1.Investigating tolerance ability for early enteral nutrition in severe burned patients

Aim: to investigate the tolerance for early enteral feeding of severe burn patients during 72h postburns at the National Institute of Burns. Method: 41 adullt burn patient admitted to the National Institute of Burn during 72h postburn with a mean of burn area was 49.87% and 15.47% of full thickness area. Naso-gastric tube was inserted as soon as admission, enteral feeding was begun by nutrition pump with Vivonex and then Ensure. Energy requirement was calculated by using curerel formular. Tolerance was canculated as actual volume compared to theory demand. Volume of gastric residual was measured at 2.6 and 24 hour of enteral nutrition. Result: the tolerance was 95.58% success with rare complication including tension apart from diarrhea rate of 19.51%, no case of aspiration was recorded. The tolerance was more successful and less complication was seen in patients fed during the first 6h postburn (p< 0.05). There was no significant relation between diarrhea rate and burn severity as well as plasma albumin level post burn. Conclusion: early enteral feeding is well tolerated with little complication. This method is safe, easy to set up and should be wildly applied for burn patients.

2.Epidemiological characteristics of trauma in labor accidents at the Medical Appraisal Council of Thanh Hoa province between 1995 to 1999.

The trauma related to traffic and fall accident accounting for majority of trauma due to labor accident (34.93% and 34.33%, respectively). the rate of trauma in males is triple than in females. It mainly occurs in the laboring age group (from 31 to 50 years old), focuses to 3 occupations, including mechanical, exploring workers and drivers. The distribution of labor accident related trauma is similar over 4 seasons, with slightly higher in spring and summer. The common places is working place and in the way between home and working place. The common injuries include foot bone fracture (19.85%), soft tissue and skin injuries (17.46%) and head trauma (15.07%).

3.Primarily results of removal surgery of stone in solitary kidney

During 5 years (1994-1998), 28 patients with renal and ureteral stones in solitary kidney were operated at the surgical department of §µ N½ng hospital, including congenital solitary kidney: 3 patients (10.7%). Acquired solitary kidney: 1 patient (3.5%). Functional solitary kidney: 24 patients (85.7%). Among them: renal stone: 27 patients (96.4%), Staghorn stone: 14 patients (50%). Pyelotomy and nephrostomy account for 68% of cases, associated with positive drainage of pelvis giving favourable results: No need for blood transfusion: 24 patients (86%). Residual stones: 1 patient (3.5%). Post-operative bleeding: 2 patients (7.0%). Short-term follow up: Good (78.6%), medium (14.0%), Bad (7.0%)

4.Preliminary results of supported chemotherapy in treatment of operated gastric cancer in Hai Phong (from January, 2001 to June, 2006)

Background: Gastric cancer is the second common disease among types of cancer, followed by lung cancer for male and breast cancer for female. The selection of appropriate regimens for illness and economic conditions of patients is always a difficult problem for clinicians. Objectives: To assess preliminary results of chemotherapy in treatment of gastric cancer in Hai Phong. Subjects and method: A longitudinal, cross-section study was conducted on 57 patients with gastric cancer (35 male, 21 female), treated in Viet Tiep hospital from January, 2002 to June, 2006. All of them were in stage of Dukes C, treated postoperative supported chemotherapy. Results: Side effects were common seen as vomit, nausea and diarrhea. They were often mild and disappeared after stopping medication. 30/57 patients responded to treatment. 21 patients were treated with regimen 5FU, however, amount of treatment cycles was very scattered and treatment effect was unclear. 36 patients were treated with multi-chemical regimens as FUFA, FAM and ELF. Their response to treatment was more clearly than regimen 5FU. During treatment, there were 6 patients who had not completely responded to treatment. In which, 4 patients developed more seriously: metastasis to liver, ovary, bone and lung. 2 patients had died. Conclusion: Regimen 5FU had a little toxicity. Regimen FUFA was more effective. The role of chemotherapy in supported treatment was unclear. Chemotherapy could be applied to patients with early stage of Dukes B.

