1.CT diagnosis and different diagnosis of prostate cancer

Objective To study CT findings, diagnosis and differential diagnosis for prostate cancer. Methods 28 cases with prostate cancer confirmed by surgery and pathology, of them 4 cases were recurrence after operation. Results In 24 cases confirmed by surgery 21 (87.5%) were diagnosed by CT. Their CT appearances were : (1) Focus: in non enhanced scanning the prostate was enlarged unevenly in 18 cases. The density of the mass was slightly lower than the normal prostate in 17 cases, and equal to normal in 11 cases. Patchy calcification was seen in 2 cases moderate. In enhanced scanning, 22 cases showed nodular enhancement but slightly less than normal, 6 cases were equal to normal. (2) Metastastic lesions: 8 cases had local thickened bladder wall, 4 cases had enlarged seminal vesicle with unneven density, 2 cases with thickened anterior wall of rectum.And pelvis mass, enlarged lymph nodes, bony destruction of pelvis and pulmonary metastasses 1 case for each respectively. Conclusion (1) It is emphasized enhanced CT study should be a routine examination in the diagnosis of prostate cancer by CT; (2)CT can define the extent of the lesion and metastases thereby helpful to predict the prognosis and evaluate the effect of treatment.
2.Effect of CYP2A6 genetic polymorphisms on serum concentration of sodium valproate

Objective To investigate the effect of cytochrome P_ 450 2A6 (CYP2A6) genetic polymorphisms on serum concentration of sodium valproate. Methods A total of 98 epileptic patients receiving sodium valproate after a period of more than 5 half-time were recruited. The genotypes of CYP2A6 of the patients were detected by nested-primer polymerase chain reaction (PCR) to examine the alleles CYP2A6*1 and CYP2A6*4. Fluorescence polarization immunoassay (FPIA) was used to measure the serum concentration of sodium valproate. Results Of the 98 cases, 73 (74.5%) were wild genotypes, 24 (24.5%) were CYP2A6*1/*4 genotypes and 1(1.0%) was CYP2A6*4/*4 genotype. According to the genotypes of CYP2A6 the patients were divided into two groups,one was group A (CYP2A6*1/*1) and the other was group B (CYP2A6*1/*4 or *4/*4). 4.Approach to Complex Wound Management and Adjunct Therapy

The ageing of our population and rise in chronic diseases has resulted in the complex profile of the patients in the community. Complex wounds such as diabetic foot ulcers, infected pressure ulcers and other complications of non-healing wounds are common encounters in the primary health settings. The challenges of these complex wounds lie in its multi-factorial nature of the person, the wound and the environment. This requires a team approach to care within the limited resources boundary. As part of the care continuum, it is essential for primary care physicians to be familiarized with the approach to care of complex wounds and the adjunct therapy. This article seeks to provide a broad framework using the systematic assessment framework via T.I.M.E (Tissue, Inflammation/Infection, Moisture imbalance, Epithelial edge of wound) for wound bed preparation to guide primary care physicians/clinicians in their approach to complex wounds. It also highlighted the complexities of chronic wound management pertaining to the person, the wound and the environment as well as the recent advances adjunct therapy in chronic wound care. In addition, it seeks to enable primary care physician and wound clinicians to translate wound-healing principles into effective management strategies to provide better clinical care to our patients.

6.Shaping effects of multi-taper nickel-titanium files combined with nickel-titanium K file in simulated S-shaped canals

Objective:To evaluate the shaping effects of multi-taper ProTaper combined with nickel-titanium(Ni-Ti) K file in simulated S-shaped canals. Methods:Thirty simulated S-shaped resinous canals were randomly divided into three groups and prepared with hand ProTaper, hand Ni-Ti K file, hand ProTaper combined with Ni-Ti K file respectively. The shaping effects were assessed by the amount of material removed from the inner and outer wall of canal as well as the width of canal after instrumentation. Results: The shaping files of ProTaper removed the least material in amount of materials from the same apical dimension. The finishing files of ProTaper removed the most amount of material at the inner side of curvature and outer side of apex. ProTaper combined with Ni-Ti K file removed the least material at the above parts and the widths of canals prepared were gradually decreased from the orifice to the apex. Conclusion:ProTaper combined with Ni-Ti K file exhibited good shaping effects in simulated S-shaped canals.
7.Infarct location predicts progressive motor deficits in patients with infarct in the perforator territory of the middle cerebral artery

