1.Research situation and expectation of clinical healthcare big data

Objective To cxplore the research and application of big data mining technology in clinical healthcare environment.Methods The characteristics of clinical heahhcare data mining methods were expounded and the current research status of healthcare data mining technology was analyzed in clinical task according to the literature mining.Meanwhile,the application of healthcare data mining in clinical environment was introduced from the following four aspects:medicine research,personalized diagnosis,risk prediction and process mining.Results The necessity and importance of research and application of clinical healthcare data mining were illustrated.Moreover,the achicvements of healthcare data mining in clinical application,as well as existing problems and technical difficulties were summarized.Furthermore,the development tendencies of healthcare data mining were predicted.Conclusion The research and application of clinical healthcare data mining have improved the healthcare quality and promoted the medical advances,which is the developmcnt direction of clinical healthcare research.

Objective:To study the correlation between in vitro chemosensitivity of Fluorouraci(5-Fu) + Mitomycia (MMC) and its clinical response and prognosis in human colorectal cancers. Method:The chemosensitivity of 5-Fu + MMC was tested in 169 Dukes B and C colorectal cancers with the MTT method,and the progression-free internal and prognosis in 4 years following the surgery were observed. Objective:To study the correlation between early postsurgery phonetic acquisition of speech stop and speech outcomes of young children.Methods:28 children with cleft palate were included in the study.An one-stage palatal repair procedure was per-formed by one surgeon for the children before the age of 1 8 months.Naming tests were used in the speech therapy room when the chil-dren were aged 23 months and 30 months.Stop consonant inventory number,percent correct consonants(PCC),percent correct man-ners(PCM)and percent correct places(PCP)were analyzed.Results:Number of stop consonant was significantly correlated with PCC,PCM and PCP at 24 and 30 months of age in the children.Coefficient of determination between stop consonant number and PCC was 0.535.Conclusion:Speech stop may be used as the “sensitive sound”for the analysis of speech development of the chinese children aged 2-3 years after cleft palate repair and as the individuation guideline to determine the best assessment and therapy time.
4.Resveratrol ameliorates intestinal barrier injury in rats with hemorrhagic shock through superoxide dismutase 2 activation

