Objective To understand current situation of live quality of old patients and to explore its influencing factors.Methods Age of all subjects more than 60 years who was aged in-patients with a chronic disease in geriatric ward,department of XiangYa Hospital. General physical examination were executed. Symptom Checklist 90(SCL-90) and Quality of Life (QOL) scales were measured.Results ⑴The problems of QOL and mentality were occurred in 78%,77 33% respectively of the patients. ⑵Multiple linear regression showed that financial condition, mentality, age and physical disease were major factors that affected the patients' quality of life . 2.Comparative Study of the Effects and the Safety of Different Dose of Simvastatin on Aged Patients With Hyperlipidemia

Objective To investigate the effects and the safety of different dose of simvastatin on aged patients with hyperlipidemia.Method The 70 patients were divided into two groups at random. 40 in group A was given orally simvastatin 20 mg/d, and 30 patients in group B was given orally simvastatin 40 mg/d,respectively; The levels serum total cholesterol(TCH), low-density lipoprotein cholesterol(LDL-C), high-density lipoprotein cholesterol(HDL-C) and triglyceride(TG) were measured following the 4th week, the 8th week and the 12th week . And the changes of the functions of the patients'liver and kidney, the serum glucose level and the activities of myocardial enzymes in the two groups before and after the simbastatin treatment were also observed. Results In the 4th , 8th and 12th week at the treatment with simvastatin 20mg/d, the patients' TCH levels were decreased by 32 1%,32 1%,36 0% respectively;the TG levels were decreased by 35 1%,49 6%,52 9% respectively;the LDL levels were decreased by 39 3%,37 3%,35 2% respectively;and the HDL-C levels were increased by 2 0%,0 8%,8 3%, respectively. While in the 4th, 8th and 12th week at the treatment of simvastatin 40mg/d,the patients' TCH levels were decreased by 41 8%,48 0%,51 0%, TG levels were decreased by 18 4%,32 3%,38 4%, the LDL levels were decreased by 42 4%,47 5%,50 8%, and the HDL-C levels were increased by 1 0%,2 0%,13 0% respectively. There were no obviously changes of the results of the tests of the liver function,the kidney function,the blood glucose level and the myocardial enzymes before and after both treatment groups in the different dose of the simvastain. Conclusion The effects and the safety of the treatments of simvastatin on aged patients with hyperlipidemia were similar at the dose of 20mg/d and the dose of 40mg/d.
3.Survey of Methadone Maintenance Treatment in Wuhan City,2006-2013

Objective To explore drug abuse prevention measures by surveying the changes before and after methadone maintenance treatment(MMT). Methods The baseline survey data were obtained from patients who participated in the MTT program for the first time at Wuhan First Health Clinic of Mental Health Center Affiliated to Tongji Medical College between March 2006 and December 2013,and the general conditions of these patients were analyzed. Results There were 1 186 drug abusers, with a male and low education(junior high school and below)dominance. After the initiation of the MMT program,the number of addicted people was highest in 2008,and then gradually decreased after 2009.MMT program achieved obvious social benefits.The proportion of injectable drug use alone was decreased and the rates of oral drug use and snorting were increased over time.Fixed salary and temporary salary were obviously increased in drug abusers after 2009. Daily drug cost was decreased over time. The proportion of community/ media propaganda through which people got to know MMT remained low. Conclusion The community support and educational propaganda should be strengthened at the same time when MMT is carried out.
4.Relationship between autophagy in spinal dorsal horn and development of morphine tolerance in rats