Objective To investigate the relationship between the pattern of infarct distribution on diffusion-weight imaging (DWI) and progressive movement deficits (PMD) in patients with infarct in the perforator territory of the middle cerebral artery (MCA). Methods The patients with new infarction in the perforator territory of MCA were analyzed retrospectively. PMD was defined as an increase of at least 2 points on the motor item of the National Institutes of Health Stroke Scale (NIHSS) score persisting for at least 24 hours within 5 days of stroke onset. The demographic characteristics, risk factors, clinical presentation, and distribution characteristics of the infarcts in the PMD and non-PMD groups were compared. Results A total of 64 patients were enrol ed in the study, including 38 females and 26 males, aged 67. 5 ± 10. 8 years. There were 28 patients in the PMD group and 36 in the non-PMD group. The proportion of the infarcts located in the posterior paraventricular region (67. 8% vs. 25. 0%; χ2 =11. 5, P<0. 05) of the PMD group and the mean baseline NIHSS score (6. 9 ± 1. 8 vs. 4. 3 ± 1. 2; t=2. 42, P<0. 05) were significantly higher than those of the non-PMD group. Multiple logistic regression analysis showed that the posterior paraventricular type infarcts had significantly independently correlation with PMD (odds ratio 6. 31, 95% confidence interval 2. 20-18. 0; P<0. 001). Conclusions The posterior paraventricular type infarcts on DWI can be used as a neuroimaging marker for predicting PMD in patients with infarction in the perforator artery territory of MCA.
8.Expression of ?-amyloid protein after reperfusion of transiently Incomplete forebrain Ischemia in rat hippocampus

Objective To elucidate the delayed neuron death and the expression of ?-amyloid protein (A?) in hippocampus after reperfusion of transiently ischemic lesion.Methods Immunohistochemical and HE technique was used to examine the delayed neuron death and expression of ?-amyloid protein at 6 h,2 d,3 d,7 d,14 d,35 d after reperfusion of transiently incomplete forebrain ischemia in rat hippocampus.Results Delayed neuron death was seen at 3 days after ischemia/reperfusin in CA 1 area of the hippocampus. A? immunoreactivity began enhanced at 2 d (A:0.082?0.011)after reperfusion, up to peak at 7 d(A:0.175?0.024), disapeared at 35 d.Conclusion Delayed neuron death was occurred after reperfusion of transiently incomplete forebrain ischemia in rat hippocampus,and at the same time, A? was up expression, which is suggested to be an important role of A? in neuron death after reperfusion of ischemic lesion.
9.Clinical analysis of 82 patients with myasthenic crisis

Objective To evaluate the precipitating factor,mortality,complications of myasthenia gravis crisis and risk factors of prolonged intubation.Methods Totally 82 case records of patients who had been adimitted for having 87 episodes of myasthenia crisis over a period of 15 years from January 1985 to February 2000 and their age, gender, precipitating factors, complications of intubation, and prolonged intubation factors were retrospectively reviewed. Results In 75% of myasthenic crisis patients having precipitating factors, infection was the most often served as the precipitating factors, and then the wound, fatigue, manopause and delivery came the next, 24% of precipitating factors were related to medication. Nine were identified with independent predictors of prolonged intubation. Among the 82 myasthenia crisis patients, 12 died, with a mortality of 14%.Conclusion The prevention and treatment for intubation complications may be the best way for further shortening intubation time and reducing the mortality.
10.Research progress of radionuclide labeled fibroblast activation protein targeted molecular probes

Fibroblast activation protein (FAP) is a marker protein of cancer-associated fibroblasts(CAFs). It is highly expressed in more than 90% of epithelial cancers and hardly expressed in normal tissues. Therefore, FAP is a very promising target. Radionuclide labeled FAP targeted molecular probes can be used for PET or SPECT imaging. In particular, 68Ga-FAP inhibitors (FAPIs) PET/CT has shown good prospects in the clinic, providing a new idea for early diagnosis, accurate staging and radionuclide treatment of tumors. In addition, FAP is also highly expressed in certain non-tumor diseases, especially those related to fibrosis. In this article, the research progress of radionuclide-labeled FAP targeted molecular probes is reviewed.
