1.X-ray Diagnosis and Pathological Study of Ductal Carcinoma in Situ of Breast

Objective To discuss the value of the mammography in the diagnosis of ductal carcinoma in situ (DCIS) and guidelines ofthe operation.Methods 107 pathologically proved DCIS were retrospectively reviewed.Results There were 62 microcacifications(57.9%),the range of microcacifications was varied , of them,13 were masses or local compact with microcacifications ; 17 were localized infiltration and bad structure (15.9%);20 were masses (18.7%) ;1 papillae enhancement (0.9%),7 were normal(6.5%).Conclusion Multiplicity microcacifications are the most frequently appearance of X-ray in DCIS;mammography could find the early microcarcinoma and the early carcinoma , especially the DCIS ; mammography could exactly reflect the multifocus , it is of value in confirming the range of the operation.
2.Expression of zinc finger protein A20 mRNA of mouse liver tissues after trauma with lipopolysaccharide infection

Objective To study the expression law of zinc finger protein A20 mRNA of mouse liver tissues in the process of trauma with lipopolysaccharide (LPS) infection. Methods A total of 95 healthy mice (either sex, from Kunming, Yunnan province) with a mean body weight of 21 g (18-24 g) were randomized into 4 groups: control, trauma only (Group A), LPS only (Group B) and trauma plus LPS (Group C). The models with closed fracture of bilateral spines as well as endotoximia were made. The expression characteristics of zinc finger protein A20 mRNA of liver tissues were measured by reverse transcription-polymerase chain reaction (RT-PCR) and all result data expressed as total gray ratio of A20 mRNA to GAPDH mRNA (?s). Results In the control group, the A20 mRNA expressed at a low level. There was low expression of zinc finger protein A20 in the Group A at various time points, with no significant difference compared with the control group. After LPS infection, the expression of A20 mRNA in the Group B was elevated more obviously than that in the control group at 0.5 hour, reached peak during 0.5-2 hours and decreased after 2 hours. But expression of A20 mRNA at each time point was higher in the Group B than that in the Group A. In the Group C, after LPS infection, the expression of A20 mRNA was elevated more significantly than that in other two groups 0.5 hour, reached the highest level during 0.5-2 hours, much higher than that in the Group B (P
3.Establishment of Drug Release Determination Method for Methylphenidate Hydrochloride Bipolar Con-trolled Release Osmotic Pump Tablets

Objective:To establish the drug release determination conditions and method for methylphenidate hydrochloride bipolar controlled release osmotic pump tablets. Methods: The drug release of the tablets was determined by HPLC using a Diamonsil C18 (250 mm × 4. 6 mm, 5 μm) column with acetonitrile-KH2 PO4 (0. 02 mol·L-1 ,and pH was adjusted to 3. 0 by 1% H3 PO4 solution) (30∶ 70) as the mobile phase at a flow rate of 1 ml·min-1 , the column temperature was 35 ℃ and the injection volume was 20 μl. The effects of release medium, release apparatus and rotation speed on the release of methylphenidate hydrochloride bipolar controlled release osmotic pump tablets were studied as well. Results:The established drug release determination method had a good linear rela-tionship within the range of 1. 0-24. 0 μg·ml-1(r=0. 999 5), and the average recovery was 100. 5%(RSD=1. 58%, n = 6). Un-der the conditions of 900 ml pH 3. 0 phosphate buffer solution as the release medium and the rotation speed of 50 r·min-1 , the drug was quickly released in 0-2h, and then the release behavior was complied with a zero-level model in vitro in 2-10h with the release e-quation of Q=5. 505t+44. 221(r=0. 994 5). Conclusion:The method is simple, accurate and reliable, and suitable for the quality control of methylphenidate hydrochloride bipolar controlled release osmotic pump tablets.
4.The effects of prostacyclin on the changes of basic fibroblast growth factor after lung injury in dogs

The separation,purification and activity determination of basic fibroblast growth factor (bFGF) with nucleic acid molecular hybridization was described and the changes of bFGF in lung injury treated with prostacy-clin were observed.It was found that bioactive bFGF and bFGF-mRNA could only be found in the injured lung tissues but not in the normal lungs.Administration of prostacyclin could slightly elevate the activity of bFGF and signficantly increase the level of bFGF-mRNA.The findings of this study suggest that there is an increase of bFGF level after lung injury,prostacyclin can influence the expression of bFGF,and bFGF plays an important role in the repair of the injured lung tissues.
5.Correlation study of effect of additional filter on radiation dose and image quality in digital mammography

