1.Analysis of blood glucose monitoring during pregnancy and pregnancy outcome in patients with gestational diabetes mellitus

Objective To discuss the effect of blood glucose monitoring during pregnancy and pregnancy outcome in patients with gestational diabetes mellitus (GDM). Methods One hundred and fifty-three GDM patients from July 2007 to July 2008 were analysed.The cases were divided into satisfactory group (74 cases)and unsatisfactory group (79 cases) based on the level of bleod glucose and compared the pregnancy outcome between the two groups. Results The incidences of pre-eclampsia, polyhydramnios, macrosomia, neonatal hypoglycemia, hyperbilirubinemia of satisfactory group were lower than those of unsatisfactory group(5.41% vs 17.72%,4.05% vs 20.25%, 12.16% vs 31.65% , 5.41% vs 20.25%,8.11% vs 30.38%). The difference is statistically significant (P <0.05). The incidence of premature rupture of membranes, premature birth, postpartum hemorrhage, fetal distress of satisfactory group were lower than those of unsatisfactory group, but there was no significant difference between the two groups. Conclusion GDM can cause great harm to maternal and child health, and satisfactory blood glucose control during pregnancy may reduce maternal and child complications.
2.Experimental study on the effect of TNF-αon periprosthetic osteolysis in diabetic model induced by polyethylene particle

Objective To study the effect of the high molecular weight polyethylene on the periprosthetic tissue in diabetic model and investigate the effect of TNF-α in periprosthetic tissue.Methods Induction of experimental diabetes in rabbit with streptozotocin was performed successfully,every rabbit's lower limb was implanted a kirschner wire plug in femur by intereondylar notch.In experimental group,15 mg polyethylene particles,dispersed in 1.5 ml normal saline solution,were injected into one knee joint.The other knee joint was injected 1.5 ml normal saline solution ag control.This procedure was repeated for 2,4,6,8,10,and 12 weeks after the implantation.All animals were killed on the 2nd week after the last injection.Knee joint capsule tissues and periprosthetie tissues were examined by optical microscope.ELISA was used for determing the concentration of TNF-α.Results The results showed that kirschner wire were surrounded by multinueleated foreign-body giant cells and osteoclagt in the experimental group.Synovial membrane proliferation was obvious in the control group.There were no polyethylene particles and multinucleated foreign-body giant cells around plug in the marrpw.New bone wag found,kitschner wire was surrounded by suosteoblast,and normal synovial membrane was appeared.The concentration of TNF-α in experimental group was higher than that in control group.Conclusion Maximal ultra high molecular weight polyethylene and diabetic mellitus can restrain diabetic rabbit pefiprosthetic bone formation and increase the secretion of TNF-α.which may play an important role in the periprosthetic osteolysis.
3.Face convexity changes in class Ⅱ malocclusion patients after Twin-block appliance therapy

BACKGROUND: Twin-block appliance has been confirmed to efficiently change hard tissue and the profile of soft tissue.Whether a linear relationship exists between soft tissue and hard tissue changes remains unclear.OBJECTIVE: To evaluate the face convexity changes of soft and hard tissues after Twin-block appliance therapy and compare with controls.METHODS: Thirty-one class Ⅱ division 1 malocclusion subjects with hand-wrist radiographs in FG-G stage were selected.Among the 31 patients,17 received Twin-block appliance therapy,serving as experimental group,and the remaining 14 subjects abandoned treatment,serving as control group.Cephalometric radiographs of each included subject were taken prior to and after treatment or observation.Face convexity of soft and hard tissues,as well as the face convexity changes,was compared between the experimental and control groups.Linear regression equation was employed to analyze the linear association between soft-and hard-tissue changes.Regression equations of experimental and control groups were compared.RESULTS AND CONCLUSION: Prior to and after Twin-block appliance therapy,a significant correlation existed between the position changes of superior and inferior alveolar sockets relative to the nose and mandible,and the position changes of upper and lower lip pits relative to the nose and mandible,i.e.,there was a linear correlation between A-E change and Ss-E change,and between B-E change and Si-E change.The largest Pearson's correlation coefficient(0.839)appeared between Si-E change and B-E change,indicating the best correlativity between these two changes.Statistical analysis revealed that the linear equations of face convexity changes of soft and hard tissues after Twin-block appliance therapy were Si-E=0.745 B-E,Ss-E=0.276 A-E.These linear equations would be helpful to explain mandible growth and face convexity change after Twin-block appliance therapy and predict the prognosis of face convexity change.
4.Study on the relationship between the resting heart rate and target organ damage in elderly patients with metabolic syndrome

