2.Terpenoids and sterols from Ricinus communis and their activities against diabetes.

Seven terpenoids and three sterols were isolated from the methanol extracts of the aerial parts of Ricinus communis by chromatography methods and their structures were identified by spectra analysis as ficusic acid( 1), phytol(2), callyspinol(3) , lupeol(4), 30-norlupan-3beta-ol-20-one(5) , lup-20(29)-en-3beta,15alpha-diol(6) , acetylaleuritolic acid( 7), stigmast4-en-3-one(8) , stig-mast-4-en-6beta-ol-3-one(9) , and stigmast4-en-3,6-dione(10). Compounds 1-3 and 5-10 were obtained from this species for the first time and 5 and 6 showed significant inhibitive activity and good selectivity against 11beta-HSD of mouse and human in vitro. 3.Expression and clinical significance of hepatocyte growth factor receptor and epidermal growth factor receptor in pancreatic cancer

Objective To determine the clinical significance of hepatocyte growth factor receptor (Met) and epidermal growth factor receptor (EGFR) in the clinicopathology and prognosis of pancreatic cancer.Methods 70 patients admitted with pancreatic cancer from 1995 to 2005 were retrospectively analyzed with clinicopathological and follow-up data.Expression of Met and EGFR in cancer nest embedded with paraffin were detected by immunohislostaining (EnVision method) and correlation with clinicopathologic parameters and mutual correlation between these two receptors were further analyzed.Prognosis and related risk factors were analyzed by Kaplan-Meier survival analysis and Cox regression analysis,respectively.Results Both Met and EGFR significantly correlated with TNM staging,tumor size and superior mesenteric vessels invasion (P<0.05).Expression level of Met positively correlated with that of EGFR (r9 =0.658,P<0.05).Both Met and EGFR significantly correlated with patients' survival (P<0.05) and Met was an independent prognostic risk factor for pancreatic cancer.Conclusions Both Met and EGFR significantly affect development and prognosis of pancreatic cancer and correlate with each other.Simultaneously targeting both Met and EGFR pathways may provide an advisable strategy of targeted therapy in pancreatic cancer.
4.Clinical observation on warm needling moxibustion plus acupoint sticking therapy for cervical radiculopathy

Objective:To observe the clinical effect of warm needling moxibustion plus acupoint sticking therapy for cervical radiculopathy.Methods:A total of 120 cases were allocated into an observation group,a warm needling group and an acupoint sticking group according to the random number table,with 40 cases in each group.Cases in the observation group received warm needling moxibustion plus acupoint sticking therapy;cases in the warm needling group received the same warm needling moxibustion in the observation group;cases in the acupoint sticking group received the same acupoint sticking therapy in the observation group.The scores of Japanese Orthopaedic Association (JOA) and visual analog scale (VAS) were recorded before and after treatment.Results:The total effective rate was 95.0% in the observation group,versus 77.5% in the warm needling group and 75.0% in the acupoint sticking group (both P<0.05).Inter-group differences in JOA and VAS between the observation group and the other two groups were statistically significant (all P<0.05).Conclusion:Warm needling moxibustion plus acupoint sticking therapy is effective in treating cervical radiculopathy,and it can significantly alleviate pain and enhance clinical efficacy,and thus is worth clinical popularization.
5.Early outcome of peripheral cutting balloon in the treatment of peripheral vessel stenosis

Objective To investigate the early clinical outcome of peripheral cutting balloon(PCB) in the management of peripheral vessel stenosis.Methods Thirteen patients with peripheral limb vessel stenosis, in which 4 stenoses in hemodialysis access and 9 at lower limb arteries,underwent angioplasty by PCB.For multiple stenosis in the same vessel,the distal one should be expanded firstly.The balloon pressure was controlled in the range of 8 atm to 10 atm(1 atm=10.108 kPa).All the patient were given continuous anticoagulant therapy after the procedure.Results All the procedure were carried out successfully on the 13 patients,and no serious complications occured.The symptoms did not recur in all patients after the procedure.The 5 months' follow-up angiography proved that no restenosis occurred in one patient with previous stenosis at the hemodialysis access.Conclusion The angioplasty with PCB was a safe and reliable procedure in management of the peripheral limb vessel stenosis.The early outcome is satisfying.
6.Group social skills training on the effects of improving psychiatric symptoms and disability and the family relationship for patients with schizophrenia

