1.Myogenic regulatory factor Myf-5 expression at different periods of denervated skeletal muscle atrophy

BACKGROUND: Myogenic regulatory factors Myf-5 is an importance gene involved in muscle occurrence process to adjust and control, start and maintain the skeleton muscle cell growth. It has a possible relation with denervated skeletal muscle atrophy. OBJECTIVE: To detect the expression of Myf-5 gene in different skeletal muscle sitas and different periods following denervatad skeletal muscle atrophy.DESIGN, TIME AND SE'I-rlNG: Randomized control animal experiment was performed in the Shanxi Medical University between March and April in 2008. MATERIALS: A total of 24 Sprague-Dudley male rats was selected and divided randomly into four groups, with six in each, Le. sham operated group (innervations), denervated 2 d group, denarvated 7 d group and denervatad 28 d group. METHODS: Sham operations were made only for the rats in the sham operated group and sciatic nerve were not cut off. Sciatic nerves were cut off more than one centimeter at the mid-level of their right lower limb for the rats in denervated groups. The rats were executed by vertebrae dislocation method at 2, 7, 28 days after denervation. The skeletal muscles of the right lower limb (tibialis anterior, soleus, gastrocnemius and plantar) were dissected and dissociated. MAIN OUTCOME MEASURES: Myf-5 mRNA expression was detected by reverse transcription-polymerase chain reaction. Immunohistochemical stain (ABC method) with polyclonal antibody against Myf-5 protein was performed and the gray value was counted.RESULTS: Expressions of Myf-5 mRNA in denervatad skeletal muscle were up-regulated at 2, 7, 28 days in the eady stage of denervation (P < 0.05). The number of Myf-5 antibody positive-stained cell nuclei was the most in satellite cells at 28 days following denervation. CONCLUSION: At the early stage of denervated skeletal muscle atrophy in SD rats, expressions of Myf-5 in different skeletal muscles are all up-regulated. Denervation of rat skeletal muscle raises the Myf-5 expression in satellites cells.
2.Effect of transcutaneous electrical stimulation on the gene expression of MyoD from denervated skeletal muscle of rats

BACKGROUND: MyoD expression of skeletal muscle of rats increases distinctly in the earlier period of denervation, which can greatly postpone denervated skeletal muscle atrophy. The clinical test testifies that electrical stimulation is an effective method to cure denervated muscle atrophy. But the influence of electrical stimulation on MyoD expression during denervated muscle atrophy is still unproved. OBJECTIVE: To discuss the influences of electrical stimulation on the gene expression of MyoD of skeletal muscle of rats. DESIGN, TIME AND SETTING: Randomized control animal experiment was performed in the Animal Experimental Center of Shanxi Medical University between July and November in 2008. MATERIALS: A total of 36 healthy Sprague-Duwley rats of either gender were divided randomly into three groups, control group, denervation group and electrical stimulation group. Each group contained 12 rats. METHODS: Standard models of right sciatic nerve dissociation and gastrocnemius denervation were established in right limb of each rat in denervation group and electrical stimulation group. Thirty-minute electrical stimulation was given to the denervated gastrocnemius muscle of each rat of electrical stimulation group once a day. The rats were executed at 2, 7, 14, 28 days after denervation to dissect and dissociate their gastrocnemius muscles of the right lower limbs. MAIN OUTCOME MEASURES: MyoD mRNA expression was detected by reverse transcription polymerase chain reaction, and MyoD protein level by immunohistochemistry. 3.The clinical analysis for 43 cases of acute superior mesenteric artery thrombosis confirmed by angiography and surgery

