1.Diagnosis and management of intraparotid facial nerve schwannoma.
West China Journal of Stomatology 2015;33(4):431-435
Intraparotid facial nerve schwannoma (IFNS) is a rare benign tumor. The management of IFNS is very challenging because of the lack of appropriate methods for preoperative diagnosis, which is often conducted intraoperatively in most cases. This article reviewed the literature on IFNS recorded in PubMed from 1958 to 2014 and described in detail its clinical manifestations, diagnoses and differential diagnoses, and treatment options. Accurate diagnosis for IFNS mainly depends on intraoperative observation and postoperative histological examination. Preoperative facial nerve function, localization, and adherence, as well as preferences of IFNS patients are important factors to consider in the decision-making process for IFNS management to optimize the outcomes. Surgical resection is usually reserved for patients with facial function of House-Brackmann grade III or worse; otherwise, conservative treatment can be adopted.

Objective To compare the number of DARPP-32 positive cells yield in the grafts based on the application of single cell suspension by neural micro-transplantation technique and by traditional cell delivery technique and to explore effects and mechanisms of different approaches. Methods Cells derived from the whole ganglionic eminence of E15 rat embryos, ubiquitously expressing Green Fluorescent Protein(GFP) were implanted into unilaterally QA-lesioned rat striatum in a single-tract with an ultra-thin glass capillary with an outer diameter of 50 μm or using traditional cannula tip with a diameter of 500 μm.Results Histological assessment at 4 months after transplantation showed that there was about two-fold DARRP-32 positive striatal-like neurons in the micro-transplantation group(TT group) than that in the traditional group(MT group).Total graft volume was similar in both groups[(2.8±0.2) mm3 vs(2.5±0.4)mm3,F =0.25,P > 0.05].And DARRP-32positive plaque volume[(0.6±0.1) mm3 vs(0.6±0.2) mm3,F =0.90,P > 0.05]and TH staining plaque volume[(1.0±0.1) mm3 vs(0.7±0.1)mm3,F =1.44,P > 0.05]also had the same performance in both groups.Number of DARRP-32 positive cells was calculated by Abercrombie correction formula, and the result showed that the number of DARRP-32 positive cells in MT group was two-fold of that in TT group[(20.1 ×103±1.2 × 103) vs(9.8 × 103±3.2 × 103),F =8.62,P < 0.05].Higher DARRP-32 positive cells in MT group indicated that grafts had a better condition of growth and development. Conclusion Micro-transplantation approach can increase the number of new born striatal-like neurons, potentially due to the enlargement of the graft-host border area intensifying the graft's exposure to host derived factors and the minimized mechanical injury.

Objective To investigate the diffusion-weighted imaging (DWI)characteristics and the correlation of diffusion coeffi-cient (ADC)and differentiation grade of rectal tubular adenocarcinoma. Methods The DWI characteristics of thirty-eight patients pathologically comfirmed rectal tubular adenocarcinomas were evaluated respectively. There were 9 highly differentiated tubular ade-nocarcinomas,1 5 moderately differentiated tubular adenocarcinomas and 14 poorly differentiated tubular adenocarcinomas. The ADC value was calculated from two different factors (0 and 1 000 s/mm2 ).The correlation between ADC value and the differentiated de-gree of tumour was statistically analysed. Results The mean ADC for highly differentiated group was (0.92±0.05)×10 -3 mm2/s, and the mean ADC for moderately differentiated group was (0.79 ± 0.10)× 10 -3 mm2/s whereas that for poorly differentiated group was (0.71±0.06)×10 -3 mm2/s.The difference among three groups was statistically significant(P <0.05).The ADC value and the differentiated degree of tumour were negatively correlated(Spearman r =-0.704)P<0.01.Conclusion DWI and ADC value can be a useful estimating for the differentiation of rectal tubular adenocarcinoma.

