and significant with HPI and MTT for every two groups (P<0.05).There were no significant differences for Kep,Vp and BV among three groups (P>0.05).According to ROC of Ktrans and Ve for decompensatory liver cirrhosis,AUC were 0.86,0.92,the sensitivity were 90%,90% and the specificity were 80%,80%.According to ROC of HPI and MTT for compensatory liver cirrho sis,AUC were 0.79,0.76,the sensitivity were 85%,85% and the specificity were 75%,75%.According to ROC of HPI and MTT for decompensatory liver cirrhosis,AUC were 0.91,0.88,the sensitivity were 95%,90% and the specificity were 80%,80%.Conclusion DCE-MRI quantitative study with exchange model can obtain multiple parameters,refleet hemodynamic changes and vascular microenvironment and evaluate severity of liver cirrhosis.
7.Serum asymmetric dimethylarginine and endothelial function after renal transplantation

Objective To investigate the relation between serum asymmetric dimethylarginine (ADMA) level and endothelial function before and after living donor kidney transplantation in uremic patients.Methods A total of 38 renal transplant patients (21males and 17 females) and 36 healthy controls (20 males and 16 females) were enrolled. Plasma ADMA, symmetric dimethylarginine (SDMA), malondialdehyde (MDA), glutathione peroxidase (SeGSHPx), C-reactive protein (CRP) were mea-sured before transplantation and on Days 1, 3, 7, 14, and 28 posttransplantation. The brachial artery flow mediated dilatation (FMD) was studied before transplantation and on the 28th day after the transplantation. Results Serum levels of ADMA,SDMA,MDA and CRP were significantly increased, and the activities of nitric oxide (NO) and SeGSHPx were decreased in uremic patients compared with age matched healthy subjects (all P<0.01). Serum levels of ADMA,SDMA,MDA and CRP decreased while the activities of NO and SeGSHPx increased significantly at the first day after the transplantation (all P<0.01). The decrement of plasma SDMA normalized on the 28th day (P>0.05). The FMD was lower in the patients than the control group (P<0.01) and improved significantly on the 28th day of posttransplantation (P<0.05). Serum levels of ADMA were positively correlated with MDA (r=0.412, P<0.01;r=0.342,P<0.01) and negatively correlated with the values of SeGSHPx (r=-0.345, P<0.01;r=-0.315, P<0.01) and FMD (r=-0.452,P<0.01;r=-0.416,P<0.01) both before and after kidney transplantation. Conclusion The level of serum ADMA is associated with endothelial function improvement in uremic patients both before and after kidney transplantation.
8.Post-operative prevention of early complications of closed spinal dysraphisms in children

ObjectiveTo investigate the preventive strategy of early post-operative complications of closed spinal dysraphisms in children.MethodsOne hundred and nine children with closed spinal dysraphisms underwent surgery from January 2004 to December 2008,were enrolled in this study.After dural closure,the wound was washed completely with saline to clear the tissue debris to prevent postoperative infection. We routinely made a horizontal incision to prevent postoperative infection. Results After operation,all patients were recovered and were discharged from hospitalNo post-operative complications,including cerebrospinal fluid leakage, wound infection and flap necrosis, were encountered. ConclusionThe sophisticated microsurgical technique and the delicate surgical manipulation is the prerequisite for the prevention of early operational complication of closed spinal dysraphisms in children.