Objective To investigate the clinical manifestations and mortality related risk factors in patients with acute superior mesenteric artery embolism (ASMAE).Methods Clinical data of forty-three confirmed ASMAE patients in the PLA General Hospital from June 2002 to June 2012 were retrospectively analyzed.All patients were classified into the survival group (28 cases) and the death group (15 cases)according to the prognosis.The prognosis associated factors were further analyzed.Results The study group consisted of 31 men (72.1%)and 12 women (27.9%),with average age of (63 ± 11) years.The majority patients with ASMAE in our study had history of atherosclerotic diseases.The main clinical manifestationsincluded abdominal pain [100% (43/43)],nausea and vomitting [55.8% (24/43)],hematochezia [32.6% (14/43)].Abdominal CT scan was performed in 74.4% (32/43) patients with a high positive result of 96.9% (31/32).Weight loss occurred more frequently in survival group than in death group [32.1% (9/28) vs 6.7% (1/15),P =0.001].Moreover,weight loss has been shown as a protective factor for ASMAE survival (OR =0.75,P =0.038) by logistic analysis.Compared with the death group,the incidence of either peritoneal irritation sign or ascites was significantly lower in survival group [respectively 7.1%(2/28) vs 66.7% (10/15),14.3% (4/28) vs 73.3% (11/15),P <0.05],which were two independent risk factors of mortality(OR =8.51,P =0.014 ; OR =3.07,P =0.028).The incidence of main artery embolism of superior mesentery artery (SMA) in death group was higher than that in survival group [93.3% (14/15) vs 60.7% (17/28),P =0.023].Main artery embolism of SMA was also an independent mortality risk factor of ASMAE patients (OR =5.05,P =0.039).A total of 18 patients were treated with enterectomy.Intestine excision length was shorter in survival group than in death group [(82.8 ± 25.2) cm vs (141.0 ± 18.1)cm,P =0.017].The time from onset to operation in survival group was shorter than that in death group [(44.8 ±29.7) h vs (69.1 ±28.0) h,P =0.013].Conclusions Patients with ASMAE based on chronic ischemia have a relative good prognosis for survival.Peritoneal irritation sign,ascites and main artery embolism of SMA were independent risk factors for death in ASMAE.Intestine excision length and the interval from onset to operation may affect the mortality of ASMAE patients.
4.Study on improving the critical relative humidity of Qubi granule

Objective:To increase the critical relative humidity(CRH) of Qubi granule,thus to improve its humidity resistance. Methods:The best prescription of Qubi granule was optimized by comparative method,and to determine its critical relative humidity. Results:The old prescription of the Qubi granule's CRH was 50%.With new prescription,the Qubi granule’ s CRH was 61%. the latter has increased by 11% as compared with the former . Conclusion:Considering above result,there is a hope to improve the hygroscopicity of Qubi granule.
5.Prokaryotic expression and identification of human astrovirus nonstructural proteins, nsP1a and nsP1a/4.

Human astrovirus (HastV) is recognized as one of the leading causes of acute viral diarrhea in infants. The HastV non-structural protein, nsPla, and C-terminal protein, nsPla/4, contain various conserved functional domains,and may play an important role in virus replication, transcription and the virus-host interactions of HastV. This study used an E. coli system to investigate the expression of nsPla and nsPla/4 proteins. Firstly,the nsPla and nsPla/4 genes of HAstV-1 were cloned into the prokaryotic expression vector,PGEX-4T-1, to build the PGEX-4T-1a and PGEX-4T-la/4 fusion protein plasmids. Then, the recombinant plasmids were transformed into Escherichia coli BL21 (DE3) and induced with isopropyl-β-D-thiogalactopyranoside (IPTG). The optimal expression conditions of the two fusion proteins were identified and then analyzed by polyacrylamide gel electrophoresis (SDS-PAGE) and western blotting, respectively. The results showed that the pGEX-4T-la fusion protein was maximally expressed at 30 °C after 12 hours of induction with 1.0 mM IPTG. The pGEX-4T-la/4 fusion protein was maximally expressed at 20 °C after 8 hours of induction with 0.5 mM IPTG. Western blot analysis showed that the two fusion proteins specificity reacted with the anti-nsPla and anti-GST monoclonal antibodies, respectively. This study successfully obtained the HAstV non-structural protein, nsP1a, and its C-terminal protein nsP1a/4 protein using an E. coli system. This novel study lays the foundation for future research into the pathogenic mechanisms of human astrovirus and the functions of its non-structural protein.

