1.Application of low molecular weight heparin combined with psychological intervention in prevention of deep venous thrombosis after lower limb fracture surgery

Objective To study the clinical application of low molecular weight heparin combined with psychological intervention in the prevention of deep venous thrombosis after lower limb fracture surgery.Methods 100 patients with lower limb fracture treated in our hospital from January 2013 to December 2016 were selected as the research object in the course of the study, and randomly divided into control group and experimental group, with 50 cases in each group,.The patients in control group were treated with routine lower limb physical pressure to prevent deep venous thrombosis after lower limb fracture.The patients in the experimental group were treated with low molecular heparin combined with psychological intervention to prevent deep vein thrombosis after lower limb fracture.Comparative analysis of patients with prothrombin time and platelet and the experimental group and the control of activated partial thromboplastin time index.Results After different treatment, the platelet index of the experimental group was(189.23±36.23)×109 /L, significantly better than that of the control group(264.34±39.56)×109/L,the difference was statistically significant(P<0.05).After treatment, the patients in the experimental group the activated partial thromboplastin time and prothrombin time index respectively(35.62±4.23)s and(16.12±2.19)s, significantly better than the control group, the difference was statistically significant(P<0.05).Conclusion low molecular weight heparin combined with application of psychological intervention in prevention of deep venous thrombosis after lower limb fracture surgery, can improve clinical curative effect on prevention of deep vein thrombosis in a large extent, improve blood hypercoagulable state of patients after surgery, with further clinical promotion and application significance.

Matrix metalloproteinase-9 (MMP-9)is a endopeptidase that can degrade extracellular matrix and plays an important role in invasion and metastasis of tumor.This article reviewed the advances in studies on RKIP and MMP-9 in colorectal cancer.
3.Berberine hydrochloride combined with sodium ozagrel in the treatment of acute cerebral infarction and its effect on glucose and lipid metabolism

Objective To investigate the effects of sodium ozagrel and berberine hydrochloride in the treatment of acute cerebral infarction and its effect on glucose and lipid metabolism.Methods According to the digital table,158 patients with acute cerebral infarction were randomly divided into the two groups:the control group was treated with the sodium ozagrel group and the observation group was treated combined with sodium ozagrel and berberine hydrochloride.After 2 weeks,Chinese stroke clinical neural function defect score scale and the National Institutes of Health Stroke Scale (NIHSS) was used to evaluate neurological function.The therapeutic effectiveness was assessed by ADL after treatment 1,3,6 months.The changes of glucose and lipid levels were detected by turbidimetric method.Results The total effective ratein the observation group was obviously higher than that of the control group (87.01% vs 69.14%) (x2 =7.316,P <0.01).Of the above two ways of nerve function score indicateed that the observation group was significantly better than that of the control group (t =3.207,1.274,all P < 0.01).ADL score of all patients was significantly better than that of before treatment with drug in the two groups after 1 month,3 months and 6 months (t =7.266 and t =8.850,t =1.429 and t =2.816,t =9.218 and t =6.739,P <0.01).Compared with the control group,ADL score of the observation group was significantly improved after 3 months and 6 months (t =1.163,0.932,all P <0.01).The significant difference was found between the control group and the observation group after 3 months and 6 months (t =7.861,5.005,all P > 0.05).After the treatment,the fasting blood glucose level was reduced significantly in the observation group (t =0.859,P < 0.01),total cholesteroland high-density lipoprotein,triglyceride,low density lipoprotein level were significantly decreased (t =0.257,0.114,0.378,all P < 0.01),but there was no significant difference in HDL-C level (t =1.960,P > 0.05).Conclusion The therapeutic effect of berberine hydrochloride combined with sodium ozagrel is better in the treatment of acute cerebral infarction,and the level of glucose and lipid metabolism was significantly decreased.
4.Design and efficacy of surgery for horizontal idiopathic nystagmus with abnormal head posture and strabismus.

