2.Drug Utilization for 710 Convalescents in Haiqin Sanatorium Area of Our Hospital

OBJECTIVE:To probe into the status quo and requirements of drug utilization in Haiqin Sanatorium Area of Nanjing Military Region Hangzhou Sanatorium.METHODS:The drug utilization in 710 convalescents in Sanatorium Area from May 2007 to April 2008 was analyzed statistically.RESULTS:A total of 10 categories of drugs were used in the 710 convalescents,chiefly including anti-cold drugs,antihypertensive drugs,drugs for external use,antimicrobial agents,ophthalmic drugs,otorhinolaryngological drugs,vitamins and gastrointestinal drugs.The rate of drug use conforming diagnosis was 87.32%.Oral administration occupied 71.13%.CONCLUSION:The drug utilization in convalescents in Haiqin Sanatorium Area is basically rational.However,there are also some unreasonable points in the drug utilization which remain to be tackled further.
3.STUDY OF SERUM TRAb IN AUTOIMMUNE THYROID DISEASE WITH ABC- ELISA

In this paper the investigation of using a new method-ABC-ELISA in assay of Autoimmune Thyroid Disease are presented. The sensitivity of ABC-ELISA is compared with that of standard ELISA; Its reliability is proven by the methods of detecting TSAb with FRTL-5. TRAb is detected by ABC-ELISA in 91% of untreated Graves'. TRAb is detected by Standard ELISA in 70% of untreated Graves'. The results of ABC-ELISA in 26 untreated Graves' are equal to that of the method of detecting TSAb with FRTL-5. Therefore, we consider that ABC-ELISA is a sensitive, reproducible, convenient method applicable to clinical practice.
4.Protective effect of adenosine on temporary threshold shift of audition caused by intensive white noise in guinea pigs

Objective: To investigate the protective effect of adenosine on hearing in acute acoustic trauma and its modulatory action in the cochlea. Methods:Guinea pigs were exposed to 118 dB SPL white noise for 30 min. During the process of exposure, artificial perilymph or artificial perilymph containing 1 mmol/L adenosine was infused into the cochlea through perilymphatic perfusion. Thresholds of compound action potentials (CAP) evoked by click and tone burst were measured before and after noise exposure. Results: Immediately after exposure, the mean threshold shifts evoked by click of the control group and adenosine treated group were (40?6.68) dB and (20?5.70) dB respectively. The threshold shifts evoked by tone burst at 2 8 kHz in the control animals were 32 50 dB, while in the adenosine treated animals, 20 35 dB. There were significant differences between the 2 groups except that at 6 kHz. Two hours after exposure, there was an increase of 4 8 dB at all frequencies in the control group; no changes in the adenosine treated group were found. There were significant differences in threshold shifts between the 2 groups at all test frequencies. Conclusion: Adenosine has protective effect on temporary threshold shift in acute acoustic trauma in guinea pigs. [
5.Analysis on minimum alveolar effective concentration of sevoflurane for laryngeal mask airway insertion under general anesthesia in premature infants

Objective To determine the minimum alveolar concentration(MAC) of sevoflurane without body movement during laryngeal mask airway(LMA)intubation in premature infants less than 37 weeks of corrected gestational age undergoing total inhalation general anesthesia induction.Methods Twenty-one ASA Ⅰ or Ⅱ premature infants less than 37 weeks of corrected gestational age undergoing elective inhalation general anesthesia were enrolled in this study.At first,the general anesthesia induction was started by inhaling 6 % sevoflurane.After the premature infant lost consciousness,the end tidal sevoflurane concentration(ET-sev)was adjusted to the predetermined concentration and maintained stable for 15 min.After that,LMA was inserted.The up-anddown sequential allocation was used to determine MAC.The initial ETsev was 2 %,which was increased or decreased by 1 gradient concentration in the next case according to the LMA insertion body movement response.The adjacent concentration gradient was 0.2%.The midpoint from th body movement response to non-body movement response was set as the balance point and the mean value of the concentrations of sevoflurane at all the balance points were calculated as MAC.Results The end tidal sevoflurane con centration without the body movement responses to LMA insertion was 1.71%.Conclusion The MAC of sevoflurane without the body movement responses to LMA insertion in premature infants less than 37 weeks of corrected gestational age is 1.71%,which is lower than that in the normal children and probably because imperfect central nervous system development in premature infants.
6. The role of radiotherapy in type B3 thymoma

Objective: To investigate the biological behavior and prognosis factors of type B3 thymoma, and evaluate the efficacy of radiotherapy in patients with type B3 thymoma. Methods: A total of 188 consecutive patients with pathologically confirmed type B3 thymoma were treated in Shanghai Chest Hospital from January 2001 to December 2010. According to Masaoka stageing criteria, 35 (20%) were stage, 20 (11%) were stage, 78 (44%) were stage, and 44 (25%) were stage. Myasthenia gravis coexisted in 54 patients. Results: After a median follow-up of 49 months (7-135 months), the 5-year overall survival rate was 84% (100.0%, 89.0%, 86.1% and 59.8% of stages and respectively), and the 5-year progression-free survival rate was 65% (100.0%, 94.0%, 67.3%, and 33.0% of stages, and respectively).The reccurence rate was 22% and the metastatic rate was 7%. In the univariate analysis, tumor size, Masaoka stage, resection margin, radiotherapy and chemotherapy were significantly associated with 5-year overall survival and progression-free survival (P < 0.05). In the multivariate analysis, Masaoka stage, resection margin and radiotherapy were independent prognostic factors of 5-year progression-free survival (P < 0.05). Radiotherapy could improve the regional control rate and the overall survival of Masaoka stages ?-? patients. The intensity-modulated radiation therapy of pleure could probably improve the overall survival of recurrent patients. Conclusion: The major failure mode for type B3 thymoma is the recurrence of pleure. Radiotherapy can improve the regional control rate and the overall survival of the advanced patients. The intensity-modulated radiation therapy may be the best therapy for the postoperative recurrence of pleure. Copyright © 2013 by TUMOR.
7.Clinical Analysis of 83 Cases of Hyperosmolar Glucose in Prevention of Persistent Ectopic Pregnancy after Laparoscopic Salpingostomy