Objective To investigate the relationship between the resting heart rate (RHR) and target organ damage (TOD) in elderly patients with metabolic syndrome(MS). Methods 264 elderly patients with MS were divided into four groups according to the level of RHR: RHR1 group, RHR<65 beats/minute (bpm) (46 cases) ;RHR2 group, 65≤RHR<75 bpm (77 cases);RHR3 group, 75 bpm≤RHR<85 bpm (89 cases);RHR4 group, RHR≥85 bpm (52 cases).Electrocardiography, echocardiography, carotid uhrasonography, crcatinine clearance rate (Ccr) and quantitative assay of 24 hours' albuminuria were performed. Results (1) Compared with RHR1, RHR2 and RHR3 groups, RHR4 group showed higher levels of carotid intima-medial thickness (IMT), carotid arterial diameter (CAD), left ventricular mass index (LVMI) and albuminuria(P< 0.05 or P<0.01), and lower levels of left ventricular ejection fraction (LVEF) and Ccr (all P< 0.01). (2) The IMT, CAD, LVMI and albuminuria were positively correlated with RHR (r=0.33, 0.23, 0.61, 0.58, respectively, all P<0.01). However, the LVEF and Ccr were negatively correlated with RHR (r=-0.59, -0.51, all P<0.01). (3) Logistic multivariate analysis showed that RHR and pulse pressure (PP) had effects on myocardial hypertrophy, coronary heart disease, heart failure, cerebral stroke and renal dysfunction(P<0.05 or P<0.01). Except heart failure, PP played a more important role than RHR. Coneinsions RHR may be an independent risk factors for TOD in elderly patients with MS,and RHR regulation is important for the development of MS in the elderly.
5.Effect of calcitonin on bone mineral density and biomechanics around the artificial prosthesis in the rabbit of diabetic osteoporosis model

Objective To investigate the influence of calcitonin on bone mineral density and biomechanics around the artificial pros-thesis in ovariectomized diabetic rabbit model. Methods Fourteen femina New Zealand white rabbits at the age of 5 months old were select-ed, which weight 2.24 -2.65kg, averaging 2.26kg. First, the model of rabbit with diabetic osteoporosis was successfully established by the compound method of ovariectomy plus streptozotocin. Osteotomy in the middle part of femur was performed in both groups, fixation of artifi-cial prosthesis was done with 3.0 kirschner wire. After that, Rabbit models with diabetic osteoporosis were randomly divided into experimen-tal group and control group. Rabbits in the experimental group were treated with calcitonin 6U intramuscular injection once every other day. In control group, intramuscular injection of normal saline solution 1.5ml once every three days. Rabbit models of two groups were sacrificed in the 24th week. The BMD of the region of interest (ROI) around the prosthesis were detected before experiment and 8, 16 and 24 weeks after injection. After rabbits were killed, experimental femurs in both groups were complete removal and soft tissues were rejected. Determi-nation of the pull-out and torsion bone biomechanics experiments of prosthesis was done in both groups respectively. Results The BMD of ROI in the experimental group before operation was (0.1863±0.004)g/cm2 and (0.1753±0.005)g/cm2 in 24 weeks after operation, in control group before operation was (0.1865±0.002)g/cm2 and (0.1638±0.005)g/cm2 in 24 weeks after operation. There were significant difference between the two groups(P < 0.05). Biomechanical show that the pull-out strength in the experimental group was (312.68±8.73 )N/cm2 and (205.43±12.45 ) N/cm2 in control group. There were significant difference between the two groups(P < 0.05). The tor-sion strength in experimental group was (80.47±2.51) N/cm2 and (38.52±0.64) N/cm2 in control group. There were significant differ-ence between the two groups(P < 0.05). Conclusion Salmon calcitonin can reduce the bone turnover rate around prosthesis and decrease bone absorption in the rabbit of diabetic osteoporosis models, accelerate the bone formation around prosthesis, and increase the BMD. It can ameliorate the quality of bone around prosthesis, improve its biomechanics property, and increase the holding power between prosthesis and body mass. It is of clinical significance for the prevention and treatment of aseptic loosening artificial prosthesis.
6.Determination of Plasma Protein Binding Rate of Bisoprolol in Human Plasma by Hollow Fiber Liquid-phase Microextraction Combined with HPLC