The design and efficacy of surgery for horizontal idiopathic nystagmus (HIN) with abnormal head posture and strabismus were investigated. Different surgical procedures were selected according to the angle of head turn in 44 cases of HIN with abnormal head posture and strabismus. For patients with a head turn of 15° or less, the Anderson procedure was used; the yoke muscles were recessed upon slow-phase. For patients with a head turn between 15° and 25°, the surgery was designed as a Kestenbaum 5-4-4-5 procedure. For patients with a head turn of 25° or more, the surgery was designed as a Parks 5-8-6-7 procedure. The surgery to correct the abnormal head posture was performed on the fixating eye while that to correct the deviation was then performed on the non-fixating eye at the same time. The amount of surgery of the horizontal rectus muscles on the non-fixating eye was sum of the angle of head turn and the degree of deviation, which was calculated as follows: recession/resection amount of medial and lateral rectis/2×5=angle of head turn±degree of deviation. The results showed as follows: (1) Visual acuity: the visual acuity in the primary ocular position increased two lines or more in 35 patients, accounting for 79.55%. Nine patients had no or only one-line improvement, accounting for 20.45% of the entire study population; (2) The degree of deviation in the primary ocular position: 37 cases had a normal primary ocular position or the degree of deviation ≤8(δ) after surgery, accounting for 84.09%. Six patients had a residual degree of deviation of 8(δ)-15(δ), accounting for 13.64%. One patient had a residual degree of deviation >20(δ), accounting for 2.27% of the patients examined; (3) Abnormal head posture: 34 patients had a normal head posture or a head turn of less than 5°, accounting for 72.27%. Eight patients had a residual head turn of 5°-15°, accounting for 18.18%. Two patients had a head turn of 15°-25°, accounting for 4.55%. It was concluded that different surgical procedures based on the angle of head turn and the relationship between deviation and null zone can eliminate anomalous head posture, correct deviation, and improve vision acuity in the primary ocular position.
5.The clinical analysis of intraocular pressure elevation after pars plana vitrectomy

Objective To investigate the risk factors of the intraocular pressure (IOP) elevation after pars plana vitrectomy (PPV). Methods Eighty eight patients (88 eyes) of postoperative ocular hypertension in a series of 339 patients who had undergone PPV with normal ocular pressure before operation were retrospectively studied. The ocular pressures in both preoperative and postoperative periods were detected by NCT examination, and the ocular hypertension was decided on the level of ≥25 mm Hg. The relationships of occurence of the time of onset and duration of persistence of postoperative ocular hypertension with the different kinds of primary diseases, the techniques of operation, and the condition whether or not the affected eyes had formerly accepted surgical intervention, were analyzed. Results The IOP elevation occures mostly within 1 to 2 weeks postoperatively (77 eyes, 87.5%). In 65 cases (65 eyes) IOP returned to normal in 1 week, and in another 14 cases (14 eyes) in 1 month after treatments. Six patients' (6 eyes ) IOP was under 25 to 30 mm Hg with the medicine. With sustained elevation of IOP over 4 to 6 months, 3 cases (3 eyes ) lost or almost lost their vision finally. Objective To investigate the clinical efficacy and ultrasonographic changes treated with anti-thyroid drugs ( ATD ) in patients with primary hyperthyroidism(PHT).Methods 83 cases of PHT patients admitted to the department of ultrasound from February 2013 to August 2015 in Zhuji People's Hospital of Zhejiang Province were selected, the patients were divided into two groups according to the results of ultrasonography, echo nonuniform group 41 cases and echo uniform group 42 cases.ATD therapy was administered to the both groups (were treated with methimazole).The thyroid related indexes and hemodynamics of thyroid right superior diagnosed by color doppler ultrasound, and the curative efficacies of the two groups were analyzed.Results After treatment, the systemic vascular resistance index(RI) of echo uniform group was (0.62 ±0.17), and the echo nonuniform group was (0.65 ±0.18), there was no significant difference between the two groups.The echo uniform group of thyroid artery diameter on the right lobe(D), the peak systolic velocity(Vmax), minimum diastolic blood flow velocity(Vmin), thyroid volume(V) and blood flow volume(Q) were significantly lower than the echo nonuniform group, the difference was statistically significant (P <0.05).The total effective rate of the echo nonuniform group (48.78%) was significantly lower than that in the echo uniform group (85.71%), the difference was statistically significant(P <0.05).Conclusion ATD has a good effect in treating PHT, and can significantly improve the thyroid hemodynamics in patients, it has a positive effect on the regulation of thyroid-related parameters in patients, the clinical treatment is effective, and the effect is better especially in patients with uniform echo.
7.Identification of hemolysin BL from Bacillus cereus of patients with endophthalmitis and its biological activity detection in vitro

