1.Heparin in the treatment of burns

Burns are difficult to treat.Burn wounds complicated with serious infection and multiple organ dysfunction with high mortality.Burns wounds often heal slowly with scars and contractures.Glycosaminoglycan(GAG) has been used in parenteral and topical applicarion studies.These studies have uncovered anticoagulative,antiinflammatory and neoangiogenic properties,which may stimulate tissue repair and reepithelializing effects.The GAGs utilized in treating bruns are heparin,dermatan sulfate,heparan sulfate,keratin sulfate,chondroitin 4 and chondroitin 6 sulfate,and hyaluronic acid.Heparin,the most sulgated and acidic GAG,has been used parenterally,topically,by inhalation,in pellet,and in bioengineered membranes.Heparin relieved pain,inhibited clotting and inflammation,restored blood flow, and enhanced healing.The healed skin was smooth.Heparin preserved lung and improved function.Heparin preserved intestinal intergrity and reduced bacterial translocation.Collagen restoration was enhanced.Potential adverse effects with heparin use are bleeding,thrombocytopenia and allergy.It is important to know when heparin is contraindicated.And the coagulation time should be tested.Although not as yet fully substantiated,topical heparin therapy of burns is a useful addition to the range of available treatments for burn wounds.
2.Clinical study of atorvastatin and sodium ferulic acid on renal interstitial fibrosis of diabetic nephropathy

Objective To observe the clinical efficacy of atorvastatin combined with sodium ferulic acid on renal interstitial fibrosis of diabetic nephropathy(DN).Methods According to the diagnostic and staging criteria of DN,56 patients of DN(Mogensen stage Ⅲ~Ⅳ)who hadn’t received lipid-lowering treatment in the last 2 months and glycemic control were within the standard were randomly divided into control group (n =28 )and experimental group(n=28).Control group were received basic treatment including diabetes diet,decreasing blood glucose level and adjusting blood pressure.Experimental group were added atorvastatin combined with sodium besides the basic treatment above-mentioned.The course is both 4 weeks in two groups.Changes of fasting plasma glucose(FPG),blood urea nitrogen(BUN),serum creatinine(SCr)and 24-hour urine albumin excretion rate (24hUAER),transforming growth factorβ1 (TGF-β1 )before and after the treatment in two groups were observed. Results The differences of FPG,BUN, SCr,24h UAER and TGF-β1 in two groups before and after treatment were statistically significant(P<0.01).Conclusion Atorvastatin combined with sodium ferulate can function cooperatively in the treatment of diabetic nephropathy and can delay renal interstitial fibrosis.
3.Influence of Lamaze childbirth method of minus pain on breast secretion

ObjectiveTo discuss influence of Lamaze childbirth method on breast milk secretion during the course of Doula delivery. Methods106 primipara of natural childbirth were divided into the observation group(52 cases) and the control group(54 cases). Lamaze childbirth method of minus pain was applied in the observation group based upon Doula delivery, the control group only received Doula delivery. The initiating time of lactation, lactation volume were observed and the result underwent t test and χ2 test. ResultsThe initiating time of lactation was earlier and the lactation volume was increased in the observa-tion group than those in the control group. ConclusionsApplication of Lamaze childbirth method of mi-nus pain during the course of Doula delivery can promote early postpartum lactation and adequate lac-tation volume.
4.A bibliometric analysis of Medical Papers published by Huai'an First People's Hospital during 2002-2011

Object Bibliometrically analyzed are the medical papers of Huai'an First People's Hospital from 2002 to 2011 on CBMdisc.Methods Our own program and excel 2007 are used to statistically analyze the annual change,distribution of periodicals,authors,department,fund papers as well as citations of medical papers published by staff of Huai'an First Hospital in 10 years.Results 1935 Chinese papers were published from 2002 to 2011.However,Huai'an First Hospital began to publish SCI papers since 2009,a total of 24,accounting for only 1.23% of the total number of papers in English.According to Price's law,there are 136 core authors who published nearly half of Chinese papers(955).The two bright academic leaders Li Yufeng and Yu Liang in Hematology swept a quarter of SCI papers.All the 136 core authors and two academic leaders are well deserved scientific research backbones of our hospital.Chinese papers' total cited rate is 47.29 %,cited frequency 2.99 times.SCI papers' cited rate is 75%,cited frequency 2.83 times.Conclusions The amount of papers,amount of papers published on core journals and SCI papers volume increased year by year.But there is a great disparity of proportion between the amount of papers published on domestic journals and SCI papers volume.It is necessary to improve papers quality.
5.Induction of apoptosis with Salvia miltiorrhiza monomer IH764-3 via downregulating focal adhesion kinase in H_2O_2-stimulated hepatic stellate cells

AIM: To investigate the effect of IH764-3 on the apoptosis of H 2O 2-stimulated hepatic stellate cells(HSCs) and the alteration of focal adhesion kinase (FAK). METHODS: The proliferation of HSCs was examined by direct cell count and the apoptosis was determined by Annexin-V/PI labeling, while the morphological change was observed by light microscopy and transmission electron microscopy. In addition, FAK mRNA was detected by RT-PCR. RESULTS: H 2O 2 promoted the proliferation of HSCs. IH764-3 induced the apoptosis of HSCs in a dose-dependent manner. The HSC apoptotic rates of different groups were 6.35%,9.28%,15.10%,19.69%,respectively, after treated with different concentrations of IH764-3 for 48 h while H 2O 2 group showed 2.30%. In 30 mg/L group, the apoptosis rates were 6.73%?10.34%?15.10% for the indicated time periods(12 h, 24 h, 48 h). In the presence of IH764-3, FAK mRNA decreased. The FAK mRNA reduction began at 2 h after adding IH764-3. CONCLUSION: IH764-3 induced the apoptosis of HSCs. Down-regulating the expression of FAK mRNA may be one of its mechanisms.
6.Inhibitory effects of Arg-Gly-Asp-Ser(RGDS) tetrapeptide on proliferation and collagen synthesis via downregulating focal adhesion kinase in hepatic stellate cell