6.Genotyping of human papillomavirus among human immunodeficiency virus-positive populations

Objective To compare the prevalence and genotype distribution of human papillomavirus (HPV) between human immunodeficiency virus (HIV)-positive and-negative populations.Methods Patients with condyloma acuminatum or persons who recently had sexual contact with patients with condyloma acuminatum were enrolled into this study,and classified into HIV-positive group (n =62) and HIV-negative group (n =2 716).GeneChip analysis was performed to detect HPV and determine HPV genotypes in mucocutaneous samples collected from the external genitalia of these subjects.Statistical analysis was carried out by chi-square test using the SPSS software version 19.0.Results The prevalence rates of HPV infection,high-risk HPV types and low-risk HPV types were significantly higher in HIV-positive persons than in HIV-negative persons (74.19% (46/62) vs.42.30% (1 149/2 716),67.74% (42/62) vs.29.57% (803/2 716),58.06% (36/62) vs.24.71% (671/2 716),respectively,all P< 0.01).The detection rate of HPV was also increased in HIV-positive men compared with HIV-negative men (92.11% (35/38) vs.37.38% (382/1 022),x2 =45.98,P < 0.01).Although the top three genotypes of low-risk HPV were types 6,43 and 11 in both HIV-positive and-negative groups,the prevalence rate of HPV 6 was 37.10% (23/62) in HIV-positive group,but only 11.12% (302/2 716) in HIV-negative group.The top five genotypes of high-risk HPV were types 16 (22.58%,14/62),52,66,58 and 18 in HIV-positive group,and types 16 (7.77%,211/2716),58,56,66 and 52 in HIV-negative group.Coinfections with multiple HPV subtypes were common in both groups,and the number of concurrent HPV genotypes was as high as 8 in HIV-positive group,and 9 in HIV-negative group.The prevalence rate of coinfections with three or more HPV genotypes in HIV-positive group was significantly higher than that in HIV-negative group (65.21% (30/46) vs.16.71% (192/1 149)).Conclusions Compared with HIV-negative populations,HIV-positive populations show elevated prevalence of HPV infection,high-risk HPV genotypes and low-risk HPV genotypes.Moreover,the prevalence of HPV is higher in HIV-positive men than in HIV-negative men.These findings are worthy of further attention in clinic.
7.The surface structure and antibacterial property of Ag-containing coating on casting cobalt chromium alloy

Objective:To study the surface structure and antibacterial property of Ag-containing coating(AgCC)on casting cobalt chromium alloy(Co-Cr).Methods:Plasma spraying technique was adopted to prepare AgCC on Co-Cr.Scanning electron microsco-py,energy dispersive analysis and X-ray diffraction analysis were used to evaluate the surface properties.The antibacterial effects of AgCC against staphylococcus aureus and candida albicans were examined by film attachment method.Fluorescence microscope was a-dopted to evaluate the bacteria adhesion property.Results:AgCC surface was uniform and compact,combined perfectly with sub-strate material.The content of the surface was mainly Ag,Cr and a small amount of Ag2 O and Cr2 O3 .The results of antibacterial as-say showed that on AgCC the number of bacterial colony was fewer(P<0.05)and bacterial adhesion was less than those on Co-Cr(P<0.05).Conclusion:AgCC surface was stable and has antibacterial property.
8.The contrast of procedure for prolapse and hemorrhoids between the local anesthesia and the combined spinal epidural anesthesia

