1.The serum level of leptin before and after rosiglitazone administration in type 2 diabetes

Objective To observe leptin level before and after rosiglitazone administration and to explore the possible mechanism of rosiglitazone's improving insulin sensitivity.Methods A total of 30 patients newly diagnosed with type 2 diabetes were separated into three groups at random,and treated with rosiglitazone(4mg/d),metform(750mg/d) and gliclazide(160mg/d),respectively.Serum leptin,insulin and GHbA1c were measured after 12 weeks.Results After 12 weeks' treatment,the fasting glucose and GHbA1c in each group declined sharply,while free insulin did not change.The leptin level in rosiglitazone group decreased and insulin sensitivity was improved after administration,but there was no change in metformin and sulfonylureas groups.Conclusion Leptin may play a role in rosiglitazone's mediating the insulin-sensitizing effects.

3.Effect of medicated serum of Sangleng and Eshu on human umbilical vein vascular endothelial growth factor and vascular endothelial cells

Objective To explore the effect of medicated serum of Sangleng and Eshu on the expression of vascular endothelial growth factor and vascular endothelial cell proliferation induced by VEGF in vitro. Methods Medicated serum of Sangleng and Eshu was used to culture human umbilical vein endothelial cell (HUVEC-1) induced by VEGF. The morphologic changes of HUVEC-1 were observed with phase contrast microscope, and cell proliferation was detected by MTT method, and the expression of vascular endothelial growth factor protein and mRNA in endothelial cells was detected by Western blotting and RT-PCR. Results The medicated serum of 5.0, 2.5 g?kg~ -1 ?d~ -1 Sangleng and Eshu could cause arrangement disorder in the normal umbilical vein endothelial cells. Study the serum level of HGF, Cys C and TGF-beta1 in type 2 diabetic nephropathy (DN), the infect of Pingshen decoction on those index. Selected 69 cases of 2 type DN and randomly divided into therapy group (36 cases) and control group (33 cases). The therapy group were treated with Pingshen decoction 1 dose/d, bid po. The control group were treated with NephritisShu tablet, 6 tablet, tid po. 8 weeks was a course. Before and after treatment, we examine the serum level of HGF, Cys C and TGF-beta1 by ELISA and immunonephelometry, and compare with 30 cases of healthy control group. The study demonstrates that before treatment, the serum level of HGF in both groups were significantly lower than healthy control group (P < 0.01), but Cys C, TGF-beta1 were significantly higher (P < 0.01). After treatment, the serum level of HGF of both groups were increased. The serum level of HGF of therapy group were significantly higher than of control group (P < 0.01), but the serum level of Cys C and TGF-beta1 were significantly lower than control group (P < 0.01). The serum level of HGF was correlated negatively with Cys C,TGF-beta1. In control group, the UAER, urine beta2-MG and quantity of 24-hour urine protein were significantly decreased after treatment (P < 0.01). The index of urine of therapy group were significantly lower than control group (P < 0.01). Results indicate that test of serum level of HGF and Cys C,TGF-beta1 of diabetic nephropathy have important clinical significance. Pingshen decoction can effectively intervene in the serum level of HGF and Cys C, TGF-beta1 and index of urine.

5.Prevention effect of CAUTI by long-acting antibacterial material JUC : a meta-analysis

Objective The prevention effect of CAUTI by long-acting antibacterial material JUC was evaluated by meta analysis.Methods China digital hospital library full-text database,Chinese biomedical literature CD database,China hosptial knowledge database (CHKD) in China national knowledge internet (CNKI),VIP information Chinese science and technology periodical database were retrieved to fully collect the literature from 1995 to 2012 regarding JUC's prevention effect of CAUTI.The inclusion and exclusion standard of literature was made,and the effect index was made from the literature.Two researchers collected the literatures independently,and the included literatures were meta-analyzed by Review Manager 4.2 software,and the risk difference of infection rate and 95% CI were calculated in both the treatment group and control group.Those literatures which could not be meta-analyzed were performed descriptive analysis to evaluate the efficacy of JUC objectively.Results 20 related articles were retrieved,including 1 multicenter randomized control study,18 monocentric randomized control studies,and 1 uncontrolled study.3172 cases were included with 1526 cases in control group (usual care) and 1646 cases in treatment group (usual care with JUC).The difference between the two groups is significant (RD =-0.15,95% CI:-0.18 ~-0.13,P < 0.05).Conclusions Long-acting antibacterial material JUC can diminish the incidence of CAUTI significantly.
6.Efficacy comparison between hyperfractionated and conventional fractionated radiotherapy for non-small cell lung cancer.

