1.Clinical analysis of the diagnosis and treatment of benign paroxysmal positional vertigo by TRV-CRP.

OBJECTIVE:
To explore the effective of the new mechanical assistant canalith repositioning procedure (TRV-CRP) to the diagnosis and treatment of the benign paroxysmal positional vertigo (BPPV).
METHOD:
The data of the cases with suspected diagnosis of BPPV from April to June of 2012 checked and treated by TRV-CRP in our vestibular function checking room were collected and analyzed. All of the cases were followed up to one to three months.
RESULT:
Total 504 cases of suspected diagnosis of BPPV were collected. Three cases were rejected for un completed procedure. All of the 501 completed procedure cases included 162 male and 339 female with age ranging from 10 years old to 86 years old (52. 8 +/- 14 years old). 169 cases (33.73%) were positive diagnosis and 332 cases were negative diagnosis (66.27%). 135 cases enrolled only one semicircular canal (SC) account for 79.89% of all the positive diagnosis that included 17 cases of left horizontal SC, 4 cases of left superior SC, 29 cases of left posterior SC, 23 cases of right horizontal SC, 4 cases of superior SC and 58 cases of right posterior SC. 25 cases enrolled more than two SC (14.80%). 9 cases of objective BPPV (5.33%) included 5 cases of right posterior SC, 3 cases of left posterior SC and one cases of right horizontal SC. Exception of 163 cases without classic BPPV history in the 332 negative diagnosis cases, 169 cases had classic BPPV history including 14 cases treated by manual canalith repositioning procedure and 33 cases which had released from the vertigo. The rate of treatment effective by TRV-CRP was 100%. There were 20 cases returned for retreatment because their unsatisfied about the release of their symptom but proved to be negative diagnosis again in 10 cases and positive diagnosis in 10 cases, however, only 4 cases proved to be the same SC disease were considered as relapse (2.37%) and others enrolled different SC. After retreated, all cases got release without relapse.
CONCLUSION
TRV-CRP could diagnosis 34% of the primary diagnosis cases of BPPV. TRV-CRP had high rate of effective and lower relapse which especially fitted to patients suffered from complex BPPV or difficult to finish the manual repositioning procedure. However, due to the expensive of TRV-CRP, it is reasonable to combine TRV-CRP and manual repositioning procedure in clinical.

4.Less invasive stabilization system(LISS)in treatment of periarticular fractures of the knee joint

Objective To discuss the less invasive stabilization system(LISS)in treatment of fractures around the knee joint.Methods Between February 2002 and December 2005,103 cases of periarticular fractures of knee joint were treated with LISS in our department.There were 58 cases of comminuted distal femoral fracture and 45 cases of comminuted proximal tibia fracture.Follow-ups were conducted once per week during the first four weeks and once per month in the following months to observe whether such complications as loosening of internal fixators and nail breakage would occur.Results All the cases were followed up for a mean time of 16.5 months(range, 6 to 27 months).Loosening and breakage of internal fixation was found in only one case,while others obtained bony union.In fractures of types A and C,the knee range of motion varied from 95?to 145?(average,130?)and 85?to 140?(average,110?)in flexion respectively.HSS score:excellent in 84 cases(81.6%),good in 15 cases (14.6%),fair in three cases(2.9%),and poor in one case(0.9%).Conclusions The LISS can be the best choice for the treatment of periarticular fractures around the knee joint,but we should take care to implant the plate and screws in a proper position and direction and avoid early weight-bearing.In application of the 13-hole plate, large incision should be made to approach the distal tibia for fear of damaging the superficial peroneal nerve.
5.Brain MRI and clinical characteristics of maple syrup urine disease