Objective To investigate the relationship between autophagy in spinal dorsal horn and development of morphine tolerance in rats.Methods Twenty-four healthy male Sprague-Dawley rats,in which intrathecal (IT) catheters were successfully placed,were randomly divided into 3 groups (n =8 each):control group (group C),morphine tolerance group (group M) and morphine + rapamycin as a reinforcing agent for autophagy group (group MR).Morphine tolerance was induced with IT morphine 20 μg twice a day for 7 consecutive days.While the equal volume of normal saline was given in group C.In addition,rapamycin 2.3μg was injected intrathecally at the second injection of morphine on 3rd day lasting for 3 consecutive days in group MR.Mechanical withdrawal threshold (MWT) to yon Frey filament stimulation was measured before IT injection and 30 min after the second IT injection on 1st,3rd,5th and 7th days.The rats were sacrificed 1 h after the last MWT measurement and the L4-6 segment of the spinal cord was removed for determination of the total mammalian target of rapamycin (mTOR) and phosphorylated mTOR(p-mTOR) and autophagy marker protein LC3 Ⅱ expression in spinal dorsal horn by Western blot.The percentage of p-mTOR expression in total mTOR expression was considered as reflection of the activity.Results MWT was gradually decreased with the prolongation of time of IT injection (P < 0.05).Compared with group C,MWT was significantly increased during IT injection,mTOR activity was decreased and LC3 Ⅱ expression was up-regulated in groups M and MR (P < 0.05).Compared with group M,MWT was significandy increased on 3rd,5th and 7th days after IT injection,mTOR activity was decreased and LC3 Ⅱ expression was up-regulated in group MR (P < 0.05).Conclusion Increased autophagy in spinal dorsal horn is the regulatory mechanism of the body during the development of morphine tolerance in rats and can delay the development of morphine tolerance.
5.Analysis on feasibility of C14-urea breath test in diagnosis of child chronic cough

Objective To explore the feasibility of C14-urea breath test in the diagnosis of child chronic cough.Methods One hundred and twenty children cases of chronic cough were collected as the research subjects and divided into the C14 group(adopting C14-urea breath test for diagnosis) and combined group(C14-urea breath test combined with routine diagnostic method) according to different diagnostic means,60 in each group.Then the positive diagnostic rate and effectiveness were analyzed and compared between the two diagnostic methods.Furthermore the ROC curve was adopted to evaluate its diagnostic value.Results The positive diagnosis rate in the C14 group was 76.67%,which was significantly lower than 95.00% in combined groups,the difference was statistically significant (P<0.001).The sensitivity and positive predictive value in the combined group were obviously higher than those in the C14 group (P<0.01),but there were no statistically significant differences in specificity and negative predictive value between the two methods (P>0.05).The expression levels of serum hs-CRP and IL-6 in the C14 group were significantly higher than those in the combined group,but the expression level of IL-10 was obviously lower than that of the combined group (P<0.05).The AUC in the ROC curve of combined group was significantly higher than that of the C14 group(P<0.05).Conclusion The C14-urea breath test can serve as a supplementary diagnosis means in child chronic cough,and its combining with the routine diagnosis method can significantly increase the diagnosis effectiveness.

7.Antibiotic Resistance of Acinetobacter baumannii in ICU

OBJECTIVE To analyse the infection of Acinetobacter baumannii(ABA) in ICU of Cardiac Surgery,and make analysis for its resistance to 14 antibiotics.METHODS The collection of clinic stains of ABA isolated from ICU in 2008.The pathogens were identified by VITEK-Jr.RESULTS The drug resistance rates of Imipenem and cefoperazone/sulbactam were 74.4% and 18.6%.The resistance rates to other drugs were more than 70%.CONCLUSIONS ABA strains isolated show the trend of multi-durg resistance.We should pay more attention to the monitor of the resistance to antibiotics.
8.Increased interferon-inducible gene expression, an useful marker in the diagnosis and disease activity evaluation of systemic lupus erythematosus

Objective To study the role of the expression levels of 5 type Ⅰ interferon (IFN)-inducible genes (LY6E, OAS1, OASL, MX1, and ISG15) in the diagnosis and disease activity evaluation of systemic lupus erythematosus (SLE). Methods Peripheral blood was obtained from 68 SLE patients, 50 patients with other connective tissue diseases and 26 normal controls, and total RNA was extracted and reverse transcribed into complementary DNA. Real-time PCR technique was used to determine gene expressions at transcription level. An IFN score for each individual was calculated according to the expression of 5 1FN genes. Comparisons of gene expression and IFN score were made among groups. The genes expression levels in patients with SLE were analyzed using receiver operative characteristic curve. The association between IFN scores and disease activity, as assessed by the SLEDAI scores and 24 h proteinuria, was analyzed using Spearman correlation analyses. Results ① The expression levels of MX1, OASL, OAS1, ISG15 and LY6E mRNA in SLE patients were significantly increased as compared with normal controls and disease controls (P all<0.01 ).② IFN scores in SLE patients (17.9±29.1) were significantly increased as compared with normal controls (0±3.3)and disease controls (3.0±8.1) (P all<0.01 ). ③ IFN scores area under the ROC curve (AUCROC) was 0.846. When The IFN scores reached 2.56, its sensitivity and specificity for the diagnosis of SLE were 93.1%and 78.3%, respectively. ④ Levels of IFN score was positively correlated with SLEDAI scores (r=0.256,P<0.05) and 24 h proteinuria (r=0.337, P<0.05). Conclusion The 5 IFN-inducible genes are highly expressed in SLE patients. IFN score level is valuable for the diagnosis of SLE and a high IFN score is usually associated with an elevated disease activity.
9.The different effect of negative emotion induced by background music on verbal and spatial working memory: evidence from ERP study

