1.ECMO combined with IABP aiding the treatment of Low Cardiac Output Syndrome after cardiac surgery

Objective To summarize the treatment experience of applying ECMO in combination with IABP in the treatment of Low Cardiac Output Syndrome after cardiac surgery.Methods Clinical data of 6 cases of applica-tion of ECMO combined with IABP to aid the treatment of Low Cardiac Output Syndrome after cardiac surgery at Henan Chest Hospital were analyzed;the basic conditions of patients before surgery,clinical diagnosis,causes of ai-ding,time of aiding,and prognosis were investigated;the effects of aiding were evaluated indirectly by observing MAP, LVEF,lactic acid and other indicators.Results 6 patients who received the treatment all separated from the machine successfully.One patient suffered serious arrhythmia in the fifth day after separating from the machine and died after unsuccessful rescue efforts.One patient of cerebral hemorrhage 7 days after weaning,automatic discharge.The other four patients was cured and discharged from the hospital.Conclusion CMO Combined with IABP to aid the treat-ment of Low Cardiac Output Syndrome after cardiac surgery has proved to be effective and can win the time to allow the recovery of cardiac function.

3.Thyroid autoantibodies: adverse effects on pregnancy outcomes and child development and its intervention

Thyroid autoantibodies (TAA) in euthyroid women were associated with several adverse pregnancy outcomes,such as miscarriage,preterm birth.Maternal TAA frequently results in transient mild elevation of serum TSH above the normal reference value in women during pregnancy and in infants born in the first month of life,and may affect neuropsychological development of their offspring.The cause is still unclear.Medical interventions on TAA involve levothyroxine or selenium supplementation. The article reviews the effects of maternal thyroid autoantibodies on pregnancy outcomes,as well as on neonatal thyroid status,and neuropsychological development of the fetus and neonate,and summarizes therapeutic strategies used to intervene maternal thyroid autoimmunity.
4.Determination of Theobromine in Compound Theophylline and Ephedrine Tablet by HPLC

Objective To establish a HPLC method for determination of theobromine in Compound Theophylline and Ephedrine Tablet. Method HPLC method was adopted on Kromasil C18 column (250 mm? 4.6 mm, 5 ?m), and temperature was set at 45 ℃. The mobile phase consisted of 0.05 mol/L KH2PO4- Methanol-Triethanolamine (77∶23∶0.2), with flow rate of 0.8 mL/min. The UV detection wavelength was set at 215 nm. Results The calibration curves of theobromine was linear in the range of 0.3 094~0.722 ?g (r=0.999 96), ant the average recovery was 100.49% (n=9) with RSD=1.1%. Conclusion This method is simple, rapid, sensitive and accurate, and can be used for determination of theobromine in Compound Theophylline and Ephedrine Tablet.
5.Three-dimensional finite element analysis on the stress of mandibular first molar restored with different locations and numbers of gold alloy posts and cores

BACKGROUND:The design of post and core of the molar is affected by various factors,including the effect of the number of posts on the retention force and the effect of the number of posts on the dentin stress and distribution.OBJECTIVE:To make a mechanical analysis on the three-dimensional finite element models of the mandibular first molar defect restored with different numbers and locations of gold alloy posts,cores and all-ceramic crown.METHODS:Cone-beam CT based three-dimensional finite element models of post and core restored first mandibular molars were established,with seven different post designs.According to the numbers and location of gold alloy post inserted into root canal,a single post was placed at the mesial buccal root in group A,a single post at the mesial tongue root in group B,a single post at the distal root in group C,two posts at the mesial buccal root and the distal root in group D,two posts at the mesial buccal root and mesial tongue root in group E,two posts at the mesial tongue root and the distal root in group F,one post respectively at the mesial buccal root,mesial tongue root and distal root in group G.The von Mises criterion was applied for comparing the maximum yon Mises stress value of dentin and stress concentration areas in the seven models which restored with golden alloy posts and cores with all-ceramic crown under the maximum (600 N),vertical (225 N),inclined (225 N) and horizontal (225 N) loads.RESULTS AND CONCLUSION:Under vertical,inclined and horizontal direct loads,the maximum stresses on the remaining dentin in group C were significantly lowest,compared with the groups A and B.The maximum stresses on the remaining dentin changed irregularly with the variation of the numbers and location of posts,when the mandibular first molar defect was restored with all-ceramic crown with gold alloy posts and cores.Under the horizontal load,the maximum Von Mises stress value of the dentin increased remarkably.Under the horizontal load,the maximum stresses on the remaining dentin in group E were the highest.To conclude,the numbers and location of posts and cores have influence on magnitude and distribution of stress,when the mandibular first molar defect is restored with all-ceramic crown with gold alloy post and core.
6.Use of partial cardiopulmonary bypass for surgery of descending thoracic aneurysm

