1.Job satisfaction and influencing factors for community health service practitioners in Futian District of Shenzhen

Three hundred and nine community health service (CHS) practitioners were selected with randomized cluster sampling for survey with the job satisfaction questionnaire-occupational stress indicator (OSI).Degree of the job satisfaction was (68 ± 12)%.Position, work experience, income,feeling of recognition, degree of motivation, the scope for career development, the space for personal ambition and the relationship between salary and work load were potential influencing factors of job satisfaction to CHS practitioners.The authors suggest to establish the dynamic mechanisms of satisfaction study and to formulate comprehensive and effective incentive measures for CHS practitioners.
2.Immediate effect of percutaneous ventricular restoration on cardiac output of ischemic cardiomyopathy patients with apical aneurysm

Objective To evaluate the immediate effect of percutaneous ventricular restoration (PVR) using the PARACHUTE system in ischemic cardiomyopathy patients with apical aneurysm.Methods The study included 25 patients who received PARACHUTE partitioning device (PVD) implantation in the Xiamen Cardiovascular Hospital between January 2015 to December 2016.Invasive left ventricular hemodynamic assessments as well as cardiac output and cardiac output index were analyzed.Results Twenty-five patients [mean age (65.4±11.9) years] suffered from left ventricular aneurysm and heart failure patients after anterior myocardial infarction were enrolled.Ventricular partitioning device implantation was successful in 24/25 (96.0%) patients.PVR was failed in 1 patient due to unable to land the PVD in a satisfactory location.After implantation, a significant increase in cardiac output [(3.83±0.72) L/min vs.(4.85±0.93) L/min, P<0.01] and cardiac index [(2.32±0.74) L/(min·m2) vs.(2.90±0.82) L/(min·m2), P<0.01] was found.Conclusions Our preliminary experience on percutaneous ventricular restoration using PARACHUTE system demonstrates its feasibility and safety with increase in cardiac output and cardiac index immediately following the device implantation.
3.Effects of interferon-?, tumor necrosis factor-? and interleukin-1? on apoptosis of human airway smooth muscle cells in vitro

AIM:To clarify if interferon-?(IFN-?), tumor necrosis factor-?(TNF-?)and interleukin-1?(IL-1?)can induce apoptosis of human airway smooth muscle cells (ASMCs) in vitro. METHODS: Human ASMCs were isolated and cultured in DMEM containing 10% fetal bovine serum. Passage 4-6 cell was used in the experiment. IFN-?,TNF-? and IL-1?, were used separately or together in the treatment of human ASMCs. The effects of IFN-?,TNF-? and IL-1? on the growth of the cells was detected by MTT method at the hour 0,24,48 and 72. Light microscopy and electron microscopy were used to examine the morphological change. DNA fragmentation was analyzed by agarose gel electrophoresis. SP immunohistological staing method was performed to detect the change of expressions of p53 , bcl-2 and bax gene. The apoptosis cell percentage were detected by in situ end labeling technique (TUNEL)of fragmental DNA. RESULTS: (1)IFN-? or IFN-? together with TNF-? and IL-1? decreased the number of viable cells in a time dependent manner. (2) Light and electron microscopic examination showed cell shrinkage, membrane blebbing, nuclear contraction, chromatin condensation and nuclear fragmentation in human ASMCs. (3) Agarose gel electrophoresis showed a characteristic"ladder"of DNA bands representing integer multiples of the internucleosomal fragments (about 180-200 bp) in cytokine cotreated human ASMCs. (4)The expression of p53 and bax gene in cytokine cotreated group was significantly higher than in control group, but the expression of bcl-2 gene was lower than in control group. (5)Stimultaneous treatment with IFN-?(4?10 5 U/L),TNF-?(4?10 5 U/L)and /or IL-1? (10?10 4 U/L) induced apoptosis of human ASMCs. Objective: To discuss the entry point of clinical pharmacists for providing pharmaceutical care in clinics. Methods:Clinical pharmacists participated in making drug treatment regimen for one child with nephritic type NS combined with hypertension,and provide pharmaceutical care and medical education for the patient. Results: According to the situation of the patient, the treatment regimen was made out, evaluated and adjusted, and ADR was dealt with in time, therefore, the individualized treatment showed obvious effect, and the child was discharged with an improved condition. Conclusion: Clinical pharmacists can participate in clinical treatment with pharmaceutical professional knowledge to ensure safer and more effective drug use in child patients.
5.Identification and characterization of ovarian cancer stem-like cells from primary tumor