Objective To investigate the inhibitory effect of Arg-Gly-Asp-Ser (RGDS) on proliferation and collagen synthesis stimulated by fibronectin (FN) and the influence on the expression of focal adhesion kinase of hepatic stellate cell (HSC), in order to provide a theoretical ground for its use in prevention and treatment of hepatic fibrosis in clinic. Methods The adhesion rates were determined by toluidine blue colorimetric assay. The effects of RGDS on the proliferation and collagen synthesis of HSC were assessed by ~3H-TdR and 3H-proline incorporation assays, respectively. The FAK mRNA of HSCs was determined by RT-PCR. Results RGDS was found to inhibit the rates of adhesion, while RGES has no such ability; ②The proliferation and collagen synthesis of HSC were inhibited by RGDS in dose and time-dependent manners; ③The expression of FAK in the (25, 50, 100mg/L) RGDS groups was significantly lower than that in the control group at mRNA level. Conclusions RGDS can inhibit proliferation and collagen synthesis of HSC, and these effects correlate with its anti-adhesion ability. Meanwhile the expression of FAK is also down-regulated significantly. As a signaling molecule, FAK may be involved in the effect of RGDS. This may be one of the underlying mechanisms of anti-fibrosis effect of RGDS.
7.Relationship between calpain-10 gene polymorphism and insulin resistance phenotypes in Chinese.

In order to determine whether the variations in the calpain-10 gene constitutes risk of type 2 diabetes (T2DM) in Chinese, the frequency of UCSNP-43, 44 in 268 adults newly diagnosed with T2DM (according to the 1999 ADA criteria) and 153 non-diabetic control subjects was investigated. For all subjects, the height, weight, waist-to-hip ratio (W/H) and blood pressure, as well as following parameters were measured: (1) 75-g oral glucose tolerance test with insulin, C-peptide, HbA1c and blood lipid profiles; (2) Genomic DNA extracted from peripheral blood lymphocytes was genotyped for UCSNP-43 (calpain-10-g. 4852 G/A) and UCSNP-44 (calpain-10-g. 4841 T/C) by sequencing a polymerase chain reaction (PCR)-amplified fragment. PCR product was selected by single strand conformation polymorphism (SSCP) and then sequenced. The results showed that there was significant difference between T2DM group and normal control group in allele frequencies, haplotype frequencies, or haplotype combinations of UCSNP-43 and -44 either. But in newly diagnosed T2DM group, it was found that the individuals with the genotype UCSNP-44 T/C + C/C had significantly increased fasting and post-challenge insulin levels (Fins and P2hIns), consistent with reduced insulin sensitivity. In the BMI> 25 subgroup, the differences were even more significant. It was demonstrated that the Calpain-10 gene polymorphism UCSNP-44 was associated with insulin sensitivity and Fins and P2hIns in newly diagnosed T2DM, although Calpain-10 doesn't appear as a major diabetes susceptible gene in this population.

8.Transurethral vaporization of prostate(TUVP)for high risk patients with benign prostate hyperplasia

Objective To Study the treatment of high risk patients with binigu prostatic hyperplasia using transurethral vaporization of prostate(TUVP). Methods 60 cases of acute or chronic urinary retention were treated by TUVP after preliminary urine diversion and adequate preoperative preparation.The average age of 60 cases was 80(72~88)years. Results The function of urination recovered well after operation.The maximum urination flow rate increased from (7.4?3.5)ml/s to (18.6?5.2)ml/s( t =-13.23, P =0.000);The avarage urination flow rate increased from(3.3?1.5)ml/s to (12.6?2.6)ml/s( t =-23.15, P =0.000);The residual urine decreased from(381.3?65.3)ml to (30.5?8.4)ml( t =36.17, P =0.000). Conclusions TUVP has the advantages of minimal invasion,safety,good efficacy and less complications and it fits for the treatment of high risk patients with severe complications.

10.Treatment of ureteral calculus with transurethral ureteroscopic pneumatic lithotripsy

Objective To investigate therapeutic effects of transurethral ureteroscopic pneumatic lithotripsy(TUPL).Methods A retrospective analysis was made on 320 cases of ureteral calculus treated with TUPL from September 2002 to August 2005.A Wolf F9.8 rigid ureteroscope and an EMS pneumatic ballistic lithotriptor were used.The probe was 0.8 mm or 1.0 mm in diameter.The pulse settings was 200~300 kPa.The fragmentation was conducted with single or multiple episode of treatment.Results The stone was pushed back to the renal pelvis in 6 cases and a conversion to extracorporeal shock wave lithotripsy(ESWL) was needed.The insertion of the ureteroscope was failed in 5 cases and a conversion to open surgery was required.The TUPL time was 2~10 min(mean,5.5 min) and the intraoperative blood loss was 3~15 ml(mean,7 ml).The stones were successfully fragmented on one session in 309 cases,the success rate being 96.6%(309/320).The length of postoperative hospital stay was 3~7 d(mean,4.5 d).There were 24 cases of mucosal injury of the ureter and 4 cases of false passage,all of which were cured with double-J catheter indwelling for 1 month.Follow-up examinations for 1~10 months(mean,4 months) showed stone clearance within 1 month and no ureteral stenosis or urinary infection.Conclusions Transurethral ureteroscopic pneumatic lithotripsy for the treatment of ureteral calculus is safe and effective.
