1.The clinical study of eight cases of nasal solitary keratoacanthoma.

The clinical and pathological data of eight cases of nasal solitary keratoacanthoma were analyzed and relevant literatures were reviewed. All of the cases were verified via pathological examination after local lesion excision, along with reconstruction of tissue defects in external nose. The patients were all primary healed. With a follow up from 8 months up to 7 years, only one case relapsed one year after first surgery, and given another operation, no recurrence happened thereafter. The other seven cases did not recur so far. The nasal solitary keratoacanthoma is a squamous tumor with potential malignancy, which manifests similarly with squamous cell carcinoma, and the correct diagnosis requires comprehensive judgment from both the clinical and histopathological characteristics. The disease shows favorable prognosis and low recurrence rate.

Methods Peripheral blood DCs of patients and normal human were isolated and cultured in serum free media. 3."Blooming too soon": A case of precocious puberty.

Precocious puberty is the onset of pubertal development at an earlier age than is expected based upon established normal standards. The cause of precocious puberty may range from a variant of normal development (eg. premature adrenarche or isolated premature thelarche) to pathologic conditions with significant risk of morbidity and even death (eg. malignant germ-cell tumor or astrocytoma). A case of an 18 month old female presenting with vaginal bleeding following a previously noted breast enlargement was described. Initial assessment based on the patient's history and physical examination is suggestive of precocious puberty. Hormonal studies indicated normal levels of FSH and LH, with an elevation in estradiol. Radiographic analysis showed a normal bone age. Cranial MRI revealed no abnormal masses. Sonographic evaluation showed bilateral cystic masses in the ovaries. A diagnosis of peripheral precocious puberty associated with functional ovarian cysts was made, and the patient was monitored for progression of pubertal development.

4.Diagnostic value of radionuclide imaging combined with routine CT in detecting hepatic focal nodular hyperplasia

Objective To investigate radionuclide imaging and routine CT in diagnosing hepatic focal nodular hyperplasia (FNH) and the combined diagnostic value of the two modalities. Methods Thirty-two patients with hepatic FNH were retrospectively studied. All patients underwent routine CT scan. Twenty-four patients were examined by 99Tcm-sulfur colloid (SC) hepatic planar scintigraphy and SPECT/CT imaging, and then patients who had abnormal foci underwent 99Tcm-diethyl iminodiacetic acid (EHIDA) triple-phase hepatobiliary imaging. x2 -test of four-table or Fisher exact probabilities in 2 × 2 table was applied for statistical analysis. Results Of all 32 patients pathologically diagnosed as FNH with single solitary nodule, 25 were classified as classic type and the rest 7 as non-classic type. Although routine CT found all hepatic lesions, only 15 cases were diagnosed pathologically as FNH classic type but the rest were either misdiagnosed or left as indeterminate. On radionuclide imaging (hepatic colloid scintigraphy plus triple-phase hepatobiliary images), 11 patients with big foci (with maximal diameter >3 cm) out of 24 patients were correctly diagnosed as FNH, with 7 diagnosed as classic type FNH and 4 as non-classic. Other 13 patients were either misdiagnosed or simply missed. The diagnosing rates of routine CT and radionuclide imaging were60.0% (15/25) and 38.9% (7/18) for FNH classic type, 0/7 and 4/6 for non-classic type,50.0% (10/20) and 73.3% (11/15) for big foci, 41.7% (5/12) and 0/9 forsmall foci (with maximal diameter≤3 cm), respectively. The total diagnosing rate of radionuclide imaging combined with routine CT was significantly higher than that of routine CT or radionuclide imaging alone ( x2 = 4. 48, P < 0. 05;x2 =4.27, P <0.05 ). Conclusion Radionuclide imaging in combination with routine CT may improve the diagnostic accuracy for hepatic FNH patients.
5.Efficacy of single dose antenatal corticosteroid on reducing the morbidity and mortality of preterm infants: A retrospective cohort study.

OBJECTIVE: To determine the efficacy of a single dose of antenatal dexamethasone on the neonatal morbidity and mortality of preterm infants born between 24 weeks to 33 weeks and six days age of gestation at a tertiary government hospital.
METHODS: A detailed chart review of both maternal and neonatal records of all neonates born between 24 weeks and 33 weeks and 6 days age of gestation at a tertiary government hospital from January 1, 2011 to December 31, 2013 was done. Patients were grouped based on maternal exposure to antenatal dexamethasone. After which, rate of neonatal deaths and morbidities were recorded. Chi-square test for categorical variables, independent t-test for continuous data and logistic regression were used for analysis.
RESULTS: Seven hundred and three maternal-neonatal dyads were included. Of these, 120 (17.1%) were not exposed to any antenatal corticosteroid prior to delivery, 347 (49.4%) were exposed to a single dose of 6-mg dexamethasone, and 236 (33.5%) received a complete course of four doses of 6-mg dexamethasone before preterm delivery. There were better neonatal outcomes from mothers who received completed doses of antenatal corticosteroids than those who received only a single dose, however in comparison to those who have not received any antenatal corticosteroids, the group that received only a single dose had significantly better neonatal outcome. Logistic regression analysis demonstrated that exposure to a single dose of dexamethasone before delivery was associated with reduction in neonatal mortality, and select neonatal morbidities.
CONCLUSION: It was observed that there was improved neonatal outcomes in neonates given a single dose dexamethasone compared to those who didn't receive any antenatal corticosteroid. Obstetrician gynecologists should not hesitate in administering antenatal dexamethasone even if completion may not seem feasible.

