1.Effect of storage duration of whole blood on the preparation of leukocyte-depleted red blood cells

【Objective】 To discuss the differences in the quality of leukocyte-depleted red blood cells(LDRBCs) prepared from fresh whole blood stored at 4℃ with different storage duration. 【Methods】 The whole blood was collected and stored at 4℃. LDRBCs were prepared on the day of collection (d0 preparation group) and the next day (d2 preparation group). The leukocyte residues, blood filtering time, blood loss during filtration, LDRBCs volume, and the Hct, MCV, RBC and Hb in the packed whole blood with different storage duration, and the K+, Na+, pH, free hemoglobin concentration(fHb) and hemolysis rate of plasma between two groups were analyzed and compared. 【Results】 The filtering time of d0 preparation group and d2 preparation group was (7.67±1.95) min and (11.55±2.43) min, respectively. The volume of LDRBCs in the two groups was (305.21±21.95) mL and (311.11±21.08) mL, respectively (P<0.05). Concerning blood loss during filtration and WBC residues, there was no difference between two groups (P>0.05). No significant differences could be noticed in the value of Hct, RBC and Hb during the storage period(P>0.05), irrespective of MCV(fL), which changed significantly when sampled immediately after blood collection (89.37±2.57), prior to preparation at day 0(89.68±2.74) and prior to preparation at day 2(90.30±3.25) (P<0.05). No difference in K+, Na+, pH value and fHb was notable between the two groups (P>0.05), while hemolysis rate of plasma in day 0 preparation group was lower than that of day 2 preparation group (P<0.05). 【Conclusion】 The RBC deformability, increasing with the extension of whole blood cold storage time, results in the differences in the volume of LDRBCs and incidence of hemolysis. LDRBCs preparation within 10-hour cold storage following whole blood collection is recommended.
2.HBV laboratory tests and demographic characteristics in 169 897 first-time blood donors

【Objective】 To investigate the HBV infection markers detection and demographic characteristics of first-time blood donors, so as to provide evidence for blood donor recruitment. 【Methods】 HBsAg was detected by ELISA, HBsAg negative samples were tested for HBV DNA, and chemiluminescence method was used to detect HBsAg, HBsAb, HBcAb, HBeAg and HBeAb in first-time blood donor HBsAg-/HBV DNA+ samples. Demographic information of HBsAg positive first-time donors was analyzed. 【Results】 From 2018 to 2022, a total of 502 739 people participated in voluntary blood donation, and first-time blood donors accounted for 33.79%. The HBsAg positive rate of first-time donors(28.37/10 000, 482/169 897)was higher than that of repeated blood donors(3.46/10 000, 115/332 842) (OR=8.23, 95%CI: 6.72~10.09), and the HBV DNA positive rate of first-time blood donors(4.83/10 000, 82/169 897)was lower than that of repeated blood donors(6.52/10 000, 217/332 842)(OR=0.74, 95%CI: 0.57~0.95). The positive rate of HBcAb in HBsAg-/HBV DNA+ samples of first-time blood donors was 73.17%. Significant differences were noticed in HBsAg-/HBV DNA+ and HBsAg positive rate among first-time blood donors among gender, age, education background and occupation (all P<0.05). 【Conclusion】 Low risk first-time blood donor recruitment is important for blood donation. Strengthening HBV screening before blood donation and detection after blood donation is beneficial to improve the safety of blood transfusion.
3.Inhibitory effect of nitric oxide-induced total ginsenosides on right ventricular hypertrophy in rats.

OBJECTIVETo observe the effect of total ginsenosides (TG) on right ventricular hypertrophy induced by monocrotaline (MCT) in rats, and study its relationship with the nitric oxide pathway.
METHODMale Sprague Dawley rats were randomly divided into the control group, the MCT model group, TG-treated (20, 40, 60 mg kg-1 d-1) groups, and the L-arginine (L-arg) th NO release, T + L-N and L-a + L-N groups were wi th NOS into study TG's effect 200 mg kg-1 d-1 group. Besides, and its relationship wi also set, intraperitoneally injected with TG 40 mg kg-1 d -1 and L-arg 200 mg kg-1 - d-1, and orally administered hibitor L-NAME 20 mg kg-1 d-1. After all of the groups were given drugs for 18 d, their right ventricular peak systolic pressure (RVSP) ventricular hypertrophy index (RVHI) and RVW/BW were determined. Ultra-structure of myocardial cells was observed with transmission electron microscope. The NO2 -/NO3 - content in myocardial tissues were detected with the nitrate reduction method. ANF and eNOS mRNA expressions in right ventricle tissues were detected by using real-time RT-PCR.
RESULTLow, middle and high doses of TG and L-arg preventive administration could significantly reduce RVSP, RVHI, RVW/BW and ANF mRNA expressions (P < 0. 05) , and ameliorate cellular mitochondrial swelling and degeneration. L-NAME could prevent the effect of L-arg on above indexes, whereas L-NAME of the same dose could not impact the reducing effect of TG 40 mg kg -1 on above indexes. TG 60 mg kg -1 could raise eNOS mRNA expression, but TG 20 mg kg-1 and 40 mg kg-1 showed no effect.
CONCLUSIONTG can significantly attenuate MCT-induced right cardiac hypertrophy in rats. Its anti-hypertrophic effect is partially realized through NO.