BACKGROUNDTo investigate the efficacy of hyperfractionated radiotherapy (HRT) for non small cell lung cancer (NSCLC).
METHODSFifty patients with stage I-III NSCLC were treated in our department from Oct. 1995 to Jan. 2000. All patients had the pathological diagnosis. The patients were divided into two groups according to their entrance sequence: 25 patients were treated by HRT as the treatment group given the tumor doses (DT) 68.9-74.6 Gy/6-6.5 weeks, mean DT 70.3 Gy/6-6.5 weeks; 25 patients were irradiated by conventional fractionation (CF) as the control group given DT 64.2-70.4 Gy/6-7 weeks, mean DT 68.6 Gy/6-7 weeks. No patients received chemotherapy or immunotherapy before this treatment.
RESULTSThe effective rate after the radiotherapy was 80% (20/25) in the HRT group and 48% (12/25) in the CF group (Chi-square=5.56,P < 0.05 ). The 1-, 2-, 3-year survival rate of the HRT group was 68% (17/25), 60% (15/25) and 36% (9/25) respectively. The 1-, 2-, 3-year survival rate of the CF group was 52% (13/25), 32% (8/25) and 20% (5/25) respectively. The 2-year survival rate was significantly different between two groups (Chi-square=3.95,P < 0.05 ). The 1-, 2-, 3-year local progression free-rate of the HRT group was 72% (18/25), 60% (15/25) and 44% (11/25) respectively. The 1 , 2 , 3 year local progression free rate of the CF group was 64% (16/25), 48% (12/25) and 32% (8/25) respectively.
CONCLUSIONSAccording to the direct calculation, the efficacy of HRT for NSCLC is higher than that of CF. It is necessary to follow up the long term survival for the patients treated with HRT.
7.Association of the Pro12Ala and C1431T polymorphism of the PPAR gamma2 gene and their haplotypes with obesity and type 2 diabetes.

OBJECTIVETo study the association of the Pro12Ala and C1431T polymorphism of the PPAR gamma2 gene and their haplotypes with obesity and type 2 diabetes in Chinese population.
METHODSPCR-restriction fragment length polymorphism was used to determine the Pro12Ala and C1431T polymorphisms in 207 patients with type 2 diabetes and 101 non-diabetic control subjects.
RESULTS(1) In non-diabetic control population, the Ala allele frequency was 0.064, the T1431 allele frequency was 0.252. Haplotype analysis showed that the Pro12Ala and C1431T polymorphisms were in linkage disequilibrium (Do=0.63, r(2)=0.074), which constituted three major haplotypes Pro-C, Pro-T and Ala-T. (2) There were no significant differences of the distribution frequencies of the Pro12Ala and C1431T polymorphism and their haplotypes between the type 2 diabetes mellitus group and non-diabetic control group (P > 0.05). (3) The Pro12Ala polymorphism was associated with blood pressure and lipidemia in diabetic patients. The Ala allele significantly decreased the diastolic blood pressure of non-obese diabetic patients (P < 0.05), but it did not benefit to the obese diabetic patients for the lipidemia (P < 0.05). The C1431T polymorphism was associated with overweight and obesity in diabetic patients. The T1431 allele frequency in the body mass index > or = 25 layer was significantly higher than that in the body mass index < 25 layer (P < 0.05).
CONCLUSIONThe Pro12Ala and C1431T polymorphisms of the PPAR gamma2 gene might not be a major etiological factor for type 2 diabetes; the C1431T polymorphism was associated with overweight or obesity in diabetic patients.

8.Curative effect of the fibrinogen gel for sacral canal arachnoid cyst.

OBJECTIVETo evaluate curative effect of the fibrinogen gel for treating sacral canal arachnoid cyst.
METHODSNineteen patients with sacral canal arachnoid cysts included 7 males and 12 females; The average age was 48.4 years ranging from 19 to 68 years. The course was from 2 weeks to 7 months. Of all the patients, 9 were in level of S1, 4 were in level of S1 to S2, 5 were in level of S2, 1 was in level of S1 to S3. Cystis wall greater partial excision adopted in 11 cases, partial resection in 8, then all patients were treated by spray painting fibrinogen gel.
RESULTSNineteen patients were followed-up for 13 to 30 months (mean 21.3 months). The clinical symptom disappeared completely in 18 patients, and only one patient urinated incapably, but after 2 weeks returned to normal. No one found recurrence by MRI after 12 months.
CONCLUSIONThis method of fibrinogen gel for treating sacral canal arachnoid cyst has advantages of easy performing, safety, achieve good results, less neck symptoms and early commencing of mobilization.

9.Effect of factors on callus biomass and synthetic mass of hypericin in Hypericum perforatum.

OBJECTIVETo study the effect of several factors on the quantity of hypericin in H. perforatum callus.
METHODHigh efficiency liquid phase chromatography and plant tissue culture were applied.
RESULT AND CONCLUSIONWhen the ratio of nitro-nitrogen to amina-nitrogen is 3:1, the callus biomass is 1.6-fold and the synthetic mass of hypericin rises. 0.1-0.20 mg x L(-1) mannose improves the content of total hypericin. The addition of PVP or PVPP can promote improvement of the growth and biosynthesis of callus.

10.Study on the changes of incidence rates on birth defects through hospital based surveillance program in Guangdong province during 1997-2007

Objective To analyze the tendency of hospital based surveillance incidence rates of birth defects in Guangdong province to provide reference evidence for related intervention program. Methods Data from hospital based birth defects surveillance of Guangdong province during 1997-2007 were analyzed retrospectively. Results The overall surveillance rates of birth defects in Guangdong province showed an increasing trend form 98.9 per 10 000 birth in 1997 to 244.2 per 10 000 birth in 2007. Among the frequently seen birth defects, the rate of congenital heart diseases increased sharply, but the rates of congenital talipes equinovarus, polydactyly, cleft lip with/without cleft palate showed a slight increasing trend.It seemed that there was a significant difference between different maternal age interval and gender of the infants. Mother's illness, drug use, and exposure to environment harmful factors in early pregnancy stage, high maternal age, negative bearing history were the suspect risk factors for birth defects. Conclusion The increasing trend of surveillance rate of birth defects seemed to be comprehensively affected by the factors as: method being adopted, target and extension of birth defects surveillance, technology used for diagnosis, kinds of risk factor,and quality control on surveillance programs.
