1.Factors Impacting Whereabouts and Referral in Patients with Acute Stroke after Discharge

Objective To study the selection of the follow-up care and related factors in stroke patients after acute treatment in general hospital. Methods A total of 230 acute stroke patients discharged from January to December, 2015, were reviewed. They were classified with age, modified Rankin Scale (mRS) score, referral options, etc., and the key consideration in the selection were compared. Results There were 125 patients needing referral. Referral increased while mRS worsening (P<0.05). The demand for the continuation of rehabilita-tion was higher in the patients with mRS scores of 0-1 and 2-3 than that of 4-5 (F=4.300, P<0.05), but the latter cared more about medical quality (F=3.464, P<0.05) and hospitalization length (F=3.692, P<0.05). The patients more than 80 years old cared more about medical qual-ity (F=4.630, P<0.05). The patients selecting continuous treatment in our hospital paid more attention on continuation (F=7.590, P<0.001), while those selecting rehabilitation institutions cared more about rehabilitation capacity (F=32.156, P<0.001), and those selecting communi-ty hospitals or nursing home cared more about hospitalization length (F=18.823, P<0.001), easy to visit (F=14.070, P<0.001), etc. Referral information mainly came from the doctors (72.0%), only 28.0% from the patients. Conclusion Severity of disability, age and the prognosis may impact the choice for the following rehabilitation in acute stroke patients. Doctors can do more for reasonable referral recommendations to the patients.

Objective To understand the current status and related factors of unexpected injuries during hospitalization in old patients in Shaoxing city. Methods By way of convenient sampling methods, 850 old patients aged over 65 years were chosen from five of grade 3 hospitals in Shaoxing city. The status and consequence of unexpected injuries during their hospitalization in past one year were retrospectively investigated with face to face questionnaire at patients home. 4.Detection of Mutations in the Wilms' Tumor Gene (WT1) in Leukemias

Objective To analyze mutations in the Wilms tumor gene (WT1) in Ieukemogenesis.Methods WT1 gene in peripheral blood was determined by PRC-SSCP technique,in 32 cases of acute leukemia,included 9 cases of acute granulocytic leukemia,8 cases of chronic granulocytic leukemia(mean age-33 years) and 16 specimens of normal subjects were also detected.Results Mutations in the WT1 gene in 3 of 32 leukemias were found .WT1 mutations were found in 11% of cases of acute lymphoblastic leukemia and in 13% of cases of acute myeloid leukemia,in which they were associtated with a poor response to chemotherapy.Conclusions The mutations in Wilms tumor gene WT1 are associated with leukemogenesis and its therapy,which WT1 transcripts may prove a significant tumor marker, as a MRD monitor in evaluating remission status and early relapse,and may be useful in prognosis of acute leukemia.
5.The diagnostic value of MCP-1 and RANTES in tuberculosis and malignant pleural effusion

Objective To determin and compare the significance of the expression of chemotactic cytokines in pleural effusions monocyte chemoattractant protein-1(MCP-1)and regulated on activation,normal,T-cell expressed and secreted(RANTES)levels in tuberculous and malignant pleural effusions.Methods The effusion and serum levels of MCP-1 and RANTES were measured in 37 patients with tuberculous pleural effusion and 35 patients with malignant pleural effusion,and selected 20 healthy people acting as control group.Results The effusion and serum levds of MCP-1 and RANTES in tuberculous effusion patients was significantly higher than that in malignant group(P<0.01);In tuberculous group,the content of MCP-1 in serum was higher than control group,the serum level of RANTES was no statistic significant difference between control group and tuberculous group.The contents of MCP-1 and RANTES were not correlated with pathologic type of lung.cancer.Conclusion MCP-1 and RANTES technique has an important clinical value in diagnosis and differential diagnosis of tuberculosis and malignant pleural effusion.
6.Investigation of the care burden and self efficacy of caregivers of patients with spinal cord injury

Objective To investigate the level of care burden and self efficacy of caregivers of patients with spinal cord injury,and analyze their correlation.Methods By convenience sampling method,150 cases of spinal cord injury patients and their caregivers were investigated with Zarit Caregiver Burden Scale and general self-efficacy scale.Results The score of care burden of caregivers with spinal cord injury was (52.91 ± 11.56) points,self-efficacy score of caregivers was (19.63 ± 4.85) points.The score of care burden of caregivers and self-efficacy was highly negatively correlated.Conclusions We should pay more attention to improve the self-efficacy of caregivers of patients with spinal cord injury (SCI),in order to mitigate the care burden,improve care ability and health level of the patients' families.
7.Investigation of the change in redox state of plasma factor Ⅺ in elderly type 2 diabetic patients with macroangiopathy

