1.The clinical effect of endodontical therapy in the treatment of cracked teeth with different crack depth

Objective:To observe the clinical effect of endodontic therapy and full crown restoration in the treatment of cracked teeth. Methods:The trace and depth of 50 cracked teeth were carefully examined under root canal microscope.According to the depth and scope of the cracks,the cracked teeth were classficated into the degree of Ⅰ,Ⅱ and Ⅲ,treated endodontically and restored with full crown.Results:During 1 year follow-up,34 cases showed successful effect,5 progressive,1 1 failure.The general effectiveness rate was 78.0%.The effectiveness rate of group Ⅰ,Ⅱ and Ⅲ was 93.3%,82.1 % and 28.6% respectively.Group Ⅰand Ⅱ vs Ⅲ,P <0.05,Ⅰvs Ⅱ,P >0.05.Conclusion:The treatment effect of cracked teeth is closely related to the depth and scope of cracks.
2.Management of cardiopulmonary bypass for infants below 10 kilograms with congenital heart diseases

Objective To summarize the management of cardiopulmonary bypass(CPB) for infants below 10 kilograms with congenital heart diseases.Methods From Jan 2010 to Apr 2011,the clinical datas of 122 infants aged from 1.5 months to 2 years with body weight 3 to 10 kilograms,who underwent open heart surgery under mild or moderate hypothermia CPB were retrospectively analyzed.Results Among all the 122 infants,CPB time was 13 ~ 118 min [(62.69 ± 21.48) min],aortic cross-clamp time was 0 to 86 min [(35.47 ± 19.51) min].All patients were spontaneous resuscitation and successfully weared from the machine,no severe complications associated with CPB occurred,3 infants died (2.46%,3/122) after operation.Conclusion Using membrane oxygenator,circuit tubing and artery filter with less priming,and highflow perfusion during CPB,maintaining hemodynamics stable,holding reasonable hematocrit and colloid osmotic pressure,good myocardial protection and ultrafiltration are the significant elements of the management of CPB in infants with weight less than 10 kilograms.
3.Silencing gene of TLR4 down-regulates the effect of TLR2 signal transduction in RAW264.7 cells to anti-Aspergillus fumigatus conidia stimulation

Objective To study the role of TLR2 and TLR4 signal transduction in RAW264.7 monocyte-macrophages stimulated by Aspergillus fumigatus conidia,and to investigate the expression of TLR2 signal transduction after silencing gene of TLR4.Methods Macrophages were randomly divided into normal group ( N group),normal+stimulated with Aspergillus fumigatus conidia ( N +Af group ),normal + transfected with TLR4-siRNA [ TLR4 (RNAi) group ],normal+transfected with TLR4-siRNA +stimulated with Aspergillus fumigatus conidia[ TLR4(RNAi) +Af group].RT-PCR and Western blot were used to assay expression levels of TLR2,TLR4,MyD88 mRNA and pro-inflammatory cytokines TNF-α protein when macrophages were stimulated 12 h by Aspergillus fumigatus conidia after tranfected 24 h with TLR4-siRNA by technology of RNAi.Results ( 1 ) Compared with N group,the expression of TLR2,TLR4,MyD88 mRNA and TNF-αprotein in N+Af group significantly increased before silencing gene of TLR4.(2) Silencing efficiency of macrophates was up to 83% after transfected with TLR4-siRNA.(3)The expression of TLR2,MyD88 mRNA in TLR4 (RNAi) group significantly decreased contrast with normal group.Meanwhile the expression of TLR2,MyD88 mRNA and TNF-α protein also obviously reduced in TLR4(RNAi) +Af group when compared with N +Af group.Compared with TLR4 (RNAi) group,the expression of MyD88 mRNA in TLR4 (RNAi) +Af group significantly increased.However,the expression of TLR2 mRNA and TNF-α protein have no significant change after silencing gene of TLR4.Conclusion Signaling pathway of TLR2 and TLR4 in macrophages was activated by given stimulus of Aspergillusfumigatus conidia and exerted the effect of anti-Aspergillus fumigatus spores stimulation through the release of pro-inflammatory cytokines TNF-α.Meanwhile,silencing gene of TLR4 down-regulate the effect of TLR2 signal transduction in RAW264.7 cells to anti-Aspergillus fumigatus conidia stimulation,and it found that TLR4 played an more important role by contrast with TLR2.
4.Root canal morphology of deciduous tooth.

OBJECTIVETo find out the morphological regularity of root canal in the primary tooth.
METHODSThrough transparent specimen of a set of deciduous teeth, the shape of root canal was observed under stereomicroscope.
RESULTSAmong the deciduous anterior teeth, 92% were single canal except for one single-double mixed canal. Maxillary deci-duous molar had 3 canals and mandibular deciduous molar had 4 canals, lateral canal and anastomosis between the root canals were popular in deciduous molar.
CONCLUSIONDeciduous root canal morphology is simple in anterior teeth and comparatively complex in molar.

5.Effect of combined application of dental endoscope and ultrasonic technique in treating concealed canals

Objective :
To investigate the efficiency of combined application of dental endoscope and ultrasonic technique in treating concealed root canals.
Methods :
87 complicated and difficult cases which were not found root canals during endodontic treatment by naked eyes were selected. The canal orifices were probed and negotiated with the aid of dental endoscope and ultrasonic instruments. And the success rate was recorded.
Results:
121 concealed root canals which from 87 complicated and difficult cases were classified into 64 calcified canals, 39 narrow curved canals and 18 canals with anatomic variation. Among them, 94 canals were found and then negotiated, and the total success rate was 77.68%. There was no significant difference between the success rates in treating calcified root canals, narrow curved canals and canals with anatomic variation (χ2=5.477, P > 0.05).
Conclusion
The combined application of dental endoscope and ultrasonic instruments can effectively improve the success rate of finding and negotiating concealed canals.
6.A Case Report of Blau Syndrome

Blau syndrome is a rare genetic disorder characterized by the a mix of granulomatous arthritis, uveitis, and dermatitis. Patients typically manifest multisystem involvement, including ocular, skin, and skeletal abnormalities. Blau syndrome is extremely rare, with a global incidence of less than one in a million among children. In this multidisciplinary consultation, we present a case of a 21-year-old young female patient having multisystemic involvement since early childhood. She was presented with multiple joint swelling, skin lesions, increased eye discharge, and accompanied by hypertension and arterial abnormalities, and received a diagnosis of uveitis. The patient had been receiving steroid treatment since the age of 6 and has tried various medications, with some improvement in joint swelling and ocular symptoms. Through this rare disease multidisciplinary consultation, we aim to provide guidance in the molecular diagnosis of the patient, multisystem assessment, and the selection and formulation of treatment plans. Additionally, we hope that by reporting this case, clinical physicians can gain a better understanding of the diagnosis and comprehensive treatment strategies for Blau syndrome, thereby improving the management and treatment of rare diseases.
