Objective To observe the effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes.Methods Sixty diabetes patients were divided into two groups randomly:treatment group were received Sitagliptin Phosphate Tablets and shenmai;the control group were given acarbose.14 weeks later,the indications FBG,2HBG,HbA1 C,TC,TG,LDL-C,HDL-C,hypoglycemia happen rate,BMI,blood rheology,transcranial Doppler ultrasound(TCD)were tested to see if there is any change.Results After treatment,two groups of FBG,2hBG,HbA1 C,TC,TG,LDL-C,HDL-C levels compared with those before treatment were statistically significant(P＜0.05 or P＜0.01),but the treatment group on lipid control more than those in the control group;treatment group therapy in reducing the risk of hypoglycaemia and weight gain than the control group(P＜0.05);the whole blood viscosity,plasma viscosity,low viscosity and hematocrit of treatment group were significantly lower(P＜0.05),while the control group showed no significant change;the abnormal rate of treatment group was 52.6％,significantly better than the control group 86.5％(P＜0.05).Conclusion The effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes was significant,compared with the traditional antidiabetic drugs,the combination is effective hypoglycemic control fat and less adverse reactions.

Diffuse large B-cell lymphoma (DLBCL) represents the most common type of malignant lymphoma. DLBCL is heterogeneous with respect to morphology, immunophenotype, biology, clinical presentation and outcome. Constitutive activity of the NF-κB pathway may contribute to the poor prognosis of patients with activated B cell-like (ABC) subgroup of DLBCL, caspase recruitment domain 11 (CARD11) is the important signal protein in the signaling pathway of NF-κB. Furthermore, various pre-clinical data have proved the importance of CARD11 in DLBCL. This review summarizes the biological characteristics of CARD11, and its relationship with NF-κB signaling transduction pathway and the outcome of DLBCL, so that we can better understanding the pathogenesis and new therapeutic target of DLBCL.

Objective To explore how the self-stigma of mental illness influence self-esteem and social support.Methods 109 mental illness from Jinan,Tianjin and Harbin were asked to finish a questionnaire integrated from the ISMI,SES,PSSS,and the data were calculated by SPSS18.0.Results According to the regression of self-stigma and its five attributes on the self-esteem,self-stigma was negative correlated with self-esteem,with self-alienated was the most significant attribute,the next being the public relations avoidance and the incompetent sensation,while the flight from reality and the discrimination experience were not remarkable attributes (Y =4.585-0.722x(F=59.508,P＜0.001) and Y =4.470-0.388x1 +0.813x2-0.198x3-0.064x4-0.192x5 (F=16.609,P＜ 0.001)).According to the regression of self-stigma and its five attributes on social support,self-stigma was negatively correlated with social support,self-alienated was the most remarkable attribute,and the next were the public relations avoidance and the discrimination experience,while the flight from reality and the incompetent sensation were not remarkable to the social support.(Y =3.710-0.354x(F=9.116,P＜0.01) ; Y =3.474-0.391x1 +0.102x2+ 0.301x3+0.047x4-0.301x5 (F=5.695,P＜0.001)).Conclusion The self-stigma of mental illness has negative influence on the self-esteem and social support.

Objective To compare the curative effect and complication of traditional open thyroid operation and minimally invasive laryngeal endoscopic thyroid operation.Methods 80 cases with thyroid disease were randomly divided into group I and groupⅡof 40 cases in each group,the group Ⅰ received the minimally invasive laryngeal endoscopic thyroid operation, while the Ⅱ group was treated with traditional open thyroid operation.The operation time,intraoperative amount of bleeding,postoperative drainage,hospitalization time and complications of two groups were compared.Results The operation time,hospitalization time of groupⅠwere (63.1 ±0.2)min,(5.7 ±0.6)d, which were significantly shorter than those of the control group[(90.4 ±1.1) min,(8.4 ±0.2) d].Amount of bleeding,postoperative drainage volume after operation in groupⅠwere (19.2 ±0.9) mL,(30.4 ±2.6) mL,which were significantly less than those of the control group[(42.1 ±1.8)mL,(46.2 ±1.2)mL] (t=9.819,12.168, 11.182,9.928,all P <0.05);The incidence of postoperative complications of group Ⅰ was 2.5%,lower than 15.0%of groupⅡ,the difference was statistically significant between the two groups(χ2 =13.197,P <0.05). Conclusion Minimally invasive laryngeal endoscopic thyroid operation has advantages in little trauma,short opera-tion time,rapid postoperative recovery.

Objective To study a method for determination of aluminum in food,and to ensure food safety and public health.Methods Microwave digestion-chrome azurol S spectrophotometric method was used to detect the aluminum content in fried dough sticks.Detection limit,linear range,precision,accuracy and maximum absorption wavelength were calculated.Results The method detection limit was 0.08 μg,precision was 0.60％-5.90％,and recovery rate was 96.0％-102.3％,maximum absorption wavelengnth was 620 mm.Aluminum content in 10 samples of fried dough sticks were all exceeded the standard,which were 709.5,980.8,1 010.3,1 151.5,1 214.0,1 243.7,1 318.1,1 374.0,1 435.0 and 1 479.4 mg/kg.Conclusion The method is simple,reliable,reproducible and easy to be used.