Objective To explore the protective effect of resveratrol (RSV) on intestine barrier injury induced by hemorrhagic shock and its mechanism in rats.Methods According to random number table method,sixty-four SPF grade male Sprague-Dawley (SD) rats were divided into four groups:Sham operation group (only the catheters were indwelled in arterial and venous passages after anesthesia),hemorrhagic shock model group (model group,the catheters were indwelled in arterial and venous passages after anesthesia,and 0.3 mL solvent was administrated after hemorrhagic shock),RSV group (the catheters were indwelled in arterial and venous passages after anesthesia,15 mg/kg RSV was administered after hemorrhagic shock),superoxide dismutase 2 (SOD2) specific inhibitor,2-Methoxyoestradiol (2-ME) group (on the basic treatment of RSV group,0.1 mmol/L 2-ME was administered).The hemorrhagic shock model was reproduced by femoral artery bleeding.After drug administration,all rats were divided into two parts.One part was used for observations on 24-hour survival rate and survival time,while in the other part,2 hours after the hemorrhagic shock,the blood was collected for determination of the content of serum D-lactic acid,and afterward the rats were executed to obtain small intestine tissues for the examination of histopathological changes and Chiu's score.Moreover,differences of expression levels of tight junction proteins (Occludin,Claudin,ZO-1) of small intestine tissue and the oxidative stress related indexes SOD2 activity and reduced glutathione (GSH),oxidized glutathione (GSSH),malonaldehyde (MDA) contents were compared among the groups.Results Compared with the sham group,the model group demonstrated decreased survival rate,SOD2 activity,GSH content,GSH/GSSH ratio,reduced survival time,significantly increased serum D-lactic acid activity,Chiu's score and MDA content,and decreased expressions of tight junction proteins in small intestine tissue.Compared with model group,the RSV group showed significant increased survival rate [75.0% (6/8) vs.37.5% (3/8)] and prolonged survival time (hours:21.0±4.3 vs.10.4±5.8,P < 0.05),significantly decreased serum D-lactic acid (μg/L:380.18 ± 70.59 vs.500.88 ± 97.53) and Chiu's score (1.75 ± 0.71 vs.4.00± 0.53) in small intestine (both P < 0.05),obviously increased expressions of tight junction proteins,SOD2 activity,GSH and GSH/GSSG [Occludin (gray value):0.89 ± 0.10 vs.0.43 ± 0.77,Claudin (gray value):0.78±0.06 vs.0.33 ± 0.05,ZO-1 (gray value):0.83 ± 0.06 vs.0.34 ± 0.07,all P < 0.05],and the elevated SOD2 activity (kU/L:0.85 ± 0.12 vs.0.51 ± 0.11,P < 0.05],as well as increased GSH content and GSH/GSSG ratio [GSH (μmol/L):7.25±1.01 vs.3.86±0.54,GSH/GSSG:6.39± 1.14 vs.1.56±0.25,both P < 0.05] in the small intestine,and markedly reduced MDA content (ng/g:5.00± 1.31 vs.8.63±0.92,P < 0.05).Compared with RSV group,the 2-ME group demonstrated significantly decreased survival rate [50.0% (4/8) vs.75.0% (6/8)] and further shorter survival time (hours:12.2 ± 5.7 vs.21.0±4.3),increased serum D-lactic acid (μg/L:463.88 ± 60.16 vs.380.18 ± 70.59),obviously elevated Chiu's score (3.13 ± 0.99 vs.1.75±0.71,P < 0.05),decreased expressions of tight junction proteins [Occludin (gray value):0.55±0.04 vs.0.89±0.10,Claudin (gray value):0.38±0.05 vs.0.78±0.06,ZO-1 (gray value):0.41±0.04 vs.0.83±0.06,all P < 0.05];moreover,the activity of SOD2,GSH content,GSH/GSSG ratio were greatly reduced [SOD2 activity (kU/L):0.58 ± 0.13 vs.0.85 ± 0.12,GSH (μmol/L):4.49 ± 0.52 vs.7.25 ± 1.01,GSH/GSSG:1.57 ± 0.39 vs.6.39 ± 1.14,all P < 0.05],and increased MDA content (ng/g:6.25 ± 1.04 vs.5.00 ± 1.31,P < 0.05).The small intestine tissue was basically normal in Sham group,and no significant pathological changes were seen;in the model group,the small intestine epithelial mierovilli were collapsed and the mucosal barrier was destroyed obviously;in the RSV group the damages of small intestine microvilli and barrier were markedly alleviated;in 2-ME group the pathological changes were more evident compared with those in the RSV group.Conclusion RSV can improve intestinal barrier injury following hemorrhagic shock in rats;its mechanism may be related to SOD2 activation.
5.The study of erythrocyte fragility and morphological changes caused by roller pump in vitro.

To observe the erythrocyte fragility and morphological changes of erythrocytes caused by roller pump. Ten tests were divided into two groups, Polystan pediatric pump group A (n = 5) and COBE pump group B (n = 5). Ten whole blood samples (each 400 ml) were circulated in the roller pump for 16 h. Erythrocyte fragility and free hemoglobin were measured before pumping and at every 2 hours during pumping. The possible morphological changes of erythrocytes caused by roller pump were observed by scanning electron microscope. The electron microscopic observation was made before pumping and at every 4 hours throughout pumping. Results showed that the erythrocyte fragility of two groups was not increased during a long period of pumping. The number of acanthocytes of two groups was 1.77/1.81% in the samples before pumping and 6.12/7.13, 9.18/8.73, 13.21/12.89, 16.53/17.21% at 4 h, 8 h, 12 h, and 16 h respectively. The free hemoglobin level of two groups was increased linearly during a long duration of pumping and the index of hemolysis of two groups was 0.296 mg/L/h and 0.3993 mg/L/h respectively. The result shows: 1. the erythrocyte fragility was not increased during a long period of pumping; 2. the erythrocyte membrane was injured or broken by roller pump directly; 3. the morphological changes of erythrocytes would be the basis of post operative hemolysis.

6.Discriminative patterns of traditional Chinese medicine in the non-interventional treatment of malignant lung tumors based on real-world data analysis

