2.Effects of blood pressure by intravenous infusion of different doses of oxytocin in cesarean section

Objective To observe the effects of blood pressure by intravenous infusion of different doses ofoxytocin in cesarean section.Methods Sixty full-term pregnant women undergoing cesarean section with continuous epidural anesthesia were divided into three groups by random digits table method with 20 cases each:group A,B and C.Three groups were injected 10 U oxytocin in uterine muscle after infant delivery.Group A,B and C received 5,10 and 20 U oxytocin (sodium lactate ringer,500 ml) continuous intravenous infusion at the speed of 10 ml/min.If happened uterine contractions bad,they were sublingual administering 0.2 mg misoprostol.If happened severe hypotension,they were intravenous injected 5 mg ephedrine.The change of mean arterial pressure (MAP) and heat rate before anesthesia (T0),after fetal childbirth (T1),5 m in (T2),10 min (T3),30 min (T4) after infusion of oxytocin and the dosage of ephedrine and misoprostol were recorded.Results There were no significant differences in MAP and heart rate at every time point between group A and B (P> 0.05).MAP decreased and heart rate increased in group C at T2,T3 compared with those in group A and B,and there were significant differences (P< 0.05).The number of cases of sublingual misoprostol were increased in group A (7 cases) compared with that in group B (2 cases) and group C(1 case).The 8 patients injected ephedrine in group C were more than group A(1 case) and group B (3 cases).Conclusion Cesarean section after the delivery of the fetus in the uterus muscle injection of oxytocin 10 U,after 10 U of oxytocin added 500 ml sodium lactate ringer injection at the speed of 10 ml/min intravenous infusion has little effect on the blood pressure and heart rate,and has good uterine contractions.
3.Clinical analysis of radio-frequency ablation in the treatment of advanced pancreatic cancer

Objective To explore the clinical effect of radio-frequency ablation in the treatment of advanced pancreatic cancer.Methods Retrospectively analyzed the clinical data of 28 cases of radio-frequency ablation in the treatment of advanced pancreatic cance between January 2008 and December 2012 in 304 hospital.And choosed made 140 cases patients who only underwent the conservative treatment as the control group at the same period.Then compared the data of two groups patients.Results The mean tumor diameter was (4.8 ± 1.6) cm.The average radio-frequency time was (17.7 ± 4.3) minutes.There was no patient occurred of pancreatic fistula or postoperative bleeding,and no patient dead.Peri-operation pain relief rate was 88.9%.Twenty-three patients were followed up,and the follow up rate was 82.1%.The patients' survival rate who underwent radio-frequency ablation of 1 month,6 months and 1 year were 95.7%,65.2% and 38.2%.The average survival time was (14.6 ± 2.2) months.And the data was significantly higher than that of the control group patients (97.4%,45.3%,19.1%,P =0.0306).Conclusions The radio-frequency ablation was one of an effective treatment method for the advanced pancreatic cancer patients.It can prolong the survival time of pancreatic cancer patients and alleviate the patient's pain.
4.Risk factors for symptomatic intracranial atherosclerotic stenosis: a comparison of anterior circulation and posterior circulation

Objective:To investigate the risk factors for anterior circulation and posterior circulation symptomatic intracranial atherosclerotic stenosis (sICAS).Methods:The clinical data of patients admitted to Hebei General Hospital for ischemic stroke or transient ischemic attack (TIA) and diagnosed with sICAS by digital subtraction angiography from May 2019 to May 2020 were retrospectively included. The patients were divided into anterior circulation group and posterior circulation group according to the stenosis sites, and the distribution of sICAS and its risk factors were analyzed.Results:A total of 134 patients with sICAS were enrolled, including 82 males (61.2%) and 52 females (38.8%). Their age was 60.28±11.46 years; 115 (85.8%) had ischemic stroke and 19 (14.2%) had TIA. There were 92 patients (68.7%) in the anterior circulation group and 42 (31.3%) in the posterior circulation group. Body mass index (BMI), systolic and diastolic blood pressure levels, as well as the proportion of patients with hypertension, diabetes, smoking and drinking in the posterior circulation group were significantly higher than those in the anterior circulation group (all P<0.05). Multivariate logistic regression analysis showed that higher BMI (odds ratio [ OR] 1.191, 95% confidence interval [ CI] 1.029-1.379; P=0.019), hypertension ( OR 4.073, 95% CI 1.135-14.616; P=0.031) and diabetes ( OR 2.783, 95% CI 1.149-6.738; P=0.023) were independently correlated with the posterior circulation sICAS. Conclusions:Compared with anterior circulation, high BMI, hypertension and diabetes are the independent risk factors for posterior circulation sICAS.
5.Early diagnosis value of peripheral blood PPARγ combined with IFN-γ detection for tuberculosis infection in patients with end-stage renal disease