Objective To discuss the effect of group social skills training on improving psychiatric symptoms and disability and the family relationship in patients with schizophrenia.Methods 101 followup patients of rehabilitation with schizophrenia in mental health center of a top three hospital and in a community were chosen from June 2011 to August 2012.Based on the principle of voluntary,45 and 56 patients were grouped into the control group and the intervention group,respectively.The scores of psychiatric symptoms,mental disability and family relationship were compared between two groups by independent sample test.Results At the end of six months,the score of positive and negative symptoms and mental disability were lower in the intervention group,and there was significant difference in different measuring time,the interaction existed between the intervention and the measuring time.Meanwhile,the total score of family concern in the intervention group was higher than that in the control group,and there was significant difference in different measuring time,no interaction existed between the intervention and the measuring time.Conclusions Group training of social skills could improve psychiatric symptoms and the family relationship of patients with schizophrenia,reduce mental disability and promote recovery of patients.
7.Life-threatening meningitis resulting from transrectal prostate biopsy.

After antibiotic prophylaxis with metronidazole and levofloxacin, a transrectal sextant biopsy was performed under the guide of transrectal ultrasonography (TRUS) for a 75-year-old suspicious patient with prostate adenocarcinoma. Although antibiotics were also given after this procedure, the patient still developed fever, anxious, agrypnia and headache. Blood cultures remained negative. Lumbar puncture was performed and was consistent with Escherichia coli bacterial meningitis.

9.Clinical study on modified sanmiao powder in treating chronic uric acid nephropathy.

OBJECTIVETo investigate the clinical efficacy of modified Sanmiao Powder (SMP) in treating chronic uric acid nephropathy (CUAN).
METHODSNinety-four patients with CUAN were equally randomized to the treated group and the control group. Conventional treatment was given to all patients and the treated group was administered with SMP additionally for 12 weeks. Changes of symptoms were observed, and laboratory indexes, as urinary protein quantity (UPro), urinary RBC count (URBC), urinary beta2 microglobulin (beta2-M), urinary beta-N-acetylglucosaminidase (NAG), blood urea nitrogen (BUN), serum creatine (SCr) and serum uric acid (SUA), were detected before and after treatment.
RESULTSThe total effective rate in the treated group was 87.2% (41/47), and in the control group was 61.7% (21/47), showing significant difference between groups (P < 0.01); significant improvement of UPro, URBC, beta2-M, NAG, BUN, SCr and SUA were shown in the treated group (P < 0.05, P < 0.01); but in the control group, only URBC count was significantly decreased (P < 0.05), no statistically significant change of other indexes was found (P > 0.05). SUA decreased in both groups (P < 0.01), which was markedly lower in the treated group than in the control group (P < 0.05); SCr and BUN were also decreased in the treated group (P < 0.01).
CONCLUSIONSCombined therapy of SMP and conventional Western medicine shows a favorable effect in treating CUAN. It could not only reduce SUA, but also alleviate the albuminuria and hematuria, lower the urinary levels of beta2-M and NAG to improve renal function.

10.Reliability and validity of addiction severity index in drug users with methadone maintenance treatment in Guizhou province, China.

OBJECTIVETo evaluate the reliability and validity of the Chinese version of addiction severity index (ASI)-5th version (ASI-C-5), in illegal drug users receiving methadone maintenance treatment (MMT) in China.
METHODSOne hundred and eighty-six heroin addicts (144 men and 42 women) receiving MMT at three clinics in Guizhou province, southwest China, were recruited. They were all interviewed with a questionnaire of ASI-C-5 and 35 were re-interviewed at an interval of seven days to assess its test-retest reliability.
RESULTSCronbach's alpha for internal consistency of CSs varied from 0.60 to 0.81 in all domains. Test-retest reliability of composite scores (CSs) of ASI-C-5 were satisfactory (r=0.38-0.97). Based on item analysis and expert's suggestions, five items were deleted and one item was modified in ASI-C-5. Criterion validity of ASI-C-5 was found acceptable, as compared to addicts' self-rating anxiety scale (SAS) and self-rating depression scale (SDS) (r=0.59 and 0.45) except for social support rating scale (SSRS).
CONCLUSIONSASI-C-5 can be used for heroin addicts receiving MMT with acceptable reliability and validity.