Objective To investigate the different effect of negative emotion induced by background music on verbal and spatial working memory. Methods Fifteen participants were exposed in relaxing or horrible background music and did the working memory task. After each emotional condition, the 9-point Self Assessment Manikin scale was registered. n-back paradigm was employed as WM task and the WM task contained two different types, the verbal WM and spatial WM. In the same time,The ERPs during each emotional condition was recorded,and the anterior LPC and posterior P300 were analyzed. Results The valence,arousal and dominance were 5.40±0.91,3.93 ±0.79 and 5.33 ± 1.29 in relaxing condition,while the same dimensionalities in negative condition were 2.00 ± 0.75,6.47 ± 0.99 and 2.33 ± 0.89. The self reported data showed that horrible background music induced the negative emotional state, which were observed from the decrease of valence (t(14)= 15. 902, P <0.01) and dominance (t(14) =6.708, P<0.01) while the increase of arousal (t (14)=9.906, P<0.01). The ERP data showed,there was a main effect in emotion with anterior LPC (F (1.14)= 7.37, P < 0.01) and in both verbal and spatial WM,the amplitude of LPC were reduced in negative emotional condition. However, there was an interaction of emotion and type on amplitude of P300 (F (1.14)= 11.25, P < 0.01), selective influence of negative emotion in spatial WM were seen (F (1.14)= 12.43, P<0.01). Conclusion The WM was influenced by negative emotion induced by negative background music ,and the spatial WM was more vulnerable to negative emotion.
10.Effect of mesenchymal stem cells transplantation on OAZ signaling pathway in patients with systemic lupus erythematosus

Objective To investigate the in vivo effects of allogeneic mesenchymal stem cells transplantation (MSCT) on OAZ signaling pathway in patients with systemic lupus erythematosus (SLE).Methods Isolated and expand human MSCs from bone marrow cells or umbilical cord of healthy donors were infused into SLE patients. Peripheral blood cells were collected from 10 pre-MSCT patients as well as 1 week and 4 week post-MSCT, and RNA was extracted and reverse transcripted to cDNA. mRNA expression levels of OAZ and Id1-3 were measured by using real-time PCR. Serum levels of IL-10, IL-12, IL-21, CCL2 and ANA were tested by ELISA. Relationships of the gene expression levels with levels of cytokines and ANA were analyzed. Results mRNA expression levels of OAZ, Id1 and Id3 gene in patients with SLE were significantly decreased at week 1(△C:12.4±1.1, 9.7±1.9, 9.7±1.9, 2.1±1.0) and at week 4 (△Ct:13.3±1.2, 10.4±1.5,10.8±1.2, 2.1±1.2) after MSCT when compared to those of the pre-MSCT (△Ct:11.0±0.9, 7.4±2.1, 7.8±2.1, 0.1±1.5 respectively, P all<0.05). Levels of IL-10, IL-21 and ANA were significantly lower 4 week after MSCT than those before (P<0.05); while level of CCL2 was higher than pre-MSCT (P<0.05). Cytokines and ANA levels 1 week after MSCT were not differentially changed comparing to those of the pre-MSCT. Alteration of OAZ mRNA expression levels pre- and post-MSCT were negatively correlated with changes of ANA, IL-21levels and positively correlated with changes of IL-12/IL-10 and CCL2 levels. Conclusions The expression of genes involving in the OAZ signaling pathway is effectively reduced along with the alteration of several cytokines and ANA after allogeneic MSCT in SLE patients. OAZ signaling pathway may play an important role in MSCT treatment for SLE.