Objective To develop a method for determination of the plasma protein binding rate of bisoprolol in human plasma by high performance liquid chromatography ( HPLC) combined with hollow fiber-based liquid-phase microextraction ( HF-LPME) . Methods Method of liquid phase microextraction was optimized. The concentration of bisoprolol in the reconstitute solution was analyzed by HPLC. The mobile phase consisted of water-methanol-acetonitrile-0. 1% phosphoric acid (50:34:6:10). The excitation wavelength was 232 nm and emission wavelength was 300 nm. Through the linear regression equations, the total and free concentrations were obtained, and then the protein binding rate was calculated. Results At low, middle, and high concentration, the protein binding rate of bisoprolol was 31. 2%, 32. 0% and 31. 8%, respectively. Conclusion The proposed method is proven to be simple, fast and reproducible, and is feasible for the determination of plasma protein binding rate of bisoprolol. Bisoprolol moderately binds with plasma protein independent of concentration.
7.Effects of qinggong changchun dan capsule on stress capability of mice with kidney-yang deficiency

BACKGROUND: Qinggong changchun dan (QGCCD) capsule has efficacy in profiting kidney-yang and bettering muscle or bone, it could treat debility, deficiency of energy, forgettery and tiredness and the ache of waist or knee in clinic.OBJECTIVE: To study the effect of QGCCD capsule on stress capability of mice with kidney-yang deficiency.DESIGN: Completely randomized controlled trial.SETTING: Institute of Traditional Chinese Medicine, Chengde Medical College (Key Laboratory of Research and Exploiture of Traditional Chinese Medicine in Hebei Province).MATERIALS: Totally 60 stirp Kunming mice, weighting 19-21 g, of either gender, of grade Ⅱ, were selected in this study.METHODS: The experiment was conducted in the Department of Pharmacology and Toxicology, Institute of Traditional Chinese Medicine of Chengde Medical College from February 2002 to December 2002. Totally 160 Kunming mice of either gender were randomly divided into three groups:Swimming endurance group (n=60), tolerance of hypoxia at normal pressure group (n=50) and single macrophage function group (n=50); and mice in each group were divided into 5 subgroups, including 0.25, 0.5,1.0 g/kg QGCCD groups, model control group and normal control group.Mice were fasted for 12 hours and injected with 25 mg/kg hydrocortisone for 7 successive days to establish yang-deficiency model. Various dosages of QGCCD were given with the same volume of 20 mL/kg for 7 successive days. Mice in model control group were treated with the same volume of saline. Forty minutes later, testes of swimming tolerance, tolerance time of anoxia and engulfment capability of macrophage were performed on mice [To take 20 μL blood in the eyepit vein plexus after the mice were injected 2 minutes and 10 minutes later respectively, and to count the charcoal particle expurgation index (K) of macrophage] to observe effect of various dosages of QGCCD on swimming tolerance, tolerance time of anoxia and engulfment capability of macrophage.RESULTS: Data of totally 160 mice was entered the final analysis without normal control group were longer than those of mice in model control group [(10.94±3.79), (13.68±5.62), (14.58±5.49), (16.12±2.35), (6.45±4.87) minutes;ia of mice in 0.25, 0.5, 1.0 g/kg QGCCD groups and normal control group were longer than those of mice in model control group [(19.45 ±4.63),(21.18±4.25), (22.58±5.21), (23.12±4.78), (12.35±4.89) minutes; t=1.566,macrophage and K index of mice in 0.25, 0.5, 1.0 g/kg QGCCD groups and normal control group were higher than those of mice in model control group (0.023 6±0.010 6, 0.029 1±0.0092, 0.030 8±0.008 6, 0.031 8±0.010 1, 0.012 5±0.008 1; t=2.63, 4.282, 4.898, 4.714, P < 0.05-0.01).CONCLUSION: QGCCD can prolong the swimming time of mice obviously, prolong tolerance time of anoxia according to dosage dependence,increase engulfment capability of macrophags, and strengthen stress capability of mice with kidney-yang deficiency.
8.The research of relationship between work pressure and mental resilience as well as mental health ;in new nurses

