1.A case report of Von-hippel-Lidau disease

The authors report a case of Von hippel Lindau disease in a 20 years old patients with presenting signs: diminution of visual acuity, intracramial hypertension with a left cerebollar syndrome and existence of an retinal angioma on the right side and papilla oedema on the left side. A scanning examination revealed a cystic tumor with a solid little mass. Angiography showed a capillary nodus coressponding to the mass lesion, with feeding arteries coming from the anterio superior cerebellar artery. Removal of the cyst and the tumor was successful. Pathological examination: Hemangi oblastoma. Negative ultrasound examination of the kidney, pancreas.

2.Role of ocular melanin in ocular drug delivery

Ocular melanin is mainly distributed in the uveal tract (including iris,ciliary body and choroid) and the retina pigmented epithelial (RPE) layer.Many drugs from different therapeutic classes can bind to melanin.Examples include antibiotics,anesthetics,β-blockers,corticosteroids,antimalarial drugs and so on.The critical factors are the acid/base status and the lipophilicity of the molecule.Lots of literatures,which refer to pharmacokinetic and pharmacodynamics of drugs from different administration routes,have demonstrated that the binding of drugs to melanin plays an important role in the drug delivery.Not only it may increase initial concentration of drug in the tissues,but also the reversible binding of most drugs to melanin result in a slow release of drugs from melanin,thus it would prolong the action time to potentially beneficial or adverse effects.In addition,the melanin content differs by species and races,even by regions of an eye,which results in different binding capacity of a drug in each individual.Nonetheless,further understanding the melanin-drug binding can provide scientific insight and guideline for transscleral ocular drug delivery.
3.Progress in the adrenal insufficient and adrenal crisis of critical children

Recent studies have showed that adrenal insufficiency are common in some status of critically ill children,like systemic inflammatory response syndrome,sepsis,shock,et al.The frequency of adrenal crisis was also higher than expectation.The currrent glucocorticoid replacement therapies are inclined to lower dose [ hydrocortisone 20 ~ 25 mg/( m2· d) ].The adrenal function could be evaluated with the free cortisol index and calculated free cortisol.A new method,temperature-controlled centrifugal ultrafiltration with chemiluminescence immunoassay,has been used to detect the free cotisol,which has improved the clinical practicality.
4.Research on the application value of the dynamic ECG monitoring in the diagnosis on patients with sleep ap-nea syndrome complicated with arrhythmia

Objective To investigate the clinical application value and feasibility in the dynamic ECG moni-toring in the diagnosis on patients with sleep apnea syndrome complicated with arrhythmia .Methods A total of 80 cases of sleep apnea syndrome confirmed by polysomnography diagnosis were randomly divided into the two groups,40 cases in each group .The patients in the control group were received routine ECG monitoring ,the observa-tion group were underwent dynamic ECG monitoring ,the arrhythmia detection rate in the two groups were compared . Results The ventricular premature beat ,atrial premature beat,ventricular in patients with Beckoning tachycardia , paroxysmal ventricular tachycardia , paroxysmal atrial tachycardia , first degree atrioventricular block , second degree atrioventricular block,premature atrial arrhythmia with sinoatrial block detected in the control group were 6 cases, 7 cases,9 cases,6 cases,5 cases,1 cases,1 cases,5 cases,respectively.However,21 cases,19 cases,25 cases, 16 cases,19 cases,7 cases,8 cases,14 cases were confirmed in the observation group ,the positive detection rate in cardiac arrhythmias in the observation group were significantly higher than that in the control group (χ2 =19.700, 17.000,13.700,21.200,22.600,53.000,50.500,26.100,all P<0.05).Conclusion Dynamic electrocardiogram can improve the positive rate of arrhythmia ,and have important clinical value in the diagnosis and treatment of sleep apnea syndrome complicated with cardiac arrhythmias ,which is worthy of clinical application .
5.Esophagus memory Ni-Ti alloy stent for malignant stenosis, esophagus and anastomotic stoma fistula in 60 cases

Sixty patients with malignant stenosis, esophagus and anastomotic stoma fistula were selected from Jiangsu General Hospital of Armed Police Forces between January 1995 and June 2008, including 46 males and 14 females, aged 36-77 years. Following pathologic diagnosis, 50 patients had squamous cell carcinoma, 8 had adenocarcinoma, and 2 had adeno-squamous carcinoma. Of 60 patients, 33 had esophageal carcinoma stenosis and esophagus-gastric cardia carcinoma stenosis, 20 had esophageal carcinoma stenosis combined with fistula, 7 had esophageal-stomach anastomosis combined with thoracic cavity fistula; in addition, there were 22 cases of first onset of fistula and 5 of multiple fistula. All patients were treated by esophagus memory Ni-Ti alloy coated stent implantation. Of 58 patients with successful implantation, 21 developed stent blockage due to dysphagia, and were alleviated after foreign body removal; 3 developed stent dislocation (removal and re-implanted); 7 developed restenosis due to tumor tissue ingrowth 3 months after implantation. Of 5 patients with multiple fistula, 3 were re-implanted to form invagination of 2 stents, which succeeded in blocking the fistula. Of 33 patients with esophageal carcinoma stenosis and esophagus-gastric cardia carcinoma stenosis, 26 underwent radiotherapy with or without chemotherapy (mean survival time: 24 months). Twenty patients with esophageal carcinoma stenosis combined with fistula survived for 6 months averagely. Seven patients with esophageal-stomach anastomosis combined with thoracic cavity fistula survived for 3 months averagely. Of 60 patients, 42 felt retrosternal pain, and 2 patients’ stents were extracted due to severe pain; 20 patients developed esophageal reflux. No bleeding, perforation or septicemia occurred. The results indicate that memory Ni-Ti alloy stent is effective in treating malignant stenosis, esophagus and anastomotic stoma fistula, showing good biocompatibility.

