2.Age-related changes of stimulated whole saliva flow rate and nitric oxide level in healthy adults

Objective:To study age-related changes of stimulated whole saliva flow rate and nitric oxide level in healthy adults.Methods:97 healthy adults were divided into four age groups:20-39 years (youth group), 40-59 years (middle-aged group), 60-79 years (elderly group), and 80 years and over (macrobian group).The volunteers were asked to restrain from breakfast in the morning.After mouth rinse with 2.5 ml/L chlorhexidine,20 ml/L citrate was dropped on the anterior third of the tongue dorsum of the subjects for two minutes at the pace of 1 drop every 20 seconds.Then the subjects rinsed their mouth 3 times with double-distilled water within 1 minute,then saliva was collected for 5 minutes. The volume of saliva was documented. Samples were frozen at -70 ℃ until assayed. The rates were calculated. Nitric oxide concentrations were measured with nitrate reductase and the Griess reagent, and then nitric oxide output (mol/min) was calculated.Results:(1) The flow rate of stimulated whole saliva in youth group was significantly higher than that in middle-aged, aged and macrobian groups(P 0.05 ). (2) There were no significant differences in nitric oxide concentrations between any two of the groups. Output of nitric oxide in youth group was significantly higher than that in middle-aged, aged and macrobian groups(P0.05). Conclusion:Nitric oxide exists in saliva of healthy adults. Stimulated whole saliva flow rate and nitric oxide output decreases with aging.
3.SYNTHESIS AND ANTICANCER ACTIVITY OF 3-[(3′-METHYL-4′-(SUBSTITUTED PHENYL)-1′,3′-BUTADIENYL] INDOLE DERIVATIVES

AIM To study the synthesis and anticancer activity of 3-[(3′-methyl-4′-(substituted phenyl)-1′,3′-butadienyl] indole derivatives. METHODS Electrophilic-substitution, aldol-condensation, selective-reduction, phase-transfer Wittig reaction and hydrolysis reaction were used in the synthesis of the title compounds. RESULTS Eleven compounds of 3-[(3′-methyl-4′-(substituted phenyl)-1′,3′-butadienyl] indole were synthesized. They are new compounds. Compound 8 showed different inhibitory effects on HL-60, HCT-8 and Bel7402 cell lines in vitro, and its inhibitory rate of antiinflammation was 100% at 10-5 mol*L-1 concentration. CONCLUSION Compound 8 showed high anticancer and antiinflammatory activities, and is worth further studying.
4.Roles and their mechanisms of neuroglobin in cerebral ischemia

Neuroglobin (Ngb) is an oxygen-carrying globin that specifically expresses in brain tissue.It is involved in energy metabolism,mitochondrial function,as well as cell survival and proliferation of signaling pathway regulation.Under physiological conditions,Ngb presents as a form of ferrous deoxy hexacoordination,which has stronger oxygen affinity.During ischemia and hypoxia,the expression of Ngb is upregulated in brain tissue and interacts through a variety of proteins of its downstream,and plays a protective role for the damaged brain tissue.
5.Clinical analysis of 102 cases with chronic heart failure complicated by hyponatremia

Objective To summarize the clinical experience of 3% sodium chloride in treatment of chronic heart failure with hyponatremia. Methods The 102 patients suffering from chronic heart failure with hyponatremia were treated with water limit, vasodilator, cardiotonic, diuretics, angiotensin-converting enzyme inhibitor, angiotensin Ⅱ receptor antagonist and 3% sodium chloride. Cardiac functional capacity classification (NYHA), treatment effect and hyponatremia level were compared between pre- and post- treatment. Results After treatment, heart function was statistically different as compared with that of pre-treatment (χ2=150.26, P<0.05). The 58 cases showed distinct effect, 37 cases moderate effect, 7 cases no effect, and the percentage of total effect was 93.1%. The hyponatremia of patients with different level of heart function were (139.4±22.6) mmol/L, (138.2±23.3) mmol/L, the difference had a statistics significance compared with pre-treatment hyponatremia which were (122.9±20.5) mmol/L, (120.5±26.8) mmol/L (t=2.02, 2.04, P<0.05). Conclusions Proper diuresis and sodium salt supplement could help to reduce the malignant risk of the patients suffering from chronic heart failure with hyponatremia and to improve the prognosis.

7.Application and evaluation of Mini-CEX in training clinical diagnostic thinking ability for interns

Objective To explore the application of mini-clinical evaluation exercise (Mini-CEX) in training the diagnostic thinking ability for clinical interns.Methods 83 interns who came to the First People's Hospital of Lianyungang for clinical practice in June 2015 were divided into the experimental group (45 interns) and the control group (38 interns) according to their schools.The two groups received the same clinical teaching according to internship program of five-year clinical medicine,while the experimental group had to take a weekly Mini-CEX training in chnical sections.The diagnostic thinking ability of the two groups was measured by the Diagnostic Thinking Inventory in Chinese version at the beginning and the end of the internship respectively.SPSS 21.0 was applied to processing the data.Measurement data were expressed as mean plus or minus standard deviation,and t test was used in the comparison between groups.Results At the end of the internship,the total score of diagnostic thinking,the score of knowledge structure in memory and the score of flexibility in thinking of the experimental group were higher than scores at the beginning statistically,and the experimental group had a statistically higher score than the control groups in knowledge structure in memory [(69.36 ± 4.28) vs.(66.95 ± 4.78),P=0.018].Conclusion Mini-CEX training can enhance the clinical diagnostic thinking ability of interns by improving their knowledge structure in memory.
8.Clinical profile and visual outcomes after treatment in patients with dysthyroid optic neuropathy