[摘 要] 目的:基于临床病历信息系统,采用数据挖掘方法从真实世界角度分析中医药非介入治疗肺恶性肿瘤的辨治规律。方法:收集2015年1月1日至2021年12月31日北京中医药大学东直门医院病历系统中诊断为肺恶性肿瘤的患者病史、刻下症和用药信息,在剔除使用介入治疗手段的病例后建立数据库。采用Microsoft Office Excel 2019对高频药物的频数、四气五味、归经及功效进行描述性统计,并对病史和刻下症反映的症状进行数据化统计;对统计得到的高频药物和症状信息借助SPSS Modeler 18.0软件中Apriori算法对核心数据进行关联规则分析,并用Web节点构建关联网状图;借助SPSS Stastistics 23.0进行聚类分析。结果:共纳入119例未进行介入治疗的肺恶性肿瘤患者,中医常见症状为咳嗽、白痰、喘憋、纳差、眠差、痰黏、便秘、乏力、胸闷、气短;药物四气以温药最多;五味以甘味最多;归经以肺经频率最高。关联规则结果显示,二项支持度最高的组合为半夏→柴胡、半夏→瓜蒌、杏仁→麻黄;三项关联支持度最高的组合为半夏→茯苓、柴胡,杏仁→麻黄、半夏,半夏→杏仁、柴胡,半夏→瓜蒌、贝母。聚类结果聚为3类,依次为肺阴虚证、痰热壅肺证和肺气虚证。结论:中医药非介入治疗肺恶性肿瘤的中医药辨治规律为:聚类证型依次为肺阴虚证、痰热壅肺证和肺气虚证。治疗多用和解少阳、行气止咳平喘和清虚热化痰饮为主,药物以半夏、甘草、黄芩、茯苓、柴胡多见。
7.1990-1992 mortality of stomach cancer in China.

OBJECTIVETo assess the impact of stomach cancer on the Chinese population by epidemiological analysis of its mortality distribution.
METHODS1990-1992 data on stomach cancer mortality collected by sampling survey involved one tenth of the total Chinese population.
RESULTSThe crude mortality rate of stomach cancer in China was 25.2 per 10(5) (32.8 per 10(5) for males and 17.0 per 10(5) for females), which comprised 23.2% of the total cancer deaths from 1990 to 1992, making stomach cancer the leading cause of cancer death. The stomach cancer mortality rate of males was 1.9 times of that of females. The Chinese mortality rates of stomach cancer adjusted by the world population were 40.8 per 10(5) and 18.6 per 10(5) of males and females, which were 4.2-7.9 (of males) and 3.8-8.0 (of females) times of those in the developed countries. Age-adjusted mortality rates of stomach cancer in China have distinct geographical difference: form the lowest 2.5 per 10(5) to the highest 153.0 per 10(5) in the 263 surveyed localities, 15.3 per 10(5) in urban areas and 24.4 per 10(5) in rural areas giving a difference of 1.9 times.
CONCLUSIONThe prevention and treatment of stomach cancer in China, especially in the countryside and the under-developed areas in the northwest, should be a long-term focus in control of cancers of the digestive system. Urgent measures for prevention and early detection of stomach cancer should be taken.

8.One case report of primary mucinous adenocarcinoma combined with signet-ring cell carcinoma of female urethra

Primary female urethral adenocarcinoma is rare. This paper reports a case of primary urethral mucinous adenocarcinoma complicated with signet ring cell carcinoma. The patient underwent urethral tumor resection in another hospital. Postoperative examination indicated that the tumor remained, and the tumor was completely removed after urethral tumor resection. After 11 months of follow-up, there was no tumor residue or recurrence.
9.Coil design and simulation analysis of magnetic induction hyperthermia in rats

A Helmholtz coil is selected to generate the alternating magnetic field,and the relevant model is established with a rat brain tumor as the research object.Based on the Pennes bioheat transfer equation,the electromagnetic field distribution and temperature field distribution are calculated in COMSOL simulation software,and the factors affecting the outcome of magnetic induction hyperthermia are analyzed.The results show that both magnetic field distribution and temperature field distribution meet the requirements for magnetic induction hyperthermia.The magnetic field distribution in the tumor treatment area is uniform,and the central magnetic induction strength is 12.847 mT.The temperature rise in the tumor area is significant,and the temperature at the tumor center is 46℃or above,basically reaching the treatment temperature.The therapeutic efficacy of magnetic induction hyperthermia is affected by the number of turns,current,radius and spacing,magnetic field frequency and other parameters.The study provides reference for the clinical application of magnetic induction hyperthermia and the coil design.
10.Challenges and countermeasures in building national regional medical centers for mental health

As guided by the policy of building national regional medical centers, it is imperative to advance the construction of the national regional medical centers for mental health to achieve the sustainable and balanced development of the mental health cause of the country. The authors summarized the current status of national regional medical centers, and analyzed challenges faced in management and operation mode, government investment and compensation mechanism, expansion of high-quality psychiatric medical resources, as well as research and innovation of mental diseases of these centers. On such basis, the authors put forward corresponding countermeasures and suggestions for the next stage of development.