Objective To investigate and evaluate the diagnostic value of transcranial doppler ultrasound (TCD) on brain death in patients with severe craniocerebral injury.Methods Forty-two cases of severe craniocerebral injury from Affiliated Hospital of Jining Medical University were divided into brain death group and survival group,according to the clinical prognosis.TCD was conducted to examine brain blood flow numerical and changed characteristics of the spectrum.Average blood flow velocity (Vm),pulse index (PI),diastolic blood flow in reverse (reverberating flow) and small systolic peaks in early systole (wave of spikes)were documented.TCD parameters were compared between the two groups with t test.Results In the brain death group,frequency of reverberating flow was 86.67％ and frequency of wave of spikes was 13.33％.In survival group,frequency of reverberating flow was 14.81％ and no wave of spikes was recorded.The difference in frequencies of reverberating flow and wave of spikes between the two groups was significant(x2 =28.25,P ＜0.01).The average speed of bilateral middle cerebral artery blood flow in brain death group((20.02±13.96) cm/s) was significantly reduced compared with survival group((56.81± 16.84) cm/s,t=2.30,P＜0.05).Pulse index (PI) values in brain death group (4.02±3.49)were significantly increased compared with survival group (1.24±1.03) (t=2.10,P＜0.05).Conclusion Reverberating flow or wave of spikes is an important marker for brain death in patients with severe craniocerebral injury.TCD may be applied to clinically confirm the diagnosis of brain death.

Objective To investigate the expression of tumor stem cell marker aldehyde dehydrogenase -1 (ALDH-1)in primary tumor and lymph node metastases of lymph node-positive invasive micropapillary carcinoma ( IMPC) of breast and its clinical significance .Methods The expression , location and distribution of ALDH-1 were examined by immunohistochemical staining in 103 cases with lymph node metastasis of IMPC and 110 cases with lymph node metastasis of invasive ductal carcinoma not otherwise specified ( IDC-NOS) without preoperative chemotherapy and radiotherapy , and their relationship with clinicopathological characteristics and prognostic sig-nificance was analyzed .Results ( 1 ) The positive expression rate of ALDH-1 was significantly higher in IMPC group of primary tumor and lymph node metastases (37.9% in primary tumor, 39/103 cases;47.6% in lymph node metastases, 49/103 cases)than in IDC-NOS group(21.8% in primary tumor, 24/110 cases; 23.6% in lymph node metastases, 26/110 cases)(P<0.05).(2)Expression of ALDH-1 had a significantly positive corre-lation with tumor size , histological grade , lymph node metastasis , ER-negative status , PR-negative status , and HER2 overexpression in primary tumor and lymph node metastases of IMPC (P<0.05).(3)The disease-free sur-vival of IMPC group patients was significantly shorter than that of IDC-NOS group patients(P=0.003).In IPMC patients with lymphode metastases , the disease-free survival of patients with ALDH-1 positive in primary tumor and lymph node was significantly lower than patients with ALDH-1 negative expression (P<0.05).In multivari-ate analysis, positive expression of ALDH-1 in metastatic lesions was associated with worse prognosis ( P =0.005).Conclusion ALDH-1 can be used as an independent factor for predicting prognosis of patients with lymph node-positive breast IMPC and the existence of stem cells in IMPC tumor cells may represent high IMPC lymphatic invasion , lymph node metastasis and high resistance .

OBJECTIVE:To strengthen application management of antibiotics in outpatients,promote rational use of antibiot-ics,and to provide reference for scientific management and decision-making in the hospital. METHODS:The proportion of outpa-tients receiving antibiotics in total outpatients was analyzed statistically during Jan. 2008-Jun. 2016. Utilization rate data of antibiot-ics in outpatients during 2008-2015 were used to establish Autoregressive integrated moving average model(ARIMA),and the data of the first half of 2016 was used to validate established model;the utilization rate trend of antibiotics in outpatients in the second half of 2016 was predicted. SPSS 20.0 statistical software was adopted for statistical analysis. RESULTS:Established ARIMA(2,1, 0)(2,1,0)12 model has higher fitting degree. There was a small difference between measured value and fitted value of utilization rate of antibiotics in outpatients in 2016. Average absolute error was 0.72%,and average relative error was 4.20%,within 95%confidence interval of fitted value. Dynamic trend of model predicted value was basically consistent with measured value. CONCLU-SIONS:ARIMA model simulates utilization rate trend of antibiotics in outpatients well,can be used for short-term prediction and dynamic analysis of utilization rate trend of antibiotics. However,for long-term prediction,various factors should be considered.