Objective To explore the clinical effect of procedure for prolapse and hemorrhoids (PPH) treating for Ⅲand Ⅳ degree hemorrhoid under the local anesthesia.Methods One hundred and fifty patients with Ⅲ and Ⅳ degree hemorrhoid were divided into two groups randomly.One group was the local anesthesia group (LA group) which included 73 cases,the other one was the combined spinal epidural anesthesia group (CSE group) which included 77 cases.Compared the safety and efficacy of different procedure.Results The original symptom of the two groups were improved.There was a significant difference in the time of hospitalization time and the hospitalization expense between the two groups (P<0.05).The LA group was(4.8±1.1)days with(3980±639)yuan,and the CSE group was(6.8±1.1) days with(5128±728)yuan.The rates of two groups of urine retention were 9.6% and 24.7% (P<0.05) after the operation respectively,and there were no significant differences in recovery normal activity time,the pain index,copracrasia and pruritus,the bleed,the anal fistula,the prolapse of hemorrhoid,the skin tag (P >0.05).Conclusion PPH under the local anesthesia is safe,compare to the combined spinal epidural anesthesia,it excels in shortening the hospitalization time and reducing the hospitalization expense,also it can reduce significantly the rate of urine retention after operation.
9.Treatment of renal calculi in pediatric patients with minimally invasive percutaneous nephrolithotomy

Objective To evaluate the technique and efficacy of minimally invasive percutaneous nephrolithotomy (MPCNL) for renal calculi in pediatric patients. Methods From April 2009 to December 2009, 12 pediatric patients (8 males and 4 females) with renal calculi were treated by MPCNL. The age ranged from 18 to 53 months (mean 32 months). All the 12 cases were diagnosed by KUB+IVU, ultrasonography and CT. The stone had average diameter of 1. 3 cm (ranged from 1. 0 to 1. 8 cm). Seven cases had simple renal pelvis stone and 5 cases had multiple renal calyx stone. UPJ stricture was not found in this series. General anaesthesia was applied. Renal transfixion pin was punctured to select renal calices by monitoring with ultrasonography. 12 - 16 F percutaneous renal access was successfully established in all cases and calculi were fragmented by pneumatic lithotripter. Results The average operative time of MPCNL was 74 min. Phase Ⅰ lithotripsy was underwent in all patients. The phase Ⅰ stone-free rate was 67%(8/12). One cases accepted second MPCNL. The calculus clearance rate reached 75%(9/12). Three cases had residual calculi ranged from 2 to 4 mm. One of whom had ESWL 2 weeks postoperatively. All cases were followed up for 1 - 7 months, all cases were in stone free status. Conclusion Regarding the advantages of less bleeding, high clearance rate, and shorter hospital stay, MPCNL is an effective and safe treatment option for renal calculi in pediatric patients.
10.Effect of Shanzha Xiaozhi Capslue on Stability of Atherosclerosis Plaques in Patients with Metabolic Syndrome and Carotid Atherosclerosis

Objective To investigate the effect of Shanzha Xiaozhi Capslue on stability of atherosclerosis plaques in patient with metabolic syndrome (MS) and carotid atherosclerosis (CAS). Methods Totally 106 patients with MS and CAS were randomly divided into 2 groups with 55 cases in the treatment group and 51 cases in the control group. The control group improved the way of life and control of relevant risk factors, while the treatment group was treated with Shanzha Xiaozhi Capslue additionally for 6 months. The occurrence of major adverse cardiovascular and cerebrovascular events of the two groups was compared. The intima-media thickness, plaques grade of carotid artery and the changes of Hs-CRP, Hcy, insulin resistance were measured before and after the treatment. Results After treatment, the occurrence of major adverse cardiovascular and cerebrovascular events in the treatment group reduced significantly compared with the control group (P=0.047). The intima-media thickness and plaques grade of carotid artery in the treatment group decreased significantly compared with the control group (P<0.05), and the serum levels of Hs-CRP, Hcy, insulin resistance were decreased significantly compared with the control group (P<0.01). Conclusion Shanzha Xiaozhi Capslue can decrease the intima-media thickness and plaques score of carotid artery, stabilize atherosclerotic plaque, thus reduce the occurrence of major adverse cardiovascular and cerebrovascular events of patients with MS and CAS.