6.Anaplasma marginale and A. platys Characterized from Dairy and Indigenous Cattle and Dogs in Northern Vietnam

Anaplasma marginale and A. platys were detected and characterized (16S rDNA sequence analysis) from dairy and indigenous cattle, and the latter in domestic dogs in Vietnam. A phylogenetic tree was inferred from 26 representative strains/species of Anaplasma spp. including 10 new sequences from Vietnam. Seven of our Vietnamese sequences fell into the clade of A. marginale and 3 into A. platys, with strong nodal support of 99 and 90%, respectively. Low genetic distances (0.2–0.4%) within each species supported the identification. Anaplasma platys is able to infect humans. Our discovery of this species in cattle and domestic dogs raises considerable concern about zoonotic transmission in Vietnam. Further systematic investigations are needed to gain data for Anaplasma spp. and members of Anaplasmataceae in animal hosts, vectors and humans across Vietnam.

Even though the combined use of ultrasound (US) and alpha-fetoprotein (AFP) is recommended for the surveillance of hepatocellular carcinoma (HCC), the utilization of AFP has its challenges, including accuracy dependent on its cut-off levels, degree of liver necroinflammation, and etiology of liver disease. Though various studies have demonstrated the utility of protein induced by vitamin K absence II (PIVKA-II) in surveillance, treatment monitoring, and predicting recurrence, it is still not recommended as a routine biomarker test. A panel of 17 experts from Asia-Pacific, gathered to discuss and reach a consensus on the clinical usefulness and value of PIVKA-II for the surveillance and treatment monitoring of HCC, based on six predetermined statements. The experts agreed that PIVKA-II was valuable in the detection of HCC in AFP-negative patients, and could potentially benefit detection of early HCC in combination with AFP. PIVKA-II is clinically useful for monitoring curative and intra-arterial locoregional treatments, outcomes, and recurrence, and could potentially predict microvascular invasion risk and facilitate patient selection for liver transplant. However, combining PIVKA-II with US and AFP for HCC surveillance, including small HCC, still requires more evidence, whilst its role in detecting AFP-negative HCC will potentially increase as more patients are treated for hepatitis-related HCC. PIVKA-II in combination with AFP and US has a clinical role in the Asia-Pacific region for surveillance. However, implementation of PIVKA-II in the region will have some challenges, such as requiring standardization of cut-off values, its cost-effectiveness and improving awareness among healthcare providers.
9.Prevalence of Opisthorchis viverrini-Like Fluke Infection in Ducks in Binh Dinh Province, Central Vietnam.

Following the first report of Opisthorchis viverrini infection in a domestic duck in Phu My District of Binh Dinh Province, Central Vietnam, many other cases were observed in the province. We determined the infection rate and intensity of O. viverrini infection in ducks in 4 districts of the province. A total of 178 ducks were randomly selected from 34 farms for examination of flukes in the liver and gall bladder. An infection rate of 34.3% (range 20.7-40.4% among districts) was found; the intensity of infection was 13.8 worms per infected duck (range 1-100). These findings show the role of ducks as a host for O. viverrini, duck genotype, which is sympatric with the human O. viverrini genotype in this province. It also stresses the need for investigations on the zoonotic potential and the life cycle of this parasite.

10.Isolation, characterization and neutralizing activity of porcine epidemic diarrhea viruses from Vietnam

Porcine epidemic diarrhea (PED) is characterized by acute enteritis, watery diarrhea, weight loss, dehydration, and death with high mortality in neonatal piglets. In this study, 3 virus isolates collected in Vietnam between 2016 and 2017 were successfully propagated in Vero cells at high virus titers. Sequence analysis of the full-length spike (S) gene revealed that all 3 isolates belong to genogroup 2a, which is closely related to other prevalent Asian strains. Amino acid sequence comparisons revealed 98.19% to 99.13% homology with the Vietnam isolates circulating during 2013–2015, suggesting that field PED viruses (PEDVs) evolve continuously. Experiments in animals demonstrated that antisera from guinea pigs immunized with the vaccine strain resulted in higher levels (5 log2) of neutralizing antibody against the homologous strain, and showed a relatively lower level of neutralizing antibody against the field isolates. This finding would be helpful in choosing a PEDV strain for vaccine development.