7.Application of serum PGⅠ,CEA and G-17 in assessment of chemotherapy effect in patients with gastric cancer

Objective To discuss the application value of serum pepsinogen Ⅰ(PGⅠ),carcinoembryonic antigen(CEA) and gastrin-17(G-17) in assessment of chemotherapy curative effect in patients with gastric cancer.Methods From Feb.2014 to Jun.2016,a total of 90 patients with gastric cancer were selected,and treated with docetaxe/cisplatin/5-fluorouracil(DCF) scheme for two courses.All patients were divided into effective group and ineffective group,according to the assessment criterion,proclaimed by World Health Organization.Serum levels of CEA,PGⅠand G-17 were detected before and after treatment.Curative effect was also assessed according to the changes of CEA,PGⅠand G-17.Receiver operating characteristic curve(ROC curve) was used to analyze the performance of PGⅠ,CEA and G-17 in the assessment of curative effect.Results Chemotherapy was effective in 29 cases,accounting for 32.22%.After chemotherapy,serum CEA and G-17 level of effective group were lower than ineffective group,while PGⅠ level was higher(P<0.05).ROC curve analysis showed that the area under ROC curve,sensitivity and specificity of combined detection for the assessment of curative effect were higher than single detection of serum PGⅠ,CEA and G-17,taking 70 g/L,7.35 g/L and 14.22 pmol/L as cut-off value(P<0.05).Conclusion Combined detection of serum PGⅠ,CEA and G-17 could be with high accuracy,specificity and sensitivity for the assessment of chemotherapy effect in patients with gastric cancer.
8.Prudent use of SCI and IF in medical research organizations

It is a popular practice for Chinas academia to regard SCI(Science Citation Index)and its IF(Impact Factor)as fundamental indexes for research competency evaluatiop.This article however holds that SCI and IF are but quantitative measurements for evaluating periodicals,and whether it is scientific to evaluate research results with such indexes as they are remains to be jusfified.It also suggests that practitioners in research organizations fully comprehend the meaning of SCI and IF,and use them correctly,so as to guide researchers to contribute their works to proper publishers.
9.A comparison of two methods for anti-human immunodeficiency virus antibody of double-antigen sandwich assay

Objective To investigate the sensitivities and specificities of enzyme-linked inununosorbent assay(ELISA)and immunogold labeling technique in the detection of anti-human immunodeficiency virus antibody.Methods(1)Sensitivity test:15 sera from patients with acquired immunodeficiency syndrome(AIDS)diagnosed by western blotting were diluted proportionately,we detected the anti-human immunodeficiency virus antibody in all diluted sera using ELISA and immunogold labeling technique,and recorded the maximum dilutions of the two methods that the anti-human immunodeficiency virus antibody was positive.(2)Specificity test:200 sera from patients with nonAIDS were detected by ELISA and immunogold labeling technique,and we recorded the negative rates of the two methods.Results(1)Sensitivity test:The maximum dilution of ELISA was significantly higher than that of immunogold labeling technique(P < 0.05).(2)Specificity test:The negative rates of the two methods were 100%,and there was no significant difference between the two methods.Conclusion The sensitivity of ELISA in the detection of antihuman immunodeficiency virus antibody is better than that of immunogold labeling technique,but the sensitivities and specificities of the two methods are consistent when the serum is detected,so the immunogold labeling technique is able to replace ELISA as a screening test for AIDS.
10.Comparison of clinical effectiveness of peripherally inserted central catheters and peripheral intravenous catheters in tumour chemotherapy

Objective To investigate the clinical effectiveness of peripherally inserted central catheters(PICC) and peripheral intravenous catheters(PIV) in tumor chemotherapy. Methods A total of 180 patients with intravenous chemotherapy were randomly divided into two groups. 108 patients in the PICC group, 72 patients in the PIV group. The holding time and complications were observed and compared. Results The rate of complication in the PICC group were much lower than the PIV group, however was much longer than the PIV group in holding time. But there was no statistical difference in the positive culture rate of bacteria of two groups. Conclusions It is very safe to use PICC which can reduce the rate of complication of catheters and venipuncture times and is worthy of spreading in practice.