Objective To investigate the potential effect of the redox state of plasma factor Ⅺ (FXI) on the pathogenesis of elderly diabetic hypercoagulability and macroangiopathy.Methods The plasma levels of reduced FXI were detected in elderly type 2 diabetic(T2DM)patients with/without macroangiopathy (T2DM group/DMAP group) and healthy subjects (control group),and variables associated with reduced FXI were analyzed.Results Elderly patients with T2DM had higher plasma levels of reduced FXI as compared with healthy controls.The level of reduced FXI was significantly higher in patients with macroangiopathy than without macroangiopathy [control group:(80.6± 15.6) %,T2DM group:(94.7 ± 16.0) %,DMAP group (142.6 ± 36.5) %,all P< 0.05].The multiple stepwise regression analysis showed that plasma levels of triglyceride,cholesterol and HDL-cholesterol were the independent predictors for reduced FXI.Conclusions The plasma level of reduced FXI is increased in elderly T2DM patients with macroangiopathy.The abnormality of lipid profiles may associate with the increment of reduced FXI.These findings maybe provide the novel mechanisms for diabetic hypercoagulability and macroangiopathy.
8.Study on the Quality Standard of Compound Vaccariae Tablets

OBJECTIVE:To conduct supplementary study for quality standard for Compound vaccariae tablets. METHODS:TLC was used for the qualitative identification of Vaccariae;HPLC was used to determine the contents of vaccarin and anthranilic acid. The column was YMC-Pack Pro C18 with mobile phase of methanol- 0.3% phosphpric acid(gradient elution)at the flow rate of 1.0 ml/min,the detection wavelength was 280 nm,column temperature was 35 ℃ and volume size was 10 μl. RESULTS:The chromatographic spots were clear and well-separated without interference from negative control. The linear range was 0.055 7-2.228 7μg(r=0.999 9)for vaccarin and 2.011 4-40.227 9 μg(r=0.999 9)for anthranilic acid;RSDs of precision,stability and reproduc-ibility tests were no more than 1.01%;average recoveries were respectively 99.69%(RSD=1.11%,n=6) and 99.84%(RSD=1.18%,n=6). CONCLUSIONS:The method is simple,rapid and accurate with good reproducibility,and can be used for quality control of Compound vaccariae tablets.
9.Qualitative study of care burden of caregivers who take care of patients with spinal cord injury

Objective To investigate the real situation of the burden of the caregivers who take care of patients with spinal cord injury.Methods Using phenomenological research methods of qualitative study,information of 15 family caregivers of patients with spinal cord injury were collected through semi-structured interviews in Jiangxi province,and data were analyzed with Colaizzi 7-step analysis method.Results Four themes were summarized:weak social support system,heavy economic burden,insufficient care ability,mental and physical exhaustion; mental and physical exhaustion concluded three sub-themes:burnout,lifestyle changes,sad/helplessness.Conclusions Care burden of caregivers of patients with spinal cord injury includes four aspects:social,physical,psychological,and less caring capacity.Care workers should be concerned about the health status of the caregivers,give targeted care intervention,so that caregivers can be adapted to the caregivers' role,maintain and promote a high level of health of the patients and their families.
10.Protective effect of Ginaton on lipopolysaccaride-induced acute lung injury in rats

Objective To investigate the effects of Ginaton on acute lung injury (ALI) induced by lipopolysaccaride (LPS).Methods The acute lung injury rat model was induced by receiving 5 mg/kg LPS injection in tail vein.A total of 63 Wistar rats were divide into 3 groups without caring sex.Saline control group,LPS treated group and Ginaton treated group.The samples of different groups were collected 2 h,6 h,and 10 h after tail vein administration.Serum soluble cell ashesion molecule-1 (sICAM-1) was measured by ELISA,immunohistochemical staining and Western blotting of NF-kB were performed on sections of lung specimens.Results The acute lung injury rat model was successfully induced by receiving LPS injection in tail vein.The sICAM-1 levels increased more in Ginaton treated group than those in Saline control group (P<0.01),and the increase in LPS treated group were the highest.By immunohistochemical staining,the positive NF-kB cells in Ginaton treated group were much less than those in LPS treated group (P<0.01),and in Saline control group were least (P<0.01).The results of the Western-blot method were consistent with immunohistochemical method.Conclusion ALI could be induced by LPS,Ginaton showed a protective effect through probably inhibiting activation of NF-kB on LPS-induced-ALI in this animal model.