Objective To maintain the good psychological state of new nurses, stabilize nursing troop and improve nursing quality, we investigate the work pressure source and mental health of new nurses and analyze the correlation among work pressure, mental resilience and mental health. Methods According to the principle of convenience sampling,a total of 116 new nurses who get the job less than one year in the first hospital of Jilin university were investigated using the method of questionnaire survey. The Conner Davidson Resilience Scale (CD-RISC), Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were involved in the questionnaire. Results Mental resilience scores of new nurses were (63.73±15.11), the scores of anxiety and depression were (34.49±6.45) and (35.59±7.24). Work pressure was negatively correlated with mental resilience (P<0.01) and positively correlated with anxiety and depression (P<0.01). New nurses worried about making mistakes in the work accounted for 89.42 percents (93/104), which was the highest score in the stress source. Conclusions New nurses have good mental resilience and psychological health level, but work pressure has certain impact on mental resilience and mental health. Hospital managers should take active measures which include reducing pressure and improving the ability to help the new nurses through the adaptation as soon as possible.
9.Prognostic factors of relapse and metastasis after radical resection of lung adenocarcinoma

Objective To explore the risk factors of local relapse and distant metastasis after radical resection of lung adenocarcinoma.Methods A total of 102 patients with lung adenocarcinoma operated in First Affiliated Hospital of Xinjiang Medical University from January 2005 to January 2010 were collected.The correlation between clinicopathological characteristics and prognosis was evaluated by single-factor and multi-factor analyses.The survival curves were plotted using Kaplan-Meier.Single-factor analysis of statistical difference was tested using Log-rank test.Multi-factor analysis of prognostic factors were produced by COX regression proportional hazards model.Results In the whole group,1,2,3 and 5 year disease-free survival rates were 74.30%,58.00%,51.50% and 44.90% respectively,and the median disease-free survival was 30 months.Single-factor analysis showed that tumor size (x2 =9.951,P =0.002),clinical type (x2 =8.460,P =0.004),differentiated degree (x2 =4.807,P =0.028),lymph node metastasis (x2 =40.516,P =0.000),pathological stage (x2 =38.769,P =0.000) were prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.Muti-factor analysis showed that tumor size (OR =1.943,95 % CI:1.091-3.463,x2 =5.082,P =0.024),differentiated degree (OR =2.570,95 % CI:1.451-4.552,x2 =10.467,P =0.001),lymph node metastasis (OR =3.196,95% CI:1.037-9.849,x2 =4.096,P =0.043) were independent prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.Conclusion Tumor size,differentiated degree and lymph node metastasis are independent prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.
10.The clinical observation of high-energy red fight combined with human-like collagen dressing in treatment of facial corticosteroid addictive dermatitis

Objective To observe the efficacy of high-energy red light combined with human-like collagen dressing in treatment of facial corticosteroid addictive dermatitis.Methods Eighty-three patients with facial corticosteroid addictive dermatitis were divided into treatment group (42 cases) and control group (41 cases) by random digits table method.All patients in 2 groups were treated with ebastine 10 mg,once a day,and vitamin E cream,twice a day.At the same time,the patients in control group were given human-like collagen dressing,once a day in the first week,then 3 times/week.On the basis of control treatment,the patients in treatment group were irradiated with high-energy red light 10 min in face,2-3 times/week.The treatment of both groups lasted for 12 weeks.The symptoms,skin lesions and untoward reaction were observed after treatment of 4,8 and 12 weeks.Results After treatment of 4 weeks,there was no statistical difference in the effective rate between 2 groups (P > 0.05).After treatment of 8 and 12 weeks,the effective rates in treatment group were significantly higher than those in control group [83.3%(35/42) vs.58.5% (24/41),90.5% (38/42) vs.65.9% (27/41)],there were statistical differences (P < 0.05).No untoward reaction was found in 2 groups.Conclusion High-energy red light combined with human-like collagen dressing is effective and safe in treatment of facial corticosteroid addictive dermatitis.