Objective To investigate the influencing factors of hemorrhagic transformation in non-thrombolysis patients after acute cerebral infarction. Methods According to Chengdu Stroke Registry Project,2598 consecutive patients with acute cerebral infarction admitted to the Department of Neurology,West China Hospital within 1 week of attack from January 2010 to December 2013 were enrolled prospectively. The patients were divided into a hemorrhagic transformation group and a non-hemorrhagic transformation group according to whether they had hemorrhagic transformation or not. As for patients with hemorrhagic transformation, they were divided into a symptomatic hemorrhagic transformation (SHT)group and an asymptomatic hemorrhagic transformation (ASHT)group according to whether they had aggravation of symptom and sign. The baseline data of all patients were collected and compared between the groups. The P<0. 1 variables of the univariate analysis result were enrolled in multivariate logistic regression analysis in order to identify the independent influencing factor of hemorrhagic transformation. Results In 2598 patients,249 (9. 6%)had hemorrhagic transformation,28 of them (1. 1%)were SHT and 221 (8. 5%)were ASHT. There were significant differences in male, hypertension, dyslipidemia, atrial fibrillation, drinking and smoking ratio, blood glucose, cholesterol, low density lipoprotein cholesterol, National Institutes of Health Stroke Scale (NHISS)scores, and the trial of Org 1072 in acute stroke treatment (TOAST)classification between the HT group and the non-HT group (all P<0. 05). There were no significant difference in the related influencing factors between the SHT group and the ASHT group (all P>0. 05). The results of multivariate logistic regression analysis showed that dyslipidemia (OR,0. 588, 95%CI 0. 374-0. 924,P=0. 021)was negatively correlated with hemorrhagic transformation. Atrial fibrillation (OR,3. 188,95%CI 2. 159-4. 707,P<0. 001),blood glucose (OR,1. 081,95%CI 1. 044-1. 119,P<0.001),and NHISS score (OR,1. 305,95%CI 1. 170-1. 455,P<0. 001)were positively correlated with hemorrhagic transformation. In TOAST classification, relative to the large atherosclerotic stroke, the small artery occlusive cerebral infarction was negatively correlated with hemorrhagic transformation (OR,0. 315, 95%CI 0. 167-0. 596,P<0. 001). After removing the influencing factor of atrial fibrillation, compared with the large artery atherosclerotic stroke, cardioembolism stroke was positively correlated with hemorrhagic transformation (OR,2. 823,95%CI 1. 946-4. 095,P<0. 001). Conclusion Dyslipidemia, atrial fibrillation, blood glucose,NHISS score and TOAST classification were independently associated with hemorrhagic transformation in non-thrombolysis patients after acute cerebral infarction.

OBJECTIVE To study the prognosis factors of sudden hearing loss.METHODS The clinical data of 249 out-patients of sudden hearing loss in Department of Otolaryngology Head and Neck Surgery of Beijing Tongren Hospital during 2003 to 2006 were retrospectively studied. The clinical data included age, initial diagnosis time, average hearing level of the first pure tone audiometry, type of audiogram, with or without vertigo and distort-product otoacoustic emissions (DPOAE).RESULTS The initial diagnosis time was 1 to 23 days after the onset of the diseases. The degree of hearing loss was evaluated according to the average hearing loss on frequency 250-4000Hz of the affected ear during the first diagnosis. There were 31(90 dB)cases with profound deafness(26.70 %). There were 72 cases with rising audiogram,81 cases with falling audiogram and 96 cases with flat audiogram. There were 96 cases with vertigo and 174 cases with tinnitus.Medicines used included vasodilators, steroid, neural nutritional agents, antiviral agents, energy mixture.Treatment period was 2 to 4 weeks.DPOAE was examined on 81 patients and was presented in 45 patients whose hearing level achieved normal after treatment.CONCLUSION The old and children patients and patients with vertigo have poor prognosis. The earlier the diagnosis, the better the hearing recovered. The rising audiogram and presentation of DPOAE indicate good prognosis.