4.Analysis of the Correlations between the Levels of HIF-1α,AQP9 and the Hemorrhagic Transformation in Patients with Acute Ischemic Stroke

Objective To explore the correlations between serum hypoxia inducible factor-1α (HIF-1α),aquaporin-9 (AQP9) and hemorrhagic transformation (HT) in patients with acute ischemic stroke (AIS) and its clinical significance. Methods 183 newly diagnosed patients with acute AIS from October 2017 to April 2019 were divided into HT group (n=84) and non HT group (n=99),the baseline data of gender,age,body mass index (BMI),proportions of hypertension,diabetes,hyperlipidemia,the baseline NIHSS score,baseline systolic blood pressure,baseline diastolic blood pressure,baseline blood glucose,thrombolytic time window and serum samples were collected. The expressions of HIF-1α and AQP9 proteins were detected by Western blot (WB);Pearson’s method was used to analyze the correlation between HIF-1α and AQP9 levels in patients with AIS in HT group;the risk factors of HT in AIS patients were analyzed by logistic regression;Using NIHSS score,thrombolysis time window,serum HIF-1α,AQP9 levels as independent variables,the working characteristic curve (ROC) of subjects was drawn to analyze the predictive value of HT in AIS patients. Results Compared with the non HT group,the baseline NIHSS score,baseline systolic blood pressure,baseline diastolic blood pressure,baseline blood glucose,thrombolytic time window,serum HIF-1α,AQP9 levels of AIS patients in HT group were significantly higher (P<0.05). There was a positive correlation between the levels of HIF-1α and AQP9 proteins in patients with AIS in HT group (r=0.679,P<0.05). NIHSS score,thrombolysis time window and serum HIF-1α,AQP9 protein levels were independent risk factors for HT in AIS patients after thrombolysis (P<0.05). The AUCs of NIHSS score,thrombolysis time window,serum HIF-1α and AQP9 in the diagnosis of HT in AIS patients were 0.707,0.790,0.881 and 0.869 respectively,the cutoff values were 13.39 points,296.31 min,0.33 and 0.32 respectively,the sensitivities were 44%,66.70%,86.90% and 83.30% respectively,and the specificities were 91.90%,87.90%,87.90% and 88.90% respectively. The AUC of the combination of four methods in the diagnosis of HT in AIS patients was 0.980,the sensitivity and specificity were 94.00% and 94.90%,respectively,and the diagnostic efficiency of the four methods was significantly higher than that of single index. Conclusion The elevated levels of HIF-1α and AQP9 were closely related to the occurrence of HT in AIS patients,and the combination of NIHSS score and thrombolysis time window could significantly improve the predictive value of HT in AIS,which may have some clinical reference significance.
5.Curative effect of modified Parks incision combined with rectus linear tucking on adults with concomitant strabismus

AIM: To explore the curative effect of modified Parks incision combined with rectus linear tucking on adults with concomitant strabismus and its influences on postoperative recovery of visual function and ocular aesthetics.METHODS: A total of 160 adults with concomitant strabismus were enrolled as the research subjects between June 2019 and June 2021. They were divided into observation group(modified Parks incision combined with rectus linear tucking, 89 cases)and control group(corneal limbal trapezoid conjunctival flap incision combined with rectus linear tucking, 71 cases)according to different surgical methods. The clinical curative effect and surgical related indexes between the two groups were compared. The changes in scores of corneal fluorescence staining, standard patient of eye dryness(SPEED), visual analogue scale(VAS)and Vancouver scar scale(VSS)before and after surgery in both groups were observed. The occurrence of complications such as poor conjunctival healing and astigmatism was statistically analyzed at 1mo after surgery.RESULTS: The total response rate of surgery in observation group was higher than that in control group, and length of surgical incision was shorter than that in control group(all P<0.05). The scores of postoperative corneal fluorescence staining and dry eye in observation group were lower than those in control group, and VAS scores at 1wk after surgery were lower than that in control group(all P<0.05). Postoperative divergent fusion range, strabismus angles of short-sighted 33cm and far-sighted 5m in observation group were greater than those in control group. Convergent fusion range was smaller than that in control group, and stereopsis of short-sighted 33cm and far-sighted 5m was lower than that in control group(all P<0.05). Postoperative break-up time in observation group was longer than that in control group, lacrimal secretion was more than that in control group, and scores of corneal surface regularity index(SRI)and VSS scores were lower than those in control group(all P<0.05). The total incidence of complications in observation group was lower than that in control group(P<0.05).CONCLUSION: The modified Parks incision combined with rectus linear tucking can not only improve surgical effect and promote the recovery of visual function and tear film function in adults with concomitant strabismus, but also relieve dry eye and postoperative pain, improve ocular aesthetics and reduce the incidence of postoperative complications.
6.Bone setting manipulative reduction for the treatment of children's distal radioulnar fracture and dorsal dislocation.