Objective To review the clinical data and visual outcomes after treatment of patients with dysthyroid op-tic neuropathy ( DON) .Methods We retrospectively examined the clinic data of the 38 patients (67 eyes) suffer-ing from DON and analysed the effect of glucocorticoids , radiation and orbital decompression .Results The study included 14 men and 24 women.32 patients ( 84.2%) received multiple treatment modalities .LogMAR vision acuity changed from (0.60 ±0.59) before treatment to (0.18 ±0.09) after treatment(P<0.01) .There were 59 eyes (88.1%) in the treatment effective group , 8 eyes (11.9%) in the ineffective group .38 eyes underwent ini-tial treatment with intravenous steroid pulse therapy and 35 eyes ( 92.1%) were effective .29 eyes were treated with other modalities and 24 eyes(82.8%) were effective(P<0.01).There were 3 eyes (5.1%) suffering from fixed eyeball movement in the effective group , while 4 eyes ( 50%) in the ineffective group ( P <0.05 ) .The thickness of the supraocular muscle group in the effective group was (7.63 ±1.19 ) mm, (8.81 ±0.83 ) mm in the ineffective group (P<0.05).Mean defect of the visual field was (2.41 ±2.82) in the effective group and (11.98 ± 7.07 ) in the ineffective group ( P<0.05 ) .Conclusions Treatment with multiple modalities effectively improved visual outcomes in cases of DON .Intravenous pulse steroid therapy was recommended as the initial modality .
9.The preoperative assessment of tumor size in breast ductal carcinomain situ:a comparative study between mammography and ultrasonography

Objective To compare the accuracy of mammography and ultrasonography in evaluating the tumor size of breast ductal carcinoma in situ before operation.Methods Eighty-seven patients with breast ductal carcinoma in situ confirmed by surgery pathology were retrospectively analyzed.All of the mammography and ultrasonography data were well-documented.The maximum diameter of the tumor was measured by mammography,ultrasonography and pathology,respectively.It was considered concordant if the difference between the imaging size and pathologic size was less than 0.5 cm.Pearson correlation analysis was used to determine the relation of imaging size with pathologic size.Results The range of the maximum diameter of the tumors measured by pathology,mammography and ultrasonography was 0.4-7.0 cm,0.8-6.9 cm and 0.5-4.8 cm,respectively.The mean value was (2.4±1.3)cm,(2.6±1.2)cm and (2.3±1.0)cm,respectively.The coefficient correlation between mammographic and pathologic size was 0.724,and between ultra-sonographic and pathologic size was 0.532.Conclusion Mammography is more accurate than ultrasonography in size assessment of breast ductal carcinoma in situ.
10.Detection of chromosome abnormality by next-generation sequencing technology of miscarried embryo in the first-trimester

Objective To investigate the clinical values of next-generation sequencing (NGS) technology in diagnosis of miscarried chorionic villi genetic disorders. Methods Patients who underwent miscarriage (n=87) were enrolled in this study. Among all patients, 32 cases were of recurrent miscarrage and 55 cases were of sporadic miscarriage. In all collected patients, 35 women were 35 years or older while other 52 women were less than 35 years old. Positive detection rate and the abnormal detection rate were compared between these two methods. Chromosomes abnormal rates were also compared among different types of miscarrage and different ages. All aborted villi tissue were analyzed by NGS of whole genome and G-band?ing karyotype. Results The successful detection rate of chorionic villi by NGS (100.00%) was higher than that of G-band?ing karyotype (74.71%), and the detection rate of abnormal chorionic villi by NGS (58.62%) was also higher than that of G-banding karyotype (50.77%). Three cases of chromosome structure anomaly were found in those 51 chromosome anomalies (5.88%). Other 48 cases of chromosome anomalies were aneuploidy anomalies (94.12%) include 39 cases of trisomy, 2 cases of double trisomy and 1 case of triple trisomy and 6 cases of monomer. On the other hand, 32 cases of chromosome aneuploi?dy anomalies were found in 33 chromosome anomalies by G-banding karyotype, which include 24 cases of trisomy, 2 cases of double trisomy, 1 case of triple trisomy, 5 cases of monomer and 1 case of chromosome structure anomaly. Most NGS re?sults (n=64) were in agreement with G-banding karyotype but with 1 case of discrepancy. Chromosomal abnormality rate de?tected by NGS in sporadic miscarrage group and recurrent spontaneous miscarrage group were 60.00%and 56.25%respective?ly. There was no significant difference (P>0.05). Chromosomal abnormality rate picked by NGS in women aged≥35 years old (71.43%) was higher than that in women<35 years old (50.00%) with statistically significant difference (P<0.05). Conclu?sion NGS technology showed highly accuracy in detecting chromosomal abnomality from villi tissue. Therefore, it could help to detect genetic disorders of miscarrage. It is useful to determine the reasons of miscarrage and guide the next pregnancy.