Objective To summarize the experience of use of partial cardiopulmonary bypass for surgery of descending thoracic aneurysm. Methods Thirty five patients were undertaken surgery of descending thoracic aneurysm using partial cardiopulmonary bypass and beating-heart technique in our hospital. The partial cardiopulmonary bypass includes left heart bypass (left atrial to femoral artery bypass), pulmonary artery to femoral artery bypass, femoral vein to femoral artery bypass. Results Only one patient died, the other thirty four patients experienced an uneventful recovery. There was no any complication related to partial cardiopulmonary bypass. Conclusion The use of partial cardiopulmonary bypass for surgery of descending thoracic aneurysm can reduce and avoid the complications of nervous system and urinary system effectively.
7.Three-dimensional finite element analysis of gold alloy post-core in the repair of structure defects of the mandibular first molar

BACKGROUND:The location of residual dentin may affect stress distribution of tooth root. <br> OBJECTIVE:To make a three-dimensional finite element analysis of gold al oy post-core and al-ceramic crown in the repair of structure defects of the mandibular first molar, and to provide a guideline for planning restoration for such kind of tooth structure defects. <br> METHODS:Three-dimensional finite element models of the restored first mandibular molars were constructed by CT image reconstruction technique. Then MIMICS software was used to separate the areas and finish the three-dimensional calculation. GEOMAGIC software was also applied to modify and generate a NURBS surface in each patch. Al components of the models were assembled under the ANSYS preprocessor. The Mohr strength theory was applied for comparing the maximum Mohr stress value of dentin and stress concentration areas in six models under different simulating loads. <br> RESULTS AND CONCLUSION:The restoration effects in six models were compared, the maximum Mohr stress value of dentin and stress concentration areas were not obviously different. The location of residual dentin has little influence on the stress value and distribution, when restored by gold al oy post-core and and al-ceramic crown.
8.Carotid artery calcification and ischemic cerebrovascular disease

Carotid artery calcification is a calcium deposition on arterial wals. It has a great clinical significance when it occurs within atherosclerotic plaques, which is a marker of the progressive atherosclerosis. In recent years, the studies on the mechanism of carotid artery calcification and imaging detection methods are continuously advancing and being updated. In addition, the correlation between carotid plaque calcification and ischemic cerebrovascular disease and the effect on its therapeutic decisions are also the research foci. However, there have been controversies about the advantages and disadvantages of carotid artery calcification. Some researchers believe that carotid artery calcifications are a protective factor, but others believe that they may promote the plaque vulnerability. This article reviews the recent progress in research on carotid artery calcification from the above aspects.
9.MSCT noninvasive imaging evaluation of the correlation between periodontal bone loss and carotid calcifications

Objective To detect the correlation between the periodontal bone loss and carotid calcifications by using MSCT.Methods Imaging materials of 270 patients who were suspected atherosclerosis and underwent MSCT were retrospectively studied.According to the score of carotid calcification,the patients were divided into three groups:slight calcification group (carotid calcification score <100),severe calcification group (carotid calcification score ≥100),and normal control group (carotid calcification score=0).The original images were post-processed with volume rendering(VR),multi planar reformation(MPR),curved planar reformation(CPR)and maximum intensity projection (MIP)by using EBW4.5 workstation,and then the residual teeth number and the periodontal bone loss teeth were recorded in different groups.Results On MSCT,the carotid calcification score and the number of periodontal bone loss were displayed clearly.The number of the periodontal bone loss teeth in the calcification groups were significantly higher than those in the normal control group(P<0.01).Furthermore,the number of the periodontal bone loss teeth was positively correlated with the severity of carotid calcification.Conclusion The periodontal bone loss is highly correlated with the severity of the carotid calcification.The volume reconstruction technique of MSCT is a non-invasive diagnostic testing to detect the periodontal desease and to quantify carotid calcifications.
10.Investigation and Analysis of the Cognition of Basic Medical Staff to the National Essential Medicine System in Xinjiang

OBJECTIVE:To provide reference for further developing the use of national essential medicine system and formulat-ing the plan for essential medicine that is consistent with the local situation. METHODS:A questionnaire survey was conducted on site to collect the 1 800 medical staffs chosen from 35 community health service institutions in Xinjiang with multi-stage stratified random cluster sampling to investigate the cognition to the national essential medicine,and it was statistically analyzed by SPSS 13.0 software. RESULTS:Totally 1 800 questionnaires were sent out and 1 642 were effective received with effective recovery of 91.2%. 26.5% acquired such specialized knowledge through professional training,and 50.0% by books and Internet;62.4% did never participated in training. Only 20.2% shared it for patients. The average score of recognition for the system was(6.09±2.86) scores,which was related to age,education,profession,work experience and the title (P<0.01). 75.2% supported this system. CONCLUSIONS:The basic medical staff just have some know of the concept of national essential medicine,but the understanding is still low. It is suggested to improve the cognition level by seminars and continuing training to strengthen reliability of all ethnic groups on essential medicine,change their bad medication habits,popularize common sense of rational drug use and form a good atmosphere for the implementation of the national essential medicine system in the whole society.