Objective To investigate the characteristics of brain MRI in the patients with maple syrup urine disease(MSUD). Methods Nine patients with MSUD were diagnosed by gas chromatography mass spectrometry,tandem mass spectrometry and gene test.The MRI,clinical featuresand laboratory tests were analyzed.Results The 9 patients onset age were 3 days to 6 years old.The symptoms were varied such as poor response,lethargy,seizures and learning difficulties.Remarkable elevations of blood levels of leucine and valine were found in all patients.All patients had MRI examination,2 of them also took the examination of proton magnetic resonance spectroscopy (1 H-MRS).Involving cerebellar hemisphere,cerebellar peduncle,cerebral peduncle,brain stem,globus pallidus in 5 cases respectively,thalamus and posterior limb of internal capsule in 4 cases,centrum semiovale in 3 cases.2 cases also showed abnormal signal in corpus callosum,occipital,deep temporal lobe,and frontal lobe,parietal lobe.All lesions showed mild low signal intensity in T1 ,mild high signal intensity in T2 and obvious high signal intensity in DWI,except one case.1 H-MRS did not show a methyl resonance peak at 0.9 ppm in the two patients.Conclusion The MSUD lacked of specific clinical features.The MRI characteristics are the myelinated white matter such as the cerebellar hemispheres,posterior limb of the internal capsule,the brainstem,cerebellar peduncle,cerebral peduncle showing high signal in DWI.
6.Rehabilitation for Apraxia with Unilateral Neglect after Corpus Callosum Infarction:Case Report

Objective To explore the clinical characteristics and rehabilitation of apraxia with unilateral neglect after corpus callosum in-farction. Methods A case of apraxia with unilateral neglect after corpus callosum infarction was discussed. Results After rehabilitation, the apraxia and unilateral neglect were improved. Conclusion For similar cases, rehabilitation can significantly improve the symptoms and ac-tivities of daily living.
7.The correlation between HPV infection, the expression of PCNA, Ki -67, p53 and clinicopathological characteristics in cervical carcinoma

Objective To explore the relationship between HPV infection ,the expression of PCNA ,Ki-67,p53 in cervical carcinoma and clinicopathologic data .Methods Seventy-six cases of cervical cancer tissues with complete clinical data were collected from the third affiliated hospital of harbin medical university from march 2012 to September 2012 .Tissue microarray technology ,immunohistochemical technique and statistic method were adopted to make comparative analysis among HPV ,PCNA,Ki-67,and p53 expression in patrents with cervical cancer.Results The expression rate of HPV in cervical carcinoma was 68.4%,which was related to the lym-phatic metastasis(P<0.05).Expression of PCNA in cervical cancer tissue was 90.1%and 100%in female pa-tionts women under the age of 35.The expression rate of Ki -67 and p53 were 48.9%and 38.2%respectively, both were correlated to lymphatic metastasis(P<0.05)and preoperative chemotherapy(P<0.01).Conclusion HPV infection with Ki -67,and p53 expression in cervicalcancer tissues are associated with the presence of lymph node metastasis .HPV promotes cervical epithelial cell proliferation ,inactivates tumor suppressor gene p 53 function ,and promotes the invasion and metastasis of cervical cancer .Preoperative chemotherapy of cervical canc-er can reduce the expression of p53,PCNA and Ki-67.
8.Application of no-rinse skin cleanser in first brush bath of the new-born

Objective To compare the effects of no-rinse skin cleanser and warm water with paraffin oil in first brush bath of the new-born. Methods About 42 newborns who were born from January to March 2016 were set as control group and used warm water with paraffin oil in first brush bath. About 43 newborns who were born from April to June 2016 were set as observation group and used no-rinse skin cleanser in first brush bath. The cleaning time and effect ,the rectal temperature before and after sponge and dermatitis were compared. Results Both groups did not develop dermatitis. But the observation group had better cleaning satisfaction , less cleaning time, faster rewarming than those of the control group with statistical significance (all p<0.05). Conclusion No-rinse skin cleanser used in the first brush for the new-born can reduce cleaning time and difference of temperature, which is safe for clinical application.
