1.Protection of the effective fraction of Hibiscus Mutabilis L. against injury of renal ischemic reperfusion in rats

BACKGROUND: Hibiscus Mutabilis L. Is a very effective Chinese traditional herb for non-specific inflammation in the clinic adopted by Professor Wei Zhixin, the eminent expert in Traumatology Department in China, lasting for near 60 years. In order to explain its pharmaceutical mechanism, the research of Hibiscus Mutabilis L. Was carried on in the model of ischemic reperfusion injury to explain its mechanism on anti-inflammation.OBJECTIVE: To observe the protection of the effective fraction of Hibiscus Mutabilis L. (MFR) on renal ischemic reperfusion injury in rats to probe into the mechanism of MFR on anti-inflammation.DESIGN: Randomized and controlled study on experimental animal.SETTING: A Municipal Institute of Traumatology and Orthopedic Department.MATERIALS: The experiment was performed in the Key Experiment Room (Institute of Traumatology and Orthopedic Department, Shanghai) of State Administration of Traditional Chinese Medicine, in which, 55 male Wistar rats were employed.METHODS: The rat model of renal ischemic reperfusion was adopted and MFR was applied for gastric perfusion to determine serum blood urea nitrogen(BUN) and serum creatinine(Scr) and the level of interleukin-1 (IL-1).MAIN OUTCOME MEASURES: ① Changes in serum BUN and Scr; ②Effect of MFR on level of serum IL-1.RESULTS: After MFR treatment, the renal function was improved remarkably after ischemic reperfusion in rats. In sham-operation group, serun BUN was (6.72 ± 1.30) mmol/L and serum Scr was (38.40 ± 6.23) μmol/L. In 24hours of reperfusion after one-hour ischemia, BUN was(60. 72±4.64)mmol/L in the control and(47.34 ± 8.32) mmol/L in the MFR treatment group, and the significant difference presented between two groups(t=2.562, P < 0.05) .Serum Scr was(347.95±95) μmol/L and(518.20 ± 41.15) μmol/L in the treatment group and the control respectively, indicating significant difference ( t = 3.69, P < 0.01 ) . Concerning to the effect on IL-1, in 1 hour of reperfusion after 1 hour renal ischemia,IL-1 was( 122.79 ±27.56) ng/L in the MER treatment group,(180. 28 ±33. 15) ng/L in the control group, indicating that IL-1 level in the treatment group was remarkably superior to that in the control group(t = 2.98, P < 0.05). In 3 hours of reperfusion after 1 hour ischemia, level of IL-1 in the treatment group and control group was(15.58±8.59) ng/L and (34. 13±± 10. 02) ng/L respectively, indicating significant difference( t = 3.14, P< 0.05).CONCLUSION: MFR provides protection on renal ischemic reperfusion injury, which probably is related to its inhibition on IL-1 formation.
3.The role of reactive oxygen species in epigallocatechin-3-gallate-induced apoptosis of human gastric cancer MGC803 cells

Background and purpose:Anticancer mechanism of epigallocatechin-3-gallate(EGCG)remains unclear.This study investigated the role of reactive oxygen species(ROS)in epigallocatechin-3-gallate(EGCG)-induced apoptosis in human gastric cancer MGC803 cells.Methods:The inhibition of MGC803 cells growth was measured by MTT assay.Apoptosis of MGC803 cells was studied by using the AO/EB fluorescence stain.Flow cytometry was used to detect the intracellular ROS level and the rate of apoptosis.Results:EGCG could induce apoptosis of MGC803 cells and increased in the intracellular ROS level.However,after treatment with N-acetyl-L-cystein and an athiol-containing antioxidant,the inhibitory effect of EGCG on MGC803 cells was significantly weakened.The apoptotic rate of the cells and the activity of the intracellular ROS level also decreased dramatically.Conclusion:EGCG can induce apoptosis of MGC803 cells.In turn,the ROS inhibitor can significantly inhibit the apoptosis induced by EGCG in MGC803 cells.These results suggest that the cellular generation of ROS plays a role in initiating EGCG-mediated apoptosis of MGC803 cells.
4.Effects ofYiqi Shuxin Pills on Coronary Flow Reserve of Patients with Acute Coronary Syndrome in Coronary Angiography

Objective To observe the effects ofYiqi Shuxin Pills on the coronary flow reserve (CFR) of patients with acute coronary syndrome (ACS) in the coronary angiography (CAG); To discuss its relevant mechanism of action.Methods Totally 101 patients with ACS were divided into control group (49 cases) and treatment group (52 cases). Patients in the treatment group were treated with western therapy andYiqi Shuxin Pills, while patients in the control group were treated with western therapy only. After treated for 6 months, CAG was reviewed to compare the CFR in both groups before and after the treatment. The blood-stasis syndrome and qi-deficiency syndrome scores and the level of NO, ET-1, hs-CRP, sVCAM-1, P-selectin in blood of both groups before and after the treatment were observed.Results The blood-stasis syndrome and qi-deficiency syndrome scores in the treatment group were significantly lower than those in the control group (P<0.01); The levels of ET-1, hs-CRP, sVCAM-1, P-selectin were obviously reduced and NO obviously increased in treatment group than those of control group (P<0.05). The CFR in the treatment group was significantly better than that of control group, with statistical significance (P<0.05). ConclusionYiqi Shuxin Pills can improve the state of blood-stasis syndrome and qi-deficiency syndrome, improve vascular endothelial function, relieve inflammatory reaction, lower the expression of platelet, stabilize atherosclerotic plaque of coronary artery, and then improve the CFR of ACS patients in CAG.
5.Role of activated protein C in lipopolysaccharide induced microglia activation