6.Hepatitis Infection among Workers of Yellow River Bureau

OBJECTIVE To understand the condition of hepatitis B infection among workers of Yellow River Bureau.METHODS On March 2007 326 workers had the physical examination by ELA test in a hospital.RESULTS The positive rate was 11.4% among which HBsAg,HBeAg and HBcAb positive together accounted for 1.5%;HBsAg,HBeAb and HBcAb positive together accounted for 1.5%.CONCLUSIONS Screening hepatitis B and vaccine inoculation are important.

Objective To observe the localization and distribution of follicle stimulating hormone receptor(FSHR) in the rat submandibular gland and stomach. Methods Immunohistochemical SABC method was used in the experiment. Results The serous glandular cells,granular convoluted epithelial cells and all other duct epithelial cells in the submandibular gland and the parietal cells of gastric gland showed FSHR positive immunoreactivity.The positive substance was distributed in the cytoplasm with negative nuclei.Conclusion The function of rat submandibular gland and gastric gland might be regulated by follicle stimulating hormone(FSH) through FSHR.
8.Biotransformation of furannoligularenone by cell suspension cultures of Nicotiana tabacum

Objective To investigate the biotransformation of furannoligularenone by cell suspension cultures of Nicotiana tabacum. Methods Furannoligularenone was added to the medium of the suspension cells of N. tabacum after precultured for 10 d, then they were co-cultured for another 4 d. The biotransformed products were detected with HPLC and isolated by various chromatographic methods. The chemical structures of biotransformed products were elucidated on the basis of their physicochemical pro-perties and spectroscopic data. Otherwise, the influence of co-cultured time on conversion ratio was investigated either. Results The substrate, furannoligularenone (Ⅰ), was successfully biotransformed by N. tabacum cultured cells. Two products, 3-oxo-eremophila-1, 7(11)-dien-12, 8-olide (Ⅱ) and 3-oxo-8-hydroxyeremophila-1, 7(11)-dien-12, 8-olide (Ⅲ) were obtained. After co-cultured for 5 d, the mole conversion ratio of the substrate reached the highest (53.1%). Conclusion It's the first report on biotransformation of furannoligularenone by N. tabacum cultured cells.
9.Effects of glycyrrhizin on activities of peritoneal macrophages of mice

Objective:To investigate the effect of Glycyrrhizin on the cytokines derived from peritoneal macrophages in mice.Methods:Glycyrrhizin was intraperitoneally administered 24 hr before the peritoneal macrophages (PMs) were harvested.The harvested PMs were then stimulated in vitro with lipopolysaccharide (LPS).The levels of tumor necrosis factor (TNF)-?,interleukin-12 (IL-12) p70,interleukin-10 (IL-10) and macrophage colony-stimulating factor (M-CSF) from culture supernatants were measured by ELISA.Results:Glycyrrhizin suppressed LPS-induced TNF-? production and increased LPS-induced IL-12 p70 production by PMs significantly. The production of IL-10 and M-CSF by PMs were not effected by Glycyrrhizin pretreatment.Conclusion:These findings demonstrate the ability of Glycyrrhizin to suppress LPS-induced TNF-? poduction and to enhance IL-12 production by peritoneal macrophages.
10.Practice and perceptions of pediatric residents in a Tertiary Government Hospital on mental healthcare as part of general practice

Objective:
This study aimed to describe mental health practice patterns, perceptions,
perceived responsibility, confidence, barriers, and supports of pediatric residents in a tertiary
government hospital to arrive at recommendations in enhancing their mental health
competencies.
Methodology:
As part of needs and baseline assessment within an existing collaboration,
written self-administered questionnaires were distributed among pediatric residents of the
hospital. Frequency of answers were tallied to see clustering and trends. RESULTS: Most of the
37 Pediatric residents had positive perceptions i.e. 78.83% - 83.78% and planned to incorporate
all aspects mental healthcare in their future practice as consultants i.e. 89.19 %- 97.29 %.
However, they did not get to practice or apply it as much during training. All respondents
perceived that diagnosing the neurodevelopmental disorders of Autism Spectrum, Attention
Deficit and Hyperactivity, Intellectual Disability and Learning was their responsibility. When it
came to confidence in diagnosing other psychiatric disorders less than half to 3/5 (45.9%-
64/9%) were confident or very confident in doing so. Approximately three fifths (56.75%
-67.56%) of them were confident in managing neurodevelopmental disorders. There was an even
lower proportion i.e. one fourth (16.21% - 32.43%) of pediatric residents who were confident in
managing other psychiatric disorders. Time was the most often perceived barrier while more
exposure and training were the supportive factors for integrating mental healthcare into
practice.
Conclusion
With such positive perceptions already in place, programs to improve knowledge
and skills can be developed to increase confidence, focusing on diagnosing and managing
psychiatric conditions. Target areas as guided by this research can be on how to use screening
tools and psychotropics, diagnosing anxiety and behavioral addictions, and managing psychosis
and trauma. These should be incorporated in a time-efficient manner into usual patient
interviews. Training and added exposure can be utilized in the short-term as well as
institutionalization of collaboration, liaison, and feedback systems in the future.