Objective To study whether the primary ovarian cancer cells containing cancer stem cells and its characterization in serum-free culture condition.Methods The primary cancer cells were isolated from one stage Ⅲ, grade 2 serous adenocareinoma tissue.Cells were cultured in serum-free culture system supplemented with epidermal growth factor,basic fibroblast growth factor,leukemia inhibitory factor and insulin.or standard serum-containing system.Methyl thiazolyl tetrazolium assay,quantitative PCR analysis,flow-cytometric analysis and xenograft experiments in vivo were performed.Results The primary cancer cells could maintain and forill cell sphere in serum-free culture system.These cells had the properties of self-renewal,overexpression of stem cell marker genes Nanog,Oct-4,Sox-2,nestin,ABCG2,CD_(133) and CD_(117) By contrast with the difierentiated cells under standard serum-containing culture conditions.these sphere-forming cells were more resistant to cisplatin and paclitaxel after treated 48 and 72 hours(61% vs.31%,73% vs.29%,P<0.05).With Hoechst 33342 exclusion assay,only 21.83%of sphere-forming cells were positive with the dye,compared with 83.04% positive cells in differentiated cells (P<0.01).Only 500 sphere-forming cells resulted subcutaneous xenograft tumors.All of these xenografts were categorized as serous adenocareinomas, overexpression of CA_(125) and cytokeratin-7 which were original tumor phenotype of ovarian cancer. Conclusion The sphere-forming cells isolated from primary ovarian cancer tissues have the characterization of cancer stem cell and may be a more reliable model system for understanding the biology of primary human tumors.
6.Study on Purpurin As A G Protein-coupled Receptor-35 Agonist

Purpurin is a common component ofRubia cordifolia L. The study on its molecular target was useful for elucidating the therapeutic material basis and action mechanism ofR. cordifolia. HT-29 cells were used in the cell culture. The highly expressed G Protein-coupled Receptor-35 (GPR35) agonist was used as target. The label-free optical biosensor cellular assay was used to investigate the agonist activity ofpurpurin at an endogenous receptor. The results showed thatpurpurin can cause DMR response in HT-29 cells. And the DMR response curve type was consistent with zaprinast. Its EC50 was 6.142± 0.189μmol·L-1. In addition,purpurinhad desensitization effect on GPR35 agonist zaprinast in HT-29 cells. GPR35 agonist ML145 blocked the DMR ofpurpurin. It was concluded thatpurpurinwas the GPR35 agonist.
7.A novel microtiter plate radioimmunoassay of insulin autoantibody

Objective Insulin autoantibody (IAA) is known to exist in sera of type 1 diabetes mellitus (T1DM) patients and pre-T1DM individuals. The aim of this study was to establish a novel microtiter plate radioimmunoassay (RIA) for IAA and evaluate its clinical value. Methods Diluted 125Ⅰ-insulin was mixed with 5 ul serum samples in a 96-well microtiter plate and then incubated for 72 h on an orbital plate shaker (4℃). The immunocomplexes were transferred to another protein a coated Millipore plate, and then the plate was washed with Tri-Buffered Saline Tween-20 (TBT) buffer. Counts per minute (CPM) was measured with liquid scintillation and luminescence counter. The positive cut-off point of IAA index was defined as ≥0.06 based on the 99-percentile of the distribution in 317 healthy individuals. The specificity and sensitivity of the assay were calculated from the samples provided by the fourth Diabetes Autoantibodies Standardization Program (DASP 2005). The IAA levels were determined in 71 T1 DM and 551 newly diagnosed type 2 diabetes (T2DM) patients, and 317 healthy controls. The t test, non-parametric test, x2 test and linear correlation analysis were performed on the data using SPSS 11.5 software. The concordance rate was estimated with Kappa value. Results (1) The optimized testing condition was described as 2×104 CPM of 125Ⅰ-insulin, 5 ul serum sample and slowly horizontal shaking for 72 h. (2) The intra-assay CV was 4.8%-8.9% and inter-assay CV was 6.4%-10.5%. Based on DASP 2005 samples, the specificity and sensitivity of the assay were 97% (97/100) and 50% (25/50), respectively. Ninety-six serum samples with different IAA levels were selected and tested to compare between our new method and a domestic IAA RIA kit. The results showed that the IAA indices from the two methods were positively correlated (r= 0.678, P<0.001). The concordance rate was 72.9 %(Kappa value=0.402). There were 25 samples with discordant results, which were positive for IAA titer using the corresponding microtiter plate RIA but negative using the novel RIA kit. (3) In TIDM group the positive rate of IAA was 19.7% (16/71), higher than the healthy controls (0.9%, x2=54.36, P<0.001). The subgroup of T1DM children (with 0-9 years) showed the highest IAA positive rate (55.6% ,x2=4.85, P<0.05). In T2DM group the frequency of IAA was 1.5% (8/551), which had no significant difference comparing with that of healthy controls (x2= 0.95, P >0.05). Conclusions Our proposed microtiter plate RIA method for IAA is highly sensitive and specific, likely to be feasible for clinical application. The frequency of IAA is high in children with T1DM.