ObjectiveTo investigate the role of active protein C (APC) in lipopolysaccharide (LPS) induced microglia activation.MethodsMicroglia from one day old Sprague-Dawley newborn rat was collected, purified and identified by primary culture and immunofluorescence staining, and then was randomly divided into four groups including LPS group (1.0μg/ml LPS plus 10μl phosphate buffered saline 12 h later), LPS+ APC group (1.0μg/ml LPS plus 0.1μg/ml APC 12 h later), APC group (10μl phosphate buffered saline plus 0.1μg/ml APC 12 h later) and control group (10μl phosphate buffered saline at each time point). The morphology of micaroglia in all groups was observed under microscope, and the expression of tumor necrosis factor-α (TNF-α) and protease-activated receptor-1 (PAR-1) were determined by immunofluorescence staining. One-way analysis of variance and LSD test were applied for statistical analysis.ResultsPrimary culture microglia was successful and the purity was no less than 99%. In LPS group, the microglia morphology was activated, and the expression of TNF-α was increased significantly than the control group (2.11±0.35 vs 1.38±0.28, LSD test,P=0.002). In LPS+APC group, the microglia morphological change was reversed, and the expression of TNF-α had no significant difference with the control group (1.35±0.36 vs 1.38±0.28, LSD test,P>0.05). The expression of PAR-1 in LPS+APC group was higher comparing with that in the control group (4.60±0.84 vs 2.64±0.41, LSD test,P=0.008) and the LPS group (2.44±0.86, LSD test,P=0.002). The expression of PAR-1 in APC group and LPS group had no obvious difference with control group (2.62±0.69, 2.44±0.86 vs 2.64±0.41, LSD test, bothP>0.05).ConclusionsBy increasing the level of PAR-1 in microglia, active protein C could inhibit the activation of miciroglia and the expression of TNF-α induced by lipopolysaccharide, therefore, protecting the brain tissues from inflammation-induced damage.
6.Changes of CD4(+) CD25 (+) regulatory T cells in peripheral blood in patients with hepatocellular carcinoma before and after TACE.

This study investigated the changes of CD4(+) CD25(+) regulatory T cells (Tregs) in peripheral blood of patients with hepatocellular carcinoma before and after transcatheter arterial chemoembolization (TACE). The proportion of CD4(+) CD25(+) Tregs among CD4(+) T lymphocytes in peripheral blood of 33 patients with hepatocellular carcinoma was determined by flow cytometry before, 1 week and 1 month after TACE. And 25 healthy volunteers served as control. One month after TACE, the patients were divided into two groups: 22 in group A, who were in stable condition or getting better; and 10 in group B, who were deteriorating. One patient died and was excluded. The results showed that the percentage of CD4(+)CD25(+) Tregs among CD4(+) T lymphocytes did not significantly change in the 33 patients 1 week after TACE as compared with that before TACE, however, the difference was significant (P<0.01) between the patients with hepatocellular carcinoma and the healthy subjects. The percentage of CD4(+) CD25(+) Tregs among CD4(+) T lymphocytes in group A 1 month after TACE was decreased significantly in comparison with that before and 1 week after TACE (P<0.01), whereas, that in group B was increased significantly 1 month after TACE (P<0.01). It was concluded that patients with hepatocellular carcinoma had a higher proportion of CD4(+)CD25(+) Tregs in peripheral blood. TACE did not significantly affect the level of CD4(+) CD25(+) Tregs within short time (such as 1 week). The proportion of CD4(+)CD25(+) Tregs in peripheral blood 1 month after TACE was related to the prognosis of hepatocellular carcinoma.

7.Process optimization of compound cotton rose hibiscus leaf ointment and its clinical efficacy and safety verification