Abstract: Objective To explore the early diagnostic value of peripheral blood peroxisome proliferator-activated receptor γ (PPARγ) combined with γ-interferon (IFN-γ) release assay (IGRA) in the diagnosis of pulmonary tuberculosis in patients with end-stage renal disease (ESRD), and to provide reference for clinical diagnosis and treatment. Methods From January 2019 to December 2021, 70 ESRD patients with suspicious symptoms of pulmonary tuberculosis were treated at Hebei Chest Hospital were selected as the research objects. According to the examination results, they were divided into ESRD group (40 cases) and ESRD complicated by pulmonary tuberculosis (40 cases, comorbidity group). In addition, 40 cases with pulmonary tuberculosis were used as the PTB group. All three groups of patients underwent IGRA test, and the peripheral blood PPARγ level was detected by enzyme-linked immunosorbent assay, and the diagnostic value of PPARγ combined with IGRA test for ESRD patients with pulmonary tuberculosis was explored. Results The expression level of PPARγ and IFN-γ content in the PTB group and the comorbidity group were obviously higher than those in the ESRD group (P<0.05), while the differences in PPARγ expression level and IFN-γ content between the PTB and comorbidity groups were not statistically significant (P>0.05). The ROC curve showed that the areas under the curve (AUC) of PPARγ and IGRA in the diagnosis of end-stage renal disease combined with tuberculosis were 0.823 (95%CI: 0.722-0.925) and 0.773 (95%CI: 0.662-0.883), respectively, and the AUC of combined detection was 0.928 (95%CI: 0.871-0.984), which was better than that of PPARγ and IGRA alone (Z/P=2.057/0.039, 2.843/0.005). The Kappa values of serum PPARγ and IGRA test compared with the clinical gold standard results in the diagnosis of ESRD complicated with pulmonary tuberculosis were 0.557 and 0.444 (P<0.05). The combined screening of ESRD with pulmonary tuberculosis was consistent with the clinical gold standard (Kappa=0.661, P<0.05). Among the 30 ESRD patients complicated with pulmonary tuberculosis, the sensitivity of PPARγ combined with IGRA test in diagnosis of ESRD complicated with pulmonary tuberculosis was 93.33% (28/30), which was higher than 70.00% (21/30) of PPARγ and 66.67% (20/30) of IGRA test alone (P<0.05). Conclusions Peripheral blood PPARγ and IGRA tests have certain diagnostic value for ESRD complicated with tuberculosis, and the combined detection of the two can improve the sensitivity and reduce the rate of missed diagnosis, which is worthy of clinical promotion.
6.Timing of the local anesthetic in ultrasound-guided modified Seldinger technique PICC puncture

Objective To explore the timing of local anesthesia in ultrasound-guided modified Seldinger technique PICC puncture, so as to provide theoretical basis for formulating unified operating standards in clinic. Methods Totals of 128 patients with PICC after operation were randomly divided into pre-anesthesia group and post-anesthesia group (64 cases in each group).In pre-anesthesia group, 0.2 ml 2%lidocaine was injected in the puncture point before puncturing; in post-anesthesia group, 0.2 ml 2% lidocaine was injected after puncturing.Body pain score in the process of puncturing and times of puncturing were compared between the two groups.Results The body pain score in pre-anesthesia group was (1.64 ±1.58),and (1.64 ±1.21)in post-anesthesia group, the difference was not statistically significant (t=0.00,P>0.05).The times of puncturing in pre-anesthesia group was (1.59 ±1.35), which higher than (1.18 ±0.50) in post-anesthesia group, and the difference was statistically significant (t=2.20,P<0.05).In pre-anesthesia group, the distance from local anesthesia point to the actual piercing point was positively related to pain score (r =0.319,P <0.05). Conclusions The pain were no different caused by local anesthesia before and after piercing, but less piercing times in condition of anesthesia after putting thread in, which can improve the success rate of puncture.
7.Osteoclast differentiation of Raw264.7 cells expressing enhanced green fluorescent protein in vitro