Objective:To explore the influence of ischemia/reperfusion (anoxia/reoxygenation)[FK(W16*2。142mmZQ1mm]on immunofunction of endothelial cells(ECs) and effect of intervention with 2-hydroxymethyl-3,5,6-trimethylpyrazine on it.Methods:Model of ECs induced by anoxia/reoxygenation was established to mimic ECs ischemia/reperfusion injury in vivo with human umbilical vein endothelial cell line ECV304.Then 2-hydroxymethyl-3,5,6-trimethylpyraxine was used to intervene the anoxia/reoxygenation process.Nuclear transcriptional factor-?B(NF-?B) was exhibited by fluorescent staining,HLA-ABC,HLA-DR,CD86 and CD54 were detected by flow cytometry.Mixed endothelial cell-lymphocyte reaction(MELR) was conducted to examine the proliferation of lymphocyte,production of IL-2 and percentage of apoptotic lymphocyte.Results:Anoxia/reoxygenation made the ECV304 cell became round,shrunk and abscised,with increased plasma NF-?B,and changed from positive cytoplasm to positive nucleus.HLA-ABC,HLA-DR and CD86 on surface of cells increased but CD54 showed unchanged.MELR showed the incorporation of ~3H-TdR and production of IL-2 increased significantly and the percentage of apoptotic lymphocyte decreased.After 2-hydroxymethyl-3,5,6-trimethylpyrazine intervention,the ECV304 cell shapes recovered,NF-?B expression didn’t down-regulated,but the percentage of positive cells decreased,changes to positve dominant.Besides,reversal changes were shown in other parameters.Conclusion:Anoxia/reoxygenation influences some important immune related molecules in ECV304 cells.2-hydroxymethyl-3,5,6-trimethylpyrazine could antagonize these influences to maintain the immune function of endothelial cells in a relative normal manner.
9.Clinical observation of tachycardia-induced cardiomyopathy after radiofrequency cather ablation

Objective To determine the baseline echocardiographic characteristics and the time course and degree of recovery of left ventricular (LV) systolic dysfunction in patients with tachycardia-induced cardiomyopathy ( TCM ) . Methods Seven hundred and fifteen patients received radiofrequency cather ablation ( RFCA ) for tachycardiarrhymias from July 2010 to July 2013 were screened in this study. Only 33 patients with reduced left ventricular ejection fraction (LVEF) (LVEF<50% and improved≥15%) were diagnosed with tachycardia-induced cardiomyopathy and were included in the study. Patients with early improvement (over 25%increase in LVEF at 1-week follow-up compared to the baseline ) were enrolled in the improved group , and the rest patients were enrolled in the improved group. All Patients received transthoracic echocardiography for LV size and function detection at 1 week and at 3,6,12 months follow-up. Results The average baseline of the LV end-diastolic diameter, and the LVEF were (55 ± 10.7)mm and (38 ± 4.6)%, repectively. Early improvement ( over 25%increase in LVEF at 1-week follow-up compared to the baseline ) in the improved group was observed in 16 patients. Patients with early improvement had higher LVEF at 12-month follow-up compared to the patients without early improvement [(69.2± 4.2)% vs (58.1 ± 6.9)%, P < 0.001]. Conclusions RFCA is proved to be a relatively safe and effecient treatment method. Atrial fibrillation related to TCM , rhythm control is superior to the rate control. The early improvement in LVEF may potentially predict the complete reversibility of LV systolic dysfunction.
10.Study on the Protective Effect of Traditional Chinese Medicine that Invigorating Vital Energy and Pro-moting Blood Flow on Model of Renal Ischemical Reperfusion Injury of Rats

OBJECTIVE:Approach to the protective effect of traditional Chinese medicine that invigorating vital energy and promoting blood flow on model of renal ischemical reperfusion injury of rats.METHODS:The rats were divided into control group,model group and medication group(traditional Chinese medicine that invigorating vital energy and promoting blood flow),the medication group were given prevention administration for7days;model of renal ischemical reperfusion injury of rats were established.The contents of complement C3and nitric oxide(NO)in serum and renal tissue and the pathological changes of renal tissues in each group were compared.RESULTS:Levels of complement C3and NO in serum and renal tissue were increased in the model group and there were obvious pathological changes in renal tissues;compared with the model group,there was a decrease in the contents of complement C3and NO in serum and renal tissues and there was an obvious improvement in the renal tissues in the medication group.CONCLUSION:The traditional Chinese medicine that invigorating vital energy and promoting blood flow has a protective effect on the model rats with renal ischemical reperfusion injury.