BACKGROUND: Compound cotton rose hibiscus leaf ointment is developed from the continuous optimization of the preparation process of traditional famous prescription "Xiaozhong San". Xiaozhong San has been used in Ruijin Hospital Affiliated to School of Medicine of Shanghai Jiaotong University for over 40 years and has achieved good clinical outcomes. OBJECTIVE: To verify the clinical efficacy and safety of compound cotton rose hibiscus leaf ointment, laying a foundation for its popularization and application in the treatment of acute soft tissue injury. METHODS: 112 cases of acute soft tissue injury were divided into trial group (n=55) and control group (n=57) according to the random number table method. The experimental group selected the compound cotton rose hibiscus leaf ointment, and the control group selected the compound Zijing ointment. RESULTS AND CONCLUSION: (1) There were no statistical differences in the primary outcomes, such as pain, tenderness, swelling, dysfunction and in the secondary outcomes, such as ecchymosis, insomnia, dry mouth, constipation, tongue coating and pulse condition, between the two groups before treatment, at 3 and 7 days of treatment and after treatment termination (P> 0.05). (2) The two groups achieved obvious clinical outcomes in the treatment of acute soft tissue injury, but there was no difference in the clinical efficacy between the two groups (P> 0.05). (3) The two groups had no obvious abnormalities in the routine of hematuria and stool and the function of the liver and kidney. (4) There were only five cases of mild skin allergy, including two cases in the trial group and three cases in the control group. To conclude, the compound cotton rose hibiscus leaf ointment has good clinical efficacy and safety, which is worthy of clinical application.

9.Single-center clinical analysis of 9 049 children hospitalized for accidental injury

Objective:To analyze the characteristics of child cases admitted to accidental injury and provide reliable basis for the prevention of accidental injury.Methods:The clinical data of children admitted to Hunan Children′s Hospital aged 0-18 due to accidental injuries from January 1, 2018 to December 31, 2021 were retrospectively analyzed. The type of accidental injury and characteristic factors such as prognosis, gender, age, time, and location of the child were analyzed.Results:A total of 9 049 children with accidental injury were admitted, accounting for 3.72%(9 049/33 697) of the total number of hospitalized children. The top three types of accidental injuries were falls/drop (3 695 cases), foreign bodies/suffocation (2 639 cases) and traffic accidents (1 165 cases), accounting for 82.87%(7 499/9 049). There were 8 760 cases (96.81%) of improvement and recovery, 178 cases (1.97%) of disability, and 111 cases (1.23%) of unhealed/dead. Among the accidental injuries, 5 833 cases (64.46%) were boys and 3 216 cases (35.54%) were girls, and the incidence ratio was 1.81∶1. There was significant difference between boys and girls in the composition ratio of the type of accidental injury such as falls/falls, foreign bodies/suffocation, poisoning, sharp object injury, drowning ( χ2 values were 3.90-20.56, all P<0.05). Among the accidental injuries, the children aged 1 to<3 years had higher accidental injuries than the other age groups (3 263 cases, accounting for 36.06%), and the composition ratio of accidental injuries in different age groups was different ( χ2 values were 12.98-573.97, all P<0.05). Among the accidental injuries, the accidental injuries occurred in the second quarter and the third quarter were higher than those in the other two quarters (4 892 cases, accounting for 54.06%), and the composition ratio of accidental injuries such as falls/falls, foreign bodies/suffocation, burn and scald, drowning occurred in different quarters was different ( χ2 values were 10.79-18.88, all P<0.05). In the case of accidental injuries, the family was the most likely place of accidental injury, with different types of accidental injuries occurring in different places ( χ2 values were 10.08-2 186.54, all P<0.05). Conclusions:Children′s unintentional injuries are most likely to occur in boys aged 1-<3 years, and fall/fall is the main injury type. Traffic accidents are the most important unintentional injury type leading to children′s unhealed/dead. Different injury types were related to child gender, age, quarter, and place of occurrence.Due to the differences in the occurrence mechanism and injury mode of accidental injuries in different countries and regions, and the majority of accidental injuries can be prevented, targeted preventive measures should be taken according to the characteristics of children′s accidental injuries in different regions, and a comprehensive prevention system for children′s accidental injuries should be constructed to ensure children′s safety.
10. Value of chest CT screening in the early COVID-19 outbreak

Objective:
In view of the difficulty of the shortage of new coronavirus nucleal acid test in the early COVID-19 outbreak, to explore the application value of chest CT in screening COVID-19 patients.
Methods:
Retrospective analysis was performed on the data of patients with fever who received chest CT and new coronavirus nucleal acid test during January 25, 2020 to February 2, 2020 in Zhongnan Hospital of Wuhan University. A total of 587 patients were enrolled, including 290 males and 297 females, aged from 11.0 to 96.0 (51.3±17.1) years old. Take the nucleic acid test results as the gold standard, the sensitivity, specificity and rate of missed diagnosis of CT screening COVID-19 were calculated.
Results:
Among the 587 patients, there were 433 positive cases (73.8%, 433/587) and 154 negative cases (26.2%, 154/587) of novel coronavirus nucleic acid test. Using CT screening, 494 cases (84.2%, 494/587) were positive and 93 cases (15.8%, 93/587) were negative. The sensitivity of CT screening COVID-19 was 97.7% (423/433), specificity was 53.9% (83/154) and rate of missed diagnosis was 2.3% (10/433).
Conclusions
In the early COVID-19 outbreak, CT screening has the advantages of high sensitivity and low rate of missed diagnosis of COVID-19, which can compensate for the shortage of new coronavirus nucleal acid test and can be used as the basis for rapid screening for early prevention and control of COVID-19 outbreak.