Objective: Medical therapy to prevent persistent ectopic pregnancy (PEP) after conservative laparoscopic operation was investigated. Methods:236 patients with ectopic pregnancy were divided into three groups according to the time of hospitalization. 83 cases were in group A, hyperosmolar glucose was injected into the fallopian tube lumen after laparoscopic salpingostomy. 81 cases were in group B, MTX was injected in the same way, 72 cases were in group C as control group. Serum β-HCG was tested for four times, before the operation, the first, third and a week after the operation respectively. Results:There were 6 patients of PEP in this study. One patient in group A, one patient in group B, the rest four patients in group C. The incidence of PEP was not statistically significant between group A and B( P>0. 05), while the difference was significant between group A and C ( P<0. 05). Serum β-HCG in three groups descended significantly in the first 24 hours after operation, but there were no differences among these groups. As for the descending of β-HCG in day 3 and 7, there was no difference between group A and B, but the difference was magnificent between group A and C( P < 0.05). Conclusions: Compared with the MTX, hyperosmolar glucose is an effective and safer method to prevent PEP.
8.Analyzing the risk factors and related psychological factors in patients with gastroesophageal reflux disease and surveying the curative effect

Objective To analyze, the risk factors and related psychological factors in patients with gastroesophageal reflux disease (GERD)and survey the curative effect. Methods All of 200 outpatients with GERD(GERD group) and 200 healthy persons (control group)from January 2006 to January 2008 were randomly selected, and the personal data were investigated. The GERD questionnaire (RDQ) and the Zung-self-rating depression scale (SDS) test were used. Results In GERD group, the proportions of high education, city living, marriage instability ,heavy working pressure, high body mass index (BMI),smoking and drinking habit, peptic disease in family, using nonsteroidal anti-inflammatory drugs (NSAIDs) were higher than those in control group (Pall < 0.01). After treatment, the patients in GERD group had lower scores in the RDQ and SDS test than those before treatment (P< 0.01). Conclusions There are many risk factors in patients with GERD, especially the psychological factors. The therapy of GERD should combine with psy-chotherapy and avoid high BMI, smoking, drinking and overusing the NSAID.
9.Effects of Huoxue Bushen Mixture on skin blood vessel neogenesis and vascular endothelial growth factor expression in hair follicle of C57BL/6 mice

OBJECTIVE: To investigate the possible stimulating mechanism of Huoxue Bushen Mixture (HXBSM), a traditional Chinese compound medicine, on hair growth of mice via measuring the variance of skin blood vessel neogenesis and vascular endothelial growth factor (VEGF) expression in the hair follicle. METHODS: Hot rosin and paraffin mixture depilation were used to induce C57BL/6 mice hair follicle to enter from telogen into anagen. Ninety C57BL/6 mice were divided into 3 groups randomly: HXBSM group, Yangxue Shengfa Capsule (YXSFC, another traditional Chinese compound medicine) group and untreated group. The mice were fed with corresponding drugs after modeling. The hair growth of the mice was observed every day. Every ten mice out of each group were executed respectively at day 4, 11 and day 17. Skin blood vessel neogenesis was counted through pathological section and VEGF expression in the hair follicle was measured via immunohistochemical method. RESULTS: The number of local blood vessel neogenisis in the HXBSM group observed was larger than that in the untreated group at day 4 (P<0.05); and evidently larger than that in the YXSFC group and the untreated group at day 11 (P<0.05). The expression of VEGF in the hair follicle was distinctively higher than that in the YXSFC group and the untreated group at day 11 and day 17 (P<0.05). CONCLUSION: HXBSM up-regulates VEGF expression to accelerate blood vessel neogenesis and hair growth.
10.Study on quality standard for Jixiangcao Buccal Tablets

AIM: To establish the quality standard for Jixiangcao Buccal Tablets(Herba Reineckea Camea, Radix Asteris, Heaba Houttugnlae, Pericarpium Papaveris, Heaba Ephedrae etc.). METHODS: TLC was used for the identification of Herba Reineckea Camea and Pericarpium Papaveris. HPLC was used for the determination of ephedrine hydrocholoride in the Buccal tablets. RESULTS: TLC identification was highly specific and the spots were clear. The linear range for ephedrine hydrocholoride was in the range of 0.404~2.02?g and its average recovery was 97.03% and RSD 2.2%, respectively. CONCLUSION: The quality standard is able to effectively control the quality of Jixiangcao Buccal Tablets.