OBJECTIVETo observe the effect of different manipulative reduction for children's distal radioulnar fracture and dorsal dislocation.
METHODSFrom June 2013 to June 2014, 80 children with distal radioulnar fracture and dorsal dislocation were treated by bone setting manipulative reduction including 51 males and 29 females with an average age of 6.5 years old ranging from 3 to 14 years old. Time from injury to treatment was 1 h to 6 d, 31 cases were on the right, 49 cases were on the left. Among them, 45 cases were type I of overlapping displacement, 35 cases were type II. The displacement of the fracture was observed by clinical manifestations and X-ray examination. Under fluoroscopy, different techniques were used for reduction and fixation. After 3 weeks of over wrist fixation, the splints were overturned and fixed again for 1 to 2 weeks, then were removed. The wrist joint function was evaluated based on Dienst criteria.
RESULTSEighty cases of fracture were successfully operated one time, all reached anatomic reduction or near anatomic reduction. Eighty children were followed up for 3 months to 1 year. All the fractures healed, and the healing time was 4 to 5 weeks with an average of 4.6 weeks. All patients removed the splint 3 months later, the results were excellent in 72 cases, good in 7 cases and fair in 1 case, the excellent and good rate was 98.75%.
CONCLUSIONSBone setting manipulation for children's distal radioulnar fracture and dorsal dislocation can get good reduction. At 1 month after the removal of the splint, wrist function and finger strength gradually recovered and returned to normal after 3 months.
8. Clinical observation on Yaotongning Capsule in treatment of lumbar hyperplasia (cold-dampness stagnation syndrome)

Objective: To observe the therapeutic effect and safety of Yaotongning Capsule in the treatment of lumbar hyperplasia. Methods: A random, double-blind, placebo controlled, multicenter clinical research method was used. The lumbar hyperplasia patients (144 cases) were selected, divided into Yaotongning Capsule group (treatment group) with 72 cases, 72 cases completed; and placebo group (control group) with 72 cases, 70 cases completed. The treatment lasted for 2 weeks. After treatment, the symptom of traditional Chinese medicine and pain relief situation were observed in the two groups. Results: The curative effects of Yaotongning Capsule in the treatment of lumbar hyperplasia disease (cold-dampness stagnation syndrome) were as follows: The control rate in treatment group was 41.5%, and that in the control group was 0.0%, superiority test (P<0.001) was established. Pain relief: the relieve rate in treatment group was 86.1%, and that in the control group was reduced to 45.7%, there was significant difference between the groups (P<0.001). Conclusion: Yaotongning Capsule in the treatment of lumbar hyperplasia (cold-dampness stagnation syndrome) has obviously curative effect, and can effectively reduce the pain caused by lumbar hyperplasia.
9. Blade needle treatment improves cervicogenic dizziness by increasing blood flow of vertebral basilar artery

OBJECTIVE: To investigate the therapeutic effect of blade needle therapy for cervicogenic dizziness (CD) and changes of blood flow of vertebral artery in patients with CD. METHODS: A total of 60 patients with CD were equally randomized into medication (16 women and 14 men, 38.9±10.9 years in age) group and blade needle group (17 women and 13 men, 40.1±12.4 years in age). Patients of the blade needle group were treated by performing blade needle stimulation (longitudinal cutting along the musculoaponeurotic layer) to the tender points (attachment points) of the bilateral superior and inferior oblique muscles of the head, major and minor posterior cephalic rectus, about 2/3 of suboccipital nuchal line, and near the cervical processes of C1-C2 segments. The treatment was conducted once every 3 days for 15 days. Patients of the medication group were ordered to take Flunarizine Hydrochloride capsules (10 mg) once every night for 15 days. Transcranial Doppler was used to measure changes of mean blood flow velocities of the left vertebral artery (LVA), right vertebral artery (RVA), and basilar artery (BA) before and after the treatment. The therapeutic effect was assessed according to the Criteria for Diagnosis and Therapeutic Effect Evaluation of Syndromes or Illnesses of Traditional Chinese Medicine (1994) and the Assessment Scale for Symptoms and Function of Cervicogenic Dizziness (2017). RESULTS: Following the treatment, of the two 30 cases in the medication and blade needle groups, 5 (16.7%) and 7 (23.3%) were cured, 16 (53.3%) and 20 (66.7%) were improved in their symptoms, 9 (30.0%) and 3 (10.0%) failed, with the effective rate being 70.0% and 90.0%, respectively. The effective rate of the blade needle was significantly superior to that of the medication (P<0.05). The scores of vertigo, neck-should pain, daily living and work, psychology, social adaptation, and total score of the assessment scale, as well as the average blood flow velocities of LVA, RVA and BA were considerably increased in both groups in comparison with their own pre-treatment (P<0.05), and obviously higher in the blade needle group than in the medication group (P<0.05). CONCLUSION: Blade needle treatment has a good clinical effect in the treatment of CD patients, which is probably associated with its function in increasing blood perfusion of the brain tissue, and thus being worthy of clinical application.