Objective To explore the effect of different additional filters on radiation dose and image quality in digital mammography.Methods Hologic company's Selenia digital mammography machine and the post-processing workstations and 5 M high resolution medical monitor were used in this study.Mammography phantoms with the thickness from 1.6 cm to 8.6 cm were used to simulate human breast tissue.The same exposure conditions,pressure,compression thickness,the anode were employed with the additional filters of Mo and Rh under the automatic and manual exposure mode.The image kV,mAs,pressure,filter,average glandular dose (AGD),entrance surface dose (ESD),signal-to-noise ratio (SNR),contrast-to-noise ratio (CNR) and image score according to ACR criteria were recorded for the two additional filters.Paired sample t test was performed to compare the indices of Mo and Rh groups by using SPSS 17.0.Results AGD and ESD of Rh and Mo group were both higher with the increase of the thickness of all the phantoms.AGD,ESD and their increased value of Rh filter(1.484 ± 1.041,7.969 ± 7.633,0.423 ± 0.190 and 3.057 ± 2.139) were lower than those of Mo filter(1.915 ± 1.301,12.516 ± 11.632,0.539 ±0.246 and 4.731 ± 3.294),in all the phantoms with different thickness (t values were 4.614,3.209,3.396 and 3.605,P <0.05).SNR,CNR,and image score of Rh and Mo group both decreased with the increase of the thickness of all the phantoms.There were no statistical difference (P > 0.05).Conclusions Compared with Mo filter,Rh filter could reduce the radiation dose,and this advantage is more obvious in the thicker phantom when the same image quality is required.
6.Clinical Study of Serum Lipoprotein(a) and Homocysteine with Type 2 Diabetes Mellitus and Insulin Resistance

Objective To analyze the levels of serum lipoprotein (a)[Lp(a)]and homocysteine (CHY)in type 2 diabetes mellitus (T2DM)patients and todetermine the relationship between them and diabetic insulin resistance.Methods A total of 108 patients with T2DM and 50 healthy volunteers (control group)were studied.T2DM group according to the presence of kidney disease complications could be divided into simple diabetes group (SDM)in 48 patients with diabetic nephropathy (DN)group of 60 cases.Collected patients general data and determined two groups of glycosylated hemoglobin (HbA1c), low density lipoprotein (LDL),urea nitrogen (BUN),serum creatinine (SCr),urinary inhibition C (CysC),Lp(a)and HCY levels;Compared differences between groups and how they related to T2DM kidney disease,and the relationship between in-sulin resistance.Results ①Compared between three groups only the weight (NC group 60.1±10.34 kg,SDM group 67.2 ±11.38 kg,DN group 68.8±1.35 kg),the difference was statistically significant (F=0.457,P=0.035).②compared be-tween the three groups,the levels of HbA1c (F=3.421,P=0.013),LDL-C (F=2.963,P=0.041),BUN (F=4.637,P=0.031),SCr (F=2.369,P=0.024),CysC (F=3.697,P=0.011),Lp(a)(F=1.236,P=0.021),HCY (F=4.317,P=0.027)were increased significantly.③FPG,FINS and HOMA-IR in SDM group were 5.99±0.94 mmol/L,61.11±28.46 pmol/L and 2.93±1.97 respectively,FPG,FINS and HOMA-IR in DN group were 9.01±2.23 mmol/L,98.56±62.14 pmol/L and 4.34±2.74 respectively.Compared between the two groups,FPG (t=0.47,P=0.017),FINS (t=0.55,P=0.039),HOMA-IR (t=0.69,P=0.024)differences were statistical significance.④The Lp(a),HCY were significantly pos-itive correlation with HOMA-IR (r=0.761,P=0.044;r=0.840,P=0.021).Conclusion Lp(a)and HCY levels in serum associated with diabetic nephropathy,and insulin resistance.And the detection of serum Lp(a)and HCY has important sig-nificance for early detection of diabetic nephropathy.
7.Current survey of readiness for hospital discharge of patients who had undergone spinal operation