Objective To observe the osteoclast differentiation of Raw264.7 strain stably expressing enhanced green fluorescent protein(EGFP).Methods Raw264.7 cells were transfected with EGFP-Lifeact gene via retrovirus.The G3 cell clone was obtained by limited dilution technique which stably expressed EGFP under the fluorescence microscope.The morphology of G3 cells were observed.The effects of transfection on receptor activator of nuclear factor kappaB ligand(RANKL)-induced osteoclastogenesis and bone resorbing function of G3 cells were examined by tartrate resistant acid phosphatase(TRAP) staining,immunoblotting detection of cathepsin K and bone pit resorption assay.The real-time images of podosome dynamics were taken by laser confocal microscopy during osteoclast differentiation of G3 cells.Results The Raw264.7 cells were successfully transfected with EGFP-Lifeact gene.The G3-EGFP cloned strain which could stably express EGFP even after 20 passages was constructed.There was no significant difference in morphology between G3-EGFP and wild Raw264.7 cells.The fusion rates of the transfection group and of the wild control group were (35±5)% and (39±5)%,respectively,which were not significantly different(P>0.05).The semi-quantitative ratio of cathepsin K/β-actin in the wild control group and in the transfection group was 0.83±0.07 and 1.02±0.08(P>0.05),respectively.Bone pit results showed that the total area of the bone resorption was respectively 272 252±36 193 and 262 408±23 243(P>0.05) and the number of the bone pits was respectively 320 ± 51 and 339± 55(P>0.05).The photos of laser confocal microscopy showed the constant cell-cell fusion during osteoclast differentiation of G3-EGFP cells.In addition,the dynamic self-organized podosome initially assembled podosome clusts,then dynamic rings,finally formed the characteristic podosome belt pattern in mature osteoclasts.Conclusions Enhanced green fluorescent protein high effectively expressed in Raw264.7.Biological character does not change after transfection.

METHODS Purifiedspindlin1proteinwasinjectedintorabbitstoproducethepolyclonalantiserumafter removing glutathione S-transferase (GST)from the fusion protein spindlin 1-GST that was expressed in Escherichia coli..The antiserum was purified through the Hitrap Protein A system,and the titer of spin-dlin 1 polyclonal antibody was detected by ELISA.The specificity of the polyclonal antibody was deter-minedbyWesternblottingandimmunohistochemistry.RESULTS Thetiterofspindlin1polyclonalanti-body was 1∶2000.Western blotting detection demonstrated that the spindlin 1 polyclonal antibody recog-nized myc-spindlin 1 reco mbinant fusion protein in HeLa cells transfected with pAdeasy-myc-spindlin 1 , which also corresponded with Myc.antibody.The HeLa cells were transfected with enhanced green fluo-rescence protein (EGFP)and spindlin 1 vector(pEGFP-C3-spindlin 1 ),which was confirmed by the in-dependent GFP fluorescence assay.The results of immunohistochemistry detection with the spindlin 1 polyclonal antibody suggested that spindlin 1 was mainly expressed in the nuclei of HeLa cells.More i m-portantly,in i mmunohistoche mical assays,the spindlin 1 antibody recognized nuclear spindlin 1 expres-sioninclinicalovariancancertissues.CONCLUSION Thespecificspindlin1polyclonalantibodyispre-pared,which may be used to detect cancer-related protein spindlin 1 in HeLa cells and ovarian cancer tissues.
9.Experimental study on preventive effect of Yougui drink on femoral head necrosis in rats under micro CT.

OBJECTIVETo explore the preventive effect of Yougui drink on femoral head necrosis in rats under micro CT.
METHODSTwenty-five SD rats were divided into steroid hormone group (group A, 10 rats ), Yougui drink group (group B,10 rats) and normal group (group C,5 rats)with random number table. Endotoxin were injected into abdominal cavity of rats in group A and B for 2 days, methylprednisolone sodium succinate were injected by gluteus for twice a week continued for 6 weeks; group B were gavaged by Yougui drink (veryday for 8 weeks; group C did not do any processing. All rats were killed on the 10th weeks,m icro CT were used to scan femoral head in vitro and preventive effect of Yougui drink (n femoral head necrosis in rats.
RESULTSThere was statistical significance in BMD, BV/TV, Tb.N, Tb, Th, Thb, Sp, BS/TV and DA but no significance in SMI between group A and B. Comparison between A and C, there was significant meaning in BMD, BV/TV, Tb.N, Tb, Th, Tb, Sp, BS/TV, DA and SMI.
CONCLUSIONYougui drink on femoral head necrosis in rats under micro CT has preventive effect from BMD BV/TV, Tb.N, Tb, Th, Tb, Sp, BS/TV and DA.

10.A study of influencing factors about fertility motivation in female patients with chronic kidney disease based on grounded theory

Objective:To investigate the infuencing factors of fertility motivation in patients with chronic kidney disease (CKD).Methods:The 21 female patients with CKD in the Eastern Theater General Hospital from November 2019 to November 2020 were selected by objective sampling method. The data were collected by semi-structured interview, recording and writing on-site notes, and the subjects were extracted by Strass and Corbin′s grounded theory data analysis method.Results:The core category is "the formation of patients′ reproductive motivation". A theoretical model including four main categories of patient characteristics, disease coping style, subjective norms and perceived behavioral control was constructed. Patients′ characteristics are dynamic factors, subjective norms are situational factors, perceptual behavior control is protective factors, and patients′ disease coping style is regulatory factors.Conclusion:This study explored the influencing factors of fertility motivation in CKD patients from the perspective of patients, aiming to provide theoretical basis for the formulation and implementation of intervention measures.