Objective To identify the hemolysin BL(HBL) gene from Bacillus cereus of patients with endophthalmitis comfirmed by API bacterial identification test strip,and detect its biological activity in vitro.Methods Three pairs of specific primers were designed according to the gene sequence of HBL(B,L1 and L2 components),then the PCR assay were established through condition optimization,and to further detect five Bacillus cereus strains isolated from clinical patients with endophthalmitis.HBL with biological activity was extracted by salt fractionation from a randomly selected strain,and a series of different concentrations of HBL were prepared and acted on sheep red blood cells(SRBC),Vero cells and Hela cells; virulence of HBL was also assessed through observating lethal effect of which on mice with intraperitoneal injection.Results Three genes of HBL were detected in all B.cereus strains from clinical patients; Strong hemolytic activity of HBL showed obvious time-and dose-dependent.In the study,we found the morphological changes of Vero and Hela cells caused by HBL were different,but cell death were the same result with contents released; Within 48 h after infection,lethality of HBL for mice showed 100% with the concentration of more than 2.0 HU/ml,and was also in a time-and dose-dependent manner.Conclusion HBL isolated from B.Cereus had high hemolysis activity and low concentration.The expression of all BL genes provided a strong basis for the clinical feature of B.Cereus infection,which was developed rapidly and with a poor prognosis.It also provided a new method for rapid diagnosis and molecular epidemiology investigation in clinical.
8.Influence of P-glycoprotein inhibitor on the amphotericin B transport across the blood brain barrier

Objective To determine the influence of P-glycoprotein (P-gp) inhibitor on the blood brain barrier (BBB) transport of amphotericin B (AmB)..Methods An in-vitro BBB model was established with brain capillary endothelia cells (BCEC). AmB was chosen as the test drug and verapamil was chosen as the inhibitor of P-gp.Cellular uptake of AmB at different time points and with series of verapamil concentrations were performed respectively after the determination of appropriate incubation time and drug dosage by the cytotoxicity assay. The AmB concentrations of series of samples were detected using high performance liquid chromatography (HPLC) method. One-way ANOVA analysis and Bonferroni test were used for data analysis.Results The cellular transport of AmB was accumulated as the time prolonged.The inhibitor group had a significant higher cellular uptake levelsof AmBat the time point of 90 min (t=6.753,P=0.001),120 min (t=3.574,P=0.016) and 150 min (t=4.759,P=0.005) as compared with the control group.The AmB cellular uptake level increased significantly when BCEC were incubated with verapamil of 2 μmol/L (P=0.000),5 μmol/L (P=0.014),10 μmol/L (P=0.000),50 μmol/L (P=0.014),75 μmol/L (P=0.000) and 100 tμmol/L (P=0.000),respectively,compared with the control group.Conclusion The P-gp inhibitor verapamil can enhance the cellular uptake of AmB which indicates that P-gp is involved in the BBP transport of AmB.
9.Catalytic metalloporphyrin protects against MPTP-induced Parkinson's disease in mice

Objective: To observe the effects of manganese( Ⅲ ) meso-tetrakis (N, N'-diethylimidazolium-2-yl) porphyrin (MnTDM) in treatment of early Parkinson's disease(PD) mouse model induced by subcutaneous injection of 1-methyl-4-phenyl1, 2, 3, 6-tetrahydropyridine(MPTP) and to discuss its possible mechanism. Methods:Forty male C57BL/6 mice were evenly randomized into 4 groups: MPTP model group(subcutaneous injection of 25 mg/kg MPTP for 3 days), MnTDM+ MPTP group (15 mg/kg MnTDM was subcutaneously injected 1 h before MPTP injection), MnTDM control group, and normal saline group. Performance of animals in the pole and swimming test was observed 3 days after the last injection. Levels of dopamine (DA) and its metabolites(3,4-dihydroxyphenylacetic acid [DOPAC] and homovanillic acid [HVA]) in the striatum of animals were measured by high-performance liquid chromatography with an electrochemical detector(HPLC-ECD). Thiobarbituric acid (TBA) method was used to examine the levels of malondialdehyde(MDA). Results: Acute injection of MPTP could be used for establishment of PD model. The striatal levels of DA, DOPAC and HVA in MPTP group were significantly lower(P<0.01)and the striatal level of MDA was significantly higher(P<0.05) than those of the control group. MPTP had no obvious effect on the behavioral performance of the animals in a short term. MnTDM could partly inhibit the above effects of MPTP. Compared with MPTP group, MnTDM+ MPTP group had significantly higher DA, DOPAC, and HVA levels and significantly lower MDA level(all P<0.05). There was no significant difference in the behavioral indices of animals between the 4 groups. Conclusion:MnTDM can inhibit lipid peroxidation and promote DA production; it has preventive and therapeutic effects on MPTP induced PD.