Objective To investigate the readiness for hospital discharge of patients who had undergone spinal operation at West China Hospital, Sichuan University, to guide the medical care and health education. Methods A total of 164 patients after spinal operation who have readying to discharge were selected and investigated by General Information Questionnaire, Activity of Daily Life Scale and Readiness for Hospital Discharge Scale. Apply the Excel for data inputting, SPSS19.0 for statistical analysis and choose rate, x±s and so on for statistical description according to the types of data. Results Of all the 164 patients, 47 cases (28.7%) weren′t well prepared, 117 cases (71.3%) were well prepared;the score of
Readiness for Hospital Discharge Scale was (142.38±34.23 points and the average score of all the articles was (7.12±1.72) points. Conclusions The readiness for hospital discharge score of patients who had undergone a spinal operation at West China Hospital, Sichuan University is low and their readiness for hospital discharge is inadequate. Medical staff can improve the patients′readiness for hospital discharge by strengthening health guidance in order to promote the recovery of patients.

9.Primary malignant lymphoma of the breast: imaging characteristics and correlation with histopathologic findings

Objective To evaluate the mammographic characteristics of primary lymphoma of the breast and to correlate the imaging features with the histopathologic findings.Methods We retrospectively reviewed records in 27 patients with histologically proven primary malignant lymphoma of the breast.Mmammography was performed before surgery in 14 cases.The clinical, pathologic, and imaging features were retrospectively reviewed and correlated in these 14 patients.All patients were female, with the mean age of 41.5 years (range 28-56).The lesion was in the right breast in 7 patients, the left in 6, and bilateral in 1.Results Histology revealed 13 Non-Hodgkin lymphoma (NHL) and 1 Hodgkin lymphoma (HL).In primary lymphoma, mammography revealed a unilateral solitary mass in 9 cases, unilateral multiple masses in 2 cases, bilateral multiple masses in 1 case, and unilateral diffusely increased opacity with skin thickening in 2 cases.The diameter of the masses ranged from 0.7 cm to 5.0 cm (mean 2.6 cm). Of the total 18 masses, the margins of the masses were well defined in 13, and partially defined in 5.Neither calcifications nor spiculations nor adjacent skin retraction were seen.Of the 13 NHL, histologic examination showed diffuse NHL in 12 cases and follicular NHL in 1.No mammographic features were identified that could helped distinguish follicular from diffuse NHL.Conclusion Primary lymphoma of the breast usually presents with clinical and mammographic findings mimicking a carcinoma or benign tumor.Although the clinical and imaging characteristics may suggest the possibility of breast NHL, none of the findings are pathognomonic.The final diagnosis depends on histopathology.
10.Enhancement characterization of breast masses of contrast-enhanced ultrasound: comparison with MRI

Objective To assess the enhancement characteristic of breast lesions of contrast-enhanced ultrasound (CEUS) in comparison with contrast-enhanced magnetic resonance imaging (MRI).Methods Between August 2011 and March 2013,72 women with 72 lesions were enrolled.All patients underwent ultrasound,CEUS and MRI.The histopathologic results obtained from ultrasound-guided core biopsy or operation excisions were used as the reference standard.CEUS section evaluations were made similar with MRI regarding the size and shape of lesions.Different contrast enhancement patterns including homogeneous/heterogeneous,the tumor areas,the perfusion defect areas,and modality of time-intensity curve were evaluated.Pearson's correlation coefficient,Student's t-tests,and the concordance test were used for evaluation.Results Of the 72 lesions,pathologic examination revealed 56 (77.8%) malignant lesions and 16 (22.2%) benign lesions.The tumor areas measured by CEUS and MRI agreed well,with a correlation of r =0.894,P =0.000.The difference between the two measurements was not significant according to a paired t test (P =0.886).The concordance tests gave a value of the coefficient Kappa =-0.153 (P =0.061),indicating a low concordance between the results obtained with CEUS and those obtained with MRI regarding the enhanced uniformity.There were statistically significant differences in the perfusion defect areas as measured by CEUS and MRI (P =0.01).The CEUS estimates [(0.837 ± 0.827)cm2] were consistently higher than the MRI estimates [(0.576 ± 0.524)cm2].The time-intensity curve patterns between the two groups showed no correlation.Conclusions The enhancement patterns evaluated by CEUS and MRI partly agreed well.There was no direct association between the two methods regarding the enhancement patterns because of the different contrast agent.
